1.Clinical effects observation of levocarnitine combined with sucrose iron injection for treatment of maintenance hemodialysis patients with renal anemia

Objective To observe the clinical effects of levocarnitine combined with sucrose iron injection for treatment of maintenance hemodialysis patients with renal anemia.Methods Forty-four cases of patients with maintenance hemodialysis merged renal anemia were randomly divided into control group (n =22) and treatment group(n=22).All patients were administrated conventional therapy.The sucrose iron injection was administrated with control group at hemodialysis, while treatment group administrated levocamitine after hemodialysis on this basis.The hemoglobin (Hb) and erythrocyte specific volume (Hct) were compared after treatment for 8 weeks,and the clinical effects were evaluated.Results After treatment for 8 weeks, the Hb and Hct(treatment group: (105.69±11.94) g/L, (0.31±0.04);control group: (86.31±11.39) g/L,0.27±0.05) had increased significantly compared with prior treatment(treatment group:(62.65± 12.74) g/L, (0.19±0.04;control group: (63.33 ± 13.23) g/L, 0.20± 0.06) in two groups, and the treatment group increased more significant than control group accompanied with significant statistically difference (t =3.197,2.978;P <0.05).Also,the total effective rate of treatment group was 86.4%(19/22) ,which higher than control group(50.0% (11/22)) accompanied with significant statistically difference (x2 =6.705, P<0.05).Conclusion The therapy of levocarnitine combined with sucrose iron injection can obviously improve anemia symptoms in patients with maintenance hemodialysis and worthy of popularization and application.

[Conclusion] GUO Lao unique method of clinical application of malt, clinical each fine effect, is worth learning to imitate.
3.Clinical observation of irbesartan combined with ambrette capsule for treatment of mild-to-moderate proteinuria in type 2 diabetic nephropathy

Objective To observe the clinical effects of irbesartan combined with ambrette capsule for treatment of mild-to-moderate proteinuria in type 2 diabetic nephropathy. Methods Foutty-six patients with diabetic nephropathy( DN),who meet the diagnostic criteria for diabetes and DN diagnostic criteria which WHO promulgated in 1999 years,were randomly divided into control group(n =23)and treatment group(n =23) . Patients in control group were administrated conventional hypoglycemic drugs,lipid,calcium antagonists step-down and diet control,while in treatment group were administrated irbesartan(75 mg,1 times/d)combined with ambrette capsule(5 pills,3 times/d)on this basis. Both the treatment duration was 8 weeks. Urine protein,blood urea nitrogen( BUN),serum creatinine( SCr)and C reactive protein( CRP)were measured respectively before treatment and 8 weeks after treatment. Results After 8 weeks treatment,the total effective rate in treatment group was 87. 0%(20/23),higher than that of control group(52. 2%(12/23);χ2 =6. 571,P﹤0. 05). The CRP had decreased significantly in treatment group( from( 11. 7 ± 0. 9 ) mg/L to( 5. 8 ± 0. 3 ) mg/L ) which compared with control group(from(10. 1 ± 0. 6)mg/Lto(9. 8 ± 0. 4)mg/L;P﹤0. 05). But BUN and SCr had no significant difference compared with pre-treatment( P ﹥ 0. 05 ). Conclusion Irbesartan combine with ambrette capsule can significantly decrease the urine protein and regulate on the micro-inflammatory state. It is worthy of popularization and application.
4.Preparation of gadolinium-loaded stearic acid grafted chitooligosaccharide and application in pancreatic tumor imaging

Objective To prepare gadolinium-loaded stearic acid grafted chitooligosaccharide (COSSA-DTPA-Gd) and evaluate its micelle properties,cytotoxicity,relaxation rate in vitro,and pancreatic tumor in vivo imaging.Methods Stear ic acid grafted chitooligosaccharide (COSSA) was synthesized by acetylation reaction between stearic acid and chitooligosaccharide.Diethylenetriaminepentaacetic dianhydride (DTPA) was conjugated to the residual amino groups of COSSA,then Gd3+ was chelated to obtain the final product.The micelle properties were measured using an electron microscopy and a laser particle sizer.The MTT assay was adopted to determine cytotoxicity.The in vitro relaxation rate and in vivo imaging of pancreatic tumor were evaluated using an MR scanner.Results COSSA-DTPA-Gd could self-assemble into stable micelles in aqueous solutions with a critical micelle concentration of (5.12±0.43)μg/ml.The micelles had positive charge and exhibited roughly spherical shape with a mean diameter of (58.3± 5.7)nm.The content of Gd3+ in COSSA-DTPA-Gd was 330.31 μmol/g.The nanoprobe and Magnevist,the commercial formulation,showed similar cytotoxicity (P>0.05).The cell survival rate within 24 h were higher than 85%.The in vitro relaxation rate of COSSA-DTPA-Gd was 8.23 mM-1 ·s-1.After intravenous injection,COSSA-DTPA-Gd showed a better positive contrast-enhancing effect for pancreatic tumor than Magnevist.The MR images at the tumor periphery was rapidly enhanced,while a slow increase in image quality was observed in tumor core.Conclusion The prepared COSSA-DTPA-Gd can be used for efficient MR imaging of pancreatic tumor.
5.Evaluation of clinical effect of ZeGuiLongShuang capsules combined with naftopidil on benign prostatic heperplasia

Objective To study the curative effect of ZeGuiLongShuang capsules combined with naftopidil on benign prostatic hyperplasia(BPH).Methods 140 cases with BPH were randomly divided into treatment group and control group.The patients in treatment group were given ZeGuiLongShuang capsules(once 2 tablets,3 times a day) combined with naftopidil(once 25 mg,once a day before sleeping).The patients in control group were given only naftopidil(once 25 mg,once a day before sleeping).Internatianal-prognosic-scoring-system(IPSS),uroflowmetry-max(Qmax),Quality-of-life(QOL),residual urine and weight of prostate before and after treatment were detected.Results IPSS,Qmax,QOL and residual urine changed in two groups.IPSS,Qmax,QOL and residual urine in treatment group were more significant than those in control group(P
6.An efficacy comparison of pirarubicin with other intravesicai chemotherapeutic agents in preventing blad der tumor recurrence:a Meta-analysis

Objective To compare the efficacy of pirarubicin with other intravesical chemothera peutic agents in preventing bladder tumor recurrence. Methods Databases such as Medline,EM Base,CBMDisc and PubMed,ScienceDirect,LWW,Springer,CJFD(Chinese Journal Fulhext Data base)and Chinese Periodicals Database of Science and Technology were systematicallv searehed for controlled studies involved with intravesical pirarubicin for the treattnent of bladder cancer.Eligible studies according to inclusion and exclusion criteria were selected,then recurrence data from included studies were retrieved for a meta-analysis. Results Fifteen studies met the inclusion criteria.Of these studies 10 compared efficacy of pirarubicin with mitomycin C,and the meta-analysis based on these 10 studies showed that pirarubicin was statistically superior to mitomycin C(OR=0.41,95%CI 0.27~0.61,P<0.01).Of 6 studies,pirarubicin was compared with thiotepa(OR=0.35,95%CI 0.11~1.13,P>0.05),adriamyein(OR=0.15,95% CI 0.03~O.76,P<0.05),epirubicin(OR=0.53,95% CI 0.23~1.23,P>0.05),hydroxycamptothecin(OR=0.25,95% CI 0.10~0.64,P<0.05)and blank control respectively,statistical analysis indicated pirarubicin was significantly better than adriamycin,hydroxycamptothecin and blank control,and not statistically different from thiotepa and epirubicin. Conclusion Pirarubicin is effective in preventing bladder tumor recurrence,with efficacy better than chemotherapeutic agents such as mitomycin C,hydroxyeamptothecin,and adriamycin,thus can be a novel optional intravescial chemotherapeutic agent for bladder cancer.
7.Combination of arterial perfusion chemotherapy and embolization for the treatment of giant carcinoma of kidney in 21 cases

Objective To explore the efficiency of arterial perfusion chemotherapy combined with embolization for the treatment of giant carcinoma of the kidney. Methods Arterial perfusion chemotherapy combined with embolization was performed through the renal artery in 21 cases of giant carcinoma of the kidney from April 1992 to April 2006. The chemotherapeutic agents contained carboplatin(300 mg), mitomycin(20 mg) and cyclophosphamide(800 mg). The embolization was conducted with anhydrous alcohol, sodium morrhuate, and lipiodol plus gelatin sponge. Results The arterial perfusion with embolization was successfully obtained in all 21 cases. Surgical resection was accomplished in 15 cases, 1 - 8 weeks after the embolization; revealing severely less or no blood supply to the tumor. Pathological findings showed marked necrosis of tumor cells with peripheral inflammatory infiltration, fibrous proliferation as well as capillary embolization. The survival rates at 1-, 2-, and 3-, year were 80%(12/15), 53.33%(8/15) and 40%(6/15) respectively. Conclusions Combination of perfusion chemotherapy and embolization through the renal artery for the treatment of giant carcinoma of the kidney offers promising clinical effects.
8.Intraocular Pressure Sensor Based on a Contact Lens.

Intraocular pressure detection has a great significance for understanding the status of eye health, prevention and treatment of diseases such as glaucoma. Traditional intraocular pressure detection needs to be held in the hospital. It is not only time-consuming to doctors and patients, but also difficult to achieve 24 hour-continuous detection. Microminiaturization of the intraocular pressure sensor and wearing it as a contact lens, which is convenient, comfortable and noninvasive, can solve this problem because the soft contact lens with an embedded micro fabricated strain gauge allows the measurement of changes in corneal curvature to correlate to variations of intraocular pressure. We fabricated a strain gauge using micro-electron mechanical systems, and integrated with the contact lens made of polydimethylsiloxane (PDMS) using injection molding. The experimental results showed that the sensitivity was 100. 7 µV/µm. When attached to the corneal surface, the average sensitivity of sensor response of intraocular pressure can be 125.8 µV/mm Hg under the ideal condition.

9.Expression and clinical significance of URAT1 in renal tissue of patients with uric acid nephrolithiasis

Objective To investigate the metabolic profile of uric acid and the significance of the altered renal expression of urate trans-porter 1(URAT1) in patients with uric acid nephrolithiasis. Methods The data of 24 patients in our hospital from January 2012 to October 2013 were analyzed retrospectively. Participants in the research were divided into three groups:patients with uric acid nephrolithiasis,other patients with nephrolithiasis and normal participants. The basic clinical data and the related data of uric acid metabolition of participants were collected,URAT1 gene expression in renal tissures of three groups was detected by Real-time PCR technique. All data were statistically ana-lyzed and compared between these groups. Results Uric acid levels in plasma,body mass index and age were significantly higher in patients with uric acid nephrolithiasis than other two groups (P<0. 05),but urine PH value was significantly reduced in patients with uric acid neph-rolithiasis. Urine and uric acid output of 24 hours urine were no significant difference among the three groups (P>0. 05). The result of Real-time PCR suggested that the URAT1 renal expression was significantly higher in patients with uric acid nephrolithiasis than other two groups (P<0. 05). Conculusion Patients with uric acid nephrolithiasis are closely related with hyperuricemia,but unrelated with renal over-ex-cretion of uric acid. The upregulated URAT1 expression in the kidney may be an important molecular mechanism of the clinical features.
10.Septal anterior ventricular exclusion operation for left ventricular anterior aneurysm:effects on left ventricular shape,volume and function by intraoperative transesophageal echocardiography

Objective To assess the effects of septal anterior ventricular exclusion(SAVE)procedure on left ventricular(LV)shape,volume and function in patients with post-infarction LV anterior aneurysm by intraoperative transesophageal echocardiography(TEE).Methods Twenty patients with LV anterior aneurysm underwent surgical ventrieular restoration(SVR)with the SAVE procedure.Intraoperative TEE was performed before cardiopulmonary bypass(CPB)and after weaning from CPB.LV volume and ejection fraction(EF)was measured using the biplane Simpson's method.LV end-diastolic and end-systolic volumes,indexed by body surface area(EDVI and ESVI,respectively)were calculated.To estimate the shape of LV,end-diastolic sphericity index(SI)of LV was determined.Results Compared with pre- CPB,after SVR,LV shape became more elliptical:SI increased from 0.76±0.04 to 0.84±0.05,P<0.001.LV size became more normal:EDVI decreased from(121.51±16.91)ml/m2 to(60.27±9.93)ml/m2,P<0.001,and ESVI decreased from(85.81±15.02)ml/m2 to (32.44±5.36)ml/m2,P<0.001,respectively.EF was increased significantly:(46.02±3.90)% vs(29.52±6.0)%,P<0.001.Conclusions The SAVE technique is easy to reshape LV to ellipsoid fogln and normal size and the resultant improved configuration may contribute to improving EF for patients with post-infarction LV anterior aneurysm.
