1.Effect of nitric oxide inhalation on neutrophil CD11b expression and postoperative function in patients undergoing cardiac valve replacement with CPB

Objective To investigate the effects of sustained nitric oxide (NO) inhalation during operation on neutrophil CD11b expression and postoperative lung function in patients undergoing cardiac valve replacement with cardiopulmonary bypass(CPB) .Methods Forty-two ASA Ⅱ or Ⅲ patients aged 35-53 yr weighing 47-69 kg undergoing cardiac valve replacement with CPB were randomly divided into 2 group:NO group(n = 21) and control group(n= 21) .In NO group the patients inhaled 40 ppm NO through the lungs and oxygenator after tracheal intubation during operation before the chest was closed. Anesthesia was induced with fentanyl 20 ?g?kg-1 and pipeceuronium 0.2 mg?kg-1 maintained with isofluarane inhalation and intermittern i.v. boluses of fentanyl and pipecuronium. A total of 5 000 000 IU aprotin was given during operation. Arterial blood samples were taken before induction of anesthesia (T0, baseline), immediately before CPB (T1 ) 10 rain after CPB was started (T2 ), immediately before aortic unclamping (T3 ) ,10 min after aortic unclamping (T4 ), at the end of operation (T5 ) , 12 h and 24 h after operation (T8 ,T9) for polylmorphonuclear(PMN) counts and determination of neutrophil CD11b expression . Blood gas analysis was performed PA-a O2 was calculated and dynamic compliance of respiratory system (Cdyn) was measured before induction of anesthesia, at the end of operation and at 4 and 8 h after operation. Changes of syndromes and therapeutic feature of the patients with steroid-dependent asthma(SDA)during steroid withdrawal were investigated.It was held that the patient presented pathological changes from deficiency of Yin and hyperactivity of evil fire before steroid withdrawal to deficiency of Yin and Yang during steroid withdrawal to deficiency of kidney-Yang after steroid withdrawal,with the syndrome of stasis of phlegm and blood throughout the whole process.Therefore,a new scheme for weaning the steroid with the TCM three-step sequential therapy was established according to the changes of syndromes,i,e., nourishing Yin and blood,clearing away excessive heat and eliminating stasis of phlegm and blood before steroid withdrawal; nourishing Yin and blood,inhibiting pathogenic wind,replenishing Qi and Yang,activating blood circulation,resolving phlegm and lowering the adverse rise of Qi to relieve asthma during steroid withdrawal;warming kidney-Yang and adjusting Qi and blood,and regulating functional activity of Qi after steroid withdrawal.Because the three-steep sequential therapy was designed based on the dynamic change of SDA during steroid withdrawal,it conformed with the whole clinical process of the occurrence,development and changes of SDA than any other single way,and reflected the essential characteristics of the pathological changes.
3.A study of maxillary canine impaction and root resorption of adjacent incisor in three-dimensional space

Objective:To localize the impacted maxillary canine and to observe the root resorption of adjacent incisor in 3 dimen-sions using cone beam computed tomography(CBCT).Methods:92 impacted maxillary canines in 63 patients were scanned by CBCT.The three-dimensional images were obtained by multiplanar reconstructions(MPR).The cusp tip of each impacted canine was localized and set to the X,Y and Z planes after the observation of sagittal,coronal and axial views.The root resorption of adjacent in-cisors was observed.Results:In the X-axis,92% of impactions were mesial by 1 0.4 to 1 5.1 mm,8% distal by 2 to 5 mm;in the Z-axis,60% of impactions were palatal by 1 to 4 mm,40% buccal by 0 to 4 mm.41 .3% of the impacted canines were without root resorption of adjacent incisor,36.5% with slight,1 4.3% with moderate and 7.9% with severe root resorption of the adjacent inci-sor.Conclusion:The most frequent location of impacted maxillary canine is palatal and mesial with high incidence of root resorption of adjacent incisor.
4.Efficacy of Twin-block appliance in Class II division 1 of adolescents: A systematic review

Objective; To evaluate the efficacy of Twin-block appliance in Class Ⅱ division 1 of adolescents. Methods: Several e-lectronic databases (PubMed, The Cochrane library, Embase, CBMdisk, CNKI, VIP)were searched. Abstract that appears to fulfill the initial selection criteria were selected by consensus and original articles were retrieved. Five Chinese journals were hand searched for possible missing articles. Randomized controlled trials(RCTs) that evaluate the efficacy of Twin-block appliance in Class Ⅱ division I adolescents without any surgical intervention or syndromic characteristics were considered. A comparable untreated control group was required to factor out normal growth changes. The quality of included studies was evaluated according to Cochrane Reviewers Handbook 4.2.6 Meta-analysis was performed by using RevMan 5.0.0 software. Results: Four RCTs including 289 patients were identified. The results of our meta-analysis showed a significant increase on SNB angle, Ar-Gn, Nasolabial angle and Labial-mental fold compared with control group(P<0.05). Whereas overjet, overbite, ANB angle and Ls-E had a significant decrease. In addition, SNA angle and Li-E didnt show statistical significant changes(P>0. 05). Conclusion; In Class II division 1 of adolescents, Twin-block appliance can effectively decrease their overjet, overbite, ANB angle, and induce anterior-posterior growth of mandible. The effect on restrain forward growth of the maxilla was unclear. More RCTs are required to evaluate the efficacy of Twin-block appliance.

Objective ToinvestigatethementaldevelopmentsituationofpreschoolchildreninTaiyuan. Methods 1174preschoolchildrenoftwourbanareasinTaiyuanwereselected.TheyweretestedwithDenverDevel-opmentScreenTest(DDST).Results Thepassingratewas94.80%in1174children.Theirindividual-social reaction ability and movement ability were good in the three groups.In the subject-touch reaction ability,the passing rate ofvertical tilt at 30°insidewas 36.36%in 4 years old group,which was higher than the other projects.In the speaking ability,the passing rates of three right in four problems with knowing preposition were 15.88% and 27. 22% in 4 and 5 years old group,which were higher than the other projects.The culture degree of the mental retar-dation children's parents were lower than those of the normal children's parents,the difference were statistically signifi-cant(χ2=4.485,P=0.034;χ2=7.577,P=0.006).Conclusion DDSTisaquicklyscreeningmethodformental retardation children,it is suggested to be used in medical and health examination for preschool children.
6.Comparison of biological characteristics of adipose-derived versus bone marrow-derived mesenchymal stem cells

Objective To compare the biological characteristics of adipose-derived mesenchymal stem cells (ADAS) and bone marrow derived mesenchymal stem cells (BMSCs).Methods The adipose and bone marrow-derived sources of mesenchymal stem cells were separated,and their phenotype,cell doubling time and the secretion of factors were compared.They were also used to detect T-cell cycle,activation,and proliferation inhibition.Results BMSCs and ADAS were similar on the cell phenotype and the differences only existed in the expression of only CD106.For the proliferation rate,ADAS grew faster than BMSCs (doubling time 28 h vs.39 h,P<0.05); ADAS and BMSCs also had the same ability to inhibit T cell proliferation,and dose-dependent effects existed in mitogen-stimulated Tcell proliferation and MLR: there was a strong inhibitory effect in 1:2,but this effect disappeared at 1: 100.Both ADAS and BMSCs could arrest most T cells in the G0/G1 phase,but the role of ADAS was weaker than that of the BMSCs.ADAS could not inhibit apoptosis of T cells.ADAS and BMSCs played the same roles in inhibiting the differentiation of TH0 to TH1 or TH2: mainly inhibiting differentiation of TH 0 to TH1 cells (IL-2-and IFN-γ-producing cells),but having no significant effect on TH2 cells (IL-4-and IL 10-producing cells).Conclusion ADAS and BMSC have a similar role in immune regulation.In the same volume,fat tissue has the number of more than 10 times of stem cell precursor cells than that of bone marrow,so adipose tissue is a more promising stem cells transplant source.
7.Epstein-Barr virus-associated and Epstein-Barr virus not-associated intestinal T-cell lymphomas:A clinicopathologic study

Purpose To investigate the clinicopathological and immunohistochemical features of Epstein-Barr virus-associated and Epstein-Barr virus not-associated primary intestinal T-cell lymphomas(ITCL) and to study their cell origins. Methods In situ hybridization for EBER1/2 and immunohistochemical staining for immunophenotypes, LMP-1,TIA-1,bcl-2 and CD21 were performed in 32 cases. The clinical data were analyzed and all patients were followed-up. Results (1) In 27 of the 32 cases, EBER1/2 were detected in the tumor cells, in which 11 presented LMP-1 positive reactions. (2) All 32 cases of ITCL revealed CD45RO positivity,in which 4(12.5%) expressed CD8,8(25.0%)expressed CD4, 9(28.1%)expressed CD56,and 31(96.9%)expressed TIA-1. There were 17(53.7%)cases with CD4-,CD8-,CD56- immunophenotype. None expressed bcl-2 and CD21. 32 ITCL were classified into pleomorphic medium and large cell(n=28), monomorphic medium-sized(n=2), pleomorphic small cell(n=2). Clinically, most patients with ITCL were young males with abdominal pain, hematochezia, fever and weight loss. The prognosis of patients with ITCL showed poor (survival median was 1.7 month). (3) The differences between EBV-associated and EBV not-associated ITCL lay in hematochezia, fever and the expression of CD3, CD8 and CD56. Conclusion Most of Chinese ITCL are EBV-associated ones with unusual clinicopathological and immunohistochemical features,which are of different lineages of T-cell subtypes, including cytotoxic T-cell or NK cell.
8.Neuroprotective Effect and Anticoagulation Effect of Total Saponins of Radix Liriopes on Focal Cerebral Ischemia

OBJECTIVE:To investigate the neuropotective effect and anticoagulation effect of total Saponins of Radix Liriopes on focal cerebral ischemia in rats.METHODS:Chemical reagent Fecl3 was locally applied on the injured vessels to establish middle cerebral artery thrombosis model,and the effects of total Saponins of Radix Liriopes on rats' behavioral disturbance,brain infarct size,the histopathological changes in brain and the expression rate of nNOS immunoreactive positive neurons were measured,and the bleeding time and coagulation time were also detected with glass tube method and tail transection method.RESULTS:Due to the use of total Saponin of Radix Liriopes(10 and 40mg? kg-1),the brain infarct size was significantly decreased,the behavioral disturbance were improved and the expression rate of the nNOS immunoreactive positive neurons in rats were decreased.At doses of 20 and 60mg? kg-1,total Saponin of Radix Liriopes significantly prolonged the coagulation time and bleeding time.CONCLUSION:Total Saponin of Radix Liriopes has nuroprotective effect on focal cerebral ischemia induced by middle cerebral artery thrombosis and significant anticoagulation effect.

10.Follicular occlusion triad in a family: a clinical survey and mutational analysis

Objective To observe the clinical features and to identify γ-secretase gene mutations in a Chinese family with follicular occlusion triad (FOT).Methods Clinical evaluation was carried out in a family with FOT through field investigation.Peripheral blood samples were obtained from the family members and 100 unrelated healthy controls.DNA was extracted from the blood samples,and PCR was performed to amplify all the coding regions of PSEN1,PSENEN and NCSTN genes followed by DNA sequencing and comparative analysis.Results There were 14 members over 3 generations in this family,of whom,6 (4 males and 2 females) were affected by FOT.FOT was inherited in an autosomal dominant manner in this family.Clinical manifestations varied greatly among the 4 surviving affected members.DNA sequencing revealed a novel missense mutation,c.647A > C (p.Q216P),in the exon 6 of NCSTN gene in the proband,which was cosegregated perfectly with affected,but not with unaffected,members in the family.The mutation was not found in any of the unrelated controls and had not been registered in the single nucleotide polymorphism (SNP) database in NCBI.Conclusions There is a novel heterozygous missense mutation,c.647A>C in the exon 6 of NCSTN gene,which may be the molecular basis of pathogenesis of FOT in this family.
