1.Optimization of Ethanol Extraction Technology for Xiaoluo Granules by Orthogonal Experiment

Objective:To optimize the ethanol extraction technology for Xiaoluo granules. Methods:The extraction technology for the ethanol dissolvable ingredients in 10 kinds of medicinal materials was optimized by orthogonal experiment. The weighted sum of ba-icalin content and the dry extract rate as the index, 4 influencing factors with 3 levels each were optimized by L9 (34 ) orthogonal table. Results:The optimal ethanol extraction conditions were as follows:the medicinal material was soaked by 10-fold 50% ethanol solution for 45min, and then extracted by heating reflux for 3 times with 1. 5h each time. Conclusion: The method can effectively extract the ethanol dissolvable ingredients in 10 kinds of medicinal materials, which can be used as the ethanol extraction technology for Xiaoluo granules.
2.Clinical analysis of 18 patients with blunt bronchial injuries

Objective To sum up the experience on diagnosis and treatment of blunt bronchial injuries .Methods From January 2002 to December 2009 ,18 patients with blunt broncheal injuries were admitted .The definitive diagnosis was confirmed by fibro-bronchoscopy .Twelve patients suffered from main bronchial injuries and the remaining 6 patients from lobe bronchial injuries .All patients managed with surgical therapies .Emergent operation was performed in 15 patients and elective operation in 3 patients .End to end bronchial anastomosis was performed in 15 patients and primary repair in 3 patients .Results There was no death in this group .One patient after emergent primary repair developed ischic necrosis in the repair site on 3 the postoperative day and under-went resection of the necrotic tissue followed by end to end anastomsis .Thirteen patients had no complications and were able to take part in normal activities .Anastomotic stricture occurred in 5 patients and managed with granulation tissue resection and cryothera-py .Four patients were healed but one patient underwent lobectomy 6 months later as a result of recurrent stricture and severe dysp-nea .Conclusion Fibrobronchoscopy is able to define the blunt bronchial injuries immediately .Early surgery after trauma can im-prove the successful rate of treatment .
3.Animal model construction of dog severe myocardial contusion and its application value

Objective To establish an animal model of dog severe myocardial contusion (MC) .Methods 12 dogs with the body weight of (11 .36 ± 1 .50)kg were selected .The BIM‐Ⅱ type biological impact machine was adopted to directly impact the bare area of the left chest by the driving pressure of 800 kPa ,the impact area was 16 .61 cm2 and the chest wall was inward compressed within 4-6 cm ,the severe myocardial contusion model was established .The pathological examination was taken at 8 h after injury and grading was performed according to the Abbreviated Injury Scale (AIS ,update in 2005) .Results The scattered large area bleeding spots appeared in the epicardium and endocardium of wound area .No obvious abnormality was found in the non - wound area .The pathological examination was performed based on the coronal plane of the left ventricular cross section .The percentage of the section 8 h necrosis area to the total area was (39 .78 ± 9 .07)% .Conclusion The established dog severe myocardial contusion model is stable and reproducible ,which can accurately simulate the temporary MC mode .
4.Analysis of incidence and mortality of prostate cancer in cancer registration area in Zhejiang province from 2000 to 2009

Objective To investigate the epidemic characteristics and trends of incidence and mortality of prostate cancer in cancer registration areas in Zhejiang province from 2000 to 2009.Methods Clinical data incidence and mortality of prostate cancer were collected from 6 cancer registration areas in Zhejiang province,including Hangzhou,Jiaxing,Jiashan,Haining,Shangyu and Xianju.Crude rates,standardized rate and change trend,age-specific rates and annual percent change (APC,95% CI) of prostate cancer were checked,sorted and analyzed in Zhejiang Cancer Center.Results The prostate cancer incidence rate from 2000 to 2009 was 9.79/100 000,age-standardized incidence rates by world standard population (ASIRW) was 6.39/100 000,and the incidence cumulative risk of males aged 0-74 was 0.72% ; while the mortality rate was 2.73/100 000,age-standardized mortality rates by world standard population was 1.74/100 000,and the mortality cumulative risk of males aged 0-74 was 0.14%.Age-specific incidence of prostate cancer remained low before 50,years old and peaked at over 85-year-old group (130.30/100 000).Age-specific mortality of prostate cancer increased after 55,and also peaked at over 85-year-old group (81.19/100 000).The annual prostate cancer incidence rate generally grew from 1.39/100 000 (2000) to 13.89/100 000 (2009),and the APC was 14.18% (95% CI,9.68%-18.98%).Meanwhile,the prostate cancer mortality rate also increased from 1.52/100 000 (2000) to 3.58/100 000 (2009),and the APC was 11.83% (95% CI,5.69%-18.33%).Conclusion Prostate cancer incidence and mortality in Zhejiang cancer registration areas increased sharply,and the prevention and treatment of prostate cancer should be strengthened.
5.Clinical efficacy of low-dose methimazole therapy in patients with subclinical hyperthyroidism complicated by paroxysmal atrial fibrillation

Twenty-seven patients with subclinical hyperthyroidism(SH)complicated by paroxysmal atrial fibrillation(PAF)were treated with methimazole plus bisoprolol.All patients were examined by Doppler echocardiogram and 24 h ambulatory electrocardiograms before and 3 months after treatment.Serum FT3,FT4,and TSH levels were measured with RIA.The results showed that low-dose methimazole therapy could improve the left ventricular diastolic function(P < 0.01)and help maintain sinus rhythm.The incidence of subclinical hypothyroidism was low.Low-dose methimazole was effective and safe in patients with SH complicated by PAF.
6.Investigation of the prevalence of primary aldosteronism among newly diagnosed patients with hypertension

Objective To observe the morbidity and clinical characteristics of primary aldosteronism (PA) in newly diagnosed patients with hypertension.Methods 255 patients with newly diagnosed high blood pressure were determined plasma renin activity (PRA),angiotensinⅡ(ATⅡ),aldosterone (PAC),cortisol (COR),adrenocortico-tropic hormone (ACTH)and 24h urinary vanilmandelic acid (VMA).They were examined renal artery color doppler and adrenal 64 row CT scan +enhancement scanning.Results 179 patients (70.20%)were diagnosed essential hypertension (EH).In EH patients,24 cases (13.41%)with hypokalemia.42 patients (16.47%)were diagnosed PA.In PA patients,18 cases (42 .86 %)with hypokalemia,25 cases(5 9 .5 2 % )with unilateral adrenal adenoma. Compared with EH patients,there were higher level of high density lipoprotein (HDL -C)(Z =-2.068,P =0.036),lower level of total cholesterol (TC),low density lipoprotein (LDL -C),fasting plasma glucose (FPG), serum kalium (Z =3.594,P =0.000;Z =2.807,P =0.005;Z =3.499,P =0.000;Z =8.435,P =0.000)in PA patients.The levels of plasma PRA and ATⅡin PA patients were lower than in EH patients(Z =3.673,P =0.000;Z =2.215,P =0.026).The levels of plasma PAC and ARR were higher than in EH patients (Z =8.562,P =0.000;Z =19.871,P =0.000).The minimum value of plasma PAC was 292.1pg/L,the ARR was 376.7 in 42 PA patients. The maximum value of plasma PAC was 311.3pg/L,and the ARR was 291.2.Conclusion There is high detection rate of PA in newly diagnosed hypertension.The unilateral adrenal adenoma is a main cause of PA.Hypokalemia is not common in PA patients.PA has little influence on glucolipid metabolism.ARR has high differential diagnosis accuracy for PA and EH.
7.Association of polymorphisms in angiotensin Ⅱ receptor gene with primary aldosteronism

Objective To investigate the relationship between the polymorphisms of angiotensin Ⅱ receptor gene and the risk of primary aldosteronism (PA).Methods Polymerase chain reaction -restriction fragment length polymorphism (PCR -RFLP)was used to examine the 1 1 66A /C polymorphism of AT1 R gene and 1 675A /G poly-morphism of AT2R gene in 85 patients with PA and 1 00 healthy controls.Results There was no significant difference of AT1 R 1 1 66A /C genotypes (AA,AC,CC)and allele (A and C)frequency among patients and controls (χ2 =0.430,P =0.806).There was obvious difference of AT2R 1 675A /G genotypes (AA,AG,GG)and allele (A and G) frequency among two groups (χ2 =6.1 21 ,P =0.01 3).The G allele was higher than A allele in PA group (χ2 =6.767,P =0.009).Conclusion Homogenic mutation of 1 675A /G site in AT2R gene may be one of risk factors of PA.
8.Clinical Observation of Needling QIN's Eight Head Acupoints for Insomnia

Objective To observe the therapeutic efficacy of needling QIN's eight head acupoints in treating insomnia.Method Sixty patients were randomized into QIN's eight head acupoints group (30 cases) and a conventional acupuncture group (30 cases). The modified Spiegel sleep questionnaire was evaluated respectively after 4-week and 8-week treatment.Result After 4-week treatment, there was a significant difference in comparing the modified Spiegel sleep questionnaire between the two groups (P<0.05), and QIN's eight head acupoints group was superior to the conventional acupuncture group; after 8-week treatment, there was no significant difference between the two groups in comparing the modified Spiegel sleep questionnaire (P>0.05).Conclusion QIN's eight head acupoints group had a more significant therapeutic efficacy compared to conventional acupuncture group after 4-week treatment.
9.Effect of Intravenous Injection of Radix Astragali seu Hedysari on Acute Myocardiac Infarction in Anesthetized Dogs

Objective:To investigate the protective effect of Intravenous Injection of Radix Astragali seu Hedysari on acute myocardiac infarction (AMI) in anesthetized dogs. Methods: Twenty health dogs were treated by ligating descending anterior of coronary with two-step method. The changes of electrocardiogram before and after ligation were recorded. After 4 hours myocardium were taken out and dyed with NBT to make sure of the extent of myocardiac infarction.Results: Intravenous Injection of Radix Astragali seu Hedysari could decrease the lethality of AMI, ameliorated ST changes on electrocardiogram induced by AMI, and reduced infarction area. Conclusions: Intravenous Injection of Radix Astragali seu Hedysari is effective in the treatment of AMI in anesthetized dogs.
10.Effect of Ganoderma Lucidum on Counteracting Lipid Peroxidation in Cultured Vascular Smooth Muscle Cells of Rats

Objective To observe the effect of Ganoderma Lucidum (GL) on counteracting lipid peroxidation in cultured vascular smooth muscle cell(VSMC)and to explore its therapeutic mechanism for atherosclerosis. Methods The effects of GL on the content of lipid peroxide(LPO)and the activity of superoxide dismutase(SOD)in medium were studied . Results GL decreased LPO content and increased SOD activity in a dose-effect manner. Conclusion GL can counteract the lipid peroxidation in VSMC and its therapeutic mechanism for atherosclerosis may be related to the counteraction of lipid peroxidation and enhancement of endogenous antioxidase activity.
