1.Research advances in the etiology and detection method of hand,foot and mouth disease

Hand,foot and mouth disease was caused by many kinds of enterovirus,while enterovirus 71 (EV71)and coxsackievirus A16(CA16)were prevalent causative agents of this disease,and each various subtypes of EV71 and CA16 were appear alternatively. Early diagnosis is significant for timely treatment and prognosis of children with hand,foot and mouth disease. Now,the laboratory detection methods of the etiology including virus isolation cul-ture,reverse transcription polymerase chain reaction,real - time polymerase chain reaction,reverse transcription loop -mediated isothermal amplification,enzyme - linked immunosorbent assay,neutralization tests. This review will summa-rized the advancement in the research of the etiology and detection method of hand,foot and mouth disease.
2.Current situation and development of community health management of hypertension

Cardiovascular disease caused by hypertension has become the first cause of death of residents in our country, the burden of disease has become more and more serious, how to manage hypertension has become an important public health issue in our country. In this review, the development and current situation of the community health management of hypertension at home and abroad are described and analyzed, the experience and practice worthy of learning are summarized to provide evidence for the prevention and control of hypertension in community.
3.High viscosityversus common viscosity bone cement systems in the treatment of osteoporotic vertebral compressive fractures:a Meta-analysis

BACKGROUND:Vertebroplasty system has been proved to be effective for osteoporotic vertebral compressive fracture (OVCF); however, bone cement leakage-related complications occur frequently. Thereafter, high-viscosity bone cement system with high safety and efficacy is developed, but there is stil a lack of large-scale and high quality research. OBJECTIVE:To systematicaly review the efficacy and safety of high viscosityversus common bone cement systems for OVCF. METHODS: PubMed, Embase, Cochrane Library, CNKI, CqVip and WanFang databases were searched to colect the literatures about randomized controled trials (RCTs) or clinical controled trials (CCTs) of high viscosityversus common bone cement systems for OVCF published before January 2016. The quality assessment of included literatures and data extraction were performed by two researchers independently according to the Cochrane system. Data analysis was conducted using RevMan 5. 2 software. RESULTS AND CONCLUSION:Four RCTs and six CCTs were enroled. The results of Meta-analysis indicate that there were no significant differences in the volume of bone cement [MD=0.17, 95%CI(-0.04, 0.38)], visual analogue scale scores [RCT:MD=-0.30, 95%CI(-0.72, 0.11); CCT:MD=-0.05, 95%CI(-0.43, 0.32)] and incidence of second fractures [OR=1.77, 95%CI(0.24, 12.86)] between two bone cements. However, patients undergoing high viscosity bone cement system have significantly lower Oswestry disability index scores [MD=-2.99, 95%CI(-5.85,-0.13)], greater recovery of Cobb angle [MD=-3.19, 95%CI(-5.27,-1.10)] and lower incidence of cement leakage [OR=0.35, 95%CI(0.24, 0.51)]. High viscosity bone cement system in the treatment of OVCF shows good results at recovery of spinal structure and function and reducing leakage. Due to the limited quantity of included literature sand high heterogeneity, more large scale and high quality RCTs are stil needed for further verification.
4.Bioinformatics analysis of VP1 protein of coxsackievirus A6

Objective:To predict the basic physicochemical properties ,structure and function ,and linear B-cell epitopes of the capsid protein VP1 of coxsackievirus A6(CVA6).Methods: The amino acid sequence of the CVA6 VP1 was analyzed using Bioedit software and various online tools including SubLoc ,TargetP and the others from ExPASy Bioinformatics Resource Portal.Results: The CVA6 VP1 protein was a hydrophilic protein with a relative molecular weight of 33.6 kD and an isoelectric point of 7.92.This protein containsed 24 phosphorylation sites , but no signal peptide , transmembrane domains and possible fatty acylation sites.Its secondary structure was characterized by the richest random coils , and 48.52 percent of its amino acid residues exposed at the solution inter-face.Epitope prediction by Bepipred showed a number of potential B cell epitopes in the protein ,the highest antigenicity index among them located in the region of amino acids residue 155-165.Conclusion:The basic physicochemical properties ,structure and function characteristics ,and potential linear B-cell epitopes of CVA 6 VP1 were successfully predicted , which laid foundations for the further study on the protein and the preparation of vaccines and immunological diagnostic reagents for CVA 6 infection.
5.Effect of CAG induction therapy in patients with acute myeloid leukemia

Objective To assess the effect of low-dose cytarabine and aclarubicin in combination with gran-ulocyte colony-stimulating factor (G-CSF) protocol (CAG) in patients with acute myeloid leukemia (AML),and to understand the potential factors affecting the outcome of CAG induction therapy, therefore to find the optimum pa-tients for CAG therapy. Methods Twenty-one AML patients were enrolled in the current study. All patients were treated with CAG regimen including cytarabine (10 mg/m~2, subcutaneously, every 12 h, days 1 - 14), lacinomycin (5~7 mg/m~2,intravenously,every day, days 1 -8) ,and G-CSF (200 μg/m~2,subcutaneously, every day,12 h be-fore Ara-C was given) priming. Results The overall complete remission (CR) rate of the 21 AML patients was 66.7% (14/21). The CR rates was 87.5% (7/8) in patients older than 60 yrs,60.0% (9/15) in the refractory or relapsed patients,83.3% (5/6) in the MDS transformed AML patients. The CR rates for patients with hyperprolif-erative BM and median to poor proliferative BM were 33.3% and 91.7% ,respectively(P =0.009). The median o-verall survival (OS) time of the 21 AML patients was 450 days. Two-year survival rate estimated by Kaplan-Meier Method was 30.6%. The overall median disease free survival (DFS) was 165 days. The median OS time for those refractory or relapsed was 435 days. The median OS time for those with poor cytogenetic state or standard or good cytogenetic state was 140 days and 620 days, respectively (P = 0.001). The median OS time for patients with hyperproliferative BM and median to poor proliferative BM was 321 days and 620 days, respectively (P = 0.05). The median recovery time of granulocytes above 1.0×10~9/L was 8 days. The median duration of fever was 3.5 days. The rate of infections exceeding WHO grade Ⅱ was 42.9%. No early death occurred. Conclusions The CAG induction therapy may have a higher CR rate in patients with refractory or relapsed AML, elderly AML and secondary AML from MDS transformation, and extend the median overall survival time in refractory or relapsed patients. CAG therapy can not improve the outcome of patients whose BM was in high grade proliferation state or whose cytogenetic state was poor. CAG therapy can shorten the duration of agranulocytosis and decrease the inci-dence of serious infection. Therefore, CAG therapy is worth recommending to patients who can not endure the rou-tine intensive chemotherapy.
6.Diagnostic value of double-balloon enteroscopy and CT enterography for Crohn disease in intestine

Objective To investigate the value of double-balloon enteroscopy and CT enterography for the diagnosis of intestinal Crohn disease ( CD ) . Methods Data of 125 patients with suspected CD undergoing double-balloon enteroscopy and CT enterography were reviewed. Diagnosis was made based on pathological,endoscopic findings and clinic follow-up results. Detection rates and diagnostic rates of double-balloon enteroscopy and CT enterography for intestinal CD were compared. Results The detection and diagnostic rates of intestinal CD by double-balloon endoscopy were 62. 4%( 78/125) and 94. 8% ( 74/78) , respectively. The sensitivity, specificity and accuracy of double-balloon enteroscopy in the diagnosis of intestinal CD were 100. 0%, 92. 2% and 96. 8%, respectively. The detection and diagnostic rates of intestinal CD by CT enterography were 44. 8%( 56/125 ) and 89. 3% ( 50/56 ) , respectively. The sensitivity, specificity and accuracy of CT enterography in the diagnosis of intestinal CD were 67. 6%, 88. 2% and 76. 0%, respectively. Both the detection rate and the diagnostic rate of double-balloon enteroscopy in intestinal CD were higher than those of CT enterography. Conclusion Double-balloon enteroscopy shows high application value for the diagnosis of intestinal CD, but CT enterography before enteroscopy can guide the selection of endoscopic approach, reduce economic expenditure and alleviate patients' pain, so CT enterography could be the first choice for intestinal CD patients with contraindications of enteroscopy.
7.Effect of different drugs inhalation on SD rats lung tissue

Objective To have SD rats inhaled with different drugs,and observe their lung pathological change of lungs through light microscopy,in order to evaluate the safety of different drugs inhaled by natural rats. Methods A total of 40 rats were randomly divided into 8 groups,and every group had 5 rats,including blank control groups,9 g/L saline group,Salbutamol group,Dingchuantang group,Shuanghuanglian group,Centamicin group,Danshen group,Silicon dioxide group,twice a day,last 56 days totally. Then,blood and bronchoalveolar lavage fluid were collected and analyzed for cell count,percent of each type of cell,to measure the severity of the inflammation. Additionally,histopathology re-vealed the lungˊs pathological change and the number of dust cell;while immunohistochemistry revealed CD163 respon-ding. Conclusions 9 g/L saline,salbutamol for atomized inhalation does not cause lung tissue damage;Long-term use of non-atomized drugs in atomization can cause lung tissue injury in SD rats,and the severity varies with specific drugs.
8.The effect of gestational diabetes mellitus on the newborn metabolism related indexes and prognosis

Objective Investigate the effect of gestational diabetes mellitus (GDM)on the neonatal metabolism and prognosis.Methods A total of 265 singleton pregnancy GDM patients were collected as the observation group,while 260 cases of healthy singleton pregnant women were chosen as the control group.Detected the levels of adiponectin,insulin,C-peptide,glycated hemoglobin and blood glucose,compared the prognosis of two groups.Results Compared with the control group,adiponcetin and blood glucose in 2 hours after birth in observation group were lower,insulin,C-peptide and glycated hemoglobin were higher,the number of cesarean section was more,the difference was statically significant (P<0.05).The incidence of low birth weight infant,macrosomia,neonatal asphyxia,neonatal hypoglycemia,neonatal hyperbilirubinemia,neonatal pneumonia,fetal death and newborn teratogenesis in the observation group was higher than that in the control group,the difference was statically significant (P<0.05).Conclusion GDM could cause metabolic disorder and affect fetal growth and development,leading to poor prognosis.
9. Clinical outcome of allogeneic hematopoietic stem cell transplantation with FLAG sequential busulfan/cyclophosphamide conditioning regimen for refractory/relapsed acute myeloid leukemia

Objective:
To investigate the therapeutic effects of allogeneic hematopoietic stem cell transplantation (allo-HSCT) with FLAG sequential busulfan/cyclophosphamide(Bu/Cy) conditioning regimen for refractory/relapsed acute myeloid leukemia.
Methods:
From February 2012 to June 2017, 21 patients with refractory/relapsed acute myeloid leukemia underwent allo-HSCT with FLAG sequential Bu/Cy conditioning regimen. Transplantation-related complications and clinical outcome were retrospectively analyzed.
Results:
After conditioning, no hepatic veno-occlusive disease (VOD) and grade Ⅲ hemorrhagic cystitis occurred. 76.2% (16/21) patients had fever with 4 septicemia. One patient died of septic shock before engraftment. Twenty patients achieved neutrophil engraftment with a median time of 13 days (range, 10 to 21 days). Seventeen patients achieved platelet engraftment with a median time of 18 days (range, 9 to 25 days). The cumulative incidence of acute graft-versus-host disease (aGVHD) was 39.5%, and 3 patients developed grade Ⅲ-Ⅳ aGVHD. Of 19 patients who survived more than 100 days after transplantation, 4 had local chronic graft-versus-host disease (cGVHD). Of 21 patients, the median survival time was 15 months (range, 0.5 to 67 months) post-transplantation. Transplantation-related mortality rate was 28.7%. Leukemia relapse occurred in 4 patients with a median time of 4 months (range, 3 to 8 months) after transplantation. The cumulative relapse rate at 1 year was 21.4%. The 1-year and 3-year overall survival (OS) rates were 60.7% and 54.9% respectively. Log-rank analysis revealed that bone marrow blasts ≥ 20% or extramedullary leukemia before transplantation, poor platelet engraftment and grade Ⅲ-Ⅳ aGVHD were significantly related to shortened OS (
10.Effects of Long-term Aerosol Inhalation of 4 Kinds of Non-aerosol Drugs to Lung Tissue of Healthy SD Rats

OBJECTIVE: To study lung tissue injury induced by long-term aerosol inhalation of 4 kinds of non-aerosol drugs in healthy SD rats, and to evaluate the safety of aerosol inhalation of non-aerosol drugs. METHODS: Totally 40 healthy SD rats (♂) were randomly divided into 8 group, i.e. blank control group, normal saline group (solvent control), budesonide group (non-aerosol drug control, 0.1 g/L) ,silicon dioxide group (lung injury drug control, 40 g/L)and 4 kinds of non-aerosol drugs [Dingchuan decoction group (15 g/mL, calculated by crude drug), cefatriaxone group (200 g/L), Qingkailing group (stoste) and Tangreqing group (stoste)], with 5 rats in each group. Except that blank control group didn’t received any treatment, other groups received aerosol inhalation, 10 mL, twice a day, for consecutive 56 days. After medication, the number of white blood cells in peripheral blood were counted and classified, and the number of white blood cells in bronchus alveolar lavage fluid were counted. The pathological changes of lung tissue were observed by HE staining and the number of dust cells was counted. The expression of leukocyte differentiation antigen 163 (CD163) in lung tissue were determined by immunohistochemical method. RESULTS: The white blood cells in peripheral blood mainly included lymphocyte and neutrophil, of which lymphocyte is the main one. Compared with blank control group, there was no statistical significance in the number of white blood cells, lymphocyte or neutrophil in peripheral blood, the number of white blood cells in alveolar lavage fluid or the number of dust cells in lung tissue of rats in normal saline group (P>0.05); the structures of bronchus and lung tissue were intact, and the expression of CD163 was negative. Compared with normal saline group, there was no statistical significance in the above indexed of rats in budesonide group(P>0.05), the structures of bronchus and lung tissue were intact, and the expression of CD163 was negative, while the number of white blood cells, lymphocyte or neutrophil in peripheral blood, the number of white blood cells in alveolar lavage fluid or the number of dust cells in lung tissue of rats in other 5 groups were all increased significantly (P<0.05); alveolar wall thickening and alveolar interstitial edema occurred in different degrees in lung tissue. The expression of CD163 was positive or strongly positive. CONCLUSIONS: The long-term aerosol inhalation of 4 kinds of non-aerosol drugs can induce pathological changes of lung tissue and increase the number of inflammatory cell and dust cell in alveolin in healthy SD rats.
