Objective To analyze lesional and clinical characteristics of Langerhans cell histiocytosis in children.Methods A clinical retrospective study was performed on 126 patients with Langerhans cell histiocytosis collected from 2006 to 2011 at the Hunan Children's Hospital.Results Of the 126 patients,the youngest was 2months old,and the oldest was 9 years old.The ratio of male to female was 2.5 ∶ 1.Clinical manifestations included eczematid,seborrheic lesions,hemorrhagic maculopapules,yellow nodules and white macules.Of the three clinical phenotypes of Langerhans cell histiocytosis,Letter-Siwe disease was the most prevalent,and most cases of LetterSiwe disease were associated with hepatosplenomegaly,abnormal chest X-ray,impaired hematopoietic function and multifocal bone injuries.The clinical grade was mainly Ⅲ and Ⅳ in patients with Letter-Siwe disease,Ⅰ in patients with eosinophilic granuloma,and varied from Ⅰ to Ⅳ in patients with Hand-Schuller-Christian disease with Ⅱ as the most common.Of these patients,those with eosinophilic granuloma had the oldest average age with bone as the only affected organ,while those with Letter-Siwe disease had the youngest average age with the greatest number of affected organs.The treatment of Langerhans cell histiocytosis included surgical operation and combined chemotherapy.Conclusions Langerhans cell histiocytosis has characteristic skin lesions and diverse clinical manifestations.Pathology has diagnostic significance to Langerhans cell histiocytosis.Therapy strategies and curative effects are dependent on the severity of,and the organs affected by Langerhans cell histiocytosis.

A girl who aged eight years and seven months presented with prunosus patches on the right buttock for 8 years,gradual unilateral enlargement of the right lower limb for more than 7 years,and multiple vegetations for 1 year.Dermatological examination showed nevus flammeus and multiple malodorous vegetations over the right lower limb with high skin temperature.The right lower limb was thicker and longer than the left lower limb.X-ray examination,magnetic resonance imaging and Doppler ultrasound examination revealed high-flow vascular malformations.Pathological examination of the vegetations showed vascular proliferation,fibroblast proliferation and erythrocyte extravasation.She was diagnosed as Parkes-Weber syndrome accompanied by pseudo-Kaposi's sarcoma.

Objective To compare the efficacy and safety of modified FOLFOX4 program with docetaxel-oxaliplatin-5-fluorouracil (DOF) program as neoadjuvant chemotherapy in stage Ⅲ gastric cancer patients.Methods In 86 cases of stage Ⅲ gastric cancer patients,42 received modified FOLFOX4 chemotherapy,44 cases were treated by DOF program.After two cycles of chemotherapy,the treatment effect and adverse reactions were evaluated.Results The total effectiveness in modified FOLFOX4 group and DOF group was 60％ (25/42) and 68％ (30/44) respectively (P ＞ 0.05).Tumor control rate was 83％ (35/42) and 89％ (39/44) respectively (P ＞ 0.05).The incidence of nausea,vomiting and leukopenia was higher in DOF group than modified FOLFOX4 group (P ＜ 0.05).The D2 lymph node dissection rate between modified FOLFOX4 group and DOF group was 80％ (20/25) and 87％ (26/30) respectively (P ＞ 0.05),the R0 resection rate was 72％ (18/25) and 83％ (25/30) respectively (P ＞ 0.05).In 43 R0 resection patients using FOLFOX4 the median survival time was 38.7 months,1-,3-year survival rate was 90％,and 60％ ; while that in DOF group the median survival time was 39.6 months,1-,3-year survival rate was 95％,and 75％.Multivariate analysis showed that postoperative TNM stage only was an independent risk factor for prognosis.Conclusions TNM stage was the independent risk factor for prognosis of gastric carcinoma patients after radical gastrectomy.

BACKGROUND: Several studies have demonstrated that bone marrow mcscnchymal stem cells (MSCs) can differentiate into osteoblasts after induction of bone morphogenetic protein (BMP).OBJECTIVE: This study was designed to compare the osteogenetic effects between MSCs, bBMP, and fibrin gel alone and their composite.DESIGN: A controlled observation experiment.SETTING: Second People's Hospital of Guangdong Province and Second Hospital Affiliated to Sun Yat-sen University.MATERIALS: This study was performed at the Second Hospital Affiliated to Sun Yat-sen University between January 2004 and June 2006. Healthy New Zealand rabbits were provided by the Laboratory Animal Center, Sun Yat-sen University.METHODS: Bone marrow was taken from the lilac bone. By density gradient centrifugation, autologous MSCs were in vitro cultured and amplified. A composite was made of fibrin gel and bovine bone morphogenetic protein (bBMP). After adding MSCs, another composite was made. Twelve rabbits underwent lumbar L3/4, LA/5, and LS/6 discectomy and interbody fusion via an anterior retroperitoneal approach. Three fusion sites in each animal were randomly assigned into three of four treatments: composite of MSCs and hBMP and fibrin gel (group A), composite of MSCs and fibrin gel (group B), composite of bBMP and fibrin gel (group C) and fibrin gel alone (group D). The interbodies were observed radiologically and histologically 3 months after surgery.MAIN OUTCOME MEASURES: Bone formation in the spinal interveRebral space of rabbits.RESULTS: Twelve weeks after surgery, bone formation was appasenfly greater in the Group A than in the other groups.Histological findings revealed continuous bone formation. The groups B and C showed equivalent radiological and histological changes. Bone formation was intermittent. Group D had fibrous union, but no bone formation. No residual fibrin gel was found in all groups, but a small amount of residual nucleus gelatinosus with a little cartilaginous tissue was visible in the intervertebral disc.X-ray examination results did not reveal vertebral posterior hyperplasia or spinal stenosis. There was significant difference in range of lumbar motion between the group B and the groups C and D (P ＜ 0.05). No pronounced difference in vertebral posterior height loss existed among the groups.CONCLUSION: The composite of MSCs, fibrin gel and bBMP can promote bone regeneration and achieve osseous spinal fusion.

Objective To evaluate the value of spiral CT volume reconstruetion(VR) technique in high complex anal fistula fistulography. Methods To analyze retrospectively internal openings and secondary tracts of 35 eases with high complex anal fistula verified by operation and follow-up above 3 months with VR,and compared with operation and outcome of follow-up. Results In 35 eases with high complex anal fistula, for detection of the presence of internal openings ,VR has a sensitivity of 72.7% ,a specificity of 100% ,a accuracy of 74.3% ,a positive predictive value of 100% ,and a negative predictive value of 18.1%. Secondary tracts have a sensitivity of 93.8% ,a specificity of 100.0% ,a accuracy of 94.6% ,a positive predictive value of 100.0% ,and a negative predictive value of 72.7%. Conclusion VR can demonstrate the 3D shape of fistula and is reliable in the diagnosis of internal openings and sec-ondary ducts, and is very valuable in the analysis of high complex anal fistula fistulography.

Objective To discuss the influence of the change in ceiling-mounted shield positions on the radiation doses to the first and second operators during percutaneous coronary intervention.Methods In this study,the entrance surface dose (ESD) rates were measured for the first and second operators at 125 cm and 155 cm height on different ceiling-mounted shield positions separately through transradial approach.Measurements were repeated 20 times for each position and T test was used for statistical analysis of dose rate arithmetic mean values.Results For the first operator,the effective dose values were obtained only on left foot position.The ESD rate values at ceiling-mounted shield position close to patient were higher than that close to operators (t125 =46.9,t155 =4.1,P ＜0.05).For the second operator,the ESD rate values on his foot position,right foot position,left anterior oblique position and right anterior oblique position at ceiling-mounted shield position close to the operator,were higher than that close to the patient separately (t125 =11.9,24.4,11.2,2.7,t155 =16.1,2.8,14.4,28.8,P ＜ 0.05).The ESD rate values on head position,left foot position,left head position at ceiling-mounted shield position close to the operator,were lower than that close to the patient (t125 =-4.3,-2.4,-80.4,t155 =-10.2,-6.7,-152.6,P ＜ 0.05).Conclusions The change in the ceiling-mounted shield positions gave rise to change in radiation dose to the operators.The changes in radiation doses caused by the changes in ceiling-mounted shield positions are different in various angiographic positions.Ceiling-mounted shield should be used in a reasonable way in agreement with different positions in percutaneous coronary intervention so as to effectively reduce operators radiation dose.

Ginger mainly contains two main categories of volatile and non-volatile components,of which the non-volatile gingerol is a general term for pungent substances in ginger,mainly containing gingerols,shogaols and other components,with a wide range of pharmacological effects.By reviewing the 2020 edition of the Pharmacopoeia of the People's Republic of China and related literature in Chinese databases such as CNKI and Wanfang,it was found that the use of the Chinese names of gingerols and shogaols was confusing,which caused certain troubles to readers and obstacles to academic exchanges.According to the naming principles of Chinese medicinal extracts,it is suggested that gingersols,6-gingerol,8-gingerol and 10-gingerol should be uniformly named as gingerols(姜酚类),6-gingerol(6-姜酚),8-gingerol(8-姜酚)and 10-gingerol(10-姜酚),respectively,and shogaols and 6-shogaol should be uniformly named as shogaols(姜烯酚类)and 6-shogaols(6-姜烯酚).The standardized use of the Chinese names of gingerols and shogaols will facilitate in-depth research and academic exchange on ginger,dried ginger,baked ginger and other ginger herbal medicines.
