1.Fat necrosis of beast :a report of 50 cases

Objective To investigate the etiology, pathology, diagnosis and management of patients with fat necrosis of breast (FNB). Method The clinical data of 50 cases of FNB confirmed pathologically in our hospital from 1966 to 2001 were analyzed retrospectively. Results All the patients in this series were females with the average age of 32.5 years and 56% of the patients over 30 years. The main clinical feature was breast lump and some of the patients had a trauma history. 48 patients were treated surgically and other 2 got an accurate diagnosis by biopsy and were treated by non operative therapy. Conclusions FNB is easily misdiagnosed and difficult to be differentiated from breast cancer, thus the surgery or needle biopsy is recommended.
2.Clinical analysis of Hashimoto's disease complicated with thyroid adenomas in 107 patients

Objective:To summarize diagnosis and treatment of hashimoto's disease(HD) complicated with thyroid adenomas(TA) in 107 patients.Methods:Clinical data of 107 patients of HD complicated with TA were analysed retrospectively.Results:Complete serum immunologic examinations,B ultrasonography and fine needle aspiration biopsy were helpful for the diagnosis of HD with TA.Thirty-eight patients(35.5%) were diagnosed before operation.Misdiagnosis rate was 64.5%(69/107).Intraoperative frozen section diagnosis rate was 81.3%(87/107).Partial,large and total resection of thyroid gland can be selected according to frozen section and local disease focus.Afteroperation,87patients(81.3%)hada goodrecovery.Threepatients with total thyroid gland resection need thyroxin tablet.Thyroid hypofunction did not happented in 104 patients.Conclusions:HD with TA should be treated surgically.Intraoperation frozen section is helpful for selecting operationmethods.
3.The treatment and diagnosis of smooth muscle tumors of the duodenum

Objective To investigate the diagnostic and theraputic problems in patients with smooth muscle tumors of the duodenum.Method The clinical data from 7 cases smooth muscle tumors were analyzed retrospectively which were treated surgically and confirmed pathologically in our hospital from 1987 to 2001.Results Of all the cases,4 had a leiomyoma in the duodenum and 3 leiomysarcoma.This disease is lack of pathognomonic sign and difficult for early diagnosis.All the cases were treated surgically and got good results.Conclusions Upper gastrointestinal bleeding is common in patients with this disease.Upper gastrointestinal tract examination and gastrduodenoscopy and CT scan are the main methods for diagnosis.Operation is the best choice for treatment.
4.Cryotherapy combining with ethanol injection therapy in the treatment of irresec table hepatic cancer

Purpose:To observe the effect and safety of c om bination therapy with Percutaneous Local Cryotherapy(PLCT)and Percutaneous Etha nol Injection Therapy( PEIT)in patients with irresectable hepatic cancer Methods:43 patients with irresectable hepatic carcinoma were enrolled 29 tumors of 14 patients were treated by PLCT,23 tumors of 16 patients were treated by PEIT,and 23 tumors of 13 patients were treated by combination therapy Results :The tumor size decreased by more than 50% 1 month later in 51 7%(15/29 )of tumors treated by PLCT alone, in 43 5%(10/23)of tumors treated by PEIT alone, 78 3%(18/23)tumors treated by combination therapy The 1-year surv ival rates were 64 3%(9/14) in PLCT group?43 8%(7/16) in PEIT group?8 4 6%(11/13)in combination therapy group The blood levels of AFP decreased by various extents Conclusions:Combination therapy with PLCT a nd PEIT is more effective than PLCT or PEIT alone
5.Problems of operations for huge goitres

Objective To sum up the problems should be taking care in operations for huge goitres. Methods Retrospectively analyzed the clinical data of 295 cases of huge goitres was made. Results All of the 295 cases had some degree of compress, replacement and bend of trachea by the goitre comfirmed by X ray examination .Among the 295 cases,162(54.9%) had trachea constriction.51(17.3%) had obvious dyspnea. Retrosternal goitres was found in 21 cases.12 complicated with secondary hyperthyroidism. All the 295 cases underwent subtotal or total thyroidectomy. Postoperative pathological diagnosis: nodular goitres was diagnosed in all the 259 cases (100%),including 11 cases(3.7%) with malignancy, 12 cases( 4.1% ) with secondary hyperthyroidism,and 39(15.1%) with adenomas. Conclusions Operation of huge goitres should choice appropriate anaesthetic method.For the main vessels of thyroid, very high position of the upper pole of thyroid, retrosternal goitres, malignancy of goitres etc,appropriate operative measures shoudd be adopted to prevent introoperative massive bleeding and damage of recurrent laryngeal nerve. If the indications of tracheostomy presents,a tracheostomy must be done.
6.Diagnosis and surgical treatment of tuberculosis in ileocecal junction in 74 cases

Objective:To summarize experience of diagnosis and surgical treatment of tuberculosis in ileocecal junction.Methods:Clinical data of 74 cases of tuberculosis in ileocecal junction were analysed retrospectively.Results:Clinical manifestations consisted of chronic abdominal pain(87.8%),right lower abdominal mass(95.9%),low fever、emaciation and anemia(36.5%),alternation of constipation and diarrhea(39.2%).Diagnosis rate of barium enema was 63.8%,colonoscopy and biopsy was 100%.Diagnosis rate before operation was 68.9%(51/74).In 74 cases,53 cases performed right hemicolectomy,15 cases performed anastomosis of end of ileum and transverse colon,6 cases performed building fistula of end of ileum.Conclusion:Tuberculosis in ileocecal junction should be diagnosed early and be identified with cancer of ileoceum.Good prognosis depend on positive surgical treatment and systematic medicine treatment.
9.Value of Dynamic Spiral CT in Prostate Cancer and Benign Prostate Hyperplasia

Objective To detect the value of dynamic spiral CT on the prostatic cancer and benign prostate hyperplasia(BPH).Methods Plain and dynamic spiral CT scans were obtained in 88 male patients with various ages due to prostate or other pelvis diseases.Out of these detailed data in 5 cases of prostate cancer and 11 cases of BPH which were confirmed by operation and pathology were available.Contrast enhancement pattern and degree were correlated with the pathological and operational findings.Results Prostate was enlarged in all cases except one prostate cancer and 3 cases of BPH within normal limits in size.The contour of the prostate was irregular in all cancer cases and one BPH.On dynamic CT scans,prostatic malignant lesions appeared iso-hypodensity with the rest of the prostate gland at the arterial stage and low density at the veinous stage.Diffuse heterogeneous central enhancement was found in 10 cases of BPH at the arterial stage,especially in the lateral lobes and further enhancement was found in 10 cases of BPH at the arterial stage,especially in the lateral lobes and further enhancement was found at the middle stage.There was little enhancement in the peripheral part at the arterial and veinous stages in BPH.The ratio of the central and the peripheral decreased.One case of BPH within normal size had spotty enhancement in the central part.On delayed CT scans 8 cases of BPH showed homogeneous enhancement in the total prostate gland.Conclusion Prostate cancer and BPH have their own characteristic enhancement patterns and degress,therefore dynamic spiral CT scan could help make differentiation between the two disease.
10.Study on the Effect of the Intervention on Depression and Negative Coping in Elderly Patients with Chronic Disease

Objective To explore the effect of intervention on depression and negative coping in elderly patients with chronic disease and provide a basis for mental health services. Methods Forty depression and negative coping of elderly patients with chronic disease were took psychological intervention for one year. Geriatric depression scale (GDS) and ways of coping scale (WCS) were used to assess the intervention effect. Results There was a high scores of GDS before the study (20.93?1.90)and the scores were reduced after intervention in Six months and the end of one year (17.88?1.80,11.80?1.60,respectively,P0.05). After intervention for one year,the patients applied more with positive coping styles such as problem sloving,help seeking and less with wince,illusion and self-criminating,there was a statistically significant difference (P
