1.STUDIES ON ANTITUMOR AND IMMUNOSUPPRESSION EFFECTS OF SODIUMPYRIDINETHIONE

Sodium pyridinethione, in vitro, possesses a significant inhibitory effect on human tumor cell strains, such as Hela, CNE-2 and Raji cell strain. In vivo, it has not inhibitory effect on the transplanted tumors, such as S180, LLC and EC, but significantly enhance the inhibitory effects of 5-Fu on S180.Sodium pyridinethione markedly decreases the weight of spleen and thymus, and serum hemolysins reaction induced by SRBC in mice and skin delayed type hypersensitivity reaction induced by DNCB in guinea pigs It also inhibits lyphocyte transformation induced by PHA. All these effects are shown clear dose-response relationship. Our experiments showed that sodium pyridinethion is a new antitumor and immuno-inhibitory compound.
2.Study on shortening the time of initial oral intake after general anesthesia in patients receiving oral and maxillofacial surgery

Objective To study the effect of shortening the time of initial oral intake and fluid intake after general anesthesia in patients receiving oral and maxillofacial surgery on promotion of postoperative recovery. Methods A total of 116 patients were divided into the control group and the observation group with 58 cases in each group by random digit table from January 2014 to December 2015. Patients in the control group were allowed to take food orally 6 h after they regain analepsia from anesthesia;while those in the observation group were allowed to drink water after 2 h upon analepsia from anesthesia and take liquid food after 4 h. The mis-aspiration, the fasting glucose, blood pressure, heart rate and the rate of hunger, thirst, nausea, vomiting as well as blood pressure and heart rate were observed. Results The rates of hunger, thirst, nausea, vomiting in the observation group were 17.2%(10/58), 5.2%(3/58), 12.1%(7/58), 1.7%(1/58), which were significantly lower than those in the control group, which were 94.8%(55/58), 91.4%(53/58), 36.2%(21/58), 10.3%(6/58), and there were significant differences(χ2=7.484-54.772, all P<0.05). The fasting plasma glucose level in the observation group [(4.93 ± 0.85)mmol/L] was higher than that in the control group [(4.05 ± 0.51)mmol/L],and there was significant difference(t=-7.152, P<0.05). There were significant differences in blood pressure 3 h and 5 h after operation compared with those of the control group(P<0.05). Conclusions Shortening the time of taking food for patients undergoing oral and maxillofacial surgery can relieve uncomfortable symptoms , prevent hypoglycemia and the hemodynamic disturbance after anesthesia and promote recovery,early postoperative feeding should be suggested on the premise that they are fully awake.

4.The Pathogencity of EC1-2 Epitope in Pemphigus Vulgaris Antigen

Objective To investigate the pathogenic significance of antigenic epitopes and their relevant antibodies in pemphigus vulgaris (PV) by neonatal mouse model. Methods The extracellular domain 1-2 (EC1-2) fusion protein was expressed and purified by glutathione affinity chromatography on the basis of construction of recombinant EC1-2 vector, and then the New Zealand white rabbits were immunized to obtain the specific antisera. The IgG fraction was transferred into the neonatal mice passively after it was purified from the antisera. After 15-18 hours of injection, the abdomen skin and the sera of the mice were examined by light microscopy, electron microscopy, direct immunofluorescence and indirect immunofluorescence. Results In the evaluation of the study group of mice, the intraepithelial vesicle formation was observed. Electron microscopy showed that intercellular spaces were widened, desmosome split and disappeared. In immunofluorescence, the fluorescence-labeled IgG deposied between the acantholytic cells. In the control group of mice there were no pathogenic changes observed, except very weak fluorescence between intercellular spaces. Conclusion The PV mouse model established shows that the EC1-2 epitope in PVA antigen and its relevant antibodies were pathogenic, and can be used as a tool in studying the pathogenesis of PV.
5.Posterior correction using pedicle screw fixation combined with bone grafting and fusion for treatment of severe and rigid adolescent idiopathic scoliosis in 20 cases

BACKGROUND: Traditional anterior release followed by posterior correction and fusion is frequently used to treat severe and rigid adolescent idiopathic scoliosis, which is considered as Cobb angle of the major curve > 65° and flexibility < 34.5%; however, there are a great majority of complications. Whether isolated posterior correction using pedicle screw fixation combining with bone grafting and fusion may provide better effects on severe and rigid adolescent idiopathic scoliosis needs to be further studied.OBJECTIVE: To evaluate isolated posterior correction using pedicle screw fixation combining with bone grafting and fusion for the treatment of severe and rigid adolescent idiopathic scoliosis. DESIGN: Case analysis.SETTING: Department of Orthopaedics, Renji Hospital, Medical College of Shanghai Jiao Tong University.PARTICIPANTS: Twenty patients with severe and rigid adolescent idiopathic scoliosis, including 8 males and 12 females, were selected from Department of Orthopaedics, Renji Hospital, Medical College of Shanghai Jiaotong University from June 1999 to August 2005. They were 12-18 years old, and the mean age was 14.6 years. All patients were finally diagnosed as X-ray of whole spine. According to King-Moe criteria, patients were classified into type Ⅰ(n =4), type Ⅱ(n =6), type Ⅲ (n =5), type Ⅳ(n =3) and type Ⅴ(n =2). Before surgery, mean Cobb angle of the major curve was 82° (75°-92°), mean flexibility was 30% (20%-40%), and mean shoulder height difference was 15 mm (5-35 mm). Moreover, according to Risser syndrome, patients were classified into degree 1 (n =3), degree 2 (n =5), degree 3 (n =6), degree 4 (n =5) and degree 5 (n =1). All patients and their relatives provided the informed consents, and the experiment was approved by the local ethical committee. Artificial bone was Osteoset provided by Wright Company, USA.METHODS: Patients underwent isolated posterior correction using pedicle screw fixation combining with bone grafting and fusion, and spinous process, lamina of vertebra, zygapophysial joints and transverse process were exposed in a preconcerted fusion area. Pedicle screw was inserted into strategy vertebra using free hand technique according to the anatomic landmark of entry point. Six patients underwent fixation with TSRH system, and the other patients with CDH M8 system. Operative time and blood loss were evaluated after surgery. At 7 days after operation, Cobb angle was measured with X-ray, and correction rate of major curve was calculated. While shoulder height difference and admission duration were evaluated simultaneously. Complications and recovery states were followed up in the next 4 years.MAIN OUTCOME MEASURES: ① Operative time and blood loss; ② Cobb angle and correction rate of major curve; ③ shoulder height difference and admission duration; ④ follow-up results.RESULTS: All 20 patients were included in the final analysis. ① Operative time and blood loss: Operative time lasted from 3.2 to 4.3 hours, and the mean time was 3.5 hours. Blood loss ranged from 660 to 1 070 mL, and the mean loss was 865 mL. ② Cobb angle and correction rate of major curve: Cobb angle of the major curve ranged from 82° (75°-92°) before surgery to 31° (22°-37°) after surgery, and the mean correction rate was 62%. ③ Shoulder height difference and admission duration: Mean should height difference before surgery was 15 mm (5-35 mm). Postoperative lateral film of spine indicated that thoracic and lumbar vertebra generally suffered from normal posterior and anterior convexity, and mean shoulder height difference after surgery was 7.5 mm (0-11 mm). The admission duration lasted from 8 to 11 days, and the mean duration was 9 days. ④ Follow-up results: All patients were followed up in the next 4 years after surgery. The cobb angle correction of major curve remained unchanged, and the instrumented segments were completely fused without instrumentation failure.CONCLUSION: Isolated posterior correction using pedicle screw fixation combining with bone grafting and fusion may effectively cure severe and rigid adolescent idiopathic scoliosis, which is considered as Cobb angle of the major curve between 75° and 92° and flexibility ≥ 20%.
6.Effects of iptakalim on the functions of endothelin system in human pulmonary artery endothelial cells

Aim To investigate the effects of iptakalim(IPT),a novel K_(ATP) opener,on the functions of endothelin system in human pulmonary artery endothelial cells.Methods Primary cultured human pulmonary artery endothelial cells were incubated with different concentrations iptakalim for 24 h.The levels of ET-1 in medium were observed by radioimmunoassay.Reverse transcription polymerase chain reaction(RT-PCR)was performed to analyze the expression of ET-1 and ECE.Results When endothelial cells were incubated with IPT at concentrations above 10 μmol·L~(-1),the levels of ET-1 release in medium and the levels of ET-1 mRNA were significantly inhibited.When endothelial cells were incubated with IPT at concentrations above 1 μmol·L~(-1),the levels of ECE mRNA were significantly inhibited.Conclusions IPT can inhibit the expression of ET-1 and ECE mRNA from human pulmonary artery endothelial cells, thus it inhibits the secretion of ET-1 from endothelial cells. Iptakalim may serve as a promising candidate drug to treat pulmonary hypertension.

Objective:To construct mouse ScFv against pemphigus vulgaris antigen (PVA) through phage displayed antibody technology Methods:Total RNA was isolated from the spleen cells of mice immunized with recombinant pemphigus vulgaris antigen (rPVA) after six weeks, and then reverse transcripted into cDNA Primers specific for mouse V H and V L were synthesized to amply the V H and V L by ploymerase chain reaction (PCR) V L gene was modified and amplified with linker primer (which contain 3′ region of V H, linker, and 5′ region of V L), the resultant gene called V L′ Purified V L′ and V H were subjected to SOE (splicing by overlap extension) ligation, and resultant ScFv gene was digested with SfiI and NotI, cloned to the identically digested pCantab 5E Library of TG1 transfected with recombinant phagemid were derived with diversity of 1 5?10 6 Results:The pemphigus vulgaris phage antibody library was constructed, and one positive TG1 clone was obtained, which can highly expresses phage displayed antibody Conclusion:Specific genetic antibody against PVA can be made from phage displayed antibody library
8.Advantages of foley catheter for nephrostomy tube after minimally invasive percutaneous nephrolithotomy

Objective:To assess the advantages and nursing experience of foley catheter for nephrostomy tube after minimally invasive percutaneous nephrolithotomy (mPCNL).Methods:From October 2015 to April 2016,the clinical data of 137 patients,who were diagnosed with upper urinary tract calculi and need to perform mPCNL,were collected and randomized into 2 groups:a foley catheter group (research group,n=69) and a normal nephrostomy tube group (control group,n=68).The patients in research group used foley catheter for nephrostomy tube,while those in the control group used normal nephrostomy tube.Bleeding volume,the days of bleeding,pipes shedding and pain degree were compared,and the experience of the nursing process was summarized.Results:The bleeding volume,the days of bleeding in the research group were significantly lower than those in the control group (both P<0.01).The pipes shedding rate in the research group were lower than that in the control group (P<0.05).There was no significant difference in postoperative pain scores between the 2 groups (P>0.05).There was no other complications and discomfortable symptoms in the experimental group.Conclusion:The use of foley catheter for nephrostomy tube after mPCNL is safe,and it can decrease the bleeding volume and pipes shedding rate.It doesn't increase the patient's postoperative pain and can reduce the difficulty and risk for postoperative nursing.
9.Correlation among Thromboxane B2, Oxidized Low Density Lipoprotein, Lipoprotein(a), and Homocysteine in Patients with Cerebral Infarction

ObjectiveTo study the relevant pathogenic factors, Thromboxane B2 (TXB2), oxidized low density lipoprotein (OxLDL), lipoprotein(a) (Lp(a)), and homocysteine (Hcy), in patients with cerebral infarction and the correlation among them. Methods205 patients and 40 health persons (the control) were measured with the plasma TXB2, 6-keto-prostaglandin F1α (PGF1α) and TXB2/6-keto-PGF1α (T/6-K), OxLDL, Lp(a), Hcy within 24 h. Results and ConclusionThe levels of plasma TXB2, T/6-K,OxLDL, Lp(a), and Hcy significantly increased compared with the controls (P<0.01). OxLDL was correlated with Lp(a); TXB2 was correlated with T/6-K and Hcy; T/6-K was correlated with OxLDL, Lp(a).
10.Molecular inversion probe-based single base mutations of hepatitis B virus resistance gene analysis

Objective To establish a molecular inversion probe ( MIP) method for detection of single base drug-resistance mutation in Hepatitis B virus ( HBV) gene.Methods The HBV wild type and YVDD mutant strain were isolated by Daping Hospital of the Third Military Medical University.The MIP was designed and applied to detect the HBV drug-resistance YVDD mutation in one case of wild type and one case of YVDD mutant HBV strain isolated previously.The results of MIP method were compared with that of sequencing to evaluate the detection accuracy.Results Thermal cycling single-base extension and connection reaction performed by Taq DNA Ligase and Ampligase DNA Ligase could ensure the specificity of the detection.The optimum probe concentration of MIP was 1 nmol/L.Through detection of the target gene with different DNA concentrations , the detection sensitivity of MIP was determined as 1 nmol/L.The results of MIP were consistent with that of sequencing method in detection of the clinical samples.Conclusion MIP is successfully used to detect single-base drug-resistance mutation in HBV gene.
