1.Reform and practice of dual-track teaching mode during the clinical pathology in Chinese Medicine Universities

Objective:Dual-track teaching mode in higher colleges and universities of Chinese medicine in clinical pathology teaching reform and practice, so as to change the phenomenon of university students of traditional Chinese medicine western medicine foundation weak, and improve the teaching quality of clinical pathology.Methods: We use the traditional teaching method (LBL) and teaching method of the problem-based learning (PBL) to make double-track recess internship teaching model in clinical pathology, and in practice use the access type, interactive type and potential mining methods to motivate the students’ learning.Results: The dual-track teaching model realizes complementary teaching effect of a variety of teaching methods, makes the teaching quality enhance unceasingly. It not only exercises the students’ ability of autonomous learning, cultivate the students’ clinical thinking, but also stimulates students’ imagination and improves the students’ ability to analyze and solve practical problems.Conclusion: The dual-track teaching mode is worth popularizing in the higher colleges and universities of traditional Chinese medicine, and it can improve the students’ comprehensive quality and innovation and make it better from students to the role of the doctor.
2.Observation of In Vitro Interaction of Furosemide On Stria Vascularis Cultureds From Guinea Pig

Objective To observe the effect of furosemide on stria vascularis cultivated in vitro from guinea pig, to explore the probable mechanism of furosemide ototoxicity.Methods 20 healthy pigmented guinea pigs was randomly divided into two groups: Furosemide group (n=16), control group (n=4). The stria vascularis explant were cultured for 24 hours using the explant-culture technique. Experimental groups were added into imediately different final concentration of Furosemide (60ug/ml?300ug/ml?600ug/ml?1250ug/ml?2500ug/ml) differently, and continuously cultivated 30 minutes or 90 minutes , then observed structure feature of the cultivated stria vascularis .Results Furosemide group did not show pathological changes such as edema of stria vascularis,shrinkage of the intermediate cells and enlargement of intercellular spaces. No significant changes were found in the stria vascularis structure features between furosemide group and control group. Conclusion Furosemide did not induce remarkable edema on cultivated stria vascularis from guinea pig.These findings suggested that furosemide might induce ototoxicity by indirect mechanism.
3.Analysis of the failure pattern in 110 patients with radical resection of gastric cancer

Objective To find the reasonable treatment strategy by analyzing the failure pattern and survival rates of radical resection of gastric cancer.Methods Data were collected from 110 patients with radical resection and adjuvant treatment of gastric cancer,counted up the number of cases that failure in different ways.The survival rate after operation was calculated by Kaplan-Meier.The chi-square test was used to find the differences in survival rates between different differentiation,location and gender.Results 1,3,5-year survival rates of 110 cases were 83.64% (92/110),46.36% (51/110),35.45% (39/110),respectively.Malignant ascites was the main failure type for postoperative of gastric cancer,approximately accounting for 41.51% (22/53),abdominal lymph node metastasis accounting for 30.19% (16/53),anastomotic recurrence accounting for 13.21% (7/53),abdominal implantation and mesenteric metastasis accounting for 9.43% (5/53),organ metastasis accounting for 5.66% (3/53).The 5-year local failure rate of concurrent chemoradiotherapy group was a little lower than that in adjuvant chemotherapy alone group (15.00% ∶22.22%).The 1-year survival rates of adjuvant chemotherapy alone and concurrent chemoradiotherapy group were 84.44% and 80.00% respectively,with no significant difference (x2 =0.236,P =0.627).However,the 3,5-year survival rates of the two groups were 66.67% vs 40.00% and 53.33% vs 20.00% respectively,with statistically significant differences (x2 =4.930,P =0.026 ; x2 =7.294,P =0.007).Conclusion Peritoneal metastasis is the most common failure pattern for the patients with gastric cancer who received radical operation and adjuvant treatment.The relapse rate of concurrent chemoradiotherapy group is lower than that in adjuvant chemotherapy alone group,but the overall survival rate is similar.
4.Differential expression and interaction of β-catenin/ICAT proteins in NSC67657 induced monocytic differentiation of HL60 cells

Aim To analyze differential expression and interaction of β-catenin/ICAT proteins in HL60 cells when they were induced into monocytic differentiation, and to figure out the mechanism of NSC67657 in cellu-lar induction. Methods HL60 cells were treated by 10 μmol · L-1 NSC67657 , and cellular differentiation could be observed by cytochemical staining and flow cytometry. Then, RT-PCR and Western blot were em-ployed to determine the differential expression of β-catenin/ICAT genes and proteins. Co-immunoprecipi-tation assay was used to confirm the interaction of β-catenin/ICAT proteins, and laser co-focus light mi-croscopy technology was used to co-indentify proteins differential expression and intracellular location. Re-sults HL60 cells could be induced into monocytic dif-ferentiation after 5 days treatment using 10μM NSC67657 . The CD14 ( +)% cells could be up to o-ver 90%, and cytochemical staining reports were con-sistent with this result. The expressions of ICAT gene and protein were up-regulated significantly ( P <0. 01 ) , but the expressions ofβ-catenin gene and pro-tein, on the contrary, were down-regulated(P<0. 05) when HL60 cells were induced into monocytic differen-tiation. From co-immunoprecipitation assay findings, ICAT protein interacted with β-catenin protein, and the absorbance of protein electrophoresis bands in-creased in differentiated cells. From laser co-focus light microscopy assay findings, the fluorescence of ICAT and β-catenin protein could be both observed in cytoplasm and nucleus. In drug treated HL60 cells, the fluorescence of ICAT protein was enhanced both in cytoplasm and nucleus, however, the fluorescence ofβ-catenin protein, which looked like transferring into different organelles, decreased significantly in nucleus, but increased in cytoplasm. Conclusions HL60 cells could be induced into monocytic differentiation by NSC67657 and β-catenin/ICAT proteins differentially expressed during cellular differentiation. The enhanced interaction of β-catenin/ICAT proteins and β-catenin protein transferring from nucleus into cytoplasm indi-cates that NSC67657 probably induces HL60 cells into monocytic differentiation through down-regulating β-catenin protein and blockingβ-catenin protein from nu-cleus.
5.Clinical Study on A Newly Designed Tenodesis with Polymolecular Polyethlene Fastener

Aim To investigate the clinical importance of a newly designed tenodesis with polymolecular polyethlene fastener. Methods Eighty-nine patients with knee ligaments injuries were reconstructed with the technique of newly designed tenodesis. Rehabilitation program started under the instruction of the therapist from the next day of the surgery.Results The tenodesis on all patients in our study were successful. The fasteners in two patients and the transplants in three patients were blocked in the bone tunnel during the operation. One fastener was taken out from the bone tunnel under arthroscopy. The patients were followed up for 19 months averagely (from 6 months to 49 months), most of them showed a stable knee without complain, except two patients feel uncomfortable after vigorous activities; seven patients show little laxity in anterior drawer test (ADT) or posterior drawer test (PDT) and three patients had 20-degree limitation of knee flexion. 6.Diagnostic value of procalcitonin in patients with infection complicated by heart failure

Objective To evaluate the diagnostic value of procalcitonin (PCT) in patients with infection complicated by heart failure.Methods A total 2454 patients,which with simple heart failure,simple bacterial infection and bacterial infection complicated with heart failure,from Sun Yat-sen University affiliated zhongshan hospital were enrolled in this case-control study.244 heathly controls were aslo evaluated at the same time.The serum PCT levels in different groups were analyzed by Cobas E601.ROC curve was employed to compare the diagnostic values of PCT between simple heart failure group and infection complicated with heart failure group,and the cutoff value of PCT testing for infection complicated by different grades heart failure was determined.The difference between test groups was analyzed using Kruskal-Walis H method.Results PCT levels in simple heart failure group (3.46 ± 3.07) were significantly higher than those in control group (0.04 ± 0.03) (t =4.262,P < 0.01).Besides,PCT levels in infection complicated by heart failure group(18.18 ± 10.33)were significantly higher than those in simple infection group(8.97 ± 6.20) (t =2.694,P < 0.01).Although PCT could still be a biomarker for diagnosis of bacterial infection in patients with or without heart failure (areas under the ROC curve were 80%,82%,85%,88%,respectively),the positive predictive values of PCT were declined in line with the heart failure degree increasing [z(1,2) =-6.24,P<0.01; z(l,3) =-4.35,P<0.01; z(1,4) =-5.19,P<0.01; z(2,3) =-5.33,P<0.01;z(2,4) =-2.86,P<0.05; z(3,4) =-2.46,P<0.05].There are significant differences between areas under ROC curve [z(1,2) =-2.55,P < 0.05 ; z(1,3) =-5.42,P < 0.01; z(2,3) =-2.90,P < 0.05],and the cutoff values of PCT to identify infection patients complicated with Ⅱ,Ⅲ,Ⅳ degrees heart failure were 0.086,0.192 and 0.657 μg/L,respectively.Conclusion PCT levels were increased in heart failure patients so we should pay more attention to the degree of heart failure and PCT level on patients which suffered both infection and heart failure.
7.Identification of a novel dynamic red blindness in human by event-related brain potentials.

Dynamic color is an important carrier that takes information in some special occupations. However, up to the present, there are no available and objective tests to evaluate dynamic color processing. To investigate the characteristics of dynamic color processing, we adopted two patterns of visual stimulus called "onset-offset" which reflected static color stimuli and "sustained moving" without abrupt mode which reflected dynamic color stimuli to evoke event-related brain potentials (ERPs) in primary color amblyopia patients (abnormal group) and subjects with normal color recognition ability (normal group). ERPs were recorded by Neuroscan system. The results showed that in the normal group, ERPs in response to the dynamic red stimulus showed frontal positive amplitudes with a latency of about 180 ms, a negative peak at about 240 ms and a peak latency of the late positive potential (LPP) in a time window between 290 and 580 ms. In the abnormal group, ERPs in response to the dynamic red stimulus were fully lost and characterized by vanished amplitudes between 0 and 800 ms. No significant difference was noted in ERPs in response to the dynamic green and blue stimulus between the two groups (P>0.05). ERPs of the two groups in response to the static red, green and blue stimulus were not much different, showing a transient negative peak at about 170 ms and a peak latency of LPP in a time window between 350 and 650 ms. Our results first revealed that some subjects who were not identified as color blindness under static color recognition could not completely apperceive a sort of dynamic red stimulus by ERPs, which was called "dynamic red blindness". Furthermore, these results also indicated that low-frequency ERPs induced by "sustained moving" may be a good and new method to test dynamic color perception competence.
8.Significance of immunofixation electrophoresis in the identification of M proteins

Objective To study the application of immunofixation electrophoresis(IFE) in typing M proteins. Methods Serum M proteins were detected in 43 patients and 20 normal controls by agarose gel IFE and rate nephelometery. Results Of 43 patients, 29 were affirmed to have M proteins, including 20 cases of IgG ?, 5 IgG ?, 2 IgM ?, 1 IgA ? and 1 ?. Conclusion The technology of IFE, characterized by easy differentiation of electrophoretic bands, simple operation and rapidity, has great value in typing M proteins.
9.Operation pathways for sphenoidal sinus by nasal endoscope and treatments for correlated diseases beyond scope of sphenoidal sinus

Objective:We probe more direct operation pathways in sphenoidal sinus and saddle area, and take proper measures for correlated diseases which surpass scope of sphenoidal sinus by combining image data, which could prevent serious complication.Method:Under nasal endoscope, the operation approaches we adopted included trans-anterior ethmoid sinus-posterior ethmoid sinus-sphenoidal sinus pathway, trans-meatus nasi superior-posterior ethmoid sinus-sphenoidal sinus pathway, trans nasal septum-sphenoidal sinus pathway and trans-natural opening of sphenoid sinus anterior into sphenoid sinus pathway by taking upper edge of posterior naris as a consistent surgical landmark. Forty-six cases who had sphenoidal sinus mass with bone erosion recieved surgical treatment.Result:Twenty-one cases with cyst and pus cyst of sphenoidal sinus had been healed by trans-meatus nasi superior pathway;1 case with hematoma and organization in sphenoidal sinus had been cleared by trans-meatus nasi superior pathway;3 cases had hematoma and organization in sphenoidal sinus and pseudoaneurysm in internal carotid artery,one of them suffered fatal hemorrhage in surgical exploration, and cured by endovascular embolization, the other two only underwent nasal endoscopic examination,the diagnosis was established by DSA and they received interventional therapy;4 cases with papilloma in sphenoidal sinus had been treated by trans-natural opening of sphenoid sinus approach,and 3 cases were cured,1 case had only partial mass resection as the papilloma offended the outer wall of sphenoidal sinus diffusely;2 cases with cholesteatoma of sphenoid sinus have been removed completely by trans-meatus nasi superior approach;1 case with encephalomeningocele of sphenoidal sinus underwent sphenoidotomy by trans-natural opening of anterior of sphenoidal sinus, and intraoperative puncture showed characteristic cerebrospinal fluid,the exposed meninges were then repaird surgically;1 case with mycosis of sphenoidal sinus had been cured by thorough clearing of the leision in sphenoidal sinus combined with antifungal therapy;3 cases with malignancy of sphenoidal sinus had received major mass resection of sphenoidal sinus by trans-anterior ethmoid sinus-posterior ethmoid sinus approach, and followed with radio therapy and chemotheraphy;5 cases with NPC in-volving sphenoidal sinus had been treated by radio therapy and chemotherapy after pathological examinacation;5 cases with post-operative cerebrospinal rhinorrhea and granulation hyperplasia of sphenoidal sinus had been repaired successfully by trans-meatus nasi superior approach or tans-nasal septum approach.Conclusion:There are various surgical pathways to deal with sphenoidal sinus and correlated diseases under nasal endoscope.The operation will be direct, safe and minimal invasive if we choose the pathway properly. Thin slice CT scan and 3D reconstruction of sella, and DSA or angiography of brain before operation is one of effective means to prevent surgical complications for leisions beyond scope of sphenoidal sinus.
10.Expression of caveolin-1 in tumor and its clinical significance

Caveolin-1(Cav-1) is a scaffold protein of caveolae that acts as a tumor modulator by interacting with cell adhesion molecules and signaling receptors.Evidence from a variety of studies indicate that caveolin-1 exhibits the heterogeneity of Cav-1 expression in different tumors.Current research has clearly established a role for Cav-1 as a a novel prognostic marker,Future studies will undoubtedly offer novel exciting opportunities to develop anti-cancer therapies.
