Objective To explore the effect of serum leptin on puberty development in schoolboys.Methods 170 health schoolboys were investigated on the first spermatorrhea time and detected the levels of serum leptin, testosterone,follicle stimulating hormone (FSH), luteinizing hormone (LH). Case-controlled study was carried by multivariable logistic regression in puberty development of schoolchildren with and without spermatorrhea. Meanwhile, follow-up studies on the independent influencing factors on the puberty development were applied to this investigation in continuous two years.Results The levels of serum leptin, testosterone,FSH and LH of schoolboys with spermatorrhea were significantly higher than those of schoolboys without spermatorrhea. Multiple logistic regression analysis demonstrated that the serum leptin,testosterone, FSH, and bodyweight were the independent influence factors in puberty of the schoolboy.The levels of serum leptin,testosterone of pre-spermatorrhea schoolboys were significantly higher than schoolboys without spermatorrhea.Conclusions Serum leptin,testosterone, FSH, and bodyweight were the independent influence factors in puberty of the schoolboy.The levels of serum leptin and testosterone had obviously changed before spermatorrhea.

Objective To investigate the efficacy and safety of Topiramate as adjunctive therapy in patients with refractory epilepsy.Methods 115 patients enrolled in this open-label,self-controlled prospective study which were designed to adopt slower dosage escalation schedule with lower initial,until reached target dosages(up to 8 weeks) or the most tolerable dosages of Topiramate,and maintained them 1~3 years treatment.The long-term efficacy,safety of Topiramate and their relationship with dosage were evaluated.Results The median percentages reduction were 56.0%,75.8%,76.1%,77.3% and 78.1% after 8 weeks,6 months,1 year,2 years and 3 years Topiramate treatment respectively for adults,and 32.1%,66.7%,68.9%,70.1% and 70.8% respectively for children.The overall efficacy for partial seizures(70.6%) was higher than that for generalized seizures(37.5%)((P

Objective To evaluate the relationship between the number of copies of genes and congenital diaphragmatic hernia by the detection of multiple loci in infants with congenital diaphragmatic hernia. Methods Multiple loci were analyzed by Microarray analysis of Affymetrix Cytoscan 750 k in 11 neonates with congenital diaphragmatic hernia, in whom 1 case was twins,and his fraternal twins were diagnosed of fetuse intestinal dilatation. Results A homozygous deletion (8 p11.22 arr[hg19]) was found in one neonate with congenital diaphragmatic hernia, and was eventually confirmed that the depolymerization of the biotin and metalloprotease (ADAM) 3A genes lead to homozygous deletion of the 1～15 exon. Conclusion The alteration of ADAM3A copy number may be the cause of congenital diaphragmatic hernia.

Objective To explore and compare the effect of sevoflurane and disoprofol to inflammatory factors in patients received abdominal surgeries.Methods Patients with sevoflurane and disoprofol respectively were anaesthetized and detected IL-6,IL-8,IL-10 and TNF-α in their serum at T1 (when anesthesia induction),T2 (after anesthesia induction),T3 (30min after beginning of operations) and T4 (after operations).Results Compared with T1,the serum level of IL-6,IL-8,IL-10 and TNF-α increased significantly in all patients(all P ＜ 0.05) ; Compared with disoprofol group,the serum level of IL-6,IL-8 and TNF-α increased significantly (all P ＜ 0.05) ; Compared with sevoflurane group,the serum level of IL-10 increased significantly(all P ＜ 0.05).Conclusion Intravenous anesthesia with disoprofol in patients receiving abdominal operations is proper for maintenance of inflammatory balance.

Objective To study the effect of clopidogrel on intimal hyperplasia of rabit arta injured by balloon and its mechanism.Methods Twelve New Zealand rabbits were established as atherosclerosis plaques rupture models and were evenly divided in to the high-cholesterol group and the clopidogrel group.Another 6 rabbits were set as the control.Serum cholesterol and triglyceride levels were taken in the beginning,before and after surgery.Meansurement alsincluded the changes in macrophage count and collagen levels in the plaques,changes of plasma fibrinogen and P-selectin levels as well as changes in activity of t-PA and PAI-1.The changes of intima thickness and the degree of stenosis were recorded.Results Compared to the control group,the high-cholesterol group and the clopidogrel group had higher average macrophages count and collagen levels in the plaques.The clopidogrel group had less macrophages count in the plaques than the high-cholesterol group;however,the clopidogrel group had higher levels of collagen in the plaques than the high-cholesterol group.The plasma level of P-selectin reduced in the clopidogrel group after plaques rupture.The activity of t-PA remained uncharged in the clopidogrel group but increased in the high cholesterol group.Compared to pre-treatment level,the activity of PAI-1 increased in the high-cholesterol group but reduced in the clopidogrel groups after atherosclerosis plaques rupture.The thickness of vessel intima increased in both the high-cholesterol group and the clopidogrel group,however,the degree of intima thickening and vessel stenosis is less in the clopidogrel group compared to the high-cholesterol group.Conclusion Clopidogrel relieves inflammation reaction by inhibitting thrombogenesis and the movement of macrophages in atherosclerosis plaques,thus minimizes the degree atherosclerosis and vessel intima hyperplasia.

Objective To observe the effect of synthetic corticotropin with vitamin B 6 on intractable epilepsy of children.Methods 20 patients (3 months to 4 years old) among which 9 children with infantile spasms and 11 children with intractable epilepsy were injected with vitamin B 6 (50～100 mg/d) for 10 days,then treated with cortrosyn 0 015~0 025 mg/Kg?d by the interval mothod of lengthening dosage,total course of treatment was about 2 months,seizure,EEG and some blood biochemical tests were observed in all patients before and after the course of treatment.Results The seizure of 14 patient children stopped after the treatment,seizure frequency was decreased in 5 patients (from 15 to 40 times/week decreasing to 3 to 14 times/week respectively),1 patient with intractable epilepsy was stopped treating because of side effect.The general clinical condition and EEG were also improved.The blood biochemical tests about liver and kidney were normal during treatment,hypokaliemia,hypocalcemia and slight dropsy were appeared in some patients.Conclusion Cortrosyn with vitamin B 6 is effective for infantile spasms and intractable epilepsy in children of low age.

Objective To study the effect of oxygen therapy combined with recombinant epidermal growth factor on cervical cancer radioactive dermatitis. Methods From October 2013 to October 2015 in Guangxi tumor hospital, 70 stage IIIB cervical cancer patients with radioactive dermatitis from intensity modulated radiation therapy (IMRT) were enrolled in the study, 35 cases with dermatitis occurring on odd date as the study group and another 35 with dermatitis occurring on even date as the control group. The former were treated with oxygen therapy combined with epidermal growth factor and the latter with recombinant epidermal growth factor alone. The two groups were compared in terms of curative effect, wound healing time and pain. Results The total effective rate of the research group was significantly higher than that of the control group (P<0.01). The healing time of the research group was significantly shorter than that of the control group (P < 0.001). Conclusion The oxygen therapy combined with recombinant epidermal growth factor is more effective in improving the cure rate, shorten healing time of dermatitis, reduce the pains from radiotherapy and help them through the radiotherapy process.

Objective To assess the enteral iron and energy intake of premature neonates during hospitalization in neonatal intensive care unit(NICU).Methods This retrospective study enrolled 208 premature neonates admitted in the NICU of Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine within 48 hours after birth from January 2012 to December 2013.The following data were recorded for all infants:clinical basic information,the amount of formula iron intake in milk,enteral and parenteral nutrition intake as well as oral iron supplementation.Results The amount of enteral iron intake in the 1st,2nd,3rd,4th week after birth and at discharge was (0.2 ± 0.3) mg/(kg · d),(0.9 ±0.7) mg/(kg · d),(1.2 ±1.0) mg/(kg · d),(1.5 ± 1.1) mg/(kg · d),and (2.2 ± 1.1) mg/(kg · d),respectively,which accounted for 10％,45 ％,60％,75 ％ and 110％ of the recommended iron intake.The energy intake in the 1st,2nd,3rd,4th week after birth and at discharge was (13.2 ± 14.8) kcal/(kg · d) (1 kcal =4.184 kJ),(46.0 ± 32.6) kcal/(kg · d),(62.2 ± 38.1) kcal/(kg · d),(71.3 ± 38.2) kca]/(kg · d) and (105.3 ± 32.1) kcal/(kg · d),which accounted for 11.0％,38.3％,51.8％,59.4％ and 87.8％ of the recommended intake,respectively.Lower enteral iron and energy intake after birth occurred in the premature infants with lower birth weight,especially those with anemia who had significantly lower enteral iron intake than the normal neonates in the first 2 weeks after birth.Conclusions Enteral iron and energy intake among premature neonates were insufficient,especially during the first 2 weeks after birth.It is necessary to develop and implement more effective nutritional support to improve the nutritional status of premature infants.

Aim To observe the effects of liver cirrho-sis on the expression of transforming growth factor-β1 ( TGF-β1 ) and ColⅠin rat myocardium and interven-tion of erythropoietin ( EPO ) . Methods Thirty-six male Sprague-Dasley rats were randomly divided into three groups:control group, liver cirrhosis group and EPO group, then the cardic hemodynamic parameters in vivo and levels of serum lactate dehydrogenase ( LDH ) as well as creatine kinase isoenzyme ( CK-MB) were measured. With Masson′s trichrome stain, changes of collagen formation of myocardial tissue in different groups were observed. Also the mRNA ex-pressions of TGF-β1 and ColⅠin myocardium were de-tected by RT-PCR. Results In contrast to control group, rats in liver cirrhosis group showed a decline in systolic and diastolic function of left ventricule, rising myocardial enzyme, a distinct increase of cardiac colla-gen deposition, as well as an elevation of TGF-β1 and ColⅠmRNA expressions. In contrast to liver cirrhosis group, rats in EPO group demonstrated an improve-ment in systolic and diastolic function of left ventricule as well as in cardiac collagen deposition, and a de-crease in both myocardial enzyme and TGF-β1 and ColⅠmRNA expressions. Conclusion Liver cirrhosis can lead to the changes of myocardial structure and function in rats,and it can accelerate myocardial inter-stitial fibrosis; EPO can protect the myocardial injury in liver cirrhosis rats.

Objective To investigate the prognostic value of heart rate turbulence (HRT) in patients with chronic heart failure (CHF). Methods From October 2006 to May 2009, a total of 96 elderly CHF inpatients were selected, and the clinic data were recorded. Based on the echocardiogram and dynamic electrocardiogram, the differences of sinus HRT index of patients with different cardiac function classification were analyzed. During 9-28 months medical follow-up, the treatment endpoint was death from heart disease. Based on Logisitc regression, the prognostic values of HRT, age, hypertension, diabetes, myocardial infarction (MI), left ventricular ejection fraction, ACEI and β- adrenergic blocker for death of CHF patients were analyzed.Results There was no significant difference in HRT between grade Ⅱ and grade Ⅲ cardiac function (χ~2 = 1.60, P>0. 05), and between grade Ⅲ and grade Ⅳ cardiac function (χ~2 = 1.43, P>0. 05). But there was significant difference in HRT between grade Ⅱ and grade Ⅳ cardiac function patients χ~2 =9.84, P<0. 05), and HRT was weaken in grade Ⅳ group. The average follow-up time was (18. 0±9.6) months. Of all 96 patients, there were 34 dead of heart disease. There were correlations of death of CHF with HRT, low LVEF (≤ 45%), age (≥65 years), diabetes, MI and classification of heart function. Conclusions The sinus HRT in CHF patients has a favorable prognostic value.
