1.History and prospect of bariatric surgery

With the coming of obesity epidemic and the development of mini-invasive bariatric surgery,we have entered an era where bariatric surgery growing in an exponential way and becoming an independent surgical division.Bariatric surgery is not only the single effective treatment for morbid obese patients (BMI > 35 kg/m2),but also transforming into metabolic surgery for the treatment of not well controlled type 2 diabetes in obese patients with BMI < 35 kg/m2.The type of bariatric procedure had changed a lot in the past decades.Current bariatric surgery is not only very safe but also very effective.Many new metabolic surgeries and procedures have also been developed and a great break-through in diabetes treatment is expected in the future.Bariatric/Metabolic surgical division is a consequence of the rapid growing bariatric surgeries.An independent Bariatric/Metabolic surgical division may focus on a high quality bariatric surgical service,provide the education and training of bariatric surgeons,and help in research and development of this field.As obesity and associated diabete is becoming the most important health issue in the modern world,a continuing development of Bariatric/Metabolic surgery is expecting.
2.Determination of Mannityl Nicotinate and Related Substances by HPLC

Objective: To establish an HPLC method for the determination of mannityl nicotinate and its related substances. Methods:The HPLC method was performed on an Agilent Extend-C18 column (250 mm × 4. 6 mm, 5 μm) with the mobile phase of 0. 01 mol·L-1 ammonium acetate solution-methanol(40∶60). The detection wavelength was 264nm. The column temperature was 30℃. The flow rate was 1. 0 ml·min-1 and the injection volume was 10 μl. Results: The linear range of mannityl nicotinate was 53. 6-1 072. 8 μg·ml-1(r=1. 000 0), and the mean recovery was 98. 9%(RSD=0. 4%,n=9). Mannityl nixotinate and its related substances could be well separated. The limit of quantitation was 0. 06 μg·ml-1 and the limit of detection was 0. 02 μg·ml-1 . Con-clusion:The method is specific, accurate and rapid. It is suitable for the quality control of mannityl nicotinate.

5.Clinical studies of transurethral bipolar plasma cutting and transurethral resection for benign prostatic ;hyperplasia

Objective To investigate clinical value of transurethral bipolar plasma cutting and transurethral resection for benign prostatic hyperplasia,to provide a reference for clinical treatment.Methods 100 cases with benign prostatic hyperplasia were selected,according to surgical treatment,they were divided into control group and observation group.50 patients in the control group received TURP surgical treatment,50 patients in the observation group were given TUPKP surgical treatment.The patients were followed up for 3 months,the indwelling catheter time, weight and other clinical indicators of glandular tissue were compared,serum sodium and hemoglobin levels were observed,the quality of life index,international Prostate Symptom Score,complications and other indicators were compared. Results In the observation group,operative time was (60.54 ±8.37)min,blood loss was (108.43 ±21.27)mL, hospital stay was (7.09 ±2.25)d,which were shorter than those of the control group (70.24 ±12.22)min,(188.76 ± 21.36)mL,(8.63 ±2.76)d,the differences were statistically significant (t =5.441,P =0.012;t =8.326,P =0.001;t =6.216,P =0.006).In the observation group after three months,maximum flow rate was (18.95 ±4.11)mL/s, residual urine volume was (28.74 ±4.55)mL,IPSS was (5.12 ±1.27)points,QOL was (1.43 ±0.24)scores, which were better than those of the control group (14.24 ±3.77)mL/s,(20.85 ±5.11)mL,IPSS(6.95 ±1.44)points, QOL(2.03 ±0.32)points,the differences were statistically significant (t =8.763,P =0.000;t =7.616,P =0.003;t =5.472,P =0.011;t =8.044,P =0.002).The incidence rate of complication in the observation group was 10.00%, which was significantly lower than 22.00% in the control group,the difference was statistically significant (χ2 =5.437,P =0.012).Conclusion TUPKP treatment for benign prostatic hyperplasia has significant clinical effect, which can effectively improve the clinical symptoms and quality of life with good safety,it is worthy of clinical application.
6.Clinical analysis of cutting balloon coronary angioplasty

Objective To evaluate the safety,efficiency and short-term results of cutting balloon coronary angioplasty.Method This procedure was performed on 63 lesions in 52 patients (male 94%,age 57?9 years).Results The overall procedural success rate for the cutting balloon angioplasty without predilation were 92.1%.Seven out of 9 cases of total occluded lesions had satisfactory results.Two cases of in-stent restenosis were also treated with CB successfully.Forty-three lesions were treated with cutting balloon followed by stents insertion.In 20 lesions treated with the cutting balloon alone,there was a marked reduction in the luminal stenosis from 78.8?11.6% to 7.2?5.6%.One patient developed pericardial tamponade 3 hours after cutting balloon angioplasty,which was resolved with pericardiocentesis.No severe dissection,no abrupt coronary closure,no emergency bypass surgery and acute myocardial infarction occurred.During 6 to 2 months' follow-up,only one patient treated by cutting balloon alone experienced angina pectoris and coronary angiography showed a restenosis, there were no further cardiac event.Conclusion Cutting balloon coronary angioplasty is a safe and effective interventional therapy with a high success rate and satisactory short-term results.
7.Carotid artery stenting with cerebral protection devices and its effect on host pathological changes and biocompatibility

Many studies have been conducted at home and abroad about carotid artery stenting to treat carotid artery stenosis. However, it is limited by the complications such as perioperative ischemia. Carotid artery stenting with cerebral protection devices can omit the procedure to block blood flow and can know the treatment condition through angiography. The results of comprehensive data show that carotid artery stenting is an efficient treatment technique, and can reduce the biocompatible responses of the host. In addition, carotid artery stenting with cerebral protection devices is a safe and effective therapy for high-riskcarotid artery atherosclerotic stenosis in the elderly.

METHODS: Sperm of caput epididymides and cauda epididymides were obtained from Kunming mice aged 6 months (n=15, as control) and 18 months ( n=15). 10.Effect of arsenic trioxide on growth of a heterologous graft model for human breast cancer in nude mice

Objective To study the inhibitory effect of arsenic trioxide(AS2O3)on the tumor growth of breast cancer line MDA-MB-435s implanted subcutaneously in nude mice and its mechanism.Methods BALB/C-nu/nu nude mice were subcutaneously injected with MDA-MB-435s breast cancer cells that are ER-positive,and treated with intraperitoneal injection of AS2O3 and DDP in different concentrations.The implanted tumor was weighed,and tumor inhibition rates were calculated.The expression of PTEN and Caspase-7 induced by AS2 03 were examined by immunohistochemical method.Results The growth of implanted tumor was markedly inhibited with DDP,low dose and high dose AS2O3,and the inhibitory rates were 48.68%,32.80%,66.67%respectively.The immunohisto chemical staining showed that the number of PTEN and Caspase-7 protein increased markedly(P<0.05).Conclu sions AS2O3 inhibits the growth of human breast cancer cell implanted tumor.The molecular mechanism of AS2O3 on induction of apoptosis of breast cancer cells may be throush increasing the expression of PrrEN and Caspase-7(P <0.05).
