1.Inhalation of nitric oxide combined with intravenous phenylephrine for improvement of arterial hypoxenua in patient with tetralogy of Fallot:a clinical observation

Objective: This study was to evaluate the effects of inhalation of nitric oxide combined with intravenous phenylephrine on the arterial hypoxemia and tissues acidosis in patient with tetralogy of Fallot. Method: The 34 pa tients were randomly divided into Ⅰ(n=17) and Ⅱ(n=17) groups. In both groups, when the patient's SpO_2 was less than 80%, the intervention began. In Ⅰ group,a bolus dose(1?g/kg) of phenylephrine was given intermittently, until the SpO_2 was large than 90% or blood pressure increased to 120%, of baseline. In Ⅱ group, inhalation of 20 ppm NC was combined with intermittent bolus of phenylephrine 1?g/kg. Result: The value of SpO_2, MAP and BE had no sig nificant differences between two groups at pre-treatment. In Ⅱ group, SpO_2 was higher, meanwhile MAP, the negative value of BE,the total dose of phenylephrine used were lower than those in Ⅰ group after treatment (P
2.Analysis on type distribution of cervical human papilloma virus in 13 156 women in Guigang area

Objective To analyze distribution of human papilloma virus(HPV) subtypes in women infected by HPV in Guigang area ,and to determine the dominant type in this region .Methods 21 genital HPV genotypes were screened in 13 156 women in out‐patient clinic in Guigang area from July 2013 to July 2014 by using rapid flow‐through hybridization technique of nucleic acid mole‐cules ,and high‐risk HPV detection and typing was carried out for screening cervical cancer .Results A total of 4 682 HPV‐positive women were identified and the infection rate was 35 .59% .All of 21 genetypes were checked out ,dominated by single infection .The percentages of HPV genotypes from high to low was HPV‐52 (16 .28% ) ,HPV‐16 (14 .01% ) ,HPV‐58 (8 .48% ) ,HPV8304 (7 .58% ) ,HPV‐6(6 .79% )and HPV‐11(5 .72% ) .Conclusion The dominant genetype of HPV of women in this area is HPV‐52 .
3.Clinical research of the therapeutic effect of esmolol on refractory ventricular fibrillation occurred after release of aortic cross-clamp during cardiopulmonary bypass

Objective To evaluate the effect of esmolol on refractory veatricular fibrillation just after aortic declamp during cardiopulmonary bypass.Methods 40 patients undergoing valve replacement occurring refractory ventricular fibrillation after release of aortic cross-clamp was randomly given esmolol (prepared to 10 mg/ml) 1 mg/kg(group esmolol,n =20),or Lidocaine (prepared to 10 mg/ml) 1 mg/kg(group control,n=20),the endocardial electric defibrillation was continued after 2min.If the fibrillation still can not be reversed after another two times defribrillation,the routine clinical method would beused.The time of aortic cross-clamp,rectal temperature,MAP,and the value of serum Lactic acid,potassium,and PH were recorded.After intervention,the times of defibrillation,heart rate and rhythm 5 min after reversal were recorded.At the end of CPB,the CPB time and the dosage of positive inotropic drugs were also recorded.Results The success rate of defibrillation was higer in group esmolol than control group(P <0.05).Heart rate after reversal in group esmolol were slower than that in control group(P < 0.05).The CPB time of control group was longer than group esmolol (P < 0.05),and the dosage of positiveinotropic drugs was significantly higher in control group(P < 0.01).Conclusion When occurring refractory ventricular fibrillation just after aortic declamp during CPB,Using esomolol by way of intravenous infusion can apparently reduce the frequency of defibrillation,and improve the rate of rewersal.It can also be favorable to cardiac function,and decrease the dependency of positive inotropic drugs,and shorten the time of CPB.

5.Protective Role of Hydrogen Sulfide on Neonatal rat Myocardial Cells by Hypoxia-Reoxygenation Injury

Objective:To investigate the direct effect of different concentrations of hydrogen sulfide(H_2S)on neonatal rat myocardial cells by hypoxia injury during different time point,and the indirect effect of reoxygenation injury in order to analyze the protective role of H_2S on myocardial cells subjected to hypoxia/reoxygenation injury. Methods:Primary cultured myocardial cells from neonatal rats were randomly divided into 9 groups:Normal control group,Hypoxia NaHS 0 μmol/L group,100 μmol/L group,200 μmol/L group,400 μmol/L group and those groups subjected to hypoxia for 24 h,48 h and 72 h. Hypoxia/reoxygenation NaHS 0 μmol/L group,100 μmol/L group,200 μmol/L group,400 μmol/L group and those groups subjected to hypoxia for 24 h,48 h,72 h and then reoxygenation for 2 h. The number of survival cells were counted and the activity of lactate dehydrogenase(LDH)were measured respectively at each time point. Results:At the same time point of 24 h,48 h,72 h hypoxia,the number of survival cells increased and the activity of LDH decreased in hypoxia NaHS treatment groups as compared with Hypoxia NaHS 0 μmol/L group(P<0.01). The protective role of Hypoxia NaHS 200 μand 400 μmol/L groups were better than NaHS 100 μmol/L group at 24 h of time poit(P<0.05~0.01). The number of survival cells by hypoxia/reoxygenation increased and the leakage of LDH from reoxygenation cells decreased in hypoxia/reoxygenation NaHS treatment groups as compared with hypoxia/reoxygenation NaHS 0 μmol/L group(P<0.01).Conclusion:NaHS 100 to 400 μmol/L had the protective effects on myocardial cells by hypoxia/reoxygenation injury. NaHS 200 to 400 μmol/L might offer the best protective effects on myocardial cells by hypoxia injury at 24 h.
6.Study of diagnostic value of neutrophil CD64 and sTREM-1 in elderly patients with community-acquired pneumonia

Objective:To explore the neutrophil CD64 and sTREM-1 in elderly patients with community-acquired pneumonia ( CAP) in the diagnosis of clinical value. Methods: 76 elderly CAP hospitalized patients were divided into severe pneumonia group (n=23) (live in 15 cases,8 deaths)and ordinary pneumonia group(n=53) according to severity;45 patients in the control group compared with healthy older persons. Peripheral blood neutrophil CD64 was measured by automatic flow cytometry,sTREM-1 levels was measured by double-antibody sandwich enzyme-linked immunosorbent assay. The receiver operating characteristic curve ( ROC curve) was used to check diagnostic value of the detection. Results: Median concentrations of CD64 and sTREM-1 in the severe pneumonia group,general pneumonia group and control group were 37. 49,18. 82 and 10. 63 MFI,and 75. 39,65. 31 and 43. 96 pg/ml, respective-ly. Although there was significant differences among the three groups ( P<0. 05 ); CD64 and sTREM-1 in Severe pneumonia survival group reduced to a normal level as his condition improved gradually,death group continued to rise as the disease,and peaked at the time of death;the area under the ROC curve of CD64,sTREM-1were respectively 0. 876,0. 843,which was 0. 917 by combination of CD64 and sTREM-1. Conclusion: Both CD64 and sTREM-1 are good markers in the diagnosis value of CAP, the dynamic changes of two may reflect the condition and prognosis of CAP.
7.Effect of nitric oxide inhalation on neutrophil CD11b expression and postoperative function in patients undergoing cardiac valve replacement with CPB

Objective To investigate the effects of sustained nitric oxide (NO) inhalation during operation on neutrophil CD11b expression and postoperative lung function in patients undergoing cardiac valve replacement with cardiopulmonary bypass(CPB) .Methods Forty-two ASA Ⅱ or Ⅲ patients aged 35-53 yr weighing 47-69 kg undergoing cardiac valve replacement with CPB were randomly divided into 2 group:NO group(n = 21) and control group(n= 21) .In NO group the patients inhaled 40 ppm NO through the lungs and oxygenator after tracheal intubation during operation before the chest was closed. Anesthesia was induced with fentanyl 20 ?g?kg-1 and pipeceuronium 0.2 mg?kg-1 maintained with isofluarane inhalation and intermittern i.v. boluses of fentanyl and pipecuronium. A total of 5 000 000 IU aprotin was given during operation. Arterial blood samples were taken before induction of anesthesia (T0, baseline), immediately before CPB (T1 ) 10 rain after CPB was started (T2 ), immediately before aortic unclamping (T3 ) ,10 min after aortic unclamping (T4 ), at the end of operation (T5 ) , 12 h and 24 h after operation (T8 ,T9) for polylmorphonuclear(PMN) counts and determination of neutrophil CD11b expression . Blood gas analysis was performed PA-a O2 was calculated and dynamic compliance of respiratory system (Cdyn) was measured before induction of anesthesia, at the end of operation and at 4 and 8 h after operation. Objective To study the features and risk factors of ischemic leukoaraiosis (LA) progression in elderly patients aged 80 years and over. Methods The 56 elderly patients with ischemic LA confirmed by MRI were enrolled in this study. The characteristics and risk factors of ischemic LA were comparatively analyzed between baseline and 3 years later by single and multi-variable logistic regression analysis. Results Ischemic LA progressed mostly in the subcortical white matter in elderly patients, especially the frontal lobes (progression of white matter lesion was present in 40% in the frontal lobes at baseline and 52% after 3 years), followed by the parietal lobes (35% at baseline and 36% after 3 years); Furthermore, previous high homocysteine (Hcy) and chronic bronchitis were risk factors of ischemic LA progression. Conclusions Ischemic LA progresses mostly in the subcortical white matter in the elderly, especially the frontal lobes; High Hcy and chronic bronchitis are risk factors of ischemic LA progression in elderly patients.
