Objective To summarize the features of multiple sclerosis (MS) in China. Methods The clinical manifestations and inducements of 413 MS patients are comprehensively analyzed using clinical statistical method. Other parameters are also analyzed by electromyography and brainstem auditory evoked potential. Results MS in China mainly invades young and middle-aged people, with a higher ratio in female than in male. MS was found mostly acute or subacute in onset and usually due to an inducement mainly consisting of common cold, fever, cyesis and fatigue. The first seen symptoms were various and complicated, but mainly showed different types of visual disturbance such as the acuity of vision decrease, constriction of visual field and diplopia. Limb paralysis, sensory abnormality and visual disturbance were found the commonest symptoms. Lhermitte’s sign was common. Paroxysmal symptoms as algospasm and seizure were relatively common. Electromyography and brainstem evoked potential showed that peripheral nervous system damage might occur in MS. Spinal cord and visual nerve were found the mostly invaded according to the clinical data. Conclusion The features of MS in China should be different from those in the western countries.

Objective TO explore the etiology and pathogenesis of multiple sclerosis. Methods The information of pa-tients from 413 MS was analyzed comprehensively by the clinical statistical methods. Results Common cold was the com-monest factor causing MS, and fever,fatigue and childbirth were also the common causes. Immunological disturbance exis-ted in MS. CD~+_4 cell decreased in most of MS patients, but the distribution of CD~+_8 cell was not regular. The level of sexhormones was abnormal in MS,but it was not the same in male and female. There were three patients having family histo-ry of MS. Conclusion Virus infection,sex hormones and heredity factors are all related to MS through facilitating immu-nological disturbance.

Objective To observe the effect of Huangqi (Radix Astragali seu Hedysari) Injectio on the abnormal expression of aquaporin-1 (AQP-1) caused by glucose lactate peritoneal dialysis solution. Objective: To analyze the clinical value of modified endoscopic nipple resection in duodenal papillary adenoma resection Methods: 40 patients with duodenal papillary adenoma were randomly divided into observation group (20 cases) and control group (20 cases)according to the random number table. The patients of control group were treated by conventional resection of papillary adenoma, while the patients of observation group were treated by modified endoscopic nipple resection. The resection rate and the implantation of stent between the two groups were compared. Besides, the occurrence rate of complication and postoperative follow-up between the two groups also were compared. Results: The complete resection rate of observation group (95.00%) was not significant difference with that of control group (90.00%), while en bloc resection rate of observation group (85.00%) significant higher than that of control group (55.00%) (x2=4.28, P<0.05).There was not statistically significant difference between the two groups about the implantation of stent (x2=0.36, P>0.05). In postoperative three months, the difference of recently complication between the two groups was significant (x2=6.14, P<0.05). While the difference of long-term complication between the two groups was no significant (x2=0.36, P>0.05) in more than postoperative three months. After the follow-up of one year, the recurrence rate of observation group (10.34%) was no significant with that of control group (14.29%)(x2=0.17, P>0.05). Conclusion: The modified endoscopic nipple resection can enhance the en bloc resection rate, and reduce the incidence of complications and the recurrence rate. Besides, it has many dominances including higher feasibility in clinical practices. Therefore, its promotion is worthy.

Objective To investigate the clinicopathological features of human immunodeficiency virus (HIV) negative plasmablastic lymphoma (PBL) with no-immunosuppression,so as to accelerate the understanding for this group of disease.Methods The histological features of 6 HIV-PBL patients with no-immunodeficiency were retrospectively analyzed.Epstein-Barr virus (EBV) status was detected by in situ hybridization.Then,immunohistochemistry and fluorescence in situ hybridization (FISH) method were used to determine the immunophenotype,latent status of EBV and MYC translocation in PBL,respectively.Results HIV-PBL showed monotonous proliferation of plasmablastoid or immunoblast-like cells.Giant cells and necrosis could be observed,with less reactive cells in the background and higher mitoses.All the cases had EBV infection and type Ⅰ latency status of EBV (LMP1-/EBNA2-),and expressed terminal B-cell differentiation immunophenotype (CD20-/CD3-/CD138+/Kappa or Lambda+).Six HIV-PBL patients were elderly (median age was 69.5 years old),had equal incidence of PBL between male and female and showed high frequency of involvement of extranodal and extraoral lesion sites (6 cases and 5 cases,respectively).Median sutvival was 25.5 months.In addition,3 HIV-PBL patients had IGH/MYC translocations.Conclusions HIV-PBL is a new entity with unique clinical features including no-HIV infection,elderly,EBV positivity,and more involvement in extranodal and extraoral sites.HIV-PBL should be distinguished from HIV+ PBL.

Objective To compare the efficacy of two doses of ambroxol in the treatment of the elderly patients with acute exacerbation of chronic obstructive pulmonary disease (COPD).Methods A hundred and ten cases of the elderly patients with an acute exacerbation of COPD were divided into low-dose group and high-dose group.Low-dose group (56 cases) were given Ambroxol 30 mg,2 times per day for 10 day and high-dose group (54 cases) were given Ambroxol 60 mg,2 times per day for 10 days.Results The effective rate in high-dose group was significantly higher than that in low-dose group.Conclusion High-dose ambroxol has significant effect in the treatment of acute exacerbation of chronic obstructive pulmonary disease.

Objective To investigate the clinical efficacy of intravitreal injection of Lucentis combined with pan retinal photocoagulation and trabeculectomy for neovascular glaucoma(NVG). Methods Seventeen eyes of 15 patients with NVG were treated by 0.5 mg Lucentis intravitreal injection first. Trabeculectomy was performed 7 days later and Pan retinal photocoagulation 2 weeks later. Iris,anterior chamber angle neovascularization,intraocu?lar pressure ,and visual acuity were observed in the following 6 months. Results Iris and anterior chamber angle neovascularization were completely regressed 7 days after the injection in 17 eyes. Six months after photocoagulation, 6 eyes had different degrees of improvement in visual acuity,10 eyes no change in visual acuity and 1 eye decreased from anterior index to immediate manual. There was significant difference in pre?treatment IOP and postoperative IOP( t = 12.75 ,10.26 ,9.73 ,10.77 ,P < 0.01 ). No serious complications were observed after intravitreal injec?tion and trabeculectomy. Conclusions Intravitreal injection of Lucentis could significantly accelerate the regres?sion of iris and anterior chamber angle neovascularization ,which could improve the successful rate of trabeculecto?my and pan retinal photocoagulation. Intravitreal injection of Lucentis combined with trabeculectomy and panretinal photocoagulation is safe and effective for NVG.

BACKGROUND: Precartilaginous stem cells (PSCs) have strong proliferation ability and differentiation potential,but they are instable and prone to differentiate.Importing exogenous gene could immortalize them and leave phenotype character unchanged.OBJECTIVE: To establish immortalized precartilaginous stem cells (PSCs) from neonatal SD rats in vitro for the further related research about the differentiation mechanism and clinical application of precartilaginous stem cells.DESIGN,TIME AND SETTING: Single sample observation.The study was carried out in the Department of Orthopedics.Tongji Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from October 2005 to September 2006.MATERIALS: Neonatal SD rats,irrespective of gender,24-hour old,were used for prepare PSCs.METHODS: By using LipofectamineTM 2000,a gene transfection reagent,plasmid pCMVSV40T/PUR containing the simian virus 40 large T antigene gene (SV40Tag) was transfected into the primary cultured PSCs isolated by immuniomagnetic beads coasted with the second antibody.Colonies were isolated by puromycin selection and expanded by many passages.MAIN OUTCOME MEASURES: Biological character of PSCs; plasmid identification; biological character of transfected cells and identification; RT-PCR; growth curve.RESULTS: Immunomagnetic beads separation system obtains PSCs,which was confirmed as fibroblast growth factor receptor-3 (FGFR-3) positive PSCs.Double restriction enzyme was cut,electrophoresis confirmed pCMV was 3 kb,SV40T was 2.3 kb.A particular anti-puromycin cell clone was acquired,which was confirmed as FGFR-3 positive PSCs.The total RNA was isolated from the positive cell clones,and a 588 bp fragment,which was specific for the SV40T antigene gene,was amplified.The transfected cells were expanded to immortalized cell strain,named as immortalized precartilaginous stem cells (IPSCs).Thepopulation doubling time of IPSCs was (22.98±2.77) hours,no significant effect of subculture,freezing and recovering had been found.CONCLUSION: Precartilaginous stem cells could be isolated from neonatal SD rats,cultured in vitro,and immortalized through the transfection of pCMVSV40T/PUR.
