2.The different expression of NF-κB p65 protein in placental tissues between premature delivery and term delivery

Objective To investigate the different expression of NF-κB p65 Protein in placental tissues between premature delivery and term delivery and to explore the significance of nuclear factor kB (NF-κB) protein expression in preterm delivery. Methods Fifty premature delivery pregnant women and thirty term delivery pregnant women ( were enrolled in this study. According to the way of delivery , the patients were divided into two groups. The expression of NF-κB p65 protein was analyzed by using immunohistochemistry method in the placental tissues. Results In the term delivery, NF-κB p65 was mainly negative or weak positive expressed in the cytoplasm of cells in the placental tissues. There was no demonstrable difference in immunostaining of the NF-kB p65 subunit in the placental tissues including chorion, umbilical cord and fetal membranes before and after labor ( P > 0.05 ). In the premature delivery, NF-κB p65 was strongly expressed in the cytoplasm and nuclear of cells. The expression of NF-κB p65 protein in chorion and fetal membranes from premature delivery( 18% ,56% ) was significantly higher than that from normal pregnant women (6.7% ,13.3%, P <0.01 ). But no significant change was found for NF-κB p65 expression in umbilical cord in pregnant women with or without premature delivery ( P > 0.05 ). And in fetal membranes and deciduas, there was a significant increase in the staining of immunoreactive NF-κB p65 in preterm(52.9% ,58.8% ) by caesarean section compared to tissues obtained from term delivery( 13.3%,6.7%, P <0.01 ). In chorion and fetal membranes, there was significant increases in the staining of immunoreactive NF-κB p65 in preterm ( 24.2%, 57.6% ) by vaginal dehvery compared to tissues obtained term delivery (6.7%, 13.3%, P <0.01 ). Conclusion In this study, the expression of NF-κB p65 did not show significant change in term delivery before and after labor. NF-κB p65 in premature delivery was higher than term delivery, and it had no relationship with the delivery ways.
3.Implementation and evaluation of group health education mode in cataract department

Objective To investigate the implementation effect of group health education mode in cataract department. Methods Through the establishment of clinical health education path and repeatedly broadcast in group health education after surgery for 1200 cases of cataract patients,effect of health education and satisfaction on surgery were assessed and surveyed. Results Knowledge of health education was known by about 88.5% of patients or their family;satisfaction with nurse care was up to 95.0%; health education time of the nurses was reduced but their work efficiency was improved. Conclusions Implementation of group health education mode and clinical health education path could satisfy patients' demands of health education in large-scale cataract surgery and ensure the quality of health education.
4.Clinical Analysis of 130 Thrombocytopenia in Pregnancy

Objective To explore the causes, management and outcome of pregnant women with thrombocytopenia. Methods Medical records from 1999 to 2003 were reviewed for diagnosis,treatment ,and neonatal outcome in 130 women with thrombocytopenia in Xinhua Hospital. Results Thrombocytopenia was mainly caused by prenancy-associated thrombocytopenia (PAT) in 94 (72 3%) cases,idiopathic thrombocytopenia(ITP) in 12 cases (9 2%),hepatic disease in pregnancy in 11 (8 5%)cases, pregnancy induced hypertension 10 cases and preeclampsia 4 (10 8%)cases, Rhesus isoimmunization and viral infection each 1(0 8%) case. Thrombocytopenia was identified initially at 17 +4 weeks of gestation,37 (28 5%) cases were found before 28 weeks,93 (71 5%) cases were found after 28 weeks. Prednisone was considered if the platelet count was less than 50,000/uL. Platelet transfusion was given to those with platelet count less than 50,000/uL before and after delivery. If the platelet count was less than 50,000/uL esarean delivery might be performed. The occurrence of postpartum hemorrhage was 2 3%. Bleeding occurred in two neonatals. One newborn suffered from cephalohematoma. Sixty-three cases PAT recovered spontaneousely within 1 week after delivery. Conclusion Pregnancy thrombocytopenia usually becomes evident in the third trimester. PAT is the most common type of thrombocytopenia during pregnancy. Pregnancy thrombocytopenia is not associated with bleeding diathesis in the mother. Degree of thrombocytopenia with complication was more severe(platelet count usually less than 70,000/uL). Half of ITP are symptomatic. Glucocorticoid is effective treatment for severe thrombocytopenia. Platelet transfusion may be considered when platelet count is still less than 50,000/uL before operation. Mother with ITP may affect infant.Besides therapy directing at the eiology, corticosteroids and platelet transfusion are effective treatment for severe thrombocytopenia during delivery. One half women return to a normal platelet count within one week after delivery.
5.Transfection of B16 Melanoma Cells with IL-3 Gene and Its in Vitro Proliferative Features

IL-3 has effects on a wide variety of cell types, including immature cells of the immune cells, and mature immune cells such as granulocytes. In the purpose of studying the immunoregulatory function of IL-3 and its potential role in cancer therapy, we established a IL-3-secreting tumor model using gene transfection to deliver locally IL-3 to tumor site. First, we constructed IL-3 expression vector BMGNeo-mIL-3, then transfected it into B16 murine melanoma cells. By G418 resistant screening and limiting dilution, we obtained a transfectant that expressed high levels of IL-3 (806U / ml) . The IL-3 expression of the transfectant was confirmed by Northern blot analyses. Although the wild-type B16 cells and the B16-Neo cells transfected with BMGNeo did not express IL-3, the IL-3 expression in B16 cells had no obvious effect on the in vitro proliferation of the transfectant. These results showed we had successfully established a IL-3-secreting tumor cell clone which would enable us to further study its in vivo tumorigenicity and immune function.
6.Prospective Study of Sonographical Scanning as Prenatal Screening for Abnormal Fetuses With Aneuploid Chromosome

Objective To sonographically measure nuchal skin thickness(NT) and biparietal diameter/femur length (BPD/FL) for normal fetuses between 10 and 24 gestational weeks,screen abnormal cases then to culture fetal cells and chromosome analysis, diagnosis aneuploid chromosome deformed fetuses Methods Ultrosonic estimation of fetal gestation according to gestational sac or CRL or BPD/Fl and AC, measuring NT, BPD/Fl of 100 cases per gestational weeks, drawing normal curve of NT, making curve of 90 th and 10 th NT and BPD/Fl. Restropectively analysis 5 cases trisomy 21.Results The average of NT was 1 76mm between 10 and 13 gestational weeks. The average of NT was 3 52mm between14 and 24 gestational weeks. The value of 50th was 2 40~1 40 between14 and 24 gestational weeks.Conclusions If the value of NT is more than 90 percent of normal fetuses, and if BPD/FL ratio is morn than 90 percent of normal fetuses,the sensitivity of trisome 21 should increase.Combining factors as elderly and high risk parturition, the sensitivity of trisomy 21 is much more increasing.
7.Analysis of the correlation between copy number difference of ADAM3A gene and congenital diaphragmatic hernia

Objective To evaluate the relationship between the number of copies of genes and congenital diaphragmatic hernia by the detection of multiple loci in infants with congenital diaphragmatic hernia. Methods Multiple loci were analyzed by Microarray analysis of Affymetrix Cytoscan 750 k in 11 neonates with congenital diaphragmatic hernia, in whom 1 case was twins,and his fraternal twins were diagnosed of fetuse intestinal dilatation. Results A homozygous deletion (8 p11.22 arr[hg19]) was found in one neonate with congenital diaphragmatic hernia, and was eventually confirmed that the depolymerization of the biotin and metalloprotease (ADAM) 3A genes lead to homozygous deletion of the 1~15 exon. Conclusion The alteration of ADAM3A copy number may be the cause of congenital diaphragmatic hernia.
8.Application of gastric tube insertion by left recumbent position for patients with exanimation

Objective To explore the practicability and clinical significance of gastric tube insertion by left recumbent position for patients with exanimation. Methods We divided 100 patients with exanimation who needed gastric tube insertion into the routien group and the left-recumbent-position group with 50 cases in each group randomly and adopted routine insertion method and insertion by left recumbent position.The success rate of tube insertion and incidence of complications were observed in the two groups.Results The one-time success rate of insertion was higher while the incidence of complication was lower in the left-recumbent-position group compared with the control group (P<0.05). Conclusion Gastric tube insertion by left recumbent position proved to be a desirable method in that it could increase the onetime success rate of insertion and decrease the complications.So it worth widely applying.
9.Correlation of retinol binding protein 4 expression in maternal serum and subcutaneous adipose tissue with insulin resistance in gestational diabetes mellitus

Objective To investigate the expression of retinol binding protein 4(RBP-4) in maternal scrota and subcutaneous adipose tissue and its relationship with insulin resistance(IR)in gestational diabetes mellitus(GDM).Methods From May 2008 to April 2009,62 pregnant women who underwent elective cesarean section in the Department of Obstetrics.Affiliated Hospital of Qingdao University Medical College,were recruited,including 32 with GDM(GDM group)and 30 with normal glucose tolerance test(control group).Enzyme-linked immunosorbent assay(ELISA)was used to determine the serum concentrations of RBP-4 and radio immunoassay to measure the serum levels of fasting insulin (FINS).Fasting plasma glucose (FPG) was tested by glucose oxidase,and the Home model insulin resistance index(HOMA-IR)Was caleulated.Reverse transcription PCR(RT-PCR)and Western blot were applied to investigate the expression of RBP-4 mRNA and protein in subcutaneous adipose tissue.The correlations between the expression of RBP4 mRNA and protein in subcutaneous adipose tissue and the serum RBP-4 concentrations and HOMA-IR were analyzed. Results (1) The serum concentrations of RBP-4、FINS、FPG、HOMA-IR in GDM group were significantly higher compared with the control group [(27. 0 ±1.2) mg/L vs. (19.4±1.8)mg/L, (12.1±1.4)mU/L vs. (8.3±0.8)mU/L, (5.3±0.9)mmoL/L vs.(4. 1±0. 6) mmol/L, 2. 5 ± 0. 2 vs. 1.5 ± 0. 1, P < 0. 05, respectively]. ( 2 ) The expression of RBP-4 mRNA and protein in subcutaneous adipose tissue in the GDM group were significantly higher than that of the control group (0. 76 ± 0. 12 vs. 0. 53 ± 0. 06, 0. 74± 0. 09 vs 0. 54 ± 0. 06, P < 0. 05). (3) In the GDM group, the expression of both RBP-4 mRNA and protein in the subcutaneous adipose tissue were positively correlated with HOMA-IR(r=0. 575 and 0. 851, P < 0. 05). The serum concentration of RBP-4 were also positively correlated with HOMA-IR (r = 0. 635, P < 0. 05 ). No correlations was found between the expressions of RBP-4 mRNA and protein in subcutaneous adipose tissue with the serume RBP-4 concentrations. Conclusion High expression of RBP-4 mRNA in subcutaneous adipose tissue and the elevation of serum RBP-4 levels in GDM women may contribute to IR.
10.Correlation of lipocalin-2 and its receptor expressions with preeclampsia

Objective To research the correlation of the expressions of lipocalin-2 (LCN-2) and its receptor (NGALR) in serum and placenta with preeclampsia.Methods From Dec.2010 to Apr.2011,64 women with preeclampsia who delivered in Affiliated Hospital of Qingdao University Medical College were recruited in the study,including 26 women with moderate preeclampsia ( MPE group) and 38 women with severe preeclampsia (SPE group).Twenty-five healthy pregnant women were taken as control group.LCN-2 and NGALR mRNA and protein expression in placenta were measured by reverse transcription-PCR ( RTPCR) and western blot,respectively.Results ( 1 ) The serum levels of LCN-2 in MPE group and SPE group [ (58 ±20),(90 ± 18) μg/L] were significantly higher than that in control group [ ( 19 ±6) μg/L,P<0.01] ; the serum LCN-2 level in SPE group was significantly higher than that in MPE group (P <0.01).(2) LCN-2 mRNA expression in placenta in MPE group and SPE group (0.55 ±0.14,0.61 ±0.14) were both significantly higher than that in control group (0.28 ±0.16,P <0.01 ) ; LCN-2 protein expression in placenta of MPE group and SPE group ( 2.2 ± 0.4,2.4 ± 0.5 ) were also significantly higher than that in control group (1.4 ±0.4,P <0,01 ),no significant difference was found between MPE group and SPE group ( P > 0.05 ),(3) No significant difference was found in the expressions of NGALR mRNA in placenta among MPE group,SPE group and control group (0.46 ±0.1l,0.46 ±0.14,0.45 ±0.15,P >0.05 ).(4) NGALR protein expressions in MPE group,SPE group and control group were 2.7 ±0.8,3.0 ±0.9,and 2.7 ± 0.9,and there were no significant difference among these three groups ( P > 0.05 ).(5) In preeclampsia,serum LCN-2 level significant associated with 24 hours total urinary protein and uric acid ( r =0.565,0.476,P<0.01).LCN-2 serum level were not associated with systolic pressure and diastolic pressure (P > 0.05 ) ; there were no association with the expressions LCN-2 mRNA aud protein in placenta ( P > 0.05).Conclusions Serum LCN-2 level is closely related to the progress of preeclampsia.Increasing expression of LCN-2 in placenta may be a compensatory response to preeclampsia.
