1.Comparison of the application of retroperitoneal laparoscopy and open surgery in ureteropelvic junction obstruction

Objective To review and analyze the application of retroperitoneal laparoscopy and open surgery in ureteropelvic junction obstruction, sum up clinical expeirence, and improve the skills of retropeirtoneal laparoscopic surgery. Methods Retroperitoneal laparoscopic surgery was carired out in 22 csaes of ureteropelvic junction obstruction(retroperitoneal laparoscopy group). Open surgery was carried out in 21 cases(Open surgery group). The two groups were compared in operation time, blood loss postoperative discharge time and postoperative hospital stay respectively. Results Ureteropelvic junction obstruction group was significantly less than open surgery group in intraoperative bleeding time,postoperative bleeding time and average length of stay. Operation time in ureteropelvic junction obstruction group was slightly longer than that in open surgery group [(32.0 ± 10.0) ml vs (70.0 ± 11.9) ml, (31.0 ±5.7)h vs(52.0 ±6.9)h,(7.0 ± 1.6) d vs(14.0 ± 1.4) d, (113.0 ±7.6) min vs(72.0 ± 6.8) min] (t = 7.06,8. 14,11.37,11.36, all P < 0.05). Conclusions Retroperitoneal laparoscopic technology was in the treatment of ureteropelvic junction obstruction for its direct way, had non interference and pollution to laparoscopic internal organs,less damage,less pain, rapid recovery and short hospitla stay. With on going training and constantly updating technical skills and equipments, retroperitoneal laparoscopic technology will shotren the operation time, and will substitute open surgery.
2.Correlation between the severity of subcortical ischemic lesions and vascular cognitive impairment in encephalorrhagia patients

Objective To investigate the correlation between the severity of subcortical ischemic lesions and vascular cognitive impairment in encephalorrhagia patients. Methods One hundred and twenty-four encephalorrhagia patients were enrolled into two groups, the normal cognition group (60 cases, without cognitive impairment) and the cognitive impairment group (64 cases, with vascular cognitive impairment). Parameters of the lesion diameters, various lesion locations, white matter lesions severity and NIHSS scale score, modified Bathel index score were compared between these two groups. The correlations between the above indexes and the vascular cognitive impairment degree were analyzed by Logistic regression proceduress. Results Compared to the normal cognition group, the indexes of lesion number of frontal area and basal ganglia, lesion diameters, white-matter severity and NIHSS scale score of cognitive impairment group were higher (P<0.05, respectively), and the modified Bathel index score was lower (P<0.05), but the lesion number in temporal, parieto-occipital and infratentorial area were not changed (P>0.05, respectively) in the cognitive impairment group. Result of Logistic regression analysis showed a positive correlation between the severity of white-matter and the degree of vascular cognitive impairment (OR =4.705, P=0.009). Conclusion The severity of white-matter can effectively assess the degree of vascular cognitive impairment of encephalorrhagia patients.
4.Treatment of a Case of Malignant Lymphoma by Autologous Peripheral Blood Stem Cell Transplantation

A success in reconstruction of the hematopoietic function of marrow in a case of malignant lymphoma attained by autologous peripheral blood stem cell transplantation after the ultra-high dose chemoradiotherapy was reported. The platelet count of patient was increased to over 20 ? 109/L at the eighth day after PBSC transplantation and was recovered to over 50?109/L at the tenth day after PBSC transplantation. The white blood cell count was increased to over 0. 5?109/ L at the ninth day after PBSC transplantation. The pelvic CT-graphy showed reduction in size of the tumor. And then the patient was outhospitalized due to remission,having been surviving for more than 6 months with routine follow-up. It was suggested that the autologous peripheral blood stem cell transplantation could be a safe and effective suplement therapy for radical cure of malignant tumor.
5.Effect of medroxyprogesterone acetate on proliferation and apoptosis of human colorectal cancer cells

Objective:To investigate the effects of medroxyprogesterone acetate(MPA) on the proliferation and apoptosis of human colorectal cancer cells.Methods: Human colorectal cancer cell line LS174T was treated with different concentrations of MPA(12.5,25,50 and 100 ?mol/L) for 120 h.The inhibitory effect of MPA was measured with MTT method;the cell cycle,cell apoptosis,and cell surface Fas expression were analyzed with flow cytometry.Results: MPA had significant inhibitory effect on the growth of LS174T human colon cancer cells.Cell growth was found to be arrested in G_(1) phase and cells in S phase and G_(2)/M phase were decreased.The natural apoptosis rate and the apoptosis rates at MPA 12.5 and 25 ?mol/L were 4.8%,15.1%and 25.6%,respectively.Cell surface Fas expression in cancer cells increased after treated with MPA.Conclusion: MPA has significant inhibitory effect on the growth of human colon cancer cells,which may be related to Fas expression upregulation on cancer cell surface and G_(1) phase arrest.

7.Current Development of Breast-Specific Gamma Imaging (BSGI) Technique.

Breast-Specific Gamma Imaging (BSGI) is an improved and optimizing nuclear medicine breast imaging technique on the basis of traditional gamma camera. It uses a high resolution, small field-of-view scintilla detector. The detector is designed with 3 073 individual detector crystals and 48 position-sensitive photomultiplier tubes. The FOV of detector is 15 cm x 20 cm, and optimal system resolution for breast imaging is 3 mm, can detect the diameter of only 2-3 mm small lesions. BSGI has better sensitivity in detecting subcentimetre or nonpalpable breast cancer. The sensitivity for the diagnosis of breast cancer is high, not influenced by the density of the breast tissue, implants, architectural distortion-or scars from prior surgery or radiation. So it is called a high resolution, small field-of-view breast-specific gamma camera.

8.Molecular cloning and expression of a stress-inducible gene highly homologous to human structural maintenance of chromosome 6

BACKGROUND: Previous studies have demonstrated that the responses of myocardium to short periods of ischemia/reperfusion are associated with changes in the expressions of a variety of genes. However, the characteristics of these changes are unknown.OBJECTIVE: To clone and analyze a stress-inducible gene characteristics in order to study the molecular mechanism of myocardium to short periods of ischemia/reperfusion damage.DESIGN: A controlled animal study.SETTING: Department of Hematology, Xiangya Hospital of Central South University.MATERIALS: Sixteen castrated male German Landrace-type domestic porcines, weighing between 21 and 39 kg, were provided by the laboratory of Department of Experimental of Cardiology at Bad Nauheim in Germany. The porcines were randomly divided into two groups ischemia/reperfusion group (n =14) and control group (n =2). The porcines in the ischemia/reperfusion group were observed at 0, 30 and 90 minutes of reperfusion after reocclusion, and ischemia and non-ischemia myocardial tissues were harvested from each pocrine.METHODS: The experiments were performed in the laboratory of Department of Experimental Cardiology of Max-Planck Institute for Physiological and Clinical Research at Bad Nauheim in Germany and the Department of Hematology, Xiangya Hospital of Central South University from July in 1998 to May in 2007. All the animals were anesthetized and thoracotomized. Following 30 minutes of stabilization, the left anterior descending coronary artery (LAD) was occluded for 10 minutes followed by 30-minute reperfusion and then another 10-minute reocclusion. The porcines were killed immediately at corresponding time points. The sham-operated animals were killed without occlusion. Experimental tissue was removed from the LAD area and control tissue from the region of the left circumflex coronary artery area. Firstly, a porcine heart cDNA library was screened, DNA and deduced amino acid sequences were then analyzed. Meanwhile, short periods of myocardium ischemia/reperfusion was performed by occluding porcine LAD followed by reperfusion as mentioned above, and total RNAs isolated from myocardium or a variety of other tissues were used for Northern blotting. Quantitation of mRNA levels was accomplished by using a PhosphorImager and Image Quant software, the ration of mRNA/18S rRNA was suggested as the level of gene expression.MAIN OUTCOME MEASURES: ① DNA and amino acid sequences analysis of the cloned gene; ② Analysis of the cloned gene expression.RESULTS: All the 16 porcines were involved in the final analysis of results. After screening, a cDNA fragment with 3 461 base pairs was obtained. DNA sequencing and searching revealed that this cDNA shared 86% identity to human structural maintenance of chromosome 6 gene (hSMC6), and 84% identity to Mus musculus SMC6 (mSMC6). Furthermore, a largest polypeptide deduced from this cDNA contained 1007 amino acid residues, and protein homology searching indicated that the predicted polypeptide carried 92% and 89% identity to hSMC6 or mSMC6 protein respectively. Thus, the cloned cDNA was referred to as a porcine homology gene to hSMC6 (pSMC6). The results of Northern blotting showed that the expression of the pSMC6 mRNA in ischemia/reperfusion myocardium was increased, and its mRNA existed in all organs tested. CONCLUSION: A stress-inducible gene of pSMC6 from porcine myocardium has been cloned, which is high homology to hSMC6 gene, and the pSMC6 or along with other molecules is possible involved in the early repair of myocardium to DNA damage caused by ischemia/reperfusion in the porcine heart.
9.The correlation between ultrasound detected fetal heart abnormality and chromosomal abnormality

ObjectiveTo evaluate the correlation between ultrasound detected fetal heart abnormality and chromosomal abnormality.Methods The data of 3307 cases of prenatal fetal echocardiography and karyotype analysis results were retrospectively analyzed.The correlation between heart abnormality and chromsomal abnormality was analyzed.ResultsIn 3307 cases d pregnant women,194cases (5.87%) were found signs of fetal heart abnormality,and 3113 cases (94.13%) were not found any sign of fetal heart abnormality.And 124 cases(3.75%) were found chromosomal abnormality.The rate of chromosomal abnormality in fetus with heart abnormality was higher than that in fetus without heart abnormality[ 19.59% (38/194) vs.2.76% ( 86/3113 ),P < 0.01 ],the relative risk ratio was 7.0903.One hundred and twenty-four cases of chromosomal abnormality in the 18-three-body(42 cases) and 21-three-body(36 cases) were the most common.ConclusionsFetal heart abnormality incidence of chromosomal abnormality is high relatively.When the signs of heart abnormality are found,it is suggested that the fetus should examine karyotype analysis.
10.Comparison laparoscopic radical hysterectomy with open surgery for early stage cervical cancer in elderly patients

Objective To compare the efficacy of laparoscopic radical hysterectomy and open surgery with early stage cervical cancer in elderly patients.Methods Performed a retrospective study of 195 cases underwent surgical treatment of early stage cervical cancer in elderly patients.According to different approach,the patients were divided into laparoscopic group (66 cases) and open group (129 cases).The operation time,intraoperative blood loss,number of resected lymph nodes,bowel function recovery time,postoperative hospital stay and postoperative complications was compared between two groups.Results The operation time,intraoperative blood loss,bowel function recovery time,postoperative hospital stay in laparoscopic group were significantly shorter than those in open group [(237.6 ±46.9) min vs.(259.3 ± 76.7) min,(214.7 ± 104.8) ml vs.(283.4 ± 137.2) ml,(1.9 ± 1.2) d vs.(2.8 ± 1.5) d,(4.1 ± 3.5) d vs.(6.7 ± 1.4) d,P < 0.05 or < 0.01].There was no significant difference in number of resected lymph nodes and postoperative complications between laparoscopic group and open group (P > 0.05).Conclusion Laparoscopic radical hysterectomy is both safe and effective in elderly patients with early stage cervical cancer.
