1.Inferior vena cave filter placement: Summary of 96 Cases

Objective To investigate the effects of filter placement for inferior vena cava in prevention of fatal pulmonary embolism(PE)and the treatment of postoperative complications. Methods Inferior vena cava filters were placed in 96 patients by the way of femoral vein. Results All the patients were followed up for 4 - 36 months after the procedure without fatal pulmonary embolism occurrance in the 96 patients, but with only 17 correlated complications. Conclusion Inferior vena cava filter placement is easy and safe which can prevent the high complication incidence of fatal pulmonary arterial embolism with further more effective prevention and management of other correlative complications.
2.Influence of Trisialoganglioside-GT1b combined with Edaravone Injection on clinical effect and serum related indicators in patients with acute cerebral infarction

Objective To investigate the influence of Trisialoganglioside-GTlb combined with Edaravone Injection on clinical effect and serum related indicators in patients with acute cerebral infarction.Methods A total of 126 cases of patients with acute cerebral infarction in our hospital from October 2010 to May 2016 were selected and divided into observation group and control group,63 cases in each group.Patients in the control group were treated with regular treatment and Edaravone Injection,and patients in the observation group were treated with regular treatment,Edaravone Injection and Trisialoganglioside-GTlb.The clinical effect,NIHSS scores,Barthel index scores,serum level of neuron specific enolase (NSE),S100β protein,superoxyde dismutase (SOD),advanced oxidation protein products (AOPP) and malondiadehyde (MDA) were compared.Results The total effective rate of the observation group (90.48%) was significantly higher than that of the control group (76.19%),the difference between the two groups was statistically significant (P < 0.05).After two weeks of treatment,two groups of NIHSS and Barthel index scores were significantly lower than before treatment,the difference was statistically significant (P < 0.05);and the NIHSS and Barthel index scores of observation group were significantly lower than that of control group,the difference was statistically significant (P < 0.05).NIHSS scores,Barthel index scores of the observation group were significantly better than that of the control group (P < 0.05);The serum level of NSE,S100,AOPP,MDA of two groups after treatment were significantly lower than that before treatment,the difference was statistically significant (P < 0.05);and the above indexes of the observation group was significantly lower than the control group,the difference was statistically significant (P < 0.05);SOD levels of two groups were significantly increased than before treatment,the difference was statistically significant (P < 0.05);and the SOD levels of observation group was significantly higher than that of control group,the difference was statistically significant (P < 0.05).Conclusion Trisialoganglioside-GT1b can synergy improve the clinical effect of Edaravone Injection in patients with acute cerebral infarction,and be good to recovery the neurologic function and ability of daily living,and these may be related to the change of the serum level of NSE,S100β protein,SOD,AOPP and MDA.
3.Effects of the inhibition of Smad3 by short hairpin RNA on the expression of Smad7 in keloid fibroblasts

Objective To construct the eukaryotic expression vector containing short hairpin RNA (shRNA) targeting the inhibition of Smad3 gene segment in keloid fibroblasts (KFBs) and to investigate the effects on the expression of Smad7 in KFBs. Methods A couple of the most effective siRNA screened from former experiments were used to recombine the plasmids of Smad3 shRNA, and then Smad3 shRNA was transfected into KFBs by LyoVec TM. The expressions of Smad3 and Smad7 at different time points (0~9 d) were detected by RT-PCR and Western blotting. Results ① The recombinant Smad3 shRNA vectors were identified by RT-PCR and Western blotting. ② The expressions of mRNA and protein of Smad3 in KFBs decreased significantly in a time-dependent manner after transfection of Smad3 shRNA and reached the lowest point at 72 hours in the experimental group. 4.Correction of severe kyphosis and kyphoscoliosis by total spinal osteotomy

[Objective]To explore the experience of the correction of severe kyphosis and kyphoscoliosis by total spinal osteotomy.[Method]To correct kyphosis and kyphoscoliosis by total spinal osteotomy and screws-rods internal fixation apparatus,then graft the bone.[Result]From 1984 to 2005,50 patients were cured with the way above.There were no neural injury observed.The clinical symptom was improved in some extent.All patients were follow up from 2 months to 15 years,averaged 2.4 years,and the postoperative X ray and CT scans verifies all screws were in a proper position.No cases of implant failure were observed,and all the patients had a solid bone fusion on radiographs taken 3 months after surgery.[Conclusion]The operation to correct severe kyphosis and kyphoscoliosis by total spinal osteotomy with efficient screws-rods internal fixation apparatus,not only have a larger operate area which made opreation easier and more safe,but combine osteotomy with internal fixation and finished at the same time,it is an efficient way to correct kyphosis and kyphoscoliosis.
5.Observation on the Curative Effect of Mycophenolate Mofetil Combined with Glucocorticosteroid on the Children with Anaphylatic Purpura Nephritis

Objective:To explore the clinical effect of mycophenolate mofetil combined with glucocorticosteroid on the children with anaphylatic purpura nephritis.Methods:70 cases of children treated and diagnosed as anaphylatic purpura nephritis in our hospital from March,2012 to September,2015 were enrolled in this study.They were randomly divided into the observation group and the control group.Conventional therapy was applied to both groups,the observation group was treated by mycophenolate mofetil combined with prednisone,the control group was given cyclophosphamide combined with prednisone.The total effective rate,disappearence time of clinical symptoms,biochemical indicators,immune function as well as the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of observation group was 94.29%,which was 85.17% in the control group,no statistical difference was found between two groups(P>0.05).After therapy,the disappearence time of albuminuria,hematuria in observation group showed no significant difference compared with those of the control group,the level of 24 h urine protein quantitation and CD19+ of both groups were significantly decreased,and the level of CD19+ of observation group was significantly lower than that of the control group (P <0.05),the level of albumin,total protein and CD3+,CD3+CD4+ of both groups were significantly increased,and the level of CD3+,CD3+CD4+of observation group were significantly higher than those of the control group (P<0.05).The incidence of adverse reactions in observation group was significantly lower than that of the control group (P <0.05).Conclusion:Mycophenolate mofetil combined with glucocorticosteroid was more effective and safe on the children with anaphylatic purpura nephritis than that of cyclophosphamide combined with prednisone.
6.Clinical significance in testing urinary NK cells in patients with glomerulonephritis

Objective To evaluate the clinical significance of detecting urinary natural killer(NK) cells in patients with general types of glomerulonephritis. Methods The contents of urinary NK cells from 54 patients with glomerulonephritis were measured by flowcytometry,while all patients were classified into two groups including acute proliferation group and none-acute proliferation group by renal biopsy results. The content of urinary NK cells was compared between the two groups. Results The content of urinary NK cells in acute proliferative glomerular disease group were( 14. 8 ±3. 3)% (30 cases) ,which was significantly higher than that of(21. 6 ±2. 9)% (24 cases) in the non-acute proliferative glomerular patients(P<0.05). Conclusions Decreasing of the contents of NK cells in urine may be an indirect indicator of the activity of glomerulonephritis.
7.Diagnosis and treatment of patients with neurogenic bladder caused by sacral nerve root lesions

Objective To investigate the clinical and pathological features of sacral nerve root lesions and the diagnosis and treatment for these clinical entities.Methods The clinical data of the patients with urine retention or refractory lower urinary tract symptoms (LUTS) caused by sacral nerve root lesions were retrospectively analyzed.Totally 27 patients were included in this study,including 4 cases of acute retention,6 chronic retention and 17 refractory LUTS.All patients had urodynamic disorders and sacral nerve root space-occupying lesions demonstrated by magnetic resonance imaging.All the patients received surgical treatment and were followed up one year or more.Results The pathological findings included simple cyst in 3 cases,ganglioneuroma in 5,cyst wall inflammation in 15,cyst wall hyaline degeneration in 15,and neurodegeneration in 15.Postoperatively,9 of the 10 patients with retention could voluntarily void and 1 remained suprapubic catheter drainage.All 17 patients with refractory LUTS improved significantly in terms of international prostate symptom score,visual analogue scale pain score,quality of life,maximal urinary flow rate and post void residual (P<0.01).Conclusions Sacral nerve root lesions can be the causes of urinary retention or refractory LUTS.Surgical treatment can improve patient's voiding function and quality of life.
8.Effects of cyclic tensile stress on biological function and degeneration of nucleus pulposus cells

Objective:To investigate the effects of cyclic tensile stress on the function and degeneration of nucleus pulposus cells.Methods:The human primary nucleus pulposus cells were isolated and cultured. The cyclic tensile stress (100 000 μ?, 10% tensile strain, 0.1 Hz, 8 640 cycles) was loaded on the cells for 24 h. The proliferation of the cells was examined by MTT method. The cell cycle and apoptosis were detected through flow cytometry. Gene expression profile chip was used to detect the differentially expressed genes between the tensile stress group and control group. The function of these gene was analyzed by bioinformatics. The expression of inflammatory related factors, TGF-β, matrix degrading enzymes and extracellular matrix molecules were examined by qRT-PCR.Results:The cyclic tensile stress significantly promoted proliferation and cell cycle of nucleus pulposus cells. The cell percentage of S phase ( t=5.336, P<0.05) and G2/M phase ( t=7.288, P<0.01) was significantly different between the tensile stress group and control group. The cyclic tensile stress inhibited apoptosis of nucleus pulposus cells (8.56%±0.48% vs 10.63%±0.32%, t=4.474, P<0.05). A total of 866 differentially expressed genes were detected. Gene ontology analysis showed the roles of these genes in cells including focal adhesion, extractable matrix, membrane raft, condensed chrome kinetochore, cytoskeleton, etc. The cyclic tensile stress significantly affected the mRNA expression of inflammatory related factors, TGF-β genes, matrix proteinase and extracellular matrix molecules. Compared with the control group, the mRNA expression of inflammatory related factors IL15 ( t=5.379, P<0.05), IGF1 ( t=5.454, P<0.05) and IGFBP7 ( t=13.57, P<0.01) were significantly decreased in the tensile stress group; The mRNA expression of TGF-β genes TGFB1 ( t=6.931, P<0.05), TGFB2 ( t= 15.56, P<0.01) and TGFB3 ( t=7.744, P<0.05) were significantly increased in the tensile stress group; The mRNA expression of matrix proteinase ADAMTS3 ( t=5.241, P<0.05) and MMP19 ( t=24.72, P<0.01) were significantly decreased, and TIMP3 ( t=8.472, P<0.01) increased in the tensile stress group; The mRNA expression of extracellular matrix molecules COL2A1 ( t=5.871, P<0.05), FLRT2 ( t=5.216, P<0.05) and FN1 ( t=4.289, P<0.05) were significantly increased. Conclusion:The cyclic tensile stress promoted cell cycle and proliferation and inhibited apoptosis of nucleus pulposus cells. The cyclic tensile stress may affect the function and degeneration of nucleus pulposus cells by regulating the expression of inflammatory related factors, TGF-β, matrix degradation enzymes and ECM molecules.
9.Thoracolumbar pedicle anatomy in Han and Uygur male population in Xinjiang Uygur Autonomous Region: a computed tomography-based morphometric study

BACKGROUND: Thoracolumbar segments (T11-L2) prone to damage due to its special anatomical and biomechanical characteristics. Therefore, fully understanding the shape of pedicle of vertebral arch and finding the visible, constant, and the point of insertion of the pedicle axis is very important to the safety of pedicle screw placement.OBJECTIVE: To measure the surgically relevant parameters of thoracolumbar pedicles between Han and Uygur males using computed tomography (CT) scan to provide some anatomic reference data for pedicle screw fixation.METHODS: The vertebral bodies and pedicles of adult males were scanned (60 cases of Han people and 60 cases of Uygur people) from T10-L3 with CT. The parameters were processed by three-dimensional reconstruction. Transverse pedicle width, pedicle axis length, transverse pedicle angle, and sagittal pedicle angle were measured by using length and angle measurement tool of browser in bone-window CT images. The age and stature information were recorded. All the data above were processed by SPSS 13.0 software.RESULTS AND CONCLUSION: (1) The mean transverse pedicle width of T12 and L1 in Han was bigger than that in the Uygur. (2) The mean transverse pedicle angle of T12 and L2 in Han was bigger than that in the Uygur. (3) The mean pedicle axis length and sagittal pedicle angle of T11 and L1 in Han were bigger than that in the Uygur (P < 0.05). (4) There were some differences among some parameters of the thoracolumbar pedicles between the Han and Uygur people.Data from any study only can be used as a guide for pedicle screw fixation. Preoperative CT evaluation may provide an individualized strategy to reduce the incidence of postoperative complications caused by misplacement.
10.Research on the bacteroides of patients with AIDS and the healthy control

Objective To investigate the bacteroides of patients with AIDS and the healthy control by real-time quantitative PCR in order to reveal the role and significance of gut microflora in the AIDS-associated molecular pathogenesis. Methods The feces of the preoperative AIDS patients (n = 30) and the healthy control (n = 30) were collected. According to the sequences of target genes, specific PCR primers were designed. Bacterial genome DNA extracted from fecal samples was quantified by real-time quantitative PCR to analyze the bacterial amounts. Results In the patients with AIDS, the level of B.fragilis (3.23 ± 1.59; 4.05 ± 1.65), B.uniformis (5.69 ± 0.95;6.70 ± 2.18), B.thetaiotaomicron (5.01 ± 1.61; 6.41 ± 2.34), B.ovatus (5.78 ± 1.03; 7.07 ± 1.75), B.distasonis (4.21 ± 1.21; 5.53 ± 2.46) and B.vulgatus (2.92 ± 1.30; 4.48 ± 1.32) were significantly higher than those of healthy controls (P < 0.05). Conclusion The amount of fecal bacteroides of AIDS patients are significantly higher than those of the healthy control. These data indicate that the gut microflora of AIDS patients was disordered.
