Objective To investigate the feasibility and safety of minimally invasive pereutaneous nephro-lithotomy(MPCNL) in treating impacted upper ureteral calculi. Methods 50 cases with impacted upper ureteral cal-culi treted with MPCNL were analyzed retrospectively. Results All cases were rendered stone free at one session, clearance rate was 100%, 1 case required transfusion,no other major complications was observed. Conclusion MPC-NL was safe and effective in treating impacted upper ureteral calculi with a high clearance rate.

Objective To detect the expression level of miRNA-575 by pancreatic cancer stem cells.Methods Pancreatic cancer cell lines were cultured with serum-free medium.The cancer stem cells were obtained and their characteristics were observed with an invasion assay,a xenograft experiment,and flow cytometry analysis.Moreover,qRT-PCR was used to detect the expression of miR-575.Results A small subpopulation of cells in both ASPC-1 and PANC-1 cells survived and formed spheres in the absence of serum.The mean number of migrated cells of PANC-1 and ASPC-1 spheres was (147.3±18.6) cells and (113.2±12.9) cells.Therefore,the spheres have higher invasive ability compared with those of parental cells.As few as 5 × 105 cells in PANC-1 and ASPC-1 spheres could initiate a xenograft tumor in NOD/SCID mice,which suggested the tumorigenicity of the spheres was 100 times those of parental cell lines.The percentages of CD24+CD44+ in spheres were 0.38％-0.43％ and 4.91％-5.21％,which showed increased expression compared with ASPC-1 and PANC-1 cell lines (P＜0.05).The expression of miR-575 in spheres was 3.71 and 3.83 times its expression in cell lines ASPC-1 and PANC-1 respectively (P＜0.05).Conclusions Pancreatic cancer cell spheres may be a source for collecting cancer stem cells in a serum-free medium culture,and MiR 575 may play an important role in regulating the biological characteristics of pancreatic cancer stem cells.

Objective To explore the clinical efficacy of NEOVEIL in postoperative pulmonary air leakage of pulmonary bullae resection by video-assisted thoracic surgery. Methods We retrospectively analyzed clinical data of 37 cases received NEOVEIL combined with ATB45 in video-assisted thoracic surgery (the NEOVEIL group), and clinical data of 40 cases by ATB45 alone(the control group) . We compared postoperative hospitalization time, keeping time of pleural cavity tubes, the pulmonary leakage time and the incidence of pulmonary infection in the two groups. Results There was no postoperative mechanical ventilation,hemothorax and death. In the NEOVEIL group, the postoperative hospitalization time was 5 to 9 days, the postoperative keeping time of pleural cavity tubes 2 to 4 days, the postoperative pulmonary leakage time 0 to 2 days with 4 patients un-leakage, and 1 patients with pulmonary infection; whereas the postoperative hospitalization time was 6 to 10 days,the postoperative keeping time of pleural cavity tubes 3 to 6 days,the postoperative pulmonary leakage time 2 to 5 days and 8 patients with pulmonary infection in the control group. There was statistical significance in the postoperative hospitalization time, keeping time of pleural cavity tubes, the pulmonary leakage time and the incidence of pulmonary inflammation in two groups(P ＜ 0. 05). The incidence of pulmonary inflammation of the NEOVEIL group (2. 7%) was significantly lower than that of the control group(20. 0%)(x2 = 4. 02, P ＜ 0. 05). Conclusion The operative effectiveness of the NEOVEIL group is better than that of the control group. The NEOVEIL can prevent the postoperative pulmonary leakage.

Objective To analyze the differences of video laryngoscope, direct laryngoscopy and fibreoptic bron﹣choscope nasal intubation on hemodynamics, inflammatory and stress response. Methods 117 patients underwent surgery from November 2013 to March 2015 were chose as research subjects and randomly divided into video laryn﹣goscope group, direct laryngoscopy group, fiberoptic bronchoscope group based on different cannula enrolled way. Then compared the hemodynamics, inflammation, stress level after intubation among the three groups. Results Com﹣pared with the T0, the three groups patients' systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), C-reactive protein (CRP), interleukin -6 (IL-6), tumor necrosis factor (TNF-α), norepinephrine (NE), plasma cortisol (Cor), angiotensinⅡ(ATⅡ) differences had no significant difference (P>0.05);compared with direct laryn﹣goscopy group, video laryngoscope group, fiberoptic bronchoscope group patients' T1, T2, T3 SBP, DBP, HR, CRP, TNF-α, IL-6, NE, Cor, ATⅡ levels were lower; compared with the video laryngoscope group, optical fiber bron﹣choscopy group patients SBP, DBP, HR, CRP, TNF-α, IL-6, NE, Cor, ATⅡ low levels at T1, T2, T3 were lower (P<0.05). Conclusions Fibreoptic bronchoscope nasal intubation has less influence on patients, will not cause severe stress and inflammatory response, it is one of the ideal instrument anesthesia induction intubation.

Objective:To investigate the effect of chronic alcohol consumption on both left ventricular myocardial collagen and diastolic function in rats,and their relationship thereof.Methods:Twenty-four male Wistar rats were randomly divided into 2 groups:control group(n=12)and ethanol group(n=12).The changes in cardiac diastolic function were evaluated by echocardiography and tissue Doppler imaging(TDI).The value of myocardial hydroxyproline content was determined by hydroxyproline reagent kit.The expressions of collagen Ⅰ and collagen Ⅲ mRNA were detected by RT-PCR analysis.Results:It was found that mitral E and mitral annulus Ea were decreased,mitral annulus Aa was increased,and isovolumic relaxation time(IVRT)was prolonged in the ethanol group compared with those in control group(P＜0.05).The value of Ea/Aa ratio was greater than 1 in control group and less than 1 in ethanol group(P＜0.01).It was found that myocardial hydroxyproline content,collagen Ⅰ,collagen Ⅲ mRNA expression and their ratio significantly increased in ethanol group compared with those in the control group(P＜0.01).There was positive correlation between hydroxyproline content,collagen Ⅰ,collagen Ⅲ mRNA expression,and collagen Ⅰ /collagen Ⅲ mRNA ratio with IVRT(P＜0.05),and negative correlation between hydroxyproline content,collagen Ⅰ,collagen Ⅲ mRNA expression,and collagen Ⅰ /collagen Ⅲ mRNA ratio with the Ea/Aa ratio(P＜0.01).Conclusion:Chronic ethanol consumption can induce increase in left ventricular myocardial collagen synthesis and impairment in diastolic function in rats.Left ventricular diastolic dysfunction correlates with increase in myocardial collagen synthesis positively.

Objective: To explore the effects of monocrotaline (MCT) on right ventricular function and expression of cardiac canonical transient receptor potential channels (TRPC) subfamily in experimental rats.
Methods: The SD male rats were randomly divided into 2 groups:Control group, the rats were normally fed and MCT group, the rats received a single dose injection of MCT 60 mg/kg to induce myocardial hypertrophy. n=10 in each group and all animals were treated for 3 weeks. The right ventricular hemodynamics parameters and right ventricular hypertrophy index (RVHI) were measured, right ventricular myocardium tissue section was observed by HE staining, the mRNA and protein expressions of TRPC subfamily were examined by RT-PCR and Western blot analysis.
Results: Compared with Control group, MCT group had increased RVSP, RVHI, RV+dp/dtmax and decreased RV-dp/dtmax, all P<0.01. In MCT group, the right ventricular myocardial cells had the thinker ifber, deeply stained nuclei with irregular shape. Compared with Control group, the mRNA and protein expressions of right ventricular TRPC6 were elevated in MCT group, n=6 and n=4 respectively, all P<0.05.
Conclusion:Right ventricular hypertrophy could be induced by 3 weeks MCT treatment, it up-regulating the mRNA and protein expressions of TRPC6 which might be involved in the occurrence and development of cardiac hypertrophy in experimental rats.

Objective To evaluate the safety of modified sputum induction in moderate to very severe COPD during exacerbation. Methods Ninety patients with moderate, severe or very severe COPD during exacerbation (27 with type Ⅰ and Ⅱ respiratory failure,18 with coronary heart disease, 38 with tachycardia) were chosen as research subjects. All the patients underwent induced sputum. During induction , all patients were given electrocardiographic monitoring and pulmonary function check every 5 minutes and FEV1, HR, SpO2 in the process of testing were recorded. Results Ninety patients underwent 224 sputum inductions progress, 222 were safety. Qualified sputum samples were collected for 216 times, sputum induction was successful in 96.43% of occasions. Heart rate rising and SpO2 reducing during sputum induction mainly occurred in the beginning 5 minutes, FEV1 did not change in the whole process. Conclusion Modified sputum induction can be safe through closely monitoring in patients with moderate-to-very severe COPD with exacerbation, even with type Ⅰ or typeⅡ respiratory failure. But the risk of sputum induction will increase if patients with coronary heart disease and tachyarrhythmia, especially within beginning 5 minutes of sputum induction.
