1.Cross Contamination of Inpatient Bed Sheet: An Evaluation

OBJECTIVE Through studying the bacteria spreading in the in-hospital patients′ mattresses,cotton quilts,and pillows,to analyze the possible factors that could lead to the cross-infections among the(patients).METHODS Using the technique stipulated in the Regulations of Sterilization Techniques(Ministry of Health,2002) GB15982-1995A_3,30 samples were collected from each group of mattresses,cotton quilts,and pillows,and were examined through bacteriological lab investigations.RESULTS There were different types of(bacteria) in the mattresses,cotton quilts,and pillows;the spread of the bacteria was various.CONCLUSIONS The(percentage) of infection in the pillows is higher than the percentage in the mattresses and the cotton quilts.There are different types of bacteria existing in the samples taken,and the spreading of the bacteria varies.
2.Treatment of patellofemoral instability in adolescent by anatomical double-bundle medial patellofemoral ligament reconstruction with double suture anchor technique

Objective:To explore the clinical outcomes of anatomical double-bundle medial patellofemoral ligament (MPFL) reconstruction with double suture anchor technique in treating patellofemoral instability in adolescent.Methods:Twenty-five young people with patellofemoral instability (25 knees) in our department from January 2013 to December 2014 were enrolled for this study.All patients were performed anatomical double-bundle MPFL reconstruction with arthroscopic-assisted suture anchors technique in the patella,and fixed in the femoral socket with absorbable interference screw.All patients are evaluated by different methods,including patient's satisfaction,patellar apprehension test,recurrent subluxation/dislocation,CT assessment of bone tunnel and patellar tilt angle.Lysholm scores,Tegner scores and Kujala scores were recorded at the final follow-up.Results:The mean follow-up was 24 (range 20-40) months.All cases were observed in negative patellar apprehension test.Infection,recurrent subluxation/dislocation and patellar fracture were not found at the last follow-up.CT results demonstrated that the tunnel position were good.The patellar tilt angle was decreased from 21.6°±2.3° to 10.5°±1.6° (P<0.05);the Lysholm scores was increased from 51.7±5.3 to 93.8±6.5 (P<0.05).Tegner scores was increased from 4.1± 1.1 to 5.5±0.6 (P<0.05).Kujala scores was increased from 53.5±6.4 to 94.6±4.3 (P<0.05).Conclusion:Arthroscopic-assisted anatomical double-bundle MPFL reconstruction with the suture anchors technique is a safe,minimal invasive and effective surgical option for treating patellofemoral instability in adolescent.
3.Autogenous tendon transplantation for repair of grade III medial collateral ligament injury

BACKGROUND:Young patients with cruciate ligament injury and high activity levels are prone to grade III medial colateral ligament injury in knee joint. Conventional construction methods such as direct restoration of injured ligaments and muscle-tendon transposition may induce the loosening of medial colateral ligament complex. OBJECTIVE:To explore the clinical effect of autologous tendon transplantation for grade III medial colateral ligament injury of the knee joint. METHODS: Forty-seven patients with grade III medial colateral ligament rupture were treated by operative repair and reconstruction. After repairing the anatomical structure of medial colateral ligaments, we liberated organic semitendinous tendon, bone-patela tendon-bone, and hamstring tendon, to reconstruct superficial medial colateral ligament. RESULTS AND CONCLUSION:Thirty-seven patients were folowed up for 48 months. Preoperative Lysholm score, IKDC score, Tegner score at the final folow-up, and the widened medial joint space on valgus stress X-Ray films were significantly improved compared with before reconstruction (P < 0.05). According to clinical efficacy scores, 12 cases achieved excelent reconstruction, 18 cases achieved good reconstruction, 4 cases achieved moderate reconstruction, and 3 cases achieved poor reconstruction. The excelent and good rate was 82%. Medial colateral ligament reconstruction based on repairing the anatomical structure is effective and satisfactory. Autologous bone-patela tendon-bone or semitendinous tendon is a good choice for medial colateral ligament reconstruction.
4.Effect of recombinant trichosanthin on proliferation of human cevical cancer Caski cells.

OBJECTIVETo observe the effects of high expression of recombinant trichosanthin (rTCS) on the cell proliferation and cell cycle of human cervical cancer Caski cells.
METHODEukaryotic expression plasmid pcDNA3.1(-)/6His-TCS was constracted and stably transfected into Caski cells. RT-PCR,Western-blot were used to select the clones with rTCS high-expressing. Using pcDNA3.1(-)-transfected cells as the control, MTT assay and flowcytometry were used to elucidate the effects of rTCS high expression on cell growth and cycle regulation in Caski cells.
RESULTThe Caski cells with stable high expression of rTCS was successfully established, which could inhibit the cell growth (P<0.01) and arrest Caski cells in G1 and G2 phases (P<0.05) obviously.
CONCLUSIONHigh expression of rTCS can inhibit the growth of Caski cervical cancer cells, which might provide a new pathway for the therapy of cervical cancer.

Objective To evaluate the appearance of CT in patients with severe acute respiratory syndrome (SARS) in the recovery phase, and to study the correlation of CT findings with pulmonary function.Methods From June to August in 2003, 100 patient with confirmed SARS accepted examination in our hospital. Among them, 91 patients (39 men, 52 women, mean age 36.4 years, age range 19- 66 years) received CT examination and pulmonary function test on the same day. The interval between SARS onset and the examination ranged from 52 to 125 days (mean 87.4 days). CT appearances of pulmonary parenchymal abnormalities including distribution and extent of involvement were quantitatively analyzed, and four levels on CT scan including the aortic arch, the tracheal carina, the pulmonary venous confluence, and the dome of right diaphragm were selected to score the lesions. The correlation of CT scores with the results of pulmonary function tests was studied.Results Of the 91 cases, 47 patients had normal CT appearance in the recovery phase, whereas the other 44 patients still had parenchymal abnormalities, including residual ground-glass opacification and reticular shadow. ObjectiveTo investigate clinical features, correlative factors and prognosis of senile coronary heart disease accompanied with depression.MethodsFifty two senile patients of coronary hart disease with depression were selected for the observation group, sixty cases without depression as control group. Clinical features, family conditions, society factors and Holter report (supervision) of two groups were analyzed.ResultsClinical features such as severe dyssomnia, anxiety agitation, emotion depressing, loss of interest, feeling waste and sense of guilty in patients of observation group were significant different with those in control group. The rate of family harmoniousness and society supporting between patients of two groups were significantly different. The incidence of ventricular arrhythmias, atrial arrhythmia and myocardial ischemia changes on ST-T in the Holter reports of patients in observation group were higher than those in control group (P<0.05 or P<0.01). The incidence of cardiac sudden death of patients in observation group was also higher than patients in control group (P<0.05).ConclusionClinical symptoms such as severs dyssomnia, anxiety agitation, emotion depressing are easily occurred in senile coronary heart disease patients with depression. Patients with depression have higher incidence of cardiac sudden death, having better relationship and society support are advantageous to prevent and reduce the depression in senile coronary heart disease patients.
7.Application of pie-crusting the medial collateral ligament release in arthroscopic surgery for posterior horn of medial meniscus in knee joint

Objective:To explore the effectiveness and safety of pie-crusting the medial collateral ligament release (MCL) in treating posterior horn of medial meniscus (PHMM) tear in tight medial tibiofemoral compartment of knee joint.Methods:Thirty-two consecutive patients with PHMM tear in tight medial tibiofemoral compartment of knee joint were admitted to our department from January,2013 to December,2014.All patients were performed pie-crusting the MCL release at its tibial insertion with 18-gauge intravenous needle.All patients were evaluated by valgus stress test and bilateral valgus stress radiograph at postoperative 1st day,4th week and 12th week.Visual Analogue Scales (VAS),Lysholm scores,Tegner scores and International Knee Documentation Committee (IKDC) scores were recorded at the 1st,3th,6th month follow-up,then follow-up every 6 months.Results:The mean follow-up was 28 (24-36) months.All cases were negative in valgus stress test.MCL rupture,femoral fracture,articular cartilage lesion and neurovascular injury were not found at the last follow-up.The median medial joint space width of affected side and unaffected side for valgus stress radiographs were 6.8 mm and 4.3 mm (P<0.05) at the 1st day,5.5 mm and 4.2 mm (P<0.05) in the 4th week and 4.8 mm and 4.3 mm (P>0.05) at the 12th week,respectively.VAS scores was changed from 4.5±1.5 preoperatively to 1.7±1.0 at the final follow-up (t=16.561,P<0.05).Lysholm scores was changed from 52.3±5.8 preoperatively to 93.2±6.3 at the final followup (t=-41.353,P<0.05).Tegner scores was changed from 4.1±1.1 preoperatively to 5.5±0.6 at the final follow-up (t=-18.792,P<0.05).IKDC scores was changed from 54.5±6.2 preoperative to 93.8±4.5 at the final follow-up (t=-38.253,P<0.05).Conclusion:Pie-crusting the medial collateral ligament release is a safe,minimal invasive and effective surgical option for posterior horn of medial meniscus tear in tight medial tibiofemoral compartment of knee joint.
8.Application of multidisciplinary combined rehabilitation in patients with sarcopenia after total hip replacement

Objective:To explore the application effect of multidisciplinary combined rehabilitation in patients with sarcopenia after total hip arthroplasty (THA) .Methods:A total of 114 patients with sarcopenia who underwent THA treatment in the Department of Joint Surgery of the First Affiliated Hospital of Zhengzhou University from May to December 2020 were selected as the research objects by the convenient sampling method. The patients were divided into the control group and the observation group by the random number table method, with 57 cases in each group. The control group received routine rehabilitation nursing, while the observation group received multidisciplinary combined rehabilitation intervention. Muscle related indexes were compared between the two groups before and after intervention. The short-form Fugl-Meyer motor function score and Harris score were used to compare the motor function of the affected limb and the recovery of hip joint function in the two groups after intervention.Results:After intervention, the whole body muscle mass, limb skeletal muscle mass, affected limb skeletal muscle mass and limb skeletal muscle mass index of patients in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . The Fugl-Meyer motor function of the affected limb in the observation group was better than that in the control group, and the difference was statistically significant ( P<0.01) . The total score of simplified Fugl-Meyer motor function score and Harris score of the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.01) . Conclusions:Multidisciplinary combined rehabilitation can effectively prevent postoperative muscle atrophy and muscle mass loss in THA patients, which is beneficial to the recovery of the motor function of the affected limb and hip joint function.
9.Investigation on the utilization of parenteral nutrition preparations in 12 Hospitals of Jiangsu

Objective To investigate the use of parenteral nutrition preparations in Jiangsu Province,and to provide reference for the standardized management of parenteral nutrition preparations.Methods 720 cases using parenteral nutrition preparations from January 2017 to June 2017 in the department of general surgery of 12 hospitals in Jiangsu province were selected.The rate of nutritional risk screening,the indications of parenteral nutrition,the way of infusion,the rationality and economy of the prescriptions were retrospectively evaluated.The calorie,amino acid content,non-protein calorie/nitrogen ratio,glycolipid ratio and cation concentration of the patients received total parenteral nutrition were calculated.Results The total costs of parenteral nutrition preparations of 720 cases were 1.614 1 millions,and 346 cases did not have the indications for parenteral nutrition.The results of prescription comment showed that only 16 patients were screened for nutritional risk by Nutritional Risk Screening 2002 tool at admission.544 cases were intravenous dripped with amino acid and fat emulsion from peripheral vein.In the 176 total parenteral nutrition prescriptions,there were 39 non-protein calorie/nitrogen ratio cases,15 glycolipid ratio cases,69 cation concentration cases,61 calorie cases and 32 amino acid content cases failing to comply with the recommendation of the guidelines.Only 31 total parenteral nutrition prescriptions were completely reasonable.Conclusion The costs of parenteral nutrition preparations used in hospitals of Jiangsu are high but the rate of rationality is low.Nutrition support team should be established to regulate the use of parenteral nutrition preparations and save medical resources.
