1.CT guided percutaneous radiofrequency ablation in treatment of patients with inoperable lung cancer

Surgical resection remains the cornerstone of therapy for early stage lung cancer. Five-year survival rates are reported as high as 92% for stage Ⅰ non-small cell lung cancer (NSCLC). However, many patients presenting with resectable early stage disease are unable to tolerate pulmonary resection, even sublobular resection, because of compromised cardiopulmonary functions or other comorbidities. Traditionally,patients deemed medically inoperable have been treated by external-beam radiation. But the results were poor with a mean survival of 20 months and a 5-years survival rate of 12%. In this scenario, we need to develop other non-surgical local therapies. One of these was image-guided percutaneous radiofrequency ablation(RFA).Many clinical trials show that RFA for lung tumors is a minimally invasive, feasible and safe technique with minor mortality and morbidity. Moreover, its efficacy seems to be promising, even in the long-term follow-up.Further experiences and comparison with other emerging minimally invasive local treatments are required to determine its rote in the treatment of medically inoperable early stage NSCLC.
2.A comparative study on tension-free inguinal hernia repair whether use of antibiotics

Objective To investigate the feasibility of tension-free inguinal hernia repair whether use of antibiotics prophylaxis.Methods A total of 204 cases of adult inguinal hernia were selected and they were divided into treatment group(51 cases)and control group(153 cases)under a hospital order by 1 ∶ 3 randomly.The patients in treatment group were treated by first-generation cephalosporin antibiotics once preoperatively,while the patients in control group received no antibiotics treatment.The incidence of early postoperative infection between two groups was compared.Results Among of 204 cases,5 cases developed surgical site infection,1 case from treatment group,infection rate was 2.0% (1/51),while 4 cases from control group,infection rate was 2.6%(4/153).There was no significant difference in the infection rate between two groups(x2 =0.000,P =1.000).No significant difference was found in the results of complete white blood cell count,neutrophilic granulocyte ratio,hospital stays and hospital expenses between two groups(P > 0.05).Conclusion There is no real need to apply regular antibiotics prophylaxis treatment for adult tension-free inguinal hernia repair without high risk factors.
3.Combined Chemotherapy of HCPT,MTX,Leucovorin and 5-Fu for Metastatic Breast Cancer

Objective:The therapeutic efficacy and side effects of combined chemotherapy of HCPT,MTX,LV and 5-Fu for metastatic or recurrent breast cancer were evaluated in our study.Methods:A total of 43 cases of advanced metastatic or recurrent breast cancer were treated with chemotherapy regimen consisting of HCPT 10mg/m2 iv gtt for dl~5,MTX 100mg iv dl,Leucovorin 150mg/m2 iv gtt for d2~4,5-Fu 500mg/m2 iv gtt for dl~5.The cycle was repeated every 4 weeks,and 2 cycles were given as one course.Results:The overall CR+PR was 47%.One year survival rate was 54% and the median survival interval was 19 months.The main side effects were bone marrow suppression and gastrointestinal reaction.Conclusion:The combined chemotherapy regimen consisting of HCPT etc is beneficial for metastatic breast cancer.
4.Application of a puncture guidance supporter in CT intervention

Objective To evaluate the application of a puncture guidance supporter in CT guided biopsy and interventional therapy. Methods Totally 468 patients underwent CT interventional biopsy (n=110) or treatment (n=358) using a puncture guidance supporter. The size of lesions was 2-15 cm. CT scanning was performed to determine the entry point, data of bi-directional needle angle α, θ and depth d before operation. After local anesthesia, the needle was punctured into skin through the puncture guidance supporter, the needle overlap CT positioning laser line in tilt angle (α) was adjusted and the puncture direction at angle θ. After CT scanning confirmed the needle aiming at target lesion, punctures were performed with the needle into the depth to hit the target. Results The total puncture hit rate was 100%, and one step hit rate was 96.79%. The average time from location scanning to hit the target was 8.4 min. The incidence of pneumothorax occurred in thorax management was 1.56%. Conclusion The application of puncture guidance supporter in CT interventional diagnosis and treatment makes operation more accurate, safe and convenient.
5.Relation between mutation of DNA-Ligase Ⅳ gene and radiosensitivity in human nasopharyngeal carcinoma cell lines

Objective To define the correlation between mutation of DNA-LigaseⅣ gene and radiosensitivity.Methods Nasopharyngeal squamous carcinoma cell line (CNE), lung adenocarcinoma cell line (SPC-A1) and breast adenocarcinoma cell line(MCF-7) after irradiation were assessed with specific biological parameters. Polymerase chain reaction, cloning and sequencing techniques were used to determine the sequence of DNA-LigaseⅣ gene in these three cell lines. Then the impact of homologous change and mutation on hydrophicity-hydrophobicity of genic products was analyzed.Results The surviving fractions derived from irradiation were different with more radiosensitivity in the CNE cell line than in the others. In three cell lines, the homology of LigaseⅣ gene were: 99.95%,99.99%,99.98%, respectively. Some mutations including transversion and transition were detected and led to alterations in the hydrophicity-hydrophobicity function of products. Higher radiosensitivity of CNE was associated with amino-acid substitutions: 313aa His→Arg,538aa Gly→Arg,579aa Lys→Arg and 585aa Asn→Ser.Conclusion These results suggest that LigaseⅣp play an important role in the ligation of DNA double strand breaks and certain mutations bring about changes in radiosensitivity.
6.In vivo high-resolution magnetic resonance imaging in severe intracranial stenosis

Objective To assess the feasibility and clinical value of high-resolution magnetic resonance imaging (HRMRI) in patients with symptomatic severe intracranial stenosis (SSIS).Methods HRMRl wasperformed with a 3.0 T MR scanner on 5 patients with symptomatic middle cerebral(n=3) or basilar (n=2) arterial stenosis of≥70% confirmed bv DSA.Image diagnosis Was made on the basis of HRMRI findings of vessel wall at the stenotic segment by 2 neuroradiologists blinded t0 patient's status.Results Three of the five patients were diagnosed to have advanced intracranial atherosclerosis based on the presence of a complex eccentric atherosclerotic plaque containing a large lipid-rich necrotic core with a heterogeneous post-contrast enhancement and with signs of ruptured fibrous cap.Two other patients were likely to suffer from non-atherosclerotic lesion.HRMRI revealed an iso-signal septum in the arterial lumen attaching to the slighfly thickened arterial wall that was iso-signal with a homogeneous post-contrast enhancement in one patient and an obviously concentrically thickened arterial wall with hypo-intense signal on T1 WI and slightly high signal on T2 WI and PDWI and without any post-contrast enhancement in the remaining patient.Conclusions In vivo HRMRI in patients with SSIS is technically feasible.It provides detailed information of intracranial arterial wall at the stenotic segment.
7.Radio frequency ablation combined with transcatherterarterial chemo embolization and ethanol injection for hepatic carcinoma

Objective To explore the effect of radio frequency ablation (RFA) combined with transcatherterarterial chemo embolization (TACE) and percutaneous puncture hydrochloric acid injection(PHI) for hepatic tumors unable to resection. Methods The clinical data of 40 cases of patients with unable resection liver cancer (URLC) treated by RFA combined with TACE and PEI were analyzed retrospectively.Results There were 30 cases of primary hepatic tumor(PHT) and 10 cases of metastasis hepatic tumor(MPT) , in this series. Examination of ultrasound, CT and MRI showed the tumors shrink or steady in 39 patients.Among 30 patients with damage by ethanol, 18 cases were AFP positive before treatment and 16 cases of them AFP decreased to normal level after operation. No severe complication was seen in the series. Conclusion RFA combined with TACE and PEI is a safe, well tolerable and effective method for hepatic cancer, and may improve the treatment efficacy of URLC.
8.Cyclin D1 G870A polymorphism and the risk of colorectal cancer: a systematic review

ObjectiveTo evaluate the association between Cyclin D1 G870A polymmphism and risk of colorectal cancer.MethodsExtensive searches of relevant studies on datebase like PubMed,EMCC and CNKI were performed.Case control studies involving unrelated subjects and genotype frequencies in control group consistent with Hardy-Weinberg equilibrium were included for the meta-analysis.Twenty-three case-control studies with 6 344 cases and 9 018 controls were analyzed by the fixed-effect or random-effect meta-analysis method.The metaanalysis was applied with RevMan 5.0 software for heterogeneity test.AA and GA were compared with those with GG genotype.Pooled OR value and 95% confidence interval (CI) were calculated.ResultsThere were statistical differences between AA & GA and GG.The pooled OR value was 1.10 (95%CI:1.01-1.19,P =0.02).The values were analyzed hierarchically according to different populations.The pooled OR value of Asian was 1.11 (95% CI:0.98-1.26,P=0.11).The pooled OR value of American was 1.13(95%CI:0.97-1.32,P=0.12).The pooled OR value of European was 1.06(95%CI:0.89-1.25,P =0.52).The pooled OR value of Oceanian was 1.05(95% CI:0.80-1.38,P=0.73).The values were analyzed hierarchically according to the comparison basis.The pooled OR value of hospital-based was 1.07 (95% CI:0.95-1.20,P =0.28).The pooled OR value of population-based was 1.13 (95%CI:1.01-1.26,P=0.04).ConclusionThe Cyclin D1 G870A polymorphism is correlated with the susceptibility of colorectal cancer risk at the aggregate level analysis.Analysis by different methods:according to different populations,every group don't support the correlation.According to comparison basis,there has no correlation in hospital-based group,but there has correlation in population-based group.
9.Preliminary study on the mechanism of reading recovery in a pure alexia by using functional MRI

Objective To observe the changes of the brain function during reading recovery by using functional MRI (fMRI),and to provide the experimental data in elucidating the mechanism on the recovery of reading and language function.Methods fMRI was performed in a native Chinese patient with pure alexia on the 45 th and 130 th day after the onset,respectively.Three kinds of Chinese characters were presented during the scan and the patient was asked to make the judgement weather he could recognize the characters or not.The brain activation maps were acquired after postprocessing,and the activated location and volume were compared between the first and second experiments.Results In both experiments,Broca area,Wernicke area,and the right extrastriate were significantly activated,while the left extrastriate around the lesion was markedly activated only in the second experiment,and the volume of activation in the right extrastriate in the second experiment was about 3 times as large as that in the first experiment.Conclusion The left extrastriate cortex is one of the key areas responsible for reading function in the brain.The recovery of reading function can be compensated in contralateral corresponding cortical area,or it can be the result of reorganization in ipsilateral peri-lesion cortex.Both mechanisms may simultaneously play important roles in reading recovery.
10.Relationship between Motor Function and Balance for Spastic Hemiplegia after Stroke

ObjectiveTo discuss the relationships among spastic paralysis and motor function, balance function and other clinical variables after stroke.MethodsAssessed the clinical spastic index of hemiplegic lower limbs with CSI scale, motor function with Fugl-Meyer Assessment, and balance function with Berg Equilibrium Scale, respectively. And then analyzed the relationship between clinical spasm index with motor function, balance function of the hemiplegic lower limbs with Pearson correlation analysis. Finally, ascertained which was the most important factor affecting the clinical spasm index of spastic lower limbs with stepwise regression analysis.ResultsThere is negative correlation between clinical spasm index with motor function and balance function, respectively. Tendon reflex, muscular tension and clonus are the deciding factors to motor function(P<0.05) and balance function(P<0.05) of the hemiplegic lower limbs.ConclusionThe clinical spasm index of hemiplegic limbs can affect the recovery of motor function and balance function significantly. So, assessing and improving the clinical spasm index of paralytic lower limbs can optimize the rehabilitation program to stroke patients.
