Objective To evaluate the safety, efficacy and short term results of cutting balloon (CB) coronary angioplasty Methods CB angioplasty was performed on 116 lesions in 102 patients (male 90%, age 57 5?11 2 years) Results The overall success rates of the primary cutting balloon angioplasty without predilatation were 94% Combined with small conventional balloon predilatation improves the procedural success rate to 97% 16 cases of in stent restenosis were also treated with CB successfully 72 lesions were treated with CB followed by stents insertion In 44 lesions treated with the CB alone, there was a marked reduction in the luminal stenosis from (80 9?11 6)% to (11 6?5 9)% One patient developed cardial tamponade 3 hours after CB angioplasty, which was resolved with pericardiocentesis One case developed severe dissection after CB angioplasty No abrupt coronary closure, emergency bypass surgery and acute myocardial infarction occurred 10 (9 8 %)patients experienced angina pectoris during 1 to 20 months (average 11 2 months) follow up, there were no further cardiac events in other patients 3 cases of angiographic restenosis were confirmed in 12 patients who underwent follow up angiography Conclusion CB angioplasty is a safe and efficient technique with a high success rate and satisfactory short term results
3.Protective effect of peritoneal dialysis on patients with acute hemorrhagic necrotic pancreatitis

Objective To explore the protective effects of peritoneal dialysis on tissue and organ damage induced by acute hemorrhagic necrotic pancreatitis (AHNP) . Methods Thirty-four AHNP patients were randomly divided into PD group ( n = 21) and control group (n=13). Control group was treated with conventional therapy, while PD group was treated with conventional therapy companied by intermittent peritonea] dialysis (IPD). Urinary and serum amylase, the plasma levels of TNF and IL-1, faecal occult blood test and serum leucocyte were examined. Functions of brain, kidney , liver and heart were observed. 4.Clinical analysis of cutting balloon coronary angioplasty

Objective To evaluate the safety,efficiency and short-term results of cutting balloon coronary angioplasty.Method This procedure was performed on 63 lesions in 52 patients (male 94%,age 57?9 years).Results The overall procedural success rate for the cutting balloon angioplasty without predilation were 92.1%.Seven out of 9 cases of total occluded lesions had satisfactory results.Two cases of in-stent restenosis were also treated with CB successfully.Forty-three lesions were treated with cutting balloon followed by stents insertion.In 20 lesions treated with the cutting balloon alone,there was a marked reduction in the luminal stenosis from 78.8?11.6% to 7.2?5.6%.One patient developed pericardial tamponade 3 hours after cutting balloon angioplasty,which was resolved with pericardiocentesis.No severe dissection,no abrupt coronary closure,no emergency bypass surgery and acute myocardial infarction occurred.During 6 to 2 months' follow-up,only one patient treated by cutting balloon alone experienced angina pectoris and coronary angiography showed a restenosis, there were no further cardiac event.Conclusion Cutting balloon coronary angioplasty is a safe and effective interventional therapy with a high success rate and satisactory short-term results.
5.Expression of IL-1ra mRNA and dysfunction of phagocytosis and intracellular killing of peripheral blood neutrophils in HD patients

Objective: To investigate the function and IL 1ra gene expression of neutrophils in hemodialysis (HD) patients. Methods: Gene expression was studied by reverse transcribe polymerase chain reaction (RT PCR) from untreated neutrophils in healthy controls and HD patients. Phagocytosis and bactericidal assays decreased in the number of viable extracellular and intracellular bacteria by colony counts. Results: Freshly isolated neutrophils spontaneously expressed icIL 1ra, sIL 1ra mRNA in HD patients. Phagocytosis and intracellular killing of neutrophils in HD patients impaired obviously when exposed to staphylococcus aureus in comparison with healthy controls. Conclusion: Untreated neutrophils in HD patients spontaneously express icIL 1ra and sIL 1ra mRNA, which is consistent with the suppressed phagocytosis and intracellular killing capacity. [
6.Genistein attenuates parathyroid hormone-induced renal interstitial fibrosis through inhibiting NF-κB signaling pathway

Objective To investigate the role of genistein (Gen) in the expression of connective tissue growth factor (CTGF) induced by parathyroid hormone (PTH) in human renal tubular epithelia cells. Methods Real-time PCR, Western blotting and reporter gene assay were employed to detect the role of Gen in PTH-induced CTGF expression in HK-2 cells. The activity of NF-κB was measured by EMSA to investigate the mechanism by which PTH induced CTGF expression in HK-2 cells. Inhibitors of NF-κB signaling pathway were used to ascertain which signal pathway was involved. Results HK-2 cells had basic amount of CTGF mRNA and protein, which, however, increased significantly after treatment with PTH, and the luciferase activity increased to a higher level as compared with control group after treatment with 10-10 mol/L PTH for 12 h (1.89±0.08 vs 0.99±0.03, P<0.01). Gen decreased the expressions of CTGF mRNA and protein induced by PTH in dose-dependent manner. The NF-κB of nucleus was inactivation without PTH, while the activity of NF-κB significantly increased after exposed to PTH, with the maximal response of PTH at a concentration of 10-10 mol/L and the best stimulating time at 30 minute. The NF-κB inhibitor PDTC reduced the increase of CTGF transcript levels in response to PTH stimulation. Gen blunted PTH-mediated NF-κB activation. Conclusion Gen inhibits CTGF expression induced by PTH through bloking NF-κB signaling pathway in human renal tubular epithelial cells.

8.Characteristics of oral glucose tolerance test in 9 803 pregnant women of different pre-pregnancy body mass index and its relationship with the incidence of gestational diabetes mellitus

Objective To investigate the values and characteristics of 75 g oral glucose tolerance test (OGTT) in women with different pre-pregnancy body mass index (BMI) and to evaluate the risk of gestational diabetes mellitus (GDM).Methods Medical records of 9 803 pregnant women attending the Peking University First Hospital and delivered between July 1, 2011 and December 31, 2013 were retrospectively analyzed.The frequency of adverse pregnancy outcomes across different degrees pre-pregnancy BMI was calculated.We divided participants into 4 groups based on pre-pregnancy BMI, underweight: BMI<18.5 kg/m2 (1 221 cases), normal weight: 18.5-23.9 kg/m2 (6 594 cases), overweight: 24-27.9 kg/m2 (1 549 cases), obesity: ≥28.0 kg/m2 (439 cases).The diagnosis of GDM was made when any one of the values was met or exceeded in 75 g OGTT.The characteristics of 75 g OGTT and the incidence of GDM were analyzed.Results (1) The average age, pre-pregnancy weight, height and pre-pregnancy BMI of the participants was (30.5±3.7) years, (57.7±9.0) kg, (162.8±4.9) cm and (21.8±3.2) kg/m2, respectively.All the values of 75 g OGTT were presented normal distribution.(2)There was statistical difference in the glucose levels among women with different pre-pregnancy BMI.The fasting, 1-hour, 2-hour glucose were (4.55± 0.34), (7.31 ± 1.54), (6.38± 1.23) mmol/L in underweight women, (4.65 ±0.38), (7.70± 1.59), (6.70± 1.27) mmol/L in normal weight women, (4.82±0.47),(8.29±1.67),(7.04±1.29) mmol/L in overweight women and (4.94±0.48), (8.56 ± 1.64), (7.10 ± 1.35) mmol/L in obesity women (P<0.01).(3) The incidence of GDM was 21.76%(2 133/9 803) in our study.There were 1 374 cases (64.42%, 1 374/2 133)with only one abnormal OGTT value while 759 cases (35.58%, 759/2 133)with two or more abnormal values.The incidence of GDM in women with underweight, normal weight, overweight and obesity was 12.53%(153/1 221),19.71%(1 300/ 6 594),32.73% (507/1 549) and 39.41% (173/439), respectively (P<0.05).Meanwhile, the incidence of women with two and more abnormal OGTT value in GDM was increased as the pre-pregnancy increasing.Conclusion The risk of GDM is increased as pre-pregnancy BMI increasing, and the risk of GDM increases significantly in women with pre-pregnancy overweight or obesity.

10.Effect of intact parathyroid hormone on residual renal function in hemodialysis patients with chronic renal failure

Objective:To investigate the effect of the intact parathyroid hormone (iPTH) on residual renal function (RRF). Methods: The relationship between iPTH and calcium, phosphorum, product Ca?P, hypertension, triglycende, cholesterol,left ventricular mass index(LVMI) and RRF in 120 hemodialysis patients with chronic renal failure. Results: The results showed that 95. 9% of the hemodialysis patients with chronic renal failure had secondary parathyroldism. It was found that iPTH was positively correlated with SBP,DBP, product Ca?P, triglycende and LVMI, and negatively correlated with endogenous creatinine clearance rate and KT/V. RRF had positive correlation with KT/V and SBP,DBP, calcium, product Ca?P, triglyceride, cholesterol and LVMI. Conclusion: iPTH level is elevated in hemodialysis patients, which may lead to RRF loss.
