1.Analysis of the influencing factor of bleeding amount after selective cesarean delivery

Objective To explore the influencing factor of amount of bleeding after selective cesarean delivery,and to provide theoretical basis to prevent postpartum hemorrhage in clinic.Methods A total of 300 delivery women,who received selective cesarean delivery because of social factors,was included this research.The amount of bleeding after selective cesarean delivery was determined by volumetric method and weight method.The related factor of amount of bleeding after selective cesarean delivery was analyzed by single-factor and multiple regression method.Results The result of single-factor and multiple regression method showed that the amount of bleeding after selective cesarean delivery was related to uterine dysplasia,uterine contraction fatigue,cervical score,placenta previa,placental adhesion/implant and neonatal weight(Test value =9.263,9.845,3.447,7.234,5.765,3.627,all P < 0.01),but not related to age,body weight,gestational duration,gravidity times,production time,abortion history,premature rupture of membrane (all P > 0.05).Conclusion Conclusion Uterine dysplasia,uterine contraction fatigue,cervical score,placenta previa,placental adhesion/implant and neonatal weight were the related factors of amount of bleeding after selective cesarean delivery,and should be predicted intensely to reduce the incidence of postpartum hemorrhage.
2.Evidence-based nursing on improving compliance of COPD patients with noninvasive positive pressure ventilation

Objective To explore the effect of evidence-based nursing on improving the compliance of chronic obstructive pulmonary disease (COPD) patients with noninvasive positive pressure ventilation. Methods Forty-five patients hospitalized in the first half of the year were assigned as control group and another 45 patients hospitalized in the second half of the year were as experiment group. The control group was treated with traditional nursing methods. In the experiment group, a nursing panel was set up to analyze the causes of noncompliance of COPD patients with noninvasive positive pressure ventilation and gave care to the patients based on the analyses. The two groups were compared in terms of adverse reactions and the compliance with treatment. Result The rates of compliance with treatment and adverse reactions such as fear and anxiety , the man-machine confrontation and flatulence in the experiment group were significantly lower than those of the control group obviously (P<0.05). Conclusion The evidence-based nursing can reduce the incidence of adverse reactions , and enhance the compliance of COPD patients with noninvasive positive pressure ventilation effectively.
3.Effect of short-term intensive treatment with insulin pump on the level of circulating endothelial progenitor cells in patients of newly diagnosed type 2 diabetes mellitus and its antioxidation mechanism

Objective To observe the protective effects of short-term continuous subcutaneous insulin infusion (CSⅡ) therapy on the peripheral blood circulating endothelial progenitor cells (EPCs) in patients with newly diagnosed type 2 diabetes mellitus (T2DM) and its antioxidation mechanism. Methods Forty T2DM patients (T2DM group) and 30 controls (control group) were involved in this study. Density gradient centrifugation was used to collect peripheral blood mononuelear cells. After 7 days' inducement and differentiation, FITC-UEA and DIL-acLDL double staining cells were identified to be differentiated EPCs by laser scanning conform microscope, and they were counted under inverted microscope. Serum levels of reactive oxygen species (ROS), malondialde-hyde (MDA), glutathione (GSH), superoxide dismutase (SOD) were examined. Results In T2DM group, the EPCs in peripheral blood were significantly reduced (46 ± 3 vs 23 ± 4, P< 0.01), BOS and MDA increased[(1125 ± 687) U/ml vs(2088 ± 765) U/ml, (4.58 ±3.03) μ mol/L vs (8.32 ± 4.15) μ mol/L, P < 0.01] , and GSH and SOD decreased [(263 ± 35) mg/L vs (206 ± 41) mg/L, (103 ± 7) U/ml vs (80 ± 15) U/ml, P < 0.05] . EPCs were negatively correlated with BOS and MDA (r = -0.7352,P < 0.01 ;r = -0.8454, P < 0.01) and was positively correlated with GSH and SOD (r =0.5435,P <0.05;r =0.6437,P <0.05). After short-term CSⅡ therapy, EPCs, GSH, and SOD in patients with T2DM significantly increased (P < 0.05),but BOS and MDA significantly decreased (P <0.05). Conclusion Short-term CSⅡ therapy can significantly reduce oxidative stress in patients with T2DM, promote the restoration of EPCs, and then prevent vascular disease.
4.Study on Decoloration Process for Crude Polysaccharides from Cordyceps militaris Link. by Macroporous Resin

Objective To study the optimum decoloration technological conditions of crude polysaccharides from Cordyceps militaris Link. (PCM) by macroporous resin. Methods With the decolorization rate and retention rate of PCM as indexes, static adsorption factor experiment was used for the optimization of seven kinds of macroporous resin. And then the optimum conditions of decolorization of PCM were investigated by the static/dynamaic adsorption single-factor experiments. Results D941 macroporous resin is the best. The optimum decoloration technological conditions were as follows:initial concentration of the sample was 50 mg/mL, pH value was 5.5, operating temperature was 45℃, the flow rate of dynamaic adsorption was 2.5 BV/h, and the sample solution volume was 5 bed volume per hour. Under the optimal conditions, decolorization rate and retention rate of PCM were ( 93.9 ± 3.4) % and (91.7 ± 2.2) %, respectively. Conclusion The resin D941 is suitable for the decoloration of PCM. The optimized method is feasible and reliable, having the prospects for popularizing.
5.Updates and amendments of the quality standard of nitroglycerin tablets in the Chinese Pharmacopoeia 2020

Objective: To propose updates and amendments for the nitroglycerin tablets quality in the Chinese pharmacopoeia 2020 due to the failure of effective separation of 4 known impurities and nonseparation of free nitrate ion and excipients.
Methods: Related substances were analyzed using gradient elution by HPLC, and free nitrate ion was determined on SAX column by different HPLC method.
Results: Using the improved method to test the related substances and free nitrate ion of nitroglycerin tablets,the content of the maximum individual impurity were not more than 0.5%, the total content was not more than 2.4% and the content of free nitrate ion was not more than 6.3%.
Conclusion: The improved method is accurate and feasible. It provided a reference for the updates and amendments of the quality standard of nitroglycerin tablets in the Chinese Pharmacopoeia 2020.
6.Effect of health education on enhanced curative effect of motor imagination therapy of patients with hyperbaric oxygen

Objective To investigate the effect of health education on enhanced curative effect of motor imagination therapy of patients with hyperbaric oxygen(HBO). Methods Thirty patients undergoing HBO combined with motor imagination therapy were given health education including contents of motor imagination,time for motor imagination,intervention of bad emotions and time for motor imagination after leaving the treatment cabin.The understanding of motor imagination-associated knowledge after education was compared to that before education. Results The understanding of motor imagination-associated knowledge after education was significantly better than before education(χ2=9.017,P=0.003). Conclusion Health education may help improve the curative effect of hypterbaric oxygen treatment(HBO)by way of improving the understanding of motor imagination-related knowledge.
7.Method comparison between INNOVANCE D-Dimer and PLUS D-Dimer

Objective To evaluate the clinical performance of INNOVANCE D-Dimer,and provide information for clinical application.Methods 402 cases of sodium citrate anticoagulant blood were tested with INNOVANCE assay and PLUS assay on CA7000 analyzer to measure plasma D-Dimer levels.VIDAS-30 immunology analyzer was also used to validate the two assays.4 patients with elevated D-Dimer were monitored continuously during 5 days using INNOVANCE assay and PLUS assay respectively,then the consistency of trend between 2 assays was analyzed.Plasma specimens added with hemoglobin,bilimbin and triglyceride were used to verify the anti-interference capability of INNOVANCE D-Dimer assay.Results In 402 specimens,the result ranges of INNOVANCE D-Dimer and PLUS D-Dimer were [2.15 (0.33,8.63)]mg/L FEU and [325.50 (123.75,974.00)] μ,g/L DDU,respectively.The consistency between two assays was poor (Z =-17.375,P =0.000),especially the results in the range of PLUS D-Dimer (201-300) μg/L DDU and (301-400) μg/L DDU,the coincidence rates were only 25% and 15%,respectively; the coincidence rate was up to 85% during PLUS D-Dimer (500-600) μg/L DDU; the coincidence rate was close to 100% when PLUS D-Dimer over 700 μg/L DDU.Totally 47 of 402 cases were unmatched between two assays.Verified by VIDAS 30,83.0% (39/47) was false negative for PLUS assay,4.3% (2/47) was false negative for INNOVANCE assay,12.7% (6/47) was false positive for PLUS assay.There were 5 false positives and 39 false negative for PLUS assay,totally 45 cases; Two false negative for INNOVANCE assay.Four patients with elevated D-Dimer were monitored and the results showed similar trend between 2 assays.For INNOVANCE assay,the capacity of anti-interference to free bilirubin,unconjugated bilirubin,hemoglobin,and triglyceride was up to 217 μmol/L,337 μmol/L,41.04 g/L,18.35 mmol/L,respectively.Conclusions INNOVANCE assay can markedly reduce false negative results of D-Dimer compared with PLUS assay.INNOVANCE D-Dimer has good performance on anti-interference to jaundice,hemolysis and lipemia samples.

Objective :To construct hepatocarcinoma specific IL-1? anti-sense RNA expression vector and to explore its effect on the growth of implanted hepatocarcinoma H22 cells in mice and the possible mechanism. Methods:Murine IL-1? anti-sense RNA expression vectors pafpIRES2-antiIL-1?1 and pafpIRES2-antiIL-1?2 under the regulation of minimal alpha-feto protein (AFP) promoter and CMV enhancer were constructed,and further verified by PCR,restriction endonuclease analysis and DNA sequencing. H22 cells transfected with pafpIRES2-antiIL-1? 1 or pafpIRES2-antiIL-1? 2 were divided into 3 groups:H22/mock,H22/antiIL-1?1 and H22/antiIL-1?2 group. Expression of IL-1? was detected by RT-PCR. Transfected H22 cells were subcutaneously injected into mice to establish tumor implanted mouse model. Tumor volume was measured; the cytotocixity of spleen NK against H22 cells was detected by MTT. Results:Hepatocarcinoma specific IL-1? anti-sense RNA expression vectors pafpIRES2-antiIL-1?1 and pafpIRES2-antiIL-1?2 were successfully constructed and were verified by PCR,restriction endonuclease analysis and DNA sequencing. IL-1? expression in H22 cells was down-regulated after transfected with IL-1? anti-sense RNA expression vectors,especially with the pafpIRES2-antiIL-1?2 vector. Hepatocarcinoma cells implanted mouse model was successfully established. Tumor volume and growth of tumor in H22/antiIL-1?2 mice was obviously smaller than that in H22/mock mice,and the cytotocixity of spleen NK against H22 cells in H22/antiIL-1?1 and H22/antiIL-1?2 mice was also greatly enhanced. Conclusion:Hepatocarcinoma specific IL-1? anti-sense RNA expression vector pafpIRES2-antiIL-1? was successfully constructed. It effectively inhibits the growth of implanted hepatocarcinoma in mice probably through specifically blocking expression of IL-1? and increasing cytotocixity of spleen NK.
9.Association between single nucleotide polymorphism of BARD1 gene and BRCA1 gene mutation in epithelial ovarian cancer

Objective To investigate the relationship between single nucleotide polymorphism (SNP) of BARD 1 gene and BRCA1 gene in epithelial ovarian cancer (EOC).Methods Nineteen EOC patients with BRCA1 gene mutation and 50 EOC cases without BRCA1 gene mutation between January 2016 and October 2016 were collected,and all EOC were diagnosed by pathological method.BARD1 gene variants were detected by next generation sequencing (NGS).The SNP of BARD1 gene was analyzed by Pearson linear correlation.Logistic regression analysis was used to research the clinicopathologic features and BRCA1 gene mutation associated with BARD1 gene SNP.Pearson's chi-square test was used to analyze the association between BARD1 gene Val507Met,Arg378Ser and Pro24Ser with different clinicopathologic features and BRCA1 gene mutation risk.Results (1) Eight BARD1 gene variants were found in 69 ovarian cancer patients,in which Val507Met,Arg378Ser and Pro24Ser were common variants,and the rate of mutation were all 54% (37/69).(2) There was a significant linear correlation among Val507Met,Arg378Ser and Pro24Ser (all P<0.01).(3) Obvious differences were found in Val507Met,Arg378Ser and Pro24Ser of BARD1 gene between BRCA1+ and BRCA1 (all P<0.05).(4) No differences were found between BARD1 gene Val507Met,Arg378Ser and Pro24Ser and the clinicopathologic features (all P>0.05),while obvious differences were found in BRCA1 gene mutation compared to the controls group.The risk of BRCA1 mutation in Val507Met and Arg378Ser were more evident in subjects with negative family history,positive menopause history,negative tubal ligation,onset age (≤60 years old) and sensitivity to platinum-based chemotherapy in EOC (all P<0.05),while Pro24Ser was only more evident in positive menopause history of EOC (P<0.05).Conclusions BARD1 Val507Met,Arg378Ser and Pro24Ser are the common genotypes,which are associated with BRCA1 mutation in EOC.The family history,menopause history,tubal ligation,onset age and sensitivity to platinum-based chemotherapy have effects on BARD1 SNP in the risk of BRCA1 gene mutation.
10.Clinical significance and distribution of BRCA genes mutation in sporadic high grade serous ovarian cancer

Objective To investigate the mutations of BRCA genes in sporadic high grade serous ovarian cancer (HGSOC) and study its clinical significance. Methods Sixty-eight patients between January 2015 and January 2016 from the Affiliated Cancer Hospital of Zhengzhou University were collected who were based on pathological diagnosis of ovarian cancer and had no reported family history, and all patients firstly hospitalized were untreated in other hospitals before. (1)The BRCA genes were detected by next-generation sequencing (NGS) method. (2)The serum tumor markers included carcinoembryonic antigen (CEA), CA125, CA199, and human epididymis protein 4 (HE4) were detected by the chemiluminescence methods, and their correlation was analyzed by Pearson linear correlation. Descriptive statistics and comparisons were performed using two-tailed t-tests, Pearson′s chi square test, Fisher′s exact tests or logistic regression analysis as appropriate to research the clinicopathologic features associated with BRCA mutations, including age, International Federation of Gynecology and Obstetrics(FIGO)stage, platinum-based chemotherapy sensitivity, distant metastases, serum tumor markers (STM). Results (1) Fifteen cases (22%, 15/68) BRCA mutations were identified (BRCA1: 11 cases; BRCA2: 4 cases), and four novel mutations were observed. (2) The levels of CEA, CA199, and HE4 were lower in BRCA mutations compared to that in control group, while no significant differences were found (P>0.05), but the level of CA125 was much higher in BRCA mutation group than that in controls (t=-3.536,P=0.003). Further linear regression analysis found that there was a significant linear correlation between CA125 and HE4 group (r=0.494,P<0.01), and the same correlation as CEA and CA199 group (r=0.897,P<0.01). (3) Single factor analysis showed that no significant differences were observed in onset age, FIGO stage, distant metastasis, and STM between BRCA+and BRCA- group (P>0.05), while significant differences were found in CA125 and sensitivity to platinum-based chemotherapy between the patients with BRCA mutation and wild type (P<0.05). The multiple factors analysis showed that the high level of CA125 was a independent risk factor of BRCA mutations in sporadic HGSOC (P=0.007). Conclusion The combination of CA125 with BRCA have great clinical significance, the mutation of BRCA gene could guild the clinical chemotherapy regiments.
