Employed staffs in military hospital are the basic power in the development of hospital, and they are playing an im-portant role in the military medical security.The article gives a detailed elaboration on the innovation and practice of employed staff man-agement in our hospital.In order to improve the efficiency and scientific management level of employed staffs, the innovative measure-ments in employment pattern, post management, education development, policy system, promotion passage are put forward in combination with current situation of employed staff in order to promote the construction of employed staffs and the development of the hospital.

Post delimitation and personnel allocation is the basis of human resource management system .Scientific and rea-sonable post delimitation and personnel allocation contribute greatly to the improvement of standardized human resource management , structural optimization , cost control and efficiency promotion .In order to realize the transformations from status management to post management , from quantity management to organization management , from extensive management to fine management in human re-source management , a series of measurements were adopted to scatter post levels , refine specialty series , clarify allocation standards and rationalize personnel quantity in order to adapt to military reform , prepare for medical services of military struggle and meet the needs of hospital development .The article analyzed the practice and effect of post delimitation and personnel allocation of human re -sources in our hospital , providing reference for other hospitals in the improvement of management mechanism , management level and management efficiency .
3.Bacterial drug resistance situation and clinical features of urinary tract infection in children

Objective To analyze the basic clinical characteristics,auxiliary diagnostic indexes,bacterial infection spectrum and drug resistance of child urinary tract infection to provide the basis for the prevention,diagnosis and treatment of child UTI.Methods The clinical data in the inpatients with UTI or complicating UTI in our hospital from January 2012 to March 2014 were collected. The repeated strains were excluded.The differences in the pathogens between the patients with complicated UTI and the patients with non-complicated UTI were comprehensively analyzed.Results The onset peak of child UTI for the first time was 0 -0.5 years old.Escherichia coli ,Klebsiella and Pseudomonas aeruginosa are the major pathogens of child UTI,in which 110 strains were Escherichia coli,accounting for 52.9%.The enterobacteriaceae pathogens of UTI had higher sensitivity to carbapenems antibacterial drugs,with the resistance rate of less than 10%;the resistance rate of others detected antibacterial drugs was more than 20%.Con-clusion UTI in the children inpatients of this area has higher drug resistance rate,in the treatment of child UTI,the antibacterial drugs should be rationally used by combining the bacterial drug resistance situation in the local place and the disease severity in or-der to avoid the aggravation of bacterial drug resistance.
4.Influence of evidence-based nursing on postoperative adverse reactions, negative emotions and quality of life for aneurysms interventional therapy

Objective To study the influence of evidence-based nursing on postoperative adverse reactions,negative emotions and quality of life for aneurysms interventional therapy.Methods A total of 90 patients with intracranial aneurysm in our hospital were divided into study group and the control group (n =45) according to random number table method.The control group was given conventional nursing measures,and the study group implemented evidence-based nursing intervention.Postoperative adverse reactions,negative emotions and quality of life were observed.Results The incidences of adverse reactions were lower than that in the control group (P < 0.05).The study group before discharge had lower SAS and SDS scores than the control group (P < 0.05).Follow-up for 3 months,energy,physical activities,social life,sleep score in the study group were higher than that in the control group (P < 0.05).Conclusion Evidence-based nursing can effectively reduce the probability of postoperative adverse reactions for aneurysms interventional therapy,relieve the patients'negative moods,and ensure the quality of life,so it is suitable for clinical promotion.

6.Influencing factors of quality of life of patients with osteoradionecrosis of the jaws

Objective To investigate the level and influencing factors of quality of life in patients with osteoradionecrosis of the jaws (ORNJ) in order to provide theory basis for nursing intervention. Methods Ninety-five patients hospitalized from January 2015 to Febury 2017 and participated in the investigation by a convenient sampling method. The functional assessment of cancer therapy-head and neck (FACT-H&N), social support rating scale (SSRS) and general information questionnaire were used to assess the quality of life, social support and basic information in ORNJ patients in our hospital respectively. Results The score of quality of life was (79.99 ±15.72). The index value of common module of quality of life was higher than that of additional concern. The average scores of each dimension of common module from low to high were as follows: functional well-being, social/family well-being, physical well-being and emotional well-being. The multiple linear regression analysis showed that the influencing factors of quality of life were social support, severe mouth opening difficulty (grade Ⅲ and Ⅳ), the course of primary disease (3 ~5 years, 5 ~10 years), which explained 35.6%variation in the quality of life. Conclusions The quality of life of ORNJ patients is generally lower than that of other head and neck cancer patients. The social support, severe mouth opening difficulty and course of primary disease can affect the patients' quality of life.The smaller the mouth opening (≤1 cm), the longer the course of primary disease (3~5 years) and the worse the quality of life is. Thus, nursing staff should pay attention to the quality of life of ORNJ patients and give prospective nursing intervention to improve the quality of life according to the characteristics of the development of the disease.
7.Research on current life quality situation of soldiers of the field operations and the influence factors

Objective To explore the situation as well as influence factors of life quality situation of soldiers of the field operations and to provide evidence for its improvement .Methods Cluster random sampling was taken .A total of 616 soldiers from North China , Central China and Southern part of China were investigated by WHOQOL-BREF(Chinese version) and Perceived Social Support Scale (PSSS).Database was established on Epidata 3.0 and was analyzed using SPSS 20.0.Results Informants got an average score of ( 57.99 ± 8.62) points in life quality, with the scores in psychology and social relationship field exceeding and in environmental field being inferior to those of the national norm (t=8.609,7.609,12.898;P<0.01).Difference of investigators with different education background and from family of different income were significant ( t=2.501, 3.373;P<0.01).Soldiers social support had an average score of (63.14 ±11.40)points;each domain of quality of life and dimensions of social support were positively correlated (r>0.3,P<0.01).Conclusions The general life quality of soldiers of the field operations is satisfactory .Education background , family income , and social support are crucial influence factors on life quality of soldiers .Quality of life is closely associated with social support .To advance social support , especially support from others is the key action to improve their life quality .
8.A Novel Anti-Microbial Peptide from Pseudomonas, REDLK Induced Growth Inhibition of Leishmania tarentolae Promastigote In Vitro

Leishmaniasis is a prevalent cause of death and animal morbidity in underdeveloped countries of endemic area. However, there is few vaccine and effective drugs. Antimicrobial peptides are involved in the innate immune response in many organisms and are being developed as novel drugs against parasitic infections. In the present study, we synthesized a 5-amino acid peptide REDLK, which mutated the C-terminus of Pseudomonas exotoxin, to identify its effect on the Leishmania tarentolae. Promastigotes were incubated with different concentration of REDLK peptide, and the viability of parasite was assessed using MTT and Trypan blue dye. Morphologic damage of Leishmania was analyzed by light and electron microscopy. Cellular apoptosis was observed using the annexin V-FITC/PI apoptosis detection kit, mitochondrial membrane potential assay kit and flow cytometry. Our results showed that Leishmania tarentolae was susceptible to REDLK in a dose-dependent manner, disrupt the surface membrane integrity and caused parasite apoptosis. In our study, we demonstrated the leishmanicidal activity of an antimicrobial peptide REDLK from Pseudomonas aeruginosa against Leishmania tarentolae in vitro and present a foundation for further research of anti-leishmanial drugs.
9.Morphological diagnosis of pediatric flexible bronchoscopy in pediatric intensive care unit

Objective To investigate the clinical application value of pediatric electronic bronchoscopy on the diagnosis of respiratory disease in the pediatric intensive care unit(PICU)by morphological diagnosis.Methods The clinical data of 124 patients with PICU respiratory critically ill patients who underwent bedside electronic bronchoscopy from July 2017 to December 2022 in Lanzhou University Second Hospital were retrospectively analyzed.Pathogen examination of 96 cases of bronchoalveolar lavage fluid(BALF),bronchoscopy guided nasal endotracheal intubation in 3 cases was conducted.The accuracy of diagnosis of lung lesions was compared between electronic bronchoscopy and chest CT,and the morphological changes of lung lesions under electronic bronchoscopy were observed.Results The accuracy of electronic bronchoscopy in the diagnosis of tracheoesophageal fistula,tracheoesophageal rupture and bronchial foreign body is significantly higher than that of chest CT(tracheoesophageal fistula:4 cases vs.0 case,tracheoesophageal rupture:4 cases vs.0 case,bronchial foreign body:4 cases vs.6 cases,all P<0.05),and there was no significant difference in the accuracy of diagnosis of lung contusion and bronchiectasis between electronic bronchoscopy and chest CT.Typical cases confirmed under endoscopic:4 cases(3.23%)had tracheoesophageal fistula.Bronchial rupture 4 cases(3.23%).Endobronchial tuberculosis was diagnosed in 1 case(0.81%)with bronchial neoplasms and white caseous material attached to the mucosa.Positive bacteria were detected in 23 cases(23.96%).Guided nasotracheal intubation was performed in 3 children with jaw or cervical fracture.Endotracheal catheterization under mechanical ventilation was performed in 13 cases.There were no serious complications and death during and after the operation.Conclusion Pediatric bendable bronchoscopy with PICU is safe and reliable.It can improve the diagnosis rate of respiratory diseases in PICU,and has high clinical application value.
10.Efficacy and safety of rituximab combined with CHOP or combined with dose fractionated CHOP in the treatment of primary gastric diffuse large B cell lymphoma.

OBJECTIVEThe aim of this study was to compare the safety and efficacy profiles of R-CHOP and R-fCHOP regimes in the treatment of primary gastric diffuse large B cell lymphoma (PG-DLBCL).
METHODSData of PG-DLBCL patients admitted in our hospital from March 2010 to March 2014 were collected retrospectively. Differences in gastrointestinal bleeding and perforation between the R-CHOP group and R-fCHOP group were compared. The influence of the gastrointestinal complication on subsequent treatment was also analyzed. Treatment outcome of the two groups was also compared.
RESULTSA total of 50 patients were included in this retrospective study. Forty of them were in the R-CHOP group, another ten were in the R-fCHOP group. Patients in the R-fCHOP group had a higher rate of Lugano late stage disease, and a relatively high rate of a deeper/larger ulcer. Fence occult blood test (FOBT) was positive in one (10.0%) patient in the R-fCHOP group, and 11 (31.4%) patients in the R-CHOP group, among them one had hematemesis and had to give up the subsequent chemotherapy. No perforation was observed in both groups. The response rate (RR) was 92.5% in the R-CHOP group and 90.0% in the R-fCHOP group (P > 0.05). The PFS was also comparable between the two groups (P > 0.05).
CONCLUSIONSR-fCHOP regimen has a good safety profile in patients with Lugano late stage and deep/large ulcers, who are of high risk of gastrointestinal bleeding or perforation, and also has a comparable efficacy profile when compared with the R-CHOP regimen in short-term follow-up.
