1.Loss of expression of succinate dehydrogenase subunit A and B in renal cell carcinoma

Objective To investigate the expression of succinate dehydrogenase subunit A(SDHA)and subunit B (SDHB) in patients with renal cell carcinoma (RCC),and its relationship with the clinicopathologic characteristics. Methods The expression of SDHA and SDHB was detected in 179 cases of RCC by immunohisto-chemistry and the relationship between the SDHA ,SDHB expression and the clinicopathologic characteristics of RCC were analyzed. Results In 179 cases of RCC,18 cases(10.1%)were shown with negative or suspicious-negative expression of SDHA and SDHB,including 2 cases(1.1%)with suspicious-negative expression of SDHA and SDHB. 12 cases(66.7%)had tumor located in left kidney and 6 cases(33.3%)had tumor located in right kidney. The well-differentiation group contained 15 cases(83.3%),moderate-differentiation group contained 1 case (5.6%)and poor-differentiation group contained 2 cases(11.1%). Most of SDH negative and suspicious negative RCC had the following characteristics that the normal renal tubule could be seen within the tumor. The tumor was composed of polygonal cells arranged in nest ,and the polygonal cells were separated by a fine fibrovascular stroma. The nuclei were shown around with vesicular chromatin. The eosinophilic flocculet material ,vacuole and eosino-philic inclusion body could be seen in the cytoplasm of tumor cells. Conclusion The RCC cases with negative expression of SDHA and SDHB protein have unique histological features ,with significantly higher incidence in the left kidney than that in the right kidney.
2. Comparison on clinical and CT features of adenoid cystic carcinoma and mucoepidermoid carcinoma in central airway

Objective: To compare CT features and clinical manifestations of adenoid cystic carcinoma (ACC) and mucoepidermoid carcinoma (MEC) in the central airway. Methods: A total of 76 patients with central airway salivary gland tumor proved by surgery or biopsy (45 patients with ACC and 31 patients with MEC) were analyzed retrospectively. All patients underwent chest CT, among them 38 patients with ACC and 29 patients with MEC underwent contrast enhanced CT. CT features of the tumor, including growth location, maximum diameter, growth pattern, enhancement, obstructive pneumonia, atelectasis, lymphadenopathy and pleural effusion were evaluated. Results: Patients with MEC were younger than those with ACC (P<0.01). There was no significant difference in incidence rate of cough, expectoration, fever, hemoptysis, difficulty breathing nor chest pain between two groups (all P >0.05). Compared with MEC, ACC was more likely to occur in trachea, behaved as infiltrative growth, less enhancement and lower incidence of obstructive inflammation(all P<0.05). However, there was no significant difference in tumor maximum diameter and the incidence of obstructive atelectasis, lymph node enlargement nor pleural effusion between MEC and ACC patients (all P>0.05). Conclusion: The age of onset of central airway salivary gland tumor is younger. ACC is more likely to occur in the trachea, most of them show invasive growth, while enhancement CT shows mild to moderate enhancement. MEC is more likely to occur in lobar and segmental bronchi, most of them show expansive growth and significantly enhancement on CT. The incidence of obstructive inflammation is higher in MEC patients than in ACC patients.
3.Effect of Intraaortic Balloon Pump Used During Emergency Percutaneous Coronary Intervention

Objective To observe the effect of intraaortic balloon pump (IABP) on tolerance of percutaneous coronary intervention (PCI), life signs of patients with acute myocardial infarction (AMI) during emergency PCI performed.Methods46 AMI patients complicated with or without cardiogenic shock were put on IABP at same time of coronary angiography when emergency PCI performed, and tolerance of PCI, blood pressure, heart rate and immediate death rate of patients were observed before and after operation.Results45 cases had IABP successful and 1 case failed. 40 patients (86.96%) could endure whole emergency PCI process, the total death rate was 10.87%. 66.67% of patients with obvious cardiogenic shock could endure emergency PCI, the death rate was 33.33%; 94.12% of patients without cardiogenic shock could endure PCI treatment and the death rate was 2.94%. 35 cases completed PCI treatment and the successful rate was 85.36%.ConclusionPCI supported by IABP can prevent AMI patients from cardiogenic shock, ease symptoms of patients complicated with cardiogenic shock, improve the tolerance and successful rate of the PCI treatment, and decrease immediate death rate.
4.Interventional therapy on below-knee arteries in patients with diabetic foot ulcers

The arterial lesions of lower extremities in patients with diabetic foot ulcers were extensive.The majority of below-knee arteries showed severe stenosis, even complete occlusion.Interventional therapy of femoral artery was effective in improving arterial blood perfusion and remission of the pain and numbness.After therapy, the skin temperature and ankle brachial index (ABI) were increased, healing of ulcer was enhanced and intermittent claudication was ameliorated.Percutaneous transluminal angioplasty of femoral artery was effective in improving arterial blood perfusion of the lower extremities with foreseen curing efficacy.
5.Long-term follow-up of 53 cases of combined pancreas-kidney transplantation

Objective To determine the long-term results after combined pancreas-kidney transplantation at a single-center institution.Methods Fifty-three consecutive patients with insulin-dependent diabetes mellitus and end-stage nephropathy were followed up for more than three years after combined pancreas-kidney transplantation. Immunosuppressive protocol consisted of tacrolimus ( TAC ),mycophenolate mofetil (MMF),and steroids,and antithymocyte globulin or anti-CD25 receptor mAb.The impact of different risk factors was analyzed on long term patient and graft survival.Results The 3-,5- and 8-year survival rate in recipients was 90.1%,89.1 % and 80.0%,respectively.The 3-,5- and 8-year survival rate of pancreas grafts was 84.9%,84.8% and 60.0%,and that of kidney grafts was 83.0%,82.6% and 53.3%,respectively.Principal causes of death were Infection (n =4),renal failure (n =2),cardiovascular events (n =1 ),and cerebrovascular accident (n =1 ).Graft failure for the pancreas was caused by death with a functioning graft (n =6),rejection (n =2),thrombosis (n =1 ) and pancreatitis (n =1 ).Graft failure for the kidney was due to rejection (n =9),and death with a functioning graft (n =9).Conclusion This series representing the largest experience with long-term follow up in China confirms an excellent long-term survival.Infection,rejection and surgical complication were the major risk factors leading to deaths and graft loss.
6.Immunohistochemistry research of interface membrane around loosening hip prosthesis

Objective Collecting the loosening periprosthetic interface-membrane, to discuss the mechanism of hip arthroplasty loosening. Methods The periprosthetic interface tissues of 29 hip arthroplasty revision cases from February 1995 to December 2003 were collected. The retrieved periprosthetic interface tissues were detected by immunohistochemistry. Some of them were studied by electronic microscope. Results (1)Transmission electronic microscope examination: the mitochondria swell. There were some substantia like lipid in the plasm of macrophages. Wear particles could be seen under scaning electronic microscope.(2)Immunohistochemistry: there were 22 IL-1? positive cases in cells of interface membrane. There were 29 IL-6 positive cases in cells of interface membrane. There were no positive results in TNF-? test. Conclusion (1)The wear particles of arthroplasty are important factors which cause biological reaction.(2)The interface membranes contain cytokine IL-1? and IL-6, which may play an important role in periprosthetic osteolysis and arthroplasty loosening.
8.Efficacy of personalized family doctor contract services on the risk factors of atherosclerotic cardiovascular disease in high-risk people

Objective:To investigate the efficacy of personalized family doctor contract services on risk factors of atherosclerotic cardiovascular disease (ASCVD) in high-risk population.Methods:Ten matched-community health centers of Shenzhen Luohu district were divided into intervention group and control group by cluster randomiztion. Subjects with high risks of ASCVD were screened out as intervention group from contracted residents who visited these centers and had complete data of the China-PAR model from August 2018 to April 2019. The control group received conventional general family doctor contract services. The individualized management were given to the intervention group after fully understanding patients′ ideas, concerns, and expectations (ICE). After 2-year intervention, score changes of ASCVD risk factors within and between groups were compared.Results:A total of 571 patients were enrolled, including 288 in the intervention group and 283 in the control group. After 2 years of intervention, 7 and 18 were lost to follow-up in two groups, respectively. Finally, 281 in the intervention group and 265 in the control group were included in the study. At baseline, there was no significant difference in ASCVD scores between the intervention group and the control group [(13.33±3.54) vs. (13.09±3.54) points; t=0.84, P=0.403], and the scores in both groups decreased significantly after the intervention [(10.89±4.01), (11.62±4.11) points], while the intervention group decreased more significantly (both P<0.05). Among the risk factors at baseline, HDL-C and diastolic blood pressure in the intervention group were lower than those in the control group, and there were no significant differences in other factors between the two groups. After the intervention, the levels of total cholesterol, systolic blood pressure and diastolic blood pressure in the two groups decreased significantly, and the number of people taking antihypertensive drugs increased significantly ( P<0.001 and P<0.05); HDL-C decreased in the control group ( P=0.023). After the intervention, compared to control group the intervention group had a higher proportion of patients taking antihypertensive drugs, with lower systolic and diastolic blood pressure ( P<0.05). After the intervention, the increase rate of HDL-C in the intervention group was more than that in the control group, and the decrease rate was less than that in the control group (χ 2=6.65, P=0.036). Conclusion:Family doctor contract services can reduce the risk factors of ASCVD, and personalized family doctor contract services can further improve the effects in the prevention and control of ASCVD. However, the effects might be insignificant and inconsistent for the ASCVD risk factors with deeper management requirements or no specific management measures, which highlights the complexity and diversity of ASCVD prevention and control, calling for multi-level and multi-faceted thinking and exploration.
9.Simultaneous determination of major coumarins′ content in processing Peucedani Radix materials by method of substitute for reference substance

(±)-Praeruptorin A(PA)was used as the standard reference substance to establish a new method of substitute for reference substance. The contents of both(±)-praeruptorin B(PB)and(+)-praeruptorin E(PE)were calculated by the method. The major content of coumarins in different processing materials could be gained by determining the content of(±)-praeruptorin A, which could provide scientific evidence for the research on pharmacology discrepancy and quality standards. The method was carried out on a Welch Materials Ultimate XB-C18 column(4. 6 mm×250 mm, 5 μm), with a gradient mobile phase of methanol and water at the flow rate of 1. 0 mL/min. The column temperature was 30 °C and detection wavelength was set at 322 nm. According to this method, the relative correlation factors(f)of(±)-praeruptorin B and(+)-praeruptorin E were determined as 0. 787 2 and 0. 969 0, respectively. After determine the contents of 15 batches of materials from different sources, no significant differences between substitute method and external standard method were observed. It was a feasible and credible method to determine the content of major coumarins in Peucedani Radix with different processing materials. The results showed lower contents of coumarins in processing materials than in raw materials.
10.Comparative Study of Two Common In Vitro Models for the Pancreatic Islet with MIN6

BACKGROUND:
Islet transplantation is currently considered the most promising method for treating insulin-dependent diabetes. The two most-studied artificial islets are alginate-encapsulated b cells or b cell spheroids. As three-dimensional (3D) models, both artificial islets have better insulin secretory functions and transplantation efficiencies than cells in twodimensional (2D) monolayer culture. However, the effects of these two methods have not been compared yet. Therefore, in this study, cells from the mouse islet b cell line Min6 were constructed as scaffold-free spheroids or alginate-encapsulated dispersed cells.
METHODS:
MIN6 cell spheroids were prepared by using Agarose-base microwell arrays. The insulin secretion level was determined by mouse insulin ELISA kit, and the gene and protein expression status of the MIN6 were performed by Quantitative polymerase chain reaction and immunoblot, respectively.
RESULTS:
Both 3D cultures effectively promoted the proliferation and glucose-stimulated insulin release (GSIS) of MIN6 cells compared to 2D adherent cells. Furthermore, 1% alginate-encapsulated MIN6 cells demonstrated more significant effects than the spheroids. In general, three pancreatic genes were expressed at higher levels in response to the 3D culture than to the 2D culture, and pancreatic/duodenal homeobox-1 (PDX1) expression was higher in the cells encapsulated in 1% alginate than that in the spheroids. A western blot analysis showed that 1% alginate-encapsulated MIN6 cells activated the phosphoinositide 3-kinase (PI3K)/serine/threonine protein kinase (AKT)/forkhead transcription factor FKHR (FoxO1) pathway more than the spheroids, 0.5% alginate-, or 2% alginate-encapsulated cells did. The 3D MIN6 culture, therefore, showed improved effects compared to the 2D culture, and the 1% alginate-encapsulated MIN6 cells exhibited better effects than the spheroids. The upregulation of PDX1 expression through the activation of the PI3K/AKT/FoxO1 pathway may mediate the improved cell proliferation and GSIS in 1% alginate-encapsulated MIN6 cells.
CONCLUSION
This study may contribute to the construction of in vitro culture systems for pancreatic islets to meet clinical requirements.
