1.The surgical treatment and defect repair of the former skull base comminuted fracture

Objective To study the surgical treatment and defect repair of the former skull base comminuted fracture.MethodsClinical data of 47 patients were retrospectively analyzed and emergency treatment,surgical debridement and defect repair were discussed consulting the literature..Results Restore score was determined according to international standards after surgical treatment for 3 months,in all cases,36 cases restored good,6 cases with light disability,3 cases with severe disability,2 cases died.Subsequent following-up revealed 31 cases completely recovered for normal work and life,14 patients survived in varying degrees of neurological function obstacles without encephalomeningocele.Conclusion The successful surgery is determined by thorough debridement,closing the damaged local subarachnoid,repairing reconstruct former dural skull base and defective bones.
2.Clinical histopathological and misdiagnostic analysis of 359 cases of intraocular tumors

Objective To probe the clinical character,the histopathological classification and misdiagnoses of intraocular tumors. Methods The clinical and pathological data of 359 patients with intraocular tumor diagnosed clinically between 1980~2000 were retrospectively analyzed. Results There were 300 cases of malignant tumor and 23 cases of benign tumor respectively. Non oncologic malady and benign tumor misdiagnosed as tumor or malignant tumor were 40 cases. The two leading malignant tumors were retinoblastoma and melanoma. Conclusion The clinical and pathological analysis of intraocular tumor is beneficial to the correct clinical diagnosis and treatment.
3.Emergency microsurgery for treatment of Hunt-Hess high grade intracranial aneurysms rupture

Objective To summarize the experience of emergency microsurgery under the guidance of computed tomographic arteriography (CTA) for treatment of Hunt-Hess high grade intracranial aneurysm rupture.Methods The clinical data of 34 patients with Hunt-Hess high grade intracranial aneurysm rupture were retrospectively analyzed.The CT and CTA were detected in the patients on admission.Emergency operation were all performed.The temporal occlusion of the parent artery,removed intracranial hematoma and clipping intracranial aneurysms by microsurgical techniques.All patients were removed skull.Results The prognosis was assessed by Glasgow outcome score (GOS),17 cases were recovered well,10 cases were mildly disabled,3 cases were severely disabled and 4 cases died.Conclusions CTA can be used as the primary diagnosis of intracranial aneurysm imaging methods under the emergency condition.The patients with HuntHess high grade intracranial aneurysm rupture should make positive surgical treatment to save live.
4.Relationship between human papilloma virus-DNA and cytomegalovirus-DNA virus content and risk factors of endometrial cancer and prediction model

Objective:To explore the relationship between human papilloma virus (HPV)-DNA and cytomegalovirus (CMV)-DNA virus content and the risk factors of endometrial cancer (EC) and their prediction models.Methods:Fifty-eight patients with EC from January 2017 to June 2020 in Suzhou Hospital Affiliated to Anhui Medical University were selected as the observation group, and 58 patients with endometrial hyperplasia (EH) during the same period were selected as the control group according to the 1∶1 matching case control principle. The clinical data, HPV-DNA, CMV-DNA positive rate and virus content of the two groups were compared, and the independent variables with P<0.05 were included in the Logistic regression model through single factor analysis. The risk factors of EC were analyzed, and the correlation between HPV-DNA, CMV-DNA and EC risk factors were evaluated. Logistic regression model was evaluated using the likelihood ratio chi-square and goodness-of-fit test, and the receiver operating characteristic (ROC) curve was used to analyze the predictive value of the model to EC. Results:The proportion of patients with age ≥ 55 years old, body mass index (BMI)≥ 24 kg/m 2, a history of hypertension and diabetes, malignant tumor family history and hormone replacement therapy history in the observation group were higher than those in the control group: 72.41%(42/58) vs. 39.66%(23/58),70.69% (41/58) vs. 43.10%(25/58), 36.21%(21/58) vs. 10.34%(6/58), 31.03%(18/58) vs. 8.62%(5/58),29.31%(17/58) vs. 5.17%(3/58), 27.93%(17/58) vs. 8.62%(5/58), and the differences were statistically significant ( P<0.05). The positive rates of HPV and CMV, the contents of HPV-DNA and CMV-DNA in the observation group were higher than those in the control group: 62.07%(36/58) vs. 29.31%(17/58),81.03%(47/58) vs. 41.38%(24/58), (471.16 ± 33.58) copies/ml vs. (240.08 ± 17.41) copies/ml, (256.19 ± 24.77) copies/ml vs. (132.27 ± 13.20) copies/ml, the differences were statistically significant ( P<0.05). Age ≥ 55 years, BMI ≥ 24 kg/m 2, history of hypertension and diabetes, family history of malignant tumors, history of hormone replacement therapy, HPV positive, CMV positive were risk factors for EC ( P<0.05). HPV-DNA virus content was positively correlated with the age of EC patients, family history of malignant tumors, and history of diabetes ( P<0.05), the viral content of CMV-DNA was positively correlated with age, family history of malignant tumors, and history of hypertension ( P<0.05). According to the ROC curve drawn by the predicted value and the true value of the Logistic regression model, the area under the curve was 0.930 (95% CI 0.885 - 0.976), when the predictive probability of individual nosocomial infection>0.528, the prediction sensitivity was 84.48%, and the specificity was 89.66%. Conclusions:The occurrence of EC is related to many factors, and the content of HPV-DNA and CMV-DNA viruses are the risk factors of its occurrence. The overall predictive value of a Logistic regression model constructed based on EC pathological characteristics and the virus content of HPV-DNA and CMV-DNA is high, and it has clinical application prospects.
5.Effects of transforming growth factor beta 1 neutralizing antibody on collagen production and adhesion formation of the flexor tendon

BACKGROUND: Studies have showed that transforming growth factor-β1 (TGF-β1) could yield to the collagen synthesis and adhesion formation of tendon cells at the process of healing. OBJECTIVE: To investigate the preventive effect of TGF-β1 neutralizing antibody on the collagen production and adhesion formation of flexor tendon. DESIGN, TIME AND SETTING: Randomized grouping observational experiments were performed in the Experimental Animal Center of Tongji Medical College between September 2005 and June 2006. MATERIALS: New Zealand white rabbits aged 2-5 months, weighing 3.5-4.5 kg. TGF was offered by Santa Cruz Biotechnology, USA. METHODS: Sheath fibroblasts, epitenon tenocytes, and endotenon tenocytes were obtained from rabbit flexor tendons. Cells were divided into two groups at random. In the experiment group, each cell culture was supplemented with 1 μg/L of TGF-β at increasing dose (0.1, 0.5, 1.0 mg/L) of TGF-β1 neutralizing antibody. No reagents were given in the control group. Collagen Ⅰ production was measured by enzyme-linked immunoabsorbent assay. Eighty-four adult New Zealand white rabbit forepaws underwent sharp transection of middle toe flexor digitorum profundus, followed by immediate repair. Thirty-six adult New Zealand white rabbit were divided into three groups randomly (n=12), injecting with the saline, 1.0 mg/L TGF-β1 neutralizing antibody and 2.0 mg/L TGF-β1 neutralizing antibody into tendon sheath respectively. Tendons were harvested at 4 and 8 weeks to conduct adhesion detection, biomechanical testing, histological evaluation and scanning electron microscopy observation. The remaining 48 New Zealand white rabbits were divided into two groups randomly (n=24), undergoing the saline and 1,0 mg/L TGF-β1 neutralizing antibody injection in tendon sheath respectively. Tendons were harvested at an increasing time interval (1, 2, 4, 8 weeks) and analyzed by in situ hybridization to determine the mRNA expression of TGF-β1 and collagen Ⅰ. MAIN OUTCOME MEASURES: Collagen production and adhesion of rabbit tendon cells. RESULTS: ELISA exhibited that TGF-β1 increased collagen Ⅰ production and the addition of neutralizing antibody significantly reduced TGF-β-induced collagen Ⅰ production in all cell cultures. The effect between antibody and collagen Ⅰ was dose dependent. At 4 and 8 weeks after operation, the gliding excursion ratio of the tendon was shortened and the simulated active flexion ratio were less in saline group compared with 1.0 and 2.0 mg/L TGF-β1 groups (P < 0.05). The tendon anastomosis breaking strength was shown no significant differences among 3 groups (P > 0.05). Scanning electron microscopy and histological observation showed that collagen fibers arranged irregularly in saline group, but arranged regularly in 1.0 and 2.0 mg/L TGF-β1 groups at 4 and 8 weeks after operation. The in situ hybridization examination revealed that TGF-β1 and collagen Ⅰ mRNA expression in 1.0 mg/L TGF-β1 group was lower than that in saline group at each time (P < 0.05). CONCLUSION: TGF-β1 neutralizing antibody can inhibit the function of the TGF-β1 effectively following the flexor tendon injury and repair, and can prevent adhesion formation.
6.Assessment of EBV antibodies and EBV-DNA in the diagnosis and stages of nasopha-ryngeal carcinoma

Objective:To evaluate the efficacy of Epstein-Barr nuclear antigen 1/immunoglobulin A (EBNA1/IgA), BamH1 Z transactivator/IgA (Zta/IgA), capsid antigen/IgA (VCA/IgA), and Epstein-Barr virus deoxyribonucleic acid (EBV-DNA) in detecting different stages of na-sopharyngeal carcinoma (NPC). The relationship between the EBV markers and stages of NPC was also analyzed. Methods:Blood sam-ples of 152 untreated patients with NPC and 675 healthy subjects were collected.ELISA was used to detect the serum levels of EBNA1/IgA, Zta/IgA, and VCA/IgA. Fluorescence quantitative PCR (FQ-PCR) was used to detect the plasma levels of EBV-DNA. ROC and correla-tion analyses were employed to assess the detection assays for NPC diagnosis. The positive rates of EBV markers in NPC patients in dif-ferent stages were analyzed statistically. Results: The positive rates of EBNA1/IgA, Zta/IgA, VCA/IgA, and EBV-DNA in NPC patients were higher than those in the healthy individuals. The expression of EBNA1/IgA was relatively high in early NPC. The sensitivity of EB-NA1/IgA was 77.8%. In advanced NPC, the level of EBV-DNA was high, and the sensitivity of EBV-DNA was 88.8%. The specificity of EBV-DNA and EBNA1/IgA could reach more than 96%. The combination of EBV-DNA and EBNA1/IgA showed the best diagnostic value, with a sensitivity of 92.1%(early stage 82.5%, advanced stage 98.9%) and a specificity of 96.9%. The positive rates of EBV-DNA were positively associated with the NPC clinic stage and N stage. The positives rates of Zta/IgA were positively associated with the NPC N stage. Conclusion:The best single index for NPC screening in an asymptomatic population is EBNA1/IgA. EBV–DNA is an ideal index for auxiliary diagnostics of advanced NPC. The combination of EBV-DNA and EBNA1/IgA shows the best diagnostic value. EBV-DNA is an important index in the stage and illness monitoring of NPC. Zta/IgA can indirectly reflect the character of lymph node metastasis, and it may be useful in assessment of NPC surveillance.
7.Study on the management and its effectiveness evaluation of 70538 patients with serious mental illness at home in Gansu province

Objective To analysis the family management of patients with serious mental illness in Gansu prov-ince, and evaluate its effectiveness to lay the foundation for the better community-based rehabilitation management and treatment for the people with serious mental illness. Method According to Treatment-management specification of severe mental illness (2012) and specification of national basic public health services (2011), the information such as home ther-apy, medicine taken, and dangerous behavior happened were collected from patients who were confirmed as serious men-tal illness in January 2015 to April 2016 in Gansu province. Result Up to April 30th, 2016, totally 70538 serious mental illnesses were identified, and about 81.10%(57195) were under management. The rate of treatment of serious mental ill-ness was 69.9%(48009/68663) in Gansu province, and Bipolar affective disorder (76.0%) was the highest (P<0.001). The rate of taking medication in patients under family-management was 42.16%(24113/57195), and the rate of taking regular medication was 24.74%(14150/57195). The rate of taking medication in Paranoiac psychosis patients (51.98%) was the highest (P<0.001), and the rate of taking regular medication in Mental disorders caused by epilepsy (31.02%) was the highest (P<0.001). Conclusion The rate of family-management, treatment, taking medication and take regular medica-tion of serious mental illness in Gansu province are all below the national average level. A special training should be giv-en to medical professionals to strengthen their capacity to treat serious mental illness.

Objective: To investigate the relationship between dietary -related behaviors of college students and their health , so as to provide the evidence for the health education for college students .Methods:A question-naire survey was conducted among 2105 college students from Xi′an Jiaotong University with stratified cluster ran-dom sampling .Results:In this survey , college students generally had unhealthy dietary behaviors , cognitive bia-ses of self-images and blind unhealthy weight -losing phenomenon as well .And there was a difference of dieting behaviors between college students of different sex and those of different body mass index .Conclusions:Unhealth-y dietary behaviors of college students were one of the main reasons leading to their abnormal BMI .Hence , it was necessary to improve health ethics education and dietary health guidance for college students , so as to introduce them to consciously assume their own health responsibilities and manage their health .

10.Effect of Yiqixue Buganshen Recipe on Endometrial Integrinβ3 Expression and Pinopode Formation in Mice with Embryo Implantation Dysfunction

Objective To observe the effect of Yiqixue Buganshen Recipes(YBR), the compound recipes of Chinese herbal medicine with the actions of replenishing Qi and blood and tonifying liver and kidney, on the endometrial integrin β3 of the mice with embryo implantation dysfunction, and to explore the molecular mechanism of YBR in improving embryo implantation. Methods Ninety mice were divided normal group, model group, and YBR group, 30 mice in each group. YBR group was given Jinghou Zengzhi granules 8mg/kg for promoting post-menstruation proliferation and Cuhuangti granules 8mg/kg for promoting the formation of corpora luteum. At 8∶00 am of pregnant day 4 (Pd4), subcutaneous injection of mifepristone was used for inducing embryo implantation dysfunction in the model group and YBR group. Twenty mice from each group were executed 12 hours after subcutaneous injection of mifepristone, and the uterus was isolated for the detection of mice endometrial integrin β3 mRNA and protein expression with real-time RT-PCR and Western blotting method respectively. Ten mice from each group at Pd4 were executed, and the uterus was isolated and then fixed in 2.5% glutaraldehyde solution for the detection of the formation of pinopode in mice endometrium under scanning electron microscope. Results The detection by real-time PCR and Western blotting method revealed that integrinβ3 mRNA and protein expression levels of the model group were significantly lower than those of the normal group (P<0.05), and YBR group had higher integrin β3 mRNA and protein expression levels than the model group(P<0.05). The formation of pinopodes in the model group was less than the normal group and YBR group, but the number of formed pinopodes in YBR group was similar to that in the normal group. Conclusion YBR can up-regulate the expression of integrin β3 in the endometrium of mice with embryo implantation dysfunction, and have positive regulatory effect on the formation of pinopodes on Pd4.
