Chinese Journal of Experimental Traditional Medical Formulae 2019;25(16):67-72
Objective: To investigate the changes of serum proneurotensin (PNT) and insulin-like growth factor-1 (IGF-1) levels in patients with diabetic peripheral neuropathy (DPN) and their relationship with nerve conduction velocity (NCV) and Toronto Clinical Scoring System (TCSS), so as to observe the clinical efficacy of modified Buyang Huanwu Tang on DPN patients and its effect on the levels of PNT and IGF-1. Method: Totally 68 patients with DPN were randomly divided into treatment group and control group, with 34 cases in each group. Both groups were given mecobalamin capsules orally in addition to routine therapy of diabetes, while the treatment group was given modified Buyang Huanwu Tang orally for 4 weeks. The sensory nerve conduction velocity (SNCV) of median nerve, motor nerve conduction velocity (MNCV) of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve, TCSS score and serum PNT, IGF-1 levels were observed before and after treatment. Result: Before treatment, there was no significant difference in SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve, TCSS score and serum levels of PNT and IGF-1 between two groups. Serum levels of PNT and IGF-1 were positively correlated with SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve and MNCV of common peroneal nerve (P<0.05), and negatively correlated with TCSS score (P<0.05). Compared with those before treatment, SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve and the levels of serum PNT and IGF-1 in both groups were significantly increased (P<0.05), while TCSS scores were significantly decreased (P<0.05). Compared with control group, SNCV of median nerve, MNCV of median nerve, SNCV of common peroneal nerve, MNCV of common peroneal nerve and the levels of serum PNT and IGF-1 in treatment group were higher than those in control group (P<0.05), TCSS score was lower than that in the control group (P<0.05), and the total effective rate in treatment group was higher than that in control group (P<0.05). Conclusion: Serum PNT and IGF-1 may be involved in the occurrence and progress of DPN in patients with diabetes mellitus. Modified Buyang Huanwu Tang can increase the levels of serum PNT, IGF-1, NCV but reduce TCSS score in patients with DPN. It has a definite curative effect on DPN.

OBJECTIVETo explore whether microRNA-200a (miR-200a) could be used as a novel biomarker of liver cancer using a rat model system.
METHODSDiethylnitrosamine abdominal injection was applied to induce liver cancer in the F344 rat strain (n =40); ten unmodeled rats served as controls. In addition, human subjects with normal healthy liver (n =10), liver cirrhosis (n =10), and liver cancer (n =10) were enrolled in the study. Blood samples from both rats and patients and rats' livers were collected for analysis. Real-time quantitative PCR and enzyme-linked immunosorbent assay were used respectively to measure the expressions of serum miR-200a and alpha-fetoprotein (AFP) for all rat and human subjects. In situ hybridization was used to detect the miR-200a expression in the rats' livers.
RESULTSComparison of normal rats and the liver cancer modeled rats showed that the latter had significantly lower expression of miR-200a (P less than 0.05), with decreasing expression following the progression of liver injury to cancer (liver cirrhosis rats less than early liver cancer rats less than advanced liver cancer rats); in contrast, the AFP levels were significantly higher in the liver cancer modeled rats only at the early and advanced stages of the liver cancer (P less than 0.05). These
RESULTSsuggested that miR-200a expression decreases during the developmental process of liver cancer, while AFP expression increases distinctly at the stage of tumor formation. Analysis of the human subjects' clinical samples showed that miR-200a expression was decreased in both liver cirrhosis patients and liver cancer patients (vs. normal liver subjects, P less than 0.05), while AFP showed abnormal expression only in the patients with liver cancer. Comparison of the normal rats and modeled rats using in situ hybridization showed the positive rates for miR-200a expression were 1.00% +/- 0.01% in rats with normal liver, 0.37% +/- 0.03% in rats with fibrotic liver, 0.14% +/- 0.01% in rats with cirrhotic liver, 0.05% +/- 0.00% in rats with early stage liver cancer, and 0.01% +/- 0.00% in rats with advanced stage liver cancer.
CONCLUSIONMiR-200a may play an important role in liver cancer development and may have diagnostic value for indicating early liver cancer.

Chinese Journal of Clinical Nutrition 2021;29(4):226-231
Objective:To explore the application of home nutrition support in children with intestinal failure. Methods:Children with intestinal failure admitted to Division of Pediatric Gastroenterology and Nutrition and Department of Pediatric Surgery in Xinhua Hospital were retrospectively enrolled since January 2009. The details of home nutrition support, nutritional status and home parenteral nutrition (HPN) associated complications were collected. Results:A total of 10 children received HPN support, 7 of whom were with short bowel syndrome (SBS) and the other 3 with pediatric intestinal pseudo-obstruction. The average length of remnant small bowel in 7 SBS children was (36.7±32.4) cm. The average age at HPN onset was (5.4±4.7) years. The average duration of follow-up was (3.1±2.1) years. The average duration of HPN was (619.5±669.1) days after (391.8±340.1) days of parenteral nutrition support in our hospital. All 10 cases started home enteral nutrition (HEN) with tube feeding (3 cases transited to oral feeding during treatment). The average duration of HEN was (536.1±429.6) days. Daily calorie intake was 104.0%±39.0% of the recommended intake according to the guideline, with 46.5%±21.3% via HPN and 57.5%±29.2% via HEN. During follow-up, 3 cases were found with severe malnutrition, 5 with moderate malnutrition and 2 with mild malnutrition. Four children suffered from catheter-related thrombosis and five children were identified with catheter-related blood stream infection. No intestinal failure associated liver disease was observed. Conclusions:HPN is feasible but needs the support of national medical insurance policy. At present, there are still frequent nutritional deficiencies and complications in HPN. Nutrition support team (NST) should provide guidance for more scientific nutrition screening and nutrition management.
