1.Overexpression of HMGA2 in triple negative breast cancer and its correlation with lymph node metastasis

Purpose To detect high mobility group protein A2 (HMGA2) expression in breast cancer, and to analyze its relationship with clinicopathological features and the levels of HMGA2 in different molecular subtypes of breast carcinomas. Methods An immu-nohistochemical study was undertaken for measuring the levels of HMGA2 in 58 breast carcinomas. Results ( 1 ) The expression of HMGA2 was 0, 62. 5%, 60. 0%, 100. 0% and 80. 0% in Lum A, Lum B, HER-2-OE, basal-like breast carcinoma (BLBC) and un-classification phenotype respectively ( triple negative breast cancer, 92. 3%) . High expression of HMGA2 was associated with the tri-ple-negative breast cancer (TNBC) subtypes (P<0. 01). (2) An association was identified between high expression of HMGA2, and high tumor grade, lymph node metastasis (P<0. 05), positive Ki-67, CK5/6 and EGFR (P<0. 05), negative ER and PR (P<0. 01), but no association was observed for tumor size and patients’age. Conclusion An association is identified between high ex-pression, and high tumor grade, lymph node metastasis, positive Ki-67, EGFR and CK5/6, negative ER and PR, that means a high expression of HMGA2 is associated with an adverse outcome in breast cancer. High expression of HMGA2 are associated with the TNBC subtypes. Thus recognizing HMGA2 as a rational target in TNBC. The results of the study have implications for therapeutic target iden-tification and the design of future clinical trials for TNBC.

Purpose To study the levels and subcellular localization of β-catenin in 5 different molecular subtypes of breast carcino-mas. Methods An immunohistochemical study was undertaken for measuring the levels and subcellular localization ofβ-catenin in 58 breast carcinomas. Results ( 1 ) The cytoplasmic expression of β-catenin was 21. 1%, 50%, 60%, 100% and 60% ( TNBC 84. 6%) in Lumina A, Lumina B, HER-2-OE, basal-like breast carcinoma ( BLBC) and uncl phenotype respectively. High cytoplas-mic expression was associated with the BLBC and TNBC subtypes ( P<0. 05 ) . 4.Evaluation of Logistic regression and ROC curve on diagnostic value of Epstein-Barr virus VCA-IgM ,AST and ALT in Epstein-Barr virus current infection*

Objective To investigate the diagnostic value of the single detection and combined detection of Epstein-Barr virus (EBV) VCA-IgM ,aspartate transaminase (AST) and alanine transaminase (ALT) in EBV current infection .Methods The VCA-IgM positive simples tested by chemiluminescence from January to October 2016 were collected .EBV-DNA was detected by RT-PCR .AST and ALT were detected by using the enzyme rate method .Then samples were divided into the EBV-DNA positive group and EBV-DNA negative group .SPSS22 .0 was used for conducting the non-parametric test .Then each indicator was analyzed by the Logistic regression and receiver operating characteristic (ROC) curve .Results The levels of VCA-IgM ,AST and ALT in the EBV-DNA positive group were higher than those in the EBV-DNA negative group ,the difference was statistically significant (P<0 .05) . VCA-IgM ,AST and ALT showed a correlation with EBV-DNA (P<0 .05) .The areas under curve (AUC) of VCA-IgM ,AST and ALT single indicator detection curve were 0 .803(95% CI:0 .735 -0 .872) ,0 .788(95% CI:0 .708 -0 .868) ,and 0 .752(95% CI:0 .671-0 .832) ,respectively ;AUC of 3-indicator combined detection were 0 .830(95% CI:0 .765 -0 .896) ,which was high than AUC of each single indicator detection .Conclusion Among the indicators in EBV current infection ,VCA-IgM is of great diagnostic value ,and is superior to AST or ALT .Furthermore ,the combined detection of these three indicators is better than single indicator detection ,which contributes to the diagnosis and prevention of EBV infection and other complicating infection .
5.Clinical Analysis of Serum Pepsinogen, Gastrin-17 and Helicobacter Pylori Antibody Typing in Physical Examination

Objective To investigate the infection status of Helicobacterpylori (HP) in the physical examination population and the contents of pepsinogen Ⅰ (PGⅠ),pepsinogen Ⅱ (PGⅡ) and gastrin-17 (G-17) in serumand the association with HP infection.Methods In this study,781 cases of physical examination were included.Serum HP antibody typing,PGl,PGⅡ and G-17 were measured by Helicobacter pylori antibody kit and enzyme-linked immunosorbent assay.The levels of PGⅠ,PGⅡ,PGⅠ/PGⅡ and G-17 were analyzed by SPSS22.0 analysis system,and the difference of HP infection rate between HP infection group and non-infected patients and different types of infection was analyzed.Analysis of the infection rate and the difference of each age group.Results There was no significant difference between the two groups (x2 =0.284,P=0.594),and there was significant difference between different age groups (x2 =8.523,P=0.014).The levels of PGⅠ,PGⅡ,PGR and G-17 in serum were statistically significant (Z=8.616~ 14.125,P=0.000) compared with those of HP positive and negative.There were significant differences in serum PGⅡ and PGR levels between HP Ⅰ type and HP Ⅱ type infection (Z=3.444,3.385,P=0.001).The levels of PGⅠ and PGⅡ in serum were significantly higher than those in other groups (Z=5.012,4.478,P=0.000).PGⅠ and PGⅡ were higher in women than in females (Z=0.444~0.941,P>0.05),with the increase of age,PGⅠ,PGⅡ and G-17 has an upward trend,while PGR showed a downward trend.Conclusion The level of serum PG was closely related to gender,age and HP infection,and was closely related to HP infection classification.The combination of HP antibody typing and serum PG and G-17 as a routine test of gastric function is of great significance in the screening and evaluation of early gastric diseases.
6.Comparison of clinical outcomes between microsurgical lumbar discectomy and microendoscopic discectomy

ObjectiveTo make a comparison between microsurgical lumbar discectomy(MSLD) and microendoscopic discectomy(MED) in terms of methods,feathers,and effectiveness for lumbar disc herniation (LDH).MethodsA prospective clinical review was conducted.From January 2006 to December 2009,900patients with single segment lumber disc herniation were randomly divided into the MSLD group and the MED group.There were 450 patients in each group.Comparison would be made in terms of the length of skin incision,the operative time,amount of bleeding,incidence of complication,duration of hospitalization,the time of recovery to ordinary work or life,pre- and post-operative assessment based on the criteria of visual analogue scales (VAS),and the Oswestry disability index(ODI).ResultsThe mean lengths of skin incision were (3.8±1.1) cm and (2.4±0.7) cm for MSLD and MED respectively; the operative time were (51.0±14.2) min and (62.0±16.3) min; and the blood loss were (60±35) ml and (106±43) ml,which showed a significant difference(P<0.05).There was no significant differences in terms of the hospitalization time and the time of recovery to ordinary work or life between the two groups (P>0.05).The results of VAS and ODI of two groups also showed no significant difference at final follow-up (P>0.05).As for the complications,the incidence of dural tear,acute hematomas of sacrospinalis,nerve roots and cauda equina injury and recurrence in MSLD group were much lower than that of MED group (P<0.01).There was no incidence of wrong segment,greater artery injury,or postoperative infection in each group.ConclusionThe clinical effects of both minimal invasive methods are satisfactory.However,MSLD has advantages of simpler maneuvering,shorter learning curve,and less complication than MED.
7.Minimum apparent diffusion coefficient value of DWI in the diagnosis of ductal carcinoma in situ and invasive cancer of breast

Objective To explore the value of the minimum apparent diffusion coefficient (ADC-min) value in the diagnosis of ductal carcinoma in situ (DCIS) and invasive cancer of breast.Methods One hundred and forty nine cases of breast cancer verified by histopathology were included in this retrospective study.All the patients underwent dynamic contrast enhanced magnetic resonance imaging (DCE-MRI) and diffusion weighted imaging (DWI) before the biopsy.The ADC min value and its correlation with invasive ductal carcinoma(IDC),DCIS and IDC-DCIS were analyzed.Results The mean ADC-min values for IDC,IDC-DCIS and DCIS were (0.95±0.16)×10-3 mm2/s,(1.07±0.13)×10-3 mm2/s and (1.24±0.18)×10-3 mm2/s,respectively.The ADC-min value of the three groups showed an increasing trend and there were significant differences (F=32.08,P<0.01).The optimal cutoff ADC-min value was 1.02 × 10-3 mm2/s to differentiate DCIS from invasive cancer with a sensitivity of 95.0% and a specificity of 63.6%.Conclusion The ADC min values are significantly different among IDC,IDC-DCIS and DCIS.It may be used as a reliable tool to differentiate DCIS and invasive cancer of breast.
8.Effect of xinmailong injection on NGAL, hs-cTnT and RAAS in elderly patients with type 2 cardio-renal syndrome

Objective Observation of the effects of Xinmailong injection on NGAL, hs-cTnT and RAAS in elderly patients with type 2 cardio-renal syndrome (CRS) , and EvaluatIon of the clinical efficacy and safety.Methods A total of 86 elderly patients who were collected from November 2015 to February 2017 were diagnosed as type 2 CRS in our department of geriatrics. According to the random number table method, they were randomly divided into two groups. Control group (43 cases) were treated with conventional medical therapy and xinmailong group (43 cases) were treated with xinmailong injection with a dose of 5 mg/kg twice a day for 15 days. The value of sneutrophil gelatin-associated apolipoprotein (NGAL) , hypersensitive troponin T (hs-cTnT) , brain natriuretic peptide (BNP) , plasma renin activity (PRA) , and angiotensin Ⅱ (Ang Ⅱ) , aldosterone (ALD) , left ventricular ejection fraction (LVEF) , six-minute walking test and other indicators were measured before and after treatment. Results After treatment, the value of NGAL, hs-cTnT, BNP, PRA, AngⅡ and ALD decreased in both groups, the value of LVEF and six-minute walking distance increased compared with those before treatment (P <0.05); The above indicators in the xinmailong group experienced a more significant alteration than in control group in the same period (P < 0.05); no side effect occurred in both two groups during the experiment. Conclusion Xinmailong injection can reduce the levels of NGAL, hs-cTnT and BNP in elderly patients with type 2 cardio-renal syndrome, improve heart and kidney function, and have curative positive effect and good safety. This study underlined the mechanism of Xinmailong injection may be related to the inhibition of RAAS activity.
9.Evaluation of Circulating Endothelial Progenitor Cells in Abdominal Aortic Aneurysms after Endovascular Aneurysm Repair

Background and Objectives:
Circulating endothelial progenitor cells (EPCs) participate in vascular repair and predict cardiovascular outcomes. The aim of this study was to investigate the correlation between EPCs and abdominal aortic aneurysms (AAAs).
Methods:
and Results: Patients (age 67±9.41 years) suffering from AAAs (aortic diameters 58.09±11.24 mm) were prospectively enrolled in this study. All patients received endovascular aneurysm repair (EVAR). Blood samples were taken preoperatively and 14 days after surgery from patients with aortic aneurysms. Samples were also obtained from age-matched control subjects. Circulating EPCs were defined as those cells that were double positive for CD34 and CD309. Rat models of AAA formation were generated by the peri-adventitial elastase application of either saline solution (control; n=10), or porcine pancreatic elastase (PPE; n=14). The aortas were analyzed using an ultrasonic video system and immunohistochemistry. The levels of CD34+/CD309+ cells in the peripheral blood mononuclear cell populations were measured by flow cytometry. The baseline numbers of circulating EPCs (CD34+/CD309+) in the peripheral blood were significantly smaller in AAA patients compared with control subjects. The number of EPCs doubled by the 14th day after EVAR. A total of 78.57% of rats in the PPE group (11/14) formed AAAs (dilation ratio >150%). The numbers of EPCs from defined AAA rats were significantly decreased compared with the control group.
Conclusions
EPC levels may be useful for monitoring abdominal aorta aneurysms and rise after EVAR in patients with aortic aneurysms, and might contribute to the rapid endothelialization of vessels.
