Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor in the GI tract. Surgical resection of the local disease is the gold standard therapy. In recent years, in virtue of the development of minimal invasive surgery such as laparoscopic surgery and endoscopic surgery, laparoscopic and endoscopic cooperative surgery (LECS) has been applicable for treatment of gastric GIST. This article reviewed the recent advances in LECS in treatment of gastric GIST.

Objective To produce an erythrocytic stage chimeric protein of Plasmodium berghei in Pichia pastoris and evaluate its immunogenicity. Methods The DNA sequences of AMA1 (Ⅲ) and MSP1-19 from P. berghei homologous to the corresponding sequences of P. falciparum chimeric antigen 2.9 (PfCP-2.9) were fused to generate a chimeric gene, designated as PbCP-2.9. The resulting gene was redesigned using Pichia preferential coden usage and expressed in P. pastoris in the secreted form. The recombinant protein was purified by Ni-NTA affinity chromatography. Three groups each with 10 BALB/c mice were im- munized subcutaneously with 20 ?g of purified PbCP-2.9 antigen formulated in Freund’s adjuvant, Montanide ISA720 and Montanide IMS 1 312, respectively. Three control groups each with 10 mice received only adjuvants emulsified with PBS. All the mice received three immunizations at 2-week intervals with the same dose of antigen. Serum samples were collected at preimmunization and one week after each immunization, and were analyzed for specific antibodies by ELISA and reaction with natural P. berghei proteins by IFAT. Results The PbCP-2.9 antigen with Mr 26 400 was successfully expressed in P. pastoris in secret- ed form. The recombinant protein can be recognized by the serum against blood stage parasites of P. berghei. High antibody responses were detected in all three PbCP-2.9-immune groups of mice by ELISA. However, mice immunized with PbCP-2.9 antigen in Freund’s adjuvant produced higher antibody titers than those with PbCP-2.9 antigen in Montanide ISA 206 and Montanide IMS 1312 adjuvants. Objective To express and evaluate the immunogenicity of ectodomain and its subdomains of Plasmodium berghei apical membrane antigen 1(PbAMA-1).Methods Sequence of PbAMA-1 gene was isolated from the genome of P.berghei,and was redesigned and divided into three overlapped fragments according to its subdomain structure.The codon-optimized DNA fragments of PbAMA-1 were synthesized and inserted into vector pET32a for expression in E.coli and the recombinant proteins were purified by Ni-NTA column,followed by refolding in vitro.Mice and rabbits were immu-nized with the recombinant proteins formulated with Freund adjuvant.Titer of the specific antibodies was detected by ELISA and IFA.The immunized mice were challenged by P.berghei to evaluate protective efficacy in vivo.Results The sequence of the PbAMA-1 gene was shown to be identical to that published before.PbAMA-1 sequence was redesigned via codon optimization and synthesized.Both ectodomain and its subdomains of PbAMA-1 were successfully expressed in E.coli after induction.The proteins were isolated with the purity of more than 90% after Ni column purification and refolding in vitro.Immunization of mice with the recombinant proteins induced high level of specific antibodies.The antibody titer to ectodomain E after the 3rd immunization showed a strong immunogenicity at(34.4?0.15)?10-4.The antibodies interacted with the parasites by indirect fluorescence.The immunized mice were partially protected from the challenge of P.berghei.Conclusion The recombinant PbAMA-1 is highly immunogenic and induces protective immunity against the challenge of P.berghei.

Objective: To evaluate the testing level of the relevant items in food inspection agency laboratory objectively, analyze the existing problems and help them enhance the ability of detection.Methods: The blind sample assessment of sodium saccharin in drinks was organized and performed.The test samples at high and low concentrations were prepared and studied by the tests of homogeneity and stability.Those met the requirements of blind sample assessment were randomly distributed to 193 laboratories and the returned data were analyzed statistically.Results: Totally 182 valid data were collected.Among the reported results, those from 85 laboratories were satisfied in the low concentration group with the satisfaction rate of 87.6% , those from 12 laboratories were not satisfied, and no results were suspicious;as for the high concentration group, the above data was 67(78.8%), 12 and 6, respectively.The overall satisfaction rate was 83.5%.The results, especially the dissatisfied and suspicious data, were analyzed by the original records of each participating laboratory, the analysis focused on 8 aspects including detection method, instrument brand, control product variety, pretreatment method, recovery rate, column selection and the other influencing factors, and suggestions were given to every laboratory to improve its own situation.Conclusion: The statistical data reflect that our food testing laboratory has strong detection ability in the determination of saccharin sodium in beverages, and can provide powerful technical support for the regulatory authorities.

Objective To evaluate the feasibility and efficacy of ultrasound-guided popliteal vein catheter thrombolysis for acute deep venous thrombosis of the lower extremity.Methods Clinical data were retrospectively analyzed on 120 patients of unilateral acute lower extremity deep vein thrombosis from April 2010 to April 2013.60 cases were included in systemic thrombolytic group; 60 cases into catheter directed thrombolysis group.Thrombolysis rate and limb swelling reduction rate were calculated and compared between the two groups.Results Due to limb swelling rate,in systemic thrombolysis group:Thigh swelling reduction rate was (77 ± 9) ％ ; Leg swelling reduction rate was (70 ± 11) ％,while in catheter directed thrombolys group,that was (87 ± 5) ％,and (80 ± 9) ％,respectively (P ＜ 0.05).The thrombolysis rate in systemic group was (59 ± 14)％,that was (71 ± 13)％ in catheter directed thrombolysis group (P ＜ 0.05).Conclusions Ultrasound guided precutaneous catheter popliteal vein thrombolysis significantly improves the short-term outcome of deep venous thrombosis in terms of leg swelling reduction rate and thrombolysis reduction rate.

_ Objective_ To evaluate the efficacy of unilateral subtotal adrenalectomy in the treatment of bilateral adrenal solitary neoplasma causing Cushing's syndrome and to elaborate the therapeutic principle. Methods From 2007 to 2013, a total of ten patients were diagnosed with Cushing's syndrome caused by bilateral solitary adrenal neoplasma. We compared patients'clinical symptoms, hormone profiles, biochemical and metabolic parameters, and imaging data before and after the surgery. Five of them chose the optimal neoplasma based on the lateralization ratio of adrenal venous sampling result and the other 5 patients chose the optimal neoplasma based on the diameter of the mass reflected by the computed tomography result and were then operated. Results After the unilateral subtotal adrenalectomy,the24-hour urinary free cortisol decreased significantly(P<0.05)and the midnight serum cortisol level also significantly reduced(P<0. 01). Plasma adrenocorticotropic hormone level increased significantly(P<0. 01). Nine patients of them did not need contralateral adrenalectomy and one patient received contralateral adrelectomy because of the remnant of Cushingnoid symptoms. Conclusion Unilateral subtotal adrenalectomy is an effective and safe way to treat Cushing's syndrome caused by bilateral solitary neoplasma.

Objective To explore the effect of the expressive psychotherapy on the feeling of inadequacy and loneliness of inpatients with chronic schizophrenia.MethodsTotally 120 chronic schizophrenia inpatients met recruited criteria were randomly divided into study group(n=60) and control group(n=60) according to random number table.57 patients in each group finished all the follow-ups finally.The patients in two groups were received normal medications while the patients in intervention group also attended expressive psychotherapy.All the patients were assessed at baseline,6 weeks and 12 weeks of treatment with the feeling of inadequacy scale (FIS) and the status and trait loneliness scale (STLS).Results(1) The FIS score(F=225.19,P<0.01),STLS total score(F=225.20,P<0.01),STLS status score(F=240.40,P<0.01) and STLS trait score(F=106.75,P<0.01) decreased significantly in both groups after intervention.(2) The FIS score of study group was much lower than those of control group at the at 6-week-follow-up(4.93±20.86,57.89±20.48,t=-2.250,P<0.01) and 12-week-follow-up(38.43±17.50,57.21±22.98,t=-4.900,P<0.01).(3) After 6 weeks treatment and 12 weeks treatment,STLS total score(6 weeks:(54.32±12.30) vs (68.46±15.53),t=-5.425,P<0.01,12 weeks:(44.16±10.81) vs (65.75±16.26),t=-8.386,P<0.01),STLS status score (6 weeks:(25.95±6.41) vs (34.31±7.91),t=-6.239,P<0.01,12 weeks:(19.15±5.49) vs (32.58±8.45),t=-10.103,P<0.01) and STLS trait score (6 weeks:(28.37±6.73) vs (34.15±8.45),t=-4.138,P<0.01,12 weeks:(25.00±6.18) vs (33.15±8.22),t=-6.019,P<0.01) in study group were lower than those in control group,and the difference was statistically significant.ConclusionExpressive psychotherapy can effectively improve the loneliness and inadequacy of patients with chronic schizophrenia.

Objective: To evaluate the effects of non-bracket invisible appliance mandibular advance(MA) on skeletal Class Ⅱ growing patients with mandibular retrusion. Methods: Nineteen skeletal Class Ⅱ growing patients with mandibular retrusion who were treated in the department of stomatology of Changshu traditional Chinese medicine hospital from January 2018 to May 2021 were selected. They had an average age of (12.32±1.51) years old. The treatment group consisted of 10 children who were treated with MA. The control group consisted of 9 children who refused to treat their malocclusion. Cephalometrics of all patients were taken before and after the observation or treatment. The cephalometric data of two groups were analyzed. Results: Compared with the control group, in the treatment gruop SNB angle, L1-NB angle, L1-NB distance, L6-MP distance, Z angle increased significantly (P<0.05);Co-Go distance, Co-Gn distance, SL increased more greatly than the control group; ANB angle, U1-SN angle, U1-NA angle, U1-NA distance, L1-MP distance, FCA angle decreased significantly (P<0.05); SE did not change significantly in the treatment group, but increased significantly in the control group (P<0.05). Conclusion MA can promote mandibular growth and improve lateral profile. The angle of the lower teeth and the angle of the mandible plane can be controlled by MA appliance.
