Polycystic ovary syndrome (PCOS) is a common endocrine metabolic disorder of reproductive age women,often accompanied by obesity,hyperandrogenism,hyperinsulinemia,insulin resistance (IR),and other metabolic syndrome (MS).Oxidative stress (OS) is considered as a potential inducing factor in the pathogenesis of PCOS at present.Glucagon-like peptide-1 (glp-1) receptor agonist and analogues as new therapeutic agents in the treatment of type 2 diabetes mellitus (T2DM),can lose weight,improve IR,resist OS and reduce the expression of inflammatory cytokines,thus they have been widely used in the clinical application of patients with PCOS.The author reviews the latest advances of glucagon-like peptide-1 on polycystic ovary syndrome.
2.The study of early enteral nutrition with different routes after double incisions for the therapy of esophageal cancer

Objective:To compare the feasibility,complications and advantages of the nose-intestine tube and the jejunostic tube in early enteral nutrition after double incisions for the therapy of esophageal cancer.Methods: 52 postoprative patients were divided into three groups.The nose-intestine tube group(A group,n =16) and the jejunostic tube group(B group,n = 21) received the enteral nutrition(Nutrison Fibre) and the conrtrol group(C group,n = 15) received intravenous isotonic glucose solution and oral liquid diet after the bowel movement recovery.The blood glucose,the function of liver and kidney,electrolytes and nutritional status were observed.The recovery of bowel movement and other digestive symptoms such as abdominal pain,distention,diarrhea,nausea and vomiting were observed during the period of study.Some complications such as acute intestinal obstruction,pnumonia and rhinolaryngitis were observed during the period of study.Results: The time for placing the jejunostic tube was faster than that for placing the nose-intestine tube(P
3.Effect of video-assisted thoracoscopic operation on postoperative cardiopulmonary complications in elderly patients with non-small cell lung cancer

Objective To compare the incidence of cardiopulmonary complications between treatment of video assisted thoracoscopic operation and traditional thoracotomy operation in elderly patients with non-small cell lung cancer.Methods 122 cases of elderly patients with non small cell lung cancer were treated in our hospital from Mar 2010 to Jun 2014,and randomly divided into observation group (therapeutic lobectomy by video assisted thoracoscopic operation) and control group (therapeutic lobectomy by traditional thoracotomy operation) (n=61,each).The postoperative cardiopulmonary complications and relevant indexes of cardiopulmonary function were analyzed and compared between the two groups.Results Before operation,there was no significant difference in the relevant indexes of cardiopulmonary function between the two groups (P>0.05).After operation,the forced expiratory volume in one second (FEV1) and maximal ventilatory volume (MMV) were higher and the heart rate was lower in observation group than in control group (t=3.682,2.243,5.677,all P<0.05).The incidence of cardiopulmonary complication was lower in observation group than in control group (13.1% vs.27.9%,x2 =4.075,P=0.044).Conclusions Video assisted thoracoscopic operation is better for the recovery of cardiopulmonary function,and can reduce cardiopulmonary complication in elderly patients with non-small cell lung cancer.
4.Prognostic factors of surgical treatment for elderly patients with non-small cell lung cancer

Objective To analyze prognostic factors of surgical treatment for elderly patients with non-small cell lung cancer,in order to provide a theoretic basis for improving the prognosis of patients with non-small cell lung cancer.Methods Clinical data of 145 elderly patients with non-small cell lung cancer who had received surgical treatment at our hospital from January 2012 to September 2012 were retrospectively analyzed.The survival rates of patients were investigated.Results The median survival time was 31.5 months,and 1-year,2-year and 3-year survival rates were 61.4%,45.5% and 40%,respectively.Single factor analysis showed that lesion size,number of metastatic lymph nodes,clinical stage,pathological type and radiotherapy were among the factors affecting prognosis.Multiple COX regression analysis showed that a lesion size >3cm,number of lymph nodes with metastasis > 2 and pathological type were independent risk factors for patient prognosis,and radiotherapy was a protective factor for prognosis.Conclusions Surgical treatment has good curative effects on non-small cell lung cancer in elderly patients.Lesion size,number of lymph nodes with metastasis,pathological type and radiotherapy are important factors affecting prognosis,and can be used for prognosis assessment.
5.Clinical studies on early postoperative enteral nutrition in patients with esophageal cancer

Objectives:To investigate the safety,feasibility and clinical effects of early EN(EEN) in postoperative patients with esophageal cancer. Methods:On the first postoperative day,40 patients were randomly divided into EEN group and TEN group(traditional enteral nutrition group), 20 patients per group.A series of parameters including plasma protein and lymphocyte count were tested before and after nutritional therapy.The complications,vital signs and resumption of gastrointestinal functions were carefully observed and recorded simultaneously. Results:Neither mortality nor serious morbidity occurred in all patients during the period of study.The concentrations of serum prealbumin and transferrin were significantly elevated after nutritional support in EEN group.The lymphocyte count was also higher in EEN group.The bowel movement resumed significantly earlier in EEN group than in TEN group. Conclusions:Postoperative EEN for esophageal cancer is safe and feasible,not only improving nutritional status and maintaining gastrointestinal function,but also ameliorating immunological function.
6.Analysis of the influencing factors of cervical dilatation balloon in the induced labor in primipara

Objective:To investigate the influencing factors and to build a model for predicting successful labor induction with cervical ripening balloon (CRB) in primipara.Methods:312 cases of primipara in late trimester of pregnancy induced by CRB between January 2018 and April 2019 in the First Affiliated Hospital of Shenzhen University were reviewed retrospectively. Factors including gravidity, age, body height, body weight, body mass index (BMI), the cervical Bishop score, neonatal birth weight, pregnant complication and other factors were collected to analyze the effect on successful labor induction.Results:Pregnancy times, cervical Bishop score and height in primipara were positively correlated with the success rate of induced labor, while BMI and neonatal weight were negatively correlated with the success rate of induced labor. The area under receiver operating characteristic (ROC) curve was 0.849 and diagnostic threshold was 0.725, with a sensitivity of 80.7% and a specificity of 75.7%.Conclusions:Among the primipara, the gravidity and cervical Bishop score are the most significant factors influencing the successful labor induction. These factors can be used as the predictive indexes for the outcome of labor induction along with pregnancy times, height, BMI and neonatal birth weight, which is beneficial to reduce the artificial interventions and improve the success rate of labor induction.
7.Clinical Research on Acupuncture Treatment of Diabetic Peripheral Neuropathy

Purpose: To investigate the mechanism of acupuncture treatment of diabetic peripheral neuropathy. Methods: Acupuncture therapy was used to treat diabetic peripheral neuropathy, and compared with oral calcium antagonist and vitamin therapy by random control observation.Electromyography was performed for analysis at the same time. Results: Acupuncture treatment alleviated symptoms such as extremity numbness, pain and paresthesia in varying degrees in diabetic patients with peripheral neuropathy. The results of electromyography showed a marked improvement in motor and sensory conduction velocities. Conclusion: It is indicated that acupuncture therapy is markedly superior to oral calcium antagonist and vitamin therapy in clinical effect on diabetic peripheral neuropathy, and electromyographic recovery.
8.Changes of high sensitive-C reactive protein and cardiac troponin I levels in patients with type 2 diabetes during glucose-lowering therapy

Objective To compare the changes of high sensitive-C reactive protein (hs-CRP) and cardiac troponin Ⅰ ( cTn Ⅰ ) levels before and after intensive therapy in patients with type 2 diabetes,and to find out the reasonable glucose-lowering rate.Methods One hundred and thirty-two cases of type 2 diabetes( T2DM group) and 135 cases of type 2 diabetes with coronary heart disease( T2DM+CHD group) received intensive therapy.After testing hs-CRP and cTn Ⅰ levels,the variations were analyzed.Results The ranges of the change in cTn Ⅰ and hs-CRP levels were different under four glucose-lowering rates in the T2DM+CHD group( P<0.05 ).cTn Ⅰ and hs-CRP levels were higher than those before intensive therapy in the T2DM+CHD group with glucose-lowering rate greater than 4.0mmol· L-1 · d-1.The other two subgroups with glucose-lowering rate less than 4.0 mmol· L-1 · d-1 showed decreased cTn Ⅰ and hs-CRP levels.While at the end of 3 months follow-up,cTn Ⅰ and hs-CRP levels were all significantly lower than those before intensive therapy in four subgroups ( P<0.05 ).Conclusions The increase of cardiovascular events after intensive therapy may be due to excessively fast glucose-lowering rate.The reasonable glucose-lowering rate for patients with type 2 diabetes should depend on whether there is accompanying coronary heart disease.For type 2 diabetes with coronary heart disease,excessively fast glucose-lowering rate could lead to acute rise ofcTn Ⅰ and hs-CRP levels,which causes myocardial injury.The mechanism of myocardial injury resulted from excessively fast glucose-lowering rate may be due to activation of the inflammatory pathway.In type 2 diabetes with coronary heart disease,long-term good control of blood glucose could alleviate inflammatory response and cardiac damage resulted from excessively fast glucose-lowering rate.
9.The influence of glucose-lowering rate on CKMB and muscle hemoglobin in type 2 diabetes mellitus with coronary heart disease

One hundred and fifty-one type 2 diabetic patients with coronary heart disease ( T2 DMC) and 142 cases of type 2 diabetes mellitus were included for analyzing the influence of different glucose-lowering rates on MB isoenzyme of creatine kinase (CKMB) and muscle hemoglobin level changes to search for the rational glucose-lowering rate.The level of CKMB in type 2 deabetes mellitus group was significantly lower( P<0.05 ) at follow-up than that before and after intensive therapy.In type 2 diabetes mellitus group,when the fasting or postprandial glucose-lowering rate was not greater than 6 mmol· L-1 · d-1,the level of CKMB and muscle hemoglobin were significantly lower at follow-up than that before intensive therapy ( P<0.05 ).When the fasting glucose-lowering rate is greater than 6 mmol· L-1 · d-1,the level of CKMB is significantly higher after intensive therapy than that before glucose-lowering ( P<0.05 ).In T2DMC group,when the fasting or postprandial glucose-lowering rate was not greater than 4 mmol· L-1 · d-1,the level of CKMB and muscle hemoglobin was significantly lower at follow-up than that before intensive therapy(P<0.05 or P<0.01 ),buthigher at follow-up when the fasting glucose-lowering rate was greater than 4 mmol· L-1 · d-1(P<0.05).
10.Oxygen saturation in the retinal vessels of glaucoma and normal eyes

Objective To observe the difference of retinal vessel oxygen saturation in glaucoma and normal eyes.Methods A cross sectional study design was performed.Fifty eyes of 30 glaucoma patients (glaucoma group) and 41 eyes of 27 age-and sex-matched healthy subjects (control group) were included.Retinal vessel oxygen saturation was measured with a spectrophotometric retinal oximeter in darkness and visual fields were obtained by Humphrey filed analyzer.The glaucoma eyes were divided into two groups:mean defect (MD) <6 dB (28 eyes) and MD≥6 dB (22 eyes) according to mean defect of visual field.Results Retinal arteriolar oxygen saturation values in glaucoma group and control group were (94.52 ±6.51) % and (93.47±6.30) % respectively.No statistical difference was found in retinal oxygen saturation in arterioles (H =-0.949,P =0.343).Retinal venous oxygen saturation values in glaucoma group and control group were (57.57 ± 7.96)% and (52.60 ± 7.70) % respectively.The retinal venous oxygen saturation values in glaucoma group was higher than that in control group (H=-3.318,P=0.001).The retinal arteriovenous difference in glaucoma group and control group were (36.59± 4.69)% and (42.41 ±6.73) % respectively.The retinal arteriovenous difference in glaucoma group was lower than that in control group (H=-4.148,P<0.01).The retinal arteriolar oxygen saturation values in glaucoma eyes with MD<6 dB and MD≥6 dB were (93.38 ± 6.33)% and (95.71 ± 6.54)% respectively,with no statistical difference (H=-1.857,P=0.063).Retinal venous oxygen saturation values in glaucoma eyes with MD<6 dB and MD≥6 dB were (54.83 ± 6.10) % and (6 1.07 ± 8.79) % respectively.The retinal venous oxygen saturation values in MD≥ 6 dB glaucoma eyes was higher than that in MD< 6 dB glaucoma eyes (H =-2.599,P=0.009).The retinal arteriovenous difference in glaucoma eyes with MD<6 dB and MD≥6 dB were (38.12± 4.34) % and (34.64 ± 4.49) % respectively.The retinal arteriovenous difference in MD≥6 dB glaucoma eyes was lower than that in MD<6 dB glaucoma eyes (H=-2.463,P<0.05).Conclusions Compared with healthy eyes,there is no change in the retinal arteriolar oxygen saturation,but the retinal venous oxygen saturation is higher and the retinal arteriovenous difference is lower.This feature is more obvious in MD≥6 dB glaucoma eyes.
