1.Analysis of the dection results of atypical lymphocytes in peripheral blood of patients with influenza

Objective To analyze testing results and values of blood heterotypic lymphocyte in flu patients.Methods 100 cases of influenza patients and 100 healthy people were collected.The observation group for flu patients and control group of healthy people.Extraction of peripheral blood in patients with microscopic examination observing leukocyte,neutrophil,lymphocyte,intermediate cells,atypical lymphocytes under a microscope,and calculated the percentage in the patients in the two group cells to make analysis and comparison.Ratio in peripheral blood of patients and analysis of specific lymphocytes were also calculated between different ages.Results Peripheral blood lymphocytes in observation group was (11.1 ± 2.6) %,which was significantly higher than than of the control group [(1.5 ± 0.5) %].The difference was statistically significant (chi-square =16.9,P < 0.05) ; White blood cells,the ratio of neutrophils,lymphocytes in observation group were obvious differenent compared with the control group(t/chi square are:13.2,11.3 and 12.6,P < 0.05) ;Three ages of influenza patients peripheral blood lymphocytes of the opposite sex ratio has no difference(P > 0.05).Conclusion Detection of peripheral blood lymphocyte heterosexual has great significance to the diagnosis of influenza with simple,convenient and feasible characters.It is worth to be used widely in clinical practice.
2.Discussion of iron metabolism abnormality and related indicators in hepatitis B

Objective To study the relationship among iron metabolism indicators in the patients with hepatitis B and to analyze their clinical significance.Methods Serum ferritin,iron and hepcidin were detected in 96 cases of hepatitis B and 58 negative control subjects.The above indicators detected results and their relationship were analyzed.Results Compared with the control group,ser-um ferritin and serum iron concentration in the hepatitis B group were increased,he difference was statistically significant (P <0.05);serum ferritin and serum iron concentration in the hepatitis B group demonstrated the positive correlation (P <0.05);serum ferritin in the patients with hepatitis B was negatively correlated with hepcidin (P =0.018).Conclusion The patient with hepatitis B often have the iron metabolism abnormality,and the iron metabolism indicators is expected to be an assessment indicator of treat-ment and prognosis in the patients with hepatitis B.
3.Highly effective transformation of Cryptococcus neoformans

Objective: To establish a highly effective transformation method of Cryptococcus neoformans. Methods: Special reagents was used to make C. neoformans take in external DNA under given condition. This chemical transformation result was compared with that of electrotransformation. The feasibility of this chemical transformation was tested by plasmid stability test. Results: The efficiency of this chemical transformation was more than 103 transformants/?g plasmid DNA, far more than that of the traditional electrotransformation. Conclusion: An appropriate transformation method is established for C. neoformans transformation, which has high transformation efficiency.
4.Study on filtering blebs imaging by anterior segment optical coherence tomography

Objective To seak a available way of evaluating morphology of filtering blebs after trabeculectomy is very important for forecasting the successful rate of surgery.At the present time,filtering blebs were commonly evaluated by using the slim lamp microscope,but it did not to exactly reflect the filtering passage.This study aimed to evaluate the morphology of filtering blebs after trabeculectomy with anterior segment optical coherence tomography (AS-OCT).MethodsThis was a prospective study.Sixty-nine eyes of 53 patients who had previously undergone trabeculectomy were selected in this study,including 25 eyes with open-angle glaucoma,38 eyes with close-angle glaucoma and 6 eyes with secondary glaucoma.These filtering blebs were classified into functional type (typeⅠandⅡ) and nonfunction type (type Ⅲ and Ⅳ) under the slit lamp microscope based on the van Buskirk grading scales.Intra-bleb morphology and structure were characterized as diffuse-like,cystic-like,encapsulating-like and flatten-like by AS-OCT in reference to the Leung method.The consistency between the two methods was evaluated by the Chi-Square test.Written informed consent was obtained from all the patients before surgery.ResultsThe average follow-up time was 10.78±11.0 months.All the patients finished the examination during the follow-up duration.The average intraocular pressure was(14.9±4.5)mmHg during the observation period.There were 38/69 (55.1%) functioning blebs and 31/69 (44.9%) non-function blebs under the slim lamp microscope.AS-OCT imaging showed diffuse-like blebs in 26 eyes (15.9%),cystic-like blebs in 11 eyes (27.5%),encapsulating-like blebs in 19 eyes (18.8%) and flatten-like in 13 eyes with the intraocular pressure of(13.16±3.77)mmHg,(15.36±2.92)mmHg,(15.77±5.07)mmHg and (16.62±5.33)mmHg,respectively,showing a significant difference among them (F=3.32,P<0.05).These patterns of different OCT presented a good consistency with the clinical outcome (χ~2=0.03,P=0.86).ConclusionAS-OCT allows the observation of filtering blebs after glaucoma surgery.It is probably to visualize the internal structure of filtering bleb and deep sclerectomy.Functional and dysfunctional filtering blebs delivery gives different OCT patterns.This could be a new way to assess the postoperative healing process.
5.The correlation between postural hypotension and myocardial infarction in the elderly population

Objective To investigate the association between the systolic/diastolic orthostatic hypotension (OH-S/OH-D) and myoeardial infarction (MI) in the elderly.Methods Health screening physical examination were carried in 1081 subjects without MI aged over 65 years in Guangzhou Military region.The orthostatic blood pressure and heart rate were measured in supine position after resting for more than 5 minutes and at 0 and 2 minutes after standing.All the cases were divided into systolic or diastolic group on the basis of definition of orthostatic hypotension and followed up by telephone or inpatient medical records with mean period of 315.8 days.The primary endpoint was MI occurrence.Results The prevalence of OH in this cohort was 24.5% ( OH-S/OH-D:19.3%/17.2% ).Significant differences in the occurrence of OH and OH-S were found in the elderly and the very elderly subjects( ≥80 years) (26.1% vs 20.1%,P=0.045; 21.0% vs 14.6%,P =0.018),while no difference was found in OH-D.The prevalence of MI in the OH positive subjects was significantly higher than that in the OH negative subjects,as well as in OH-S or OH-D group.After adjustment of age,supine blood pressure,creatinine and cerebrovascular history by logistic regression,the association was observed between MI and OH ( HR 15.635,95 % CI 3.299-74.091,P=0.001),OH-S(HR 8.760,95%CI2.487-30.851,P=0.001)and OH-D(HR 3.889,95% CI 1.097-13.790,P =0.035 ).Conclusion OH-S and OH-D hypotension are robust predictors for MI in the elderly.
6.Clinical application of transcatheter closure of secundum atrial septum defect with Amplatzer occluder in patients over 50 years of age

Objective To elucidate the efficacy and safety of transcatheter closure of secondary atrial septal defect (ASD) with Amplatzer septal occluder (ASO) in people over 50 years of age. Methods Retrospective comparison of 41 patients (14 males, 27 females) with ASD over 50 years of age ranging from 50 to 75(mean age 55.5?7.6, elder group) was conducted with 132 patients younger than 50 years(control group). After diagnosis of ASDs and evaluation of pulmonary artery systolic pressure and right atrial and ventricular dimensions by transthoracic echocardiography (TTE), all patients underwent transesophageal echocardiography (TEE) for complete assessment of ASD size, margins and anatomic relationship of the defect before closure of ASD. Each case was treated with ASO through the percutaneous transcatheter procedure under fluoroscopy and TTE or TEE. Early follow up(3 months) by echo was taken after the intervention. Results There was no difference of mean defect diameter measured by TEE and the balloon-stretched defect diameter of the ASDs between 2 groups. 7.Evaluation of the therapeutic effects of trabeculectomy combined with Schlemm canal suture implantation in primary open angle glaucoma

Objective To evaluate the therapeutic effects and safety of trabeculectomy combined with Schlemm canal suture implantation in patients POAG). Design Retrospective, noncomparative case series. Participants 11 patients (19 eyes)with POAG. Methods Two layers of scleral flap were made and the deeper one was dissected. The outer wall of Schlemm canal was cut away. 6-0 nylon thread was implanted between the two cut ends of the canal. Penetrating trabeculectomy was performed and followed with peripheral iridectomy. Superficial scleral flap was sutured. Main Outcome Measures The postoperative observation included intraocular pressure (IOP), formation of the anterior chamber, inflammatory reaction, visual acuity, complications and situation of filtering bleb. Results The mean postoperative follow-up period was 18.05?12.78 months (ranging from 6 to 48 months). There was significant statistical difference between preoperative and postoperative IOPs(31.23?9.72mmHg VS 15.11?6.13mmHg) (t=6.769, P=0.000). The postoperative investigation of IOP for 11 cases (19 eyes) after using medicine showed that IOP≥22mmHg only 1 case. The efficiency was 94.74%. Filtering blebs formed in 6 cases (8 eyes)(42.11%), including the case with IOP≥22mmHg. There was no significant difference in change of preoperative and postoperative IOP in patients with or without filtering blebs with t-test (t=-1.662, P=0.071). The anterior chamber formed well in each case postoperatively. Hyphemia appeared in 4 eyes (21.05%). Hypertonia oculi (IOP≥22mmHg) and hypotonia oculi (IOP≤6mmHg) appeared occasionally in 3 eyes(15.78%) and 11 eyes(57.89%) respectively. Postoperatively,no fibrinous exudates in anterior chamber, choroidal detachment and other complications occurred in any case. Conclusions The control of IOP appears to be good after the surgery of trabeculectomy combined with Schlemm canal suture implantation. It is a promising approach for glaucoma and good with fewer postoperative complications, easier postoperative management and deserve further development.
8.Ultrasound imaging findings of anatomical relationship between femoral artery and vein in children of different ages

Objective To investigate the ultrasound imaging findings of anatomical relationship between femoral artery and vein in children of different ages.Methods Sixty-five children aged 4 months-7 years were enrolled in this study.The children were divided into 3 age groups: group Ⅰ< 1 yr;group Ⅱ 1-3 yr and group Ⅲ> 3,≤ 7 yr.A protable ultrasound machine was used.The probe was placed at the level of inguinal ligament and 2 and 4 cm below inguinal ligament.The children were placed in supine position.The legs were placed in 2 positions:(1)extended and in standard anatomical position and(2)flexed and 45° abducted and 45° laterally rotated.Results The examination showed that at the level of inguinal ligament,the femoral vein lay behind and lateral to femoral artery in 91% of children.At the level of 4 cm below inguinal ligament,the femoral vein lay posterior and lateral to the femoral artery in all children.When the leg was placed in abducted and laterally rotated,the depth of femoral vein was reduced and the vein was less overlapped by artery in all children,especially in preschool children.Conclusion At the level of 4 cm below inguinal ligament,the femoral vein lies posterior and lateral to the femoral artery in children.When the leg is placed in abducted and laterally rotated,the depth of femoral vein is reduced and the vein is less overlapped by artery.It is indicated that femoral vein puncture should be performed at the level of 4 cm below inguinal ligament with the leg flexed and abducted in all children,especially in preschool children.
9.Comparative study of intravenous thrombolysis treatment of cardiogenic cerebral infarction and noncardiac cerbral infarction

Objective To compare the efficacy and safety of intravenous thrombolysis on cardiogenic cerebral infarction and noncardiac infarction by recombinant tissue plasminogen activator (rt-PA). Methods Comparations of NIHSS, mRS and adverse events before and after treatment were made between the cardiogenic group and the noncardiac group. Results No significant differences in the NIHSS and mRS were found between the two groups. The incidence of brain hernia and dermatorrhagia in the cardiogenic group was higher than that in the noncardiac group. Conclusion Rt-PA therapy in cardiogenic cerebral infarction was effective and safe in spite of higher incidence of hemorrhage and brain hernia.
10.Clinical analysis of prenatal diagnosis of choledochal cyst in 37 infants

Objectives To explore the clinical intervention of prenatal diagnosed congenital choledochal cyst in infants. Methods 37 infants with prenatally diagnosed congenital choledochal cyst from September 2006 to February 2013 were di-vided into early (0-3 month) operation group (Group A, n=20) and late (>3 month) operation group (Group B, n=17) according to the timing of surgery. The clinical characteristics, the preoperative and postoperative liver function, postoperative com-plications and pathological examination of liver biopsy were retrospectively analyzed. Results Five infants in group A and 2 infants in group B became jaundiced after birth. No statistical difference was found in alanine aminotransferase (ALT) and aspartate aminotransferase (AST) between two groups (P>0.05). Preoperative and postoperative levels of total bilirubin (TBIL) and direct bilirubin (DBIL) in group A were signiifcantly higher than those in group B (P<0.05). Two cases had anastomotic stricture and one case had bile leakage in group A while no postoperative complication was found in group B. Liver biopsy re-sults showed that there were 11 cases of biliary cirrhosis, including 4 cases in group A (36.36%) and 7 cases (63.64%) in group B, but no statistical difference in incidence of cirrhosis between two groups (P=0.160). Conclusions Infants with prenatally diagnosed congenital choledochal cyst should be closely observed. Once jaundice, clay-coledstool, increased ALT and AST, fast growing enlarged cysts were found, infants should be surgically treated as soon as possible to reduce liver damage and the incidence of liver cirrhosis.
