1.Clinical analysis of 56 cases of pregnant women with thrombocytopenia purpura

Objective To investigate the causes and methods of clinical treatments on pregnancy combined with thrombocytopenia.Methods A retrospective analysis received 56 cases of pregnant women with thrombocytopenia purpura maternal (group A),compared with the same period 56 cases of normal pregnancy (group B),the situation of childbirth,newborns and pregnancy result were observed.Results In group A maternal bleeding was 12.5 %,significantly higher than 3.6% in group B(x2 =3.4356,P < 0.05),and there was a statistically significant difference in the other group A vaginal delivery maternal bleeding rate of 3.4%,lower than 26.1% of the rate of cesarean section bleeding(P < 0.05).Neonatal mortality,intracranial hemorrhage number of cases,thrombocytopenia example number three in group A were higher than those in group B(x2 =3.8796,3.4556,3.4676,all P <0.05).Conclusion Pregnancy with thrombocytopenic purpura is a serious threat on mother and child,which are likely to lead the mother child thrombocytopenia,intracranial hemorrhage and maternal or fetal death.
2.Rituximab combined with autologous hematopoietic stem cell transplantation for therapy of CD_(20),positive B-cell lymphoma

Autologous hematopoietic stem cell transplantation(AHSCT) has a good curative effect for high-risk or middle-risk aggressive lymphoma,but some patients who receive AHSCT still died of recurrence.The main reason for this is the presence of minimal residual disease (MRD), including residual tumor cells in vivo and contamination of grafts with tumor cells.Rituximab can be targeted to remove CD_(20) positive B cells,so application of rituximab before and after AHSCT may play a role in removal of residual disease and in vivo purging.Bituximab combined with AHSCT may further enhance the curative effect for CD_(20) positive B cell lymphoma.
3.Protective Role of Hydrogen Sulfide on Neonatal rat Myocardial Cells by Hypoxia-Reoxygenation Injury

Objective:To investigate the direct effect of different concentrations of hydrogen sulfide(H_2S)on neonatal rat myocardial cells by hypoxia injury during different time point,and the indirect effect of reoxygenation injury in order to analyze the protective role of H_2S on myocardial cells subjected to hypoxia/reoxygenation injury. Methods:Primary cultured myocardial cells from neonatal rats were randomly divided into 9 groups:Normal control group,Hypoxia NaHS 0 μmol/L group,100 μmol/L group,200 μmol/L group,400 μmol/L group and those groups subjected to hypoxia for 24 h,48 h and 72 h. Hypoxia/reoxygenation NaHS 0 μmol/L group,100 μmol/L group,200 μmol/L group,400 μmol/L group and those groups subjected to hypoxia for 24 h,48 h,72 h and then reoxygenation for 2 h. The number of survival cells were counted and the activity of lactate dehydrogenase(LDH)were measured respectively at each time point. Results:At the same time point of 24 h,48 h,72 h hypoxia,the number of survival cells increased and the activity of LDH decreased in hypoxia NaHS treatment groups as compared with Hypoxia NaHS 0 μmol/L group(P<0.01). The protective role of Hypoxia NaHS 200 μand 400 μmol/L groups were better than NaHS 100 μmol/L group at 24 h of time poit(P<0.05~0.01). The number of survival cells by hypoxia/reoxygenation increased and the leakage of LDH from reoxygenation cells decreased in hypoxia/reoxygenation NaHS treatment groups as compared with hypoxia/reoxygenation NaHS 0 μmol/L group(P<0.01).Conclusion:NaHS 100 to 400 μmol/L had the protective effects on myocardial cells by hypoxia/reoxygenation injury. NaHS 200 to 400 μmol/L might offer the best protective effects on myocardial cells by hypoxia injury at 24 h.
4.Value of Xpert MTB/RIF test in bronchial alveolar lavage fluid in the discovery of sputum smear negative pulmonary tuberculosis

Objective To compare the detection rate of Xpert MTB/RIF test and sputum smear and culture method, and to investigate the value of the detection of the smear negative pulmonary tuberculosis.Methods 69 patients with tuberculosis diagnosed in Tianjin Haihe Hospital from July 2014 to July 2015 were examined by fiberoptic bronchoscopy ( and check of tubercle bacillus in sputum smear negative three times ) .With final diagnosis of pulmonary tuberculosis patients the number of statistical smear, culture method and Xpert MTB/RIF test their tuberculosis detection rate.Results A total of 69 patients with sputum smear negative patients with final diagnosis of 54 cases of pulmonary tuberculosis, confirmed 54 cases of tubercle bacillus in sputum smear negative pulmonary tuberculosis patients, Xpert MTB/RIF test was positive in 33 cases accounted for 61.11%(33/54).Among the 35 cases, 17 patients with Xpert MTB/RIF positive, can improve the detection rate of bacterial negative tuberculosis 31.48%(17/54), 19 cases of patients with negative smear positive, 16 cases of Xpert MTB/RIF positive, and the culture method was 84.21%(16/19).BALFXpert MTB/RIF test of tuberculosis found in 61.11%(33/54), the culture method of tuberculosis found 35.19%(19/54).The report is more than 50%, also has reached to 80%, and the selection has a great relationship.Xpert method and culture method, the difference was statistically significant (P<0.05).The positive rate of Xpert test was positive in 2 cases of positive MTB/RIF in the patients with positive smear positive 100%.Conclusion It is a simple, rapid and accurate method to do Xpert MTB/RIF test for BALF, and it can improve the smear negative tuberculosis patients found rate of 48.75%.The positive rate of the sputum smear negative is 59.62%(14+17/35+17), which is better than that of sputum smear and culture.
5.Effects of Gypenosides on contraction of isolated duodenum smooth muscle of rabbits

AIM:To investigate the efftct of Gypenosides on contraction of isolated duodenum smooth mustle of rabbits.METHODS:The routine experimental method of isolated duodenal smooth muscle of rabbits were used.The contraction curves of isolated duodenum smooth mustle before and after administration of different concentrations of Gypenosides(0.01%-1.00%)were recorded by Medlab biological information collecting system.RESULTS:Different concentrations of Gypenosides significantly reduced the contraction amplitude of isolated duodenum smooth mustle of rabbits,and showed a dose-dependent relationship.CONCLUSION:Gypenosides can inhibit the contraction of isolated duodenum smooth muscle of rabbits.
6.Leptin regulates expression of Agouti-related protein in hypothalamus:recent progress

Hypotha is the center of energy homeostasis in the organism.The central step of its endocrinology regulation network is that it can receive the leptin signal provided by peripheral adipose tissues,and subsequently modulate the expression of Agouti-related protein(AgRP) and proopiomelanocortin(POMC),maintaining energy homeostasis by regulating energy intake and consumption.Transcription factors FoxO1 and STAT3(signal transducer and activator of transcription 3) are key molecules in leptin-mediated transcriptional regulation of AgRP;they directly participate in the regulation of energy homeostasis.FoxO1 can enhance AgRP expression and activated STAT3 can inhibit AgRP expression.Leptin can modulate AgRP and energy homeostasis through inhibiting FoxO1 activity and activating STAT3.The article reviews the regulatory mechanism of leptin upon AgRP.
7.Exploration of longitudinal clinical practice for professional degree postgraduate students in pediatric surgery

For professional degree graduate students, we improved the clinical teaching of pe-diatric surgery, on the basis of traditional forms of practice teaching, combining application of longitu-dinal cross major pediatric surgical practice. We had the student tracking patients from outpatient ser-vice and by going through the process of after examination, treatment, follow-up, and summarizing. students learnt all characteristics of the disease, expended their knowledge and grasped the correct, novel clinical thinking of diagnosis and treatment. The implementation of these measures obtained sat-isfactory teaching effect.
8.Clinical Analysis of 19 Cases of Abdominal Wall Endometriosis

Objective To investigate the clinical characteristics, treatment, and factors of recurrence of abdominal wall endometrioms ( AWE). Methods 19 cases of AWE diagnosed at Shaoxing People' s Hospital from January 2005 to April 2009 were reviewed retrospectively. Results All patients had a history of cesarean section. Eleven patients (57.9%) had the typical complaint of an enlarging mass and pain during menstruation. All patients received surgical excision and postoperative adjuvant therapy of gonadotropin releasing hormone agonist (GnRHa) or Mifepristone treatment for 6 months. The diagnosis was confirmed by histological examination in all patients. At follow- up, ranging from 1 month to 3 years, there was 1 case of recurrence of endometrioms during a follow -up of 2 years. Conclusion AWE could be diagnosed easily by its typical clinical manifestations preoperatively. Sonography or MRI may be helpful in identifying the exact anatomical location of the lesion and in excluding other surgical conditions, however it lacks specificness. Surgical excision is the only effective treatment, and complete and wide local excision is the key point to prevent recurrence. Combination surgical excision with postoperative adjuvant therapy might reduce the recurrence, although its effectiveness need being testified in the future.

mRNA and protein expression of interleukin 1? converting enzyme (ICE) were determined with RF PCR and immunohisto chemistry in brain tissue after complete ischemia followed by reperfusion of the forebrain of gerbil.The effect of radix salviae miltorrhizae was also studied. It was found that increased expression of ICEmRNA was induced during the 4 days after ischemia and reperfusion. Meanwhile, the increases in ICE positive reactivity were observed in glial cells in the regions of CA1 and cortex 1~4 days after ischemia and reperfusion, but the ICE positive reaction was inhibited under the effect of radix salviae miltiorrhizae(P
