1.Observation of doxofylline the treatment of bronchial asthma and clinical intervention

Objective To observe the clinical nursing effect of doxofylline in the treatment of bronchial asthma and intervention. Methods 98 cases of bronchial asthma patients in our hospital from June 2014 to November 2016 received as the object of nursing, the patients were divided into control group and treatment group, 49 cases in each group, using multiple theophylline treatment, the control group was given routine care, the treatment group on the basis of the comprehensive nursing, the clinical effects of two groups were compared before and after, adverse reactions and treatment of pulmonary function. Results The total effective rate of treatment group was 89.80%, control group total effective rate was 73.47%, the total effective rate of treatment group was significantly better than the control group (P<0.05), the difference was statistically significant; the treatment group adverse reaction rate was 6.12%, the control group adverse reaction rate was 18.37%, incidence of adverse reaction in treatment group was significantly better than the control group (P<0.05), the difference was statistically significant; two groups of patients after treatment of FEV1, FFV1/FVC and PEF indexes were improved, but the treatment group was significantly better than the control group (P<0.05), the difference was statistically significant. Conclusion Doxofylline the treatment of bronchial asthma in the process of the comprehensive nursing has a good effect, and less adverse reactions. It is worthy of clinical application.
2.Clinical Study on Treatment of Constipation Caused by Antipsychotic Drugs with Acupuncture and Tuina Combined with Laxative Suppository

Objective: To observe the therapeutic results of Acupuncture and Tuina combining with laxative suppository in treating constipation caused by antisychotic drugs. Methods: 240 patients were randomly divided into Group A, B, C, and D, 60 in each group. Group A were treated with Acupuncture and Tuina combining with laxative suppository, Group B with Acupuncture, Group C with Tuina and Group D with laxative suppository. Results: The effective rate in Group A was 98.3%, Group B 95.0%, Group C 91.7% and Group D 60.0%. Univariate χ2 was used for analysis. There were significant differences among the 4 groups. Group A was compared with Group B and C (P<0.05) and compared with D (P<0.01). The efficacy time( (-χ)± S) was 2.41 ± 1.87h in Group A, 2.47 ± 1.89h in Group B,9.81 ± 6.12h in Group C and 15.13 ± 6.17h in Group D. The t-test was used for analysis. The efficacy time was significant different in Group A and B (P<0.05), significant different in Group A and C (P<0.05), marked different in Group A and D (P<0.01) and no significant different in Group B and C (P>0.05). Conclusion: Acupuncture and Tuina were effective, yet Acupuncture and Tuina combined with laxative suppository were more effective, and induce quick defecation.
3.Preparation of scopolamine hydrobromide nanoparticles-in-microsphere system

This study is to prepare scopolamine hydrobromide nanoparticles-in-microsphere system (SH-NiMS) and evaluate its drug release characteristics in vitro. SH nanoparticles were prepared by ionic crosslinking method with tripolyphosphate (TPP) as crosslinker and chitosan as carrier. Orthogonal design was used to optimize the formulation of SH nanoparticles, which took the property of encapsulation efficiency and drug loading as evaluation parameters. With HPMC as carrier, adjusted the parameters of spray drying technique and sprayed the SH nanoparticles in microspheres encaposulated by HPMC was formed and which is called nanoparticles-in-microsphere system (NiMS). SH-NiMS appearances were observed by SEM, structure was obsearved by FT-IR and the release characteristics in vitro were evaluated. The optimized formulation of SH nanoparticles was TPP/CS 1:3 (w/w), HPMC 0.3%, SH 0.2%. The solution peristaltic speed of the spray drying technique was adjusted to 15%, and the temperature of inlet was 110 degrees C. The encapsulation product yeild, drug loading and particle sizes of SH-NiMS were 94.2%, 20.4%, and 1256.5 nm, respectively. The appearances and the structure of SH-NiMS were good. The preparation method of SH-NiMS is stable and reliable to use, which provide a new way to develop new dosage form.
4.Relationship between airway inflammation and bronchial hyperreactivity in rats and the effect of theophylline

AIM: To determine the relationship between antigen-induced airway inflammation characterized by pulmonary eosinophilia and bronchial hyperreactivity in rats, and to evaluate the effect of theophylline at different doses. METHODS: In ovalbumin (OA)-sensitized rats, bronchiole wall area, eosinophils around bronchi, and the responses to methacholine (MCh) aerosol were measured after 1% OA aerosol challenge with computer-assisted techniques. RESULTS: OA challenge caused both inflammation and airway hyperreactivity, and there was a significantly positive correlation between them. Oral theophylline (1-12.5 mg/kg, bid for 7 days) attenuated antigen-induced inflammation (swelling of bronchiole walls and pulmonary eosinophilia) and bronchial hyperreactivity. CONCLUSION: These findings confirm that bronchial hyperreactivity positively correlates to airway inflammation in the rat, and suggest that theophylline at relatively lower doses has anti-inflammatory effect in airway allergic reaction.
5.Effect of affected side upper limb pillow position on hemodynamic and safety of patients undergoing cerebellopontine angle tumor resection

Objective:To investigate the effect of upper limb pillow position on hemodynamic and safety of patients undergoing cerebellopontine angle tumor resection.Methods:Eighty-four patients receiving cerebellopontine angle tumor resection in our hospital from January 2016 to December 2019 were randomly divided into the experimental group (42 cases) and the control group (42 cases). Patients in the control group were placed in routine upper limb position, while patients in the experimental group were placed in upper limb pillow position. The data including systolic pressure, diastolic pressure, heart rate, saturation of blood oxygen were recorded on admission of operation room, completing placing body position, 30 minutes and 60 minutes after operation and after finishing the operation. The numbness/soreness of upper limbs and pressure injury rate was compared between the experimental group and the control group.Results:The rate of numbness/soreness of upper limbs were 2.4% (1/42) in the experimental group, 19.1%(8/42) in the control group, the differences were statistically significant ( χ2 value was 6.098, P<0.05). The stage 1 pressure injury were 2 cases in the experimental group, stage 1 and 2 pressure injury were 6 cases and 2 cases, respectively in the control group, the differences were statically significant ( Z value was 2.039, P<0.05). Conclusion:Upper limb pillow position of the operation side can reduce postoperative complication of patients undergoing cerebellopontine angle tumor resection, but will not increase the risk of abnormal hemodynamic fluctuation.
6.Application of Cystatin C-based formulas for prediction of glomerular filtration rate

Objective To investigate the applicability of Cys C-based formulas for prediction of GFR in Chinese patients with CKD.Methods A total of 176 adult patients with CKD including 90 males and 86 females collected from 4 hospitals located in different geographic regions of China (Beijing,Shanghai,Dalian and Changsha) were enrolled in this study from September 2007 to July 2009.The rGFR was measured using 99mTc-DTPA clearance rate two-sample method.Cystantic C was measured by PETIA and PENIA respectively.The results of eGFR in the Larsson formula,Grubb formula,Hoek formula,Filler formula,Stevens formula and Hojs formula were compared with the rGFR to evaluate the calculation coherence,bias,precision,accuracy and the performance of correct phasing of the formulas.Results The mean 99mTc-DTPA clearance was [40.70 ( 19.09 - 86.49)] ml · min-1 · ( 1.73m2 ) -1.Significant difference was witnessed in the evaluation of GFR estimation formulas calculated by PETIA and PENIA (P <0.01).ICC and Spearman correlation analysis revealed a significant correlation between eGFR and rGFR.The ICCs of eGFR and rGFR ranged from 0.874 to 0.938.Compared with rGFR,the 30% accuracy of all the eight evaluation formulas using PETIA and PENIA method were lower than 60%.The percentages of correct phasing in all the 5 stages of CKD were not ideal.With these formulas,percentages of correct phasing from CKD stage 2 to CKD stage 4 were lower than 65%.The eGFRs were underestimated by formulas evaluated by PENIA in CKD stage 1.All the eGFRs were overestimated remarkably by all equations in CKD stage 5.Conclusions None of the eight Cys C based formulas are ideal for estimation of GFR in Chinese CKD patients,and they can not be applied to Chinese patients directly.For this patient population,further studies will be needed to develop a more accurate Cys C-based GFR estimation formula that includes ethnicity,age and other factors.
7.Continuous improvement of nursing care quality of orthopedic demonstration wards by taking hospital accreditation as an opportunity

Objective To explore the methods for quality management and continuous improvement of nursing care quality in the orthopedic demonstration ward by taking the hospital accreditation as an opportunity. Methods From July 2012 to June 2013, the continuous care quality improvement in the ward was carried out to find out the problems with PDCA (plan, do, check, action) cycle method, including enhancing the function of orthopedic nursing quality management groups, conducting all-staff training and improving the knowing rate by referring to the standards of hospital assessment standards. Results After the performance of whole-process quality management, the percentage of indexes assessed at level A, B and C was increased from 42.2%to 50.0%, 17.2%to 14.7%and 40.2%to 35.3%, respectively. The score of nurses' responsibility accreditation was increased from 92 to 95. The rates of patient and nursing staff satisfaction were increased from 91.8%to 98.9%and 92.57%to 97.7%, respectively. Conclusion In accordance with the standards for hospital accreditation, the continuous improvement of nursing quality in the orthopedic demonstration wards can improve the specialist care of orthopedic care, improve patients' and nurses' satisfaction, thus making the daily work more scientific and standardized.
8.Association of serum levels of matrix metalloproteinase-12 and a functional polymorphism in the promoter-82A/G of the matrix metaiioproteinase-12 gene with acute atherosclerotic cerebral infarction

Objective To investigate the association of serum levels of matrix metalloproteinase-12 (MMP-12) and a functional polymorphism in the promoter - 82A/G of the MMP-12 gene with acute atherosclerotic cerebral infarction (ACI).Methods All 608 cascs of acute ACI and 374 healthy controls were included in the study.Serum levels of MMP-12 were measured using the enzyme-linked immunosorbent assay (ELISA). At the same time,polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was performed on the - 82A/G polymorphism in the MMP-12 gene.Results The serum levels of MMP-12 in ACI group (( 17.36 ± 9.12) ng/ml) were not significantly higher than those in the healthy control group ( ( 17.42 ± 7.70) ng/ml,t = 0.047,P =0.962).The frequency of the AG + GG genotypes was not significantly different between the two groups (7.6% vs 5.9%,x2 =0.281,P =0.584),and the frequencies of the G allele were 3.8% and 2.9% in the ACI group and the control group respectively which were also not significantly different between the two groups ( x2 =0.746,P =0.374).Conclusion There is no correlation between serum level with genetic polymorphism in MMP-12 promoter (-82A/G) and cerebral infarction.
9.A clinicopathologic and immunohistochemical study of diffuse large B-cell lymphoma.

OBJECTIVETo study the clinicopathologic and immunohistochemical features of diffuse large B-cell lymphoma (DLBCL) and the significance of immunohistochemistry in diagnosis and differential diagnosis of DLBCL.
METHODS60 cases of DLBCL were studied and immunohistochemical staining for LCA, L26, BLA-36, CD30, bcl-6 were carried out with the EnVision 2 step method.
RESULTSThe age range of 76.7% (46/60) patients was 40 - 70 years. The location of the lesion includes nodal and extranodal sites. 90.0% (54/60) were in clinical stages of II (24/54), III (21/54), IV (9/54). Histopathologic morphology presented as centroblastic (88.3%, 53/60), immunoblastic (3.3%, 2/60), anaplastic large B cell type (3.3%, 2/60) and T cell rich B cell type (5.0%, 3/60). Immunostaining showed 100% (60/60) DLBCL were positive for LCA, L26, BLA36, 3.3% (2/60) DLBCL positive for CD30, 95% (57/60) expressed bcl-6 protein.
CONCLUSIONSDLBCL is an aggressive lymphoma which shows cytologic variability from case to case. The evaluation of pathologic features and immunohistochemistry in DLBCL are useful and practical for diagnostic purposes, but cannot delineate distinctive morphologic subtypes.

10.Influence of plasma matrix metalloproteinase-7 levels and genetic polymorphism of -181A/G on the stability of carotid plaque

Objective To explore the influence of plasma matrix metalloproteinase-7 ( MMP-7 ) levels and genetic polymorphism of MMP-7 - 181 A/G on the stability of carotid plaque.Method According to carotid ultrasound examination, 503 patients with carotid atherosclerotic lesions were consecutively recruited and divided into vulnerable plaque group (n = 118) and stable plaque group (n = 385).Plasma MMP-7 levels were measured by enzyme-linked immunosorbent assay (ELISA), and MMP-7 -181 A/G genotypes were determined by polymerase chain reaction-restiction fragment length polymorphism (PCR-RFLP).Results Plasma MMP-7 levels in carotid vulnerable plaque group were significantly enhanced as compared to stable plaque group (t =5.49, P =0.00).The frequency of MMP-7 -181G allele in vulnerable plaque group was significantly higher than that in stable plaque group (11.4% vs 7.0% ,χ2 = 4.78, P= 0.029).Compared to AA genotype, the genotypes with - 181G allele (AG + GG) significantly increased susceptibility to carotid vulnerable plaque ( χ2 = 5.01, OR = 1.81, P = 0.025 ) .When further analyzing the relationship between genotype and plasma MMP-7 levels, no significant differences of plasma MMP-7 levels were observed between AA genotype and AG + GG genotype in stable plaque group.However, in vulnerable plaque group, plasma MMP-7 levels of AG + GG genotype were significantly higher than that of AA genotype( t = 2.62, P = 0.01).Conclusion The present findings suggest that plasma MMP-7 level may be a biomarker for carotid vulnerable plaque.Genetic polymorphism of - 181 A/G in MMP-7 promoter may affect the expression of MMP-7, and seems to be implicated in susceptibility to carotid vulnerable plaque.
