1.Occupational Infection among Supply Office Staff:Investigation and Prevention

OBJECTIVE To strengthen the self-protection consciousness of supply office staff to reduce the occupationol infection.METHODS From Sep 2006 to Aug 2007,97 supply office persons in 8 third-level hospitals and 2 second-level hospitals were carried on the related questionnaire survey.RESULTS 85.6% Of the staff were contaminated by blood,body fluid,secretion and the excreta,74.2% of them were contaminated for many times.Sharp instrument injunry occurcd among 77.3% of the staff,in which 14.4% were injured more than 5 times,About the knowledge of prevention,43.3% was familiar,47.4% only heared about them but not familiar and 9.3% never heared it.CONCLUSIONS The supply office staff is the high dangerous infection crowd,who are the suscepitable population of the sharp instrument injury.It is important to adopt the universal prevention and the standard preventive measures to reduce occupational infection occurrence.
2.Injection of pingyangmycin and selective arterial embolization for juvenile nasopharyngeal angiofibroma

OBJECTIVE To explore the effect of pingyangmycin injection after selective arterial embolization on the early-stage and recurrent juvenile nasopharyngeal angiofibroma.METHODS Twelve cases of juvenile nasopharyngeal angiofibroma (JNA) were selected to receive digital substraction angiography (DSA) and successfully treated by selective permanent intrarterial embolization.Each case was injected with 12mg pingyanymycin after the embolization every 12 to 20 days,total 4 to 8 times.RESULTS All cases were followed up for at least one year.Six patients were cured or effective, 5 were improved,and one case was no effective. CONCLUSION Pingyangmycin injection combined with selective intrarterial embolization appears to be a safe and effective method in the treatment of early- stage and a part of recurrent juvenile nasopharyngeal angiofibroma.
3.Inhibition of airway mucin hypersecretion by ipratropium bromide i n chronic bronchitis of rats

Objective To investigate the effect of ipratropi um bromide and atropine on airway mucin hypersecretion in the chronic bronchiti s model of rats. Methods The model was established by intratrac heal instillation of small dose of lipopolysaccaride (200 μg) ipratropium bromi de and atropine were administrated 3-21 d after the model was established. Patho logical changes, mucin in bronchoalveolar lavage fluid (BALF) and tracheal ring culture medium were examined before and after the adnsinistration of iprat ropium bromide and atropine. Results Characteristic pathologica l manifestions of chronic brochitis were found after instillation of LPS. Sig nificant decrease in the number of tracheal epithelia goblet cells, secretion of mucin were observed in ipratropium bromide and atropine treated rats. The eff ect of inhibiting the secretion of mucin of ipratropium bromide was higher than atropine. Conclusion It suggests that muscarinic acetylcholine receptor plays an important role in airway mucin hypersecretion in chronic bro nchitis model of rat. Ipratropium bromide exhibit a stronger inhibition effect on mucin hypersecretion than atropine, moreover no inhibition effect on mucocil iary clearance which was observed in atropine.
4.The study of increased sensitivity to cisplatin by nm23-H1 in vitro

Objective To study the mechanism of increased sensitivity to cisplatin by nm23-H1.Methods The plasmid,pCMV-BamH1-Neo-nm23-H1,was transfected to cell line Tca8113 by lipofection strategy and was selected by G418 for 5 weeks.The stable products of nm23-H1 gene were identified by Immunohistofluorescence and Western-blotting.Using MTT and flow cytometer,we detected the changes of cell mortality rate,apoptosis and mitochondrial membrane potential.Results We successfully established the cell line of stable overexpression of nm23-H1.In vitro,the cell mortality rate and apoptosis were increased significantly in stable expression cell line of nm23-H1,compared with those in Tca8113 cell line of non-transfection;this effect could be inhibited by oubain which is an inhibitor of Na(+)/K(+)-ATPase.Mitochondrial membrane potential was decreased significantly in stable expression cell line of nm23-H1.Conclusion Nm23-H1 can increase the sensitivity of cisplatin on Tca8113 cell line;The mechanism may be the mitochondrial membrane potential is decreased by nm23-H1 so that intracellular platinum are increased to finally cause the apoptosis and necrosis of cells.
5.Survival condition and predictive model in a large data of esophageal squamous cell carcinoma

Objective The survival and prognosis factors of thoracic esophageal squamous cell carcinoma patients after radical resection was investigated.Methods 1923 patients of thoracic esophageal squamous cell carcinoma after radical resection were included in our study from January 1th 2000 to January 1th 2010 in Zhejiang Cancer Hospital,1 670 male and 253 female.the age in the majority with 40 to 59 years old(1 076/1 923,56.0%).Eighteen prognosis factors were collected.A multivariate analysis of these selected variables was performed using Cox proportional model and prognosis index.We used life table for accumulated survival rate.Results The accumulated survival rate for all patients were 82%,48% and 35% in 1 year,3 years and 5 years,respectively.Median survival time was 35.42 months.The significant prognosis factors included body mass index,length of tumor,depth of invasion,differentiation degree,lymph node metastatic degree and region,complication of surgery.Conclusion The prognosis of thoracic esophageal squamous cell carcinoma was affected by multi-factors and prognosis index can predict survival condition.
6.Protective effect of human umbilical cord-derived mesenchymal stem cells against severe acute pancreatitis in rats

Objective To study the protective effects of human umbilical cord-derived mesenchymal stem cells (ucMSCs)against severe acute pancreatitis (SAP) in rats.Methods A total of 135 Sprague-Dawley male rats were randomly divided into Sham group,SAP group and SAP+ucMSCs group (45 each).SAP+ucMSCs group:Severe acute pancreatitis was induced by injecting 5% sodium taurocholate (0.1ml/100g) into the common bilio-pancreatic duct and then CM-DiI-labeled ucMSCs at 1 × 107cells/kg were injected via the tail vein.All the rats were sacrificed 12,24 and 72 hours after SAP.The 72h death rate was counted.Pathological changes in the pancrease were detected by HE staining and pathological score was graded,ucMSCs colonization was observed by fluorescence microscopy.The serum levels of amylase,lipase,TNF-α,IL-1β,IL-4 and IL-10 were determined by ELISA.Results ucMSCs colonize the injured area of pancreatic tissue,the 72h death rate was reduced,and the serum amylase and lipase were also reduced significantly.Moreover,ucMSCs significantly reduced the pathological score of the pancrea and the level of proinflammatory cytokines (TNF-α and IL-1β),but the levels of anti-inflammatory cytokines were increased (IL-4 and IL-10).Conclusion Transplantation of ucMSCs can reduce the severity of pancreatic injury and inflammation in SAP rats.
7. Effect of Modified Buyang Huanwu Tang on Serum PNT and IGF-1 in Patients with Diabetic Peripheral Neuropathy

Objective: To investigate the changes of serum proneurotensin (PNT) and insulin-like growth factor-1 (IGF-1) levels in patients with diabetic peripheral neuropathy (DPN) and their relationship with nerve conduction velocity (NCV) and Toronto Clinical Scoring System (TCSS), so as to observe the clinical efficacy of modified Buyang Huanwu Tang on DPN patients and its effect on the levels of PNT and IGF-1. Method: Totally 68 patients with DPN were randomly divided into treatment group and control group, with 34 cases in each group. Both groups were given mecobalamin capsules orally in addition to routine therapy of diabetes, while the treatment group was given modified Buyang Huanwu Tang orally for 4 weeks. The sensory nerve conduction velocity (SNCV) of median nerve, motor nerve conduction velocity (MNCV) of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve, TCSS score and serum PNT, IGF-1 levels were observed before and after treatment. Result: Before treatment, there was no significant difference in SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve, TCSS score and serum levels of PNT and IGF-1 between two groups. Serum levels of PNT and IGF-1 were positively correlated with SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve and MNCV of common peroneal nerve (P<0.05), and negatively correlated with TCSS score (P<0.05). Compared with those before treatment, SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve and the levels of serum PNT and IGF-1 in both groups were significantly increased (P<0.05), while TCSS scores were significantly decreased (P<0.05). Compared with control group, SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve and the levels of serum PNT and IGF-1 in treatment group were higher than those in control group (P<0.05), TCSS score was lower than that in the control group (P<0.05), and the total effective rate in treatment group was higher than that in control group (P<0.05). Conclusion: Serum PNT and IGF-1 may be involved in the occurrence and progress of DPN in patients with diabetes mellitus. Modified Buyang Huanwu Tang can increase the levels of serum PNT, IGF-1, NCV but reduce TCSS score in patients with DPN. It has a definite curative effect on DPN.
8.Significance of the expression of microRNA in the peripheral blood of patients with acute pancreatitis

Objective To predict the severity of acute pancreatitis by detecting the changes of microRNA in the serum,and whether paracentesis catheter drainage (PCD) should be applied to the patients.Methods The peripheral blood of 120 patients with acute pancreatitis who were admitted to the General Hospital of Chengdu Military Command from October 2013 to March 2014 were collected.Thirty-five patients with severe acute pancreatitis (SAP) or moderately severe acute pancreatitis (MSAP) were in the group A,and 85 patients with mild acute pancreatitis (MAP) were in the group B.The scores of the acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ),RANSON and bedside index for severity in acute pancreatitis (BISAP) were assessed.The expressions of the miR-146a,miR-10b,miR-21 and miR-26a in the serum were detected by the real-time quantitative PCR.The differences in the expressions of the 4 kinds of microRNAs in the 2 groups were compared,and the correlation between the 3 exaluation system was analyzed.Factors influencing the timing of the PCD for the PCD patients were analyzed.The measurement data were analyzed using the t test,and the relationship between the variables was analyzed using the linear correlation analysis or the rank correlation analysis.Univariate or multivariate analysis was done by the Logistic regression analysis.Results (1) The scores of the APACHE Ⅱ,RANSON and BISAP were 8.28 ±0.61,3.42 ±0.54 and 1.71 ±0.32 in the group A,and 3.18 ±0.52,1.43 ±0.25 and 0.37 ±0.06 in the group B,with significant differences between the 2 groups (t =4.266,7.809,4.113,P < 0.05).(2) The expressions of the miR-146a,miR-10b,miR-21 and miR-26a were 1.41 ± 0.21,2.94 ± 0.49,1.62 ± 0.25,1.21 ± 0.20 in the group A,and 6.29 ± 0.91,0.52 ± 0.09,2.82 ± 0.33 and 3.57 ± 0.64 in the group B.There were significant differences in the expression of the miR-146a and miR-10b between the 2 groups (t=-2.156,2.110,P <0.05),while no significant difference in the miR-21,miR-26a was detected between the 2 groups (t =-1.114,-1.571,P > 0.05).(3) There was correlation between the expressions of the miR-146a,miR-10b and the APACHE Ⅱ,RANSON,BISAP in the group A (r =-0.826,0.837,-0.874,0.866,-0.833,0.899,P < 0.05),while no correlation was detected between the expressions of the miR-21,miR-26a and the 3 exaluation systems in the group A (r =0.642,0.321,0.701,0.750,0.716,0.716,P > 0.05).There was no significant difference between the miR-146a,miR-10b,miR-21,miR-26a and the APACHE Ⅱ (r =0.067,0.347,0.133,0.111,P>0.05),RANSON (r=0.178,0.078,0.092,0.142,P >0.05) and BISAP (r =0.103,0.260,0.216,0.285,P > 0.05) in the group B.(4) The results of univariate analysis showed that miR-10b,RANSON and BISAP were the factors influencing the timing of PCD (OR =4.170,5.612,2.500,95 % confidence interval:1.092-15.932,1.232-21.622,1.190-5.254,P < 0.05).The results of multivariate analysis showed that miR-10b was the factor influencing the timing of PCD (OR =2.374,95% confidence interval:1.115-5.056,P < 0.05).Conclusions miR-10b and miR-146a might be the predictors of severity of severe acute pancreatitis; miR-10b might be the indicator in judging whether PCD should be applied.
9.Value of quantitative tissue velocity imaging in the detection of regional myocardial function in dogs with acute subendocardial ischemia.

This study evaluated the application of quantitative tissue velocity imaging (QTVI) in assessing regional myocardial systolic and diastolic functions in dogs with acute subendocardial ischemia. Animal models of subendocardial ischemia were established by injecting microspheres (about 300 microm in diameter) into the proximal end of left circumflex coronary artery in 11 hybrid dogs through cannulation. Before and after embolization, two-dimensional echocardiography, QTVI and real-time myocardial contrast echocardiography (RT-MCE) via intravenous infusion of self-made microbubbles, were performed, respectively. The systolic segmental wall thickening and subendocardial myocardial longitudinal velocities of risk segments before and after embolization were compared by using paired t analysis. The regional myocardial video intensity versus contrast time could be fitted to an exponential function: y=A.(1-exp(-beta.t)), in which the product of A and beta provides a measure of myocardial blood flow. RT-MCE showed that subendocardial normalized A.beta was decreased markedly from 0.99+/-0.19 to 0.35+/-0.11 (P<0.05) in 28 left ventricular (LV) myocardial segments after embolization, including 6 basal and 9 middle segments of lateral wall (LW), 8 middle segments of posterior wall (PW) and 5 middle segments of inferior wall (IW). However, there was no statistically significant difference in subepicardial layer before and after embolization. Accordingly, the ratio of A.beta of subendocardial myocardium to subepicardial myocardium in these segments was significantly decreased from 1.10+/-0.10 to 0.31+/-0.07 (P<0.05). Although the systolic wall thickening did not change 5 min after the embolization in these ischemic segments (29%+/-3% vs 31%+/-5%, P>0.05), the longitudinal peak systolic velocities (Vs) and early-diastolic peak velocities (Ve) recorded by QTVI were declined significantly (P<0.05). Moreover, the subendocardial velocity curves during isovolumic relaxation predominantly showed positive waves, whereas they mainly showed negative waves before the embolization. This study demonstrates that QTVI can more sensitively and accurately detect abnormal regional myocardial function and post-systolic systole caused by acute subendocardial ischemia.

10.Effects of Notch signal on the expressions of HIF-α and autophagy- related genes Beclin1, LC3I, LC3II in oxygen-glucose deprivation induced myocardial cell injury.

OBJECTIVE:
To investigate the effects of Notch signal on hypoxic induction factor (HIF-1α) and autophagy-associated genes Beclin1, LC3I, LC3II in oxygen-glucose deprivation (OGD) induced myocardial cell injury.
METHODS:
The OGD model was established using hypoxic culture box and hypoglycemic DMEM medium. The cells were divided into normal control group, OGD group, OGD + NC siRNA group, OGD + Notch1 siRNA group and OGD + HIF-1α siRNA group. Western blot was used to detect the interference effects of HIF-1α siRNA and Notch1 siRNA. The effects of Notch1 siRNA and HIF-1α siRNA on the activity of myocardial cells in OGD model were detected by the CCK-8 assay. The effects of Notch1 siRNA and HIF-1α siRNA on autophage-associated genes Beclin1, LC3I and LC3II expression were detected by Western blot.
RESULTS:
The results of Western blot showed that HIF-1α siRNA could effectively knock down the expression of HIF-1α in myocardial cells in OGD model, and Notch1 siRNA could effectively knock down the expression of Notch1 and HIF-1α in myocardial cells in OGD model. The result of CCK-8 assay showed that Notch1 siRNA and HIF-1α siRNA reduced the activity of myocardial cells in OGD model, and there was no statistical difference between the two groups. Western blot results showed that Notch1 siRNA and HIF-1α siRNA could reduce the expressions of the autophagy-associated genes Beclin1, LC3I and LC3II, and reduce the ratio of LC3II to LC3I at mRNA level.
CONCLUSION
Notch1 plays a role in myocardial protection by regulating the expression of HIF-1α to regulate the autophagy in OGD model cells.
