3.Study of late recurrence in non-small cell lung cancer patients after complete resection

Objective To investigate the risk fairs and significance of late recurrence in non-small cell lung cancer (NSCLC)patients who had undergone complete resection and remained recurrence-free for5 years.Methods 496 individuals diagnosed and surgically treated for Stage Ⅰ and Ⅱ A NSCLC were included.We examined patients at 3-month intervals for the first 2 years after surgery and typically at 6-month intervals thereafter on an outpatient basis.The follow-up evaluation included physical examination,inaging examination and tumor narkers.Pathological examination had also been performed when needed.Of these,336 patients remained recurrence-free for 5 years were followed up continuously for 3 years.Clinicopathologic factors were collected including age,sex,smoking history,preoperative serum carcinoembryonic antigen (CEA) level,surgical apprach,maximum tumor dimension on resected specimen,histologic type,histologic differentiation,intratumoral lymphatic permeation,intratumoral vascular invasion,pleural invasion and pathologic stage.The primary end event was lung cancer recurrence.The data collection ended in January 2011.Kaplan-Meier method was used for survival rate.Survival difference wasevaluated bylog-rank test.Multivariate Cox regression was used to test the relationship of recurrence-free probability to various clinicopathologic factors.Results At 5 years after resection,109 patients had died of disease and 33 had died of other causes.18 patients were alive with disease.The remaining 3 - 36 patients were alive and recurrence-free for the first 5 years.The median follow-up period of these 5-year recurrence free survivors was 27 months (range,3-36 months).34 (10.1%)patients developed a late recurrence at 3 years among the 336 patients.Recurrence was locoregional in 13 (38.2%)patients:6 patients in mediastinal lymph nodes,3 patients in ipsilateral lung,2 patients with pleural dissemination and 2 patients in other locations.Recurrence was distant in 21 (61.8%) patients:8 patients with multiple-organ metastases,13 patients with single-organ metastasis,and the latter consisted of 5 patients in contralateral lung,3 patients in liver,2 patients in central nervous system and 3 patients in other locations.Smoking history,preoperative serum CEA level,histologic differentiation,intratumoral lymphatic invasion,intratumoral vascular invasion,pleural invasion and pathologic stage were risk factors for late recurrence in univariate analysis.Multivariate Cox analysis demonstrated that intratumoral vascular invasion and intratumoral lymphatic invasion were independent risk factors for late recurrence.Conclusion NSCLC patients have a significant risk of late recurrence after complete resection 5 years,especially for the patients with intratumoral vascular invasion and intralumoral lymphatic invasion.It is needed and significant for the patients to follow up continuously after 5 years of complete resection.
4.Acute sensory ataxia neuropathy: a case report

The clinical, neuroelectrophysiological, neuropathological characteristics and outcome of a case of acute sensory axonopathy are reported. The patient was a 32-year-old female. She presented with acute onset sensory abnormalities, sensory ataxia, pseudoathetosis and areflex. Lab testing showed cerebrospinal fluid albumin cytologic dissociation. Electromyography studies and sural nerve pathology were consistent with acute sensory axonopathy. Treatment including intravenous immunoglobulin and steroid therapy was provided. Her symptoms recovered slowly without relapse, so did tibial H-reflex amplitudes.
5.An analysis of clinical and imaging characteristics of atopic myelitis

Objective To study the clinical and imaging characteristics of Chinese atopic myelitis (AM) patients.Methods Three diagnosed AM patients were retrospectively analyzed for the clinical data,serum IgE level,antigen specific IgE,cerebrospinal fluid,spinal MRI and therapeutic efficacy profiles.Results All the three patients were male and presented as subacute AM with the onset at 25,47 and 49 years old respectively.Two patients were allergic to pollen and other drugs,while another patient suffered from allergic rhinitis.Elevated serum total IgE and mite antigen specific IgE were found in all cases.Paraesthesia in limb extremities and positive Lhermitte sign were the main clinical features,while no optic,motor,urinary and defecation disturbance were found.Oligoclonal banding of cerebrospinal fluid and serum aquaporin 4 (AQP4) antibody were both negative in all cases.Spinal MRI showed lesions were hypointense on T1 and hyperintense on T2 at the posterior column of T2-3 segment with abnormal enhancement in case 1,hypointense on T1 and hyperintense on T2 at C2/3 segment with mild swelling in case 2 and hypointense on T1 and hyperintense on T2 at C3-5 segments with swelling and abnormal enhancement in case 3.Vitamin B were used in one patient,while the other two patients improved after the treatment with high-dose corticosteroids.Conclusions Subacute myelitis predominantly presents as paraesthesia in limb extremities with elevated serum total IgE and mite antigen specific IgE,while severe motor disorders are rare.Swelling and abnormal enhancement lesions at the posterior column of cervical cord are the common imaging features.Treatment with corticosteroids is recommended to be sustained for 3-6 months.
6.Inhibitory Effect Analysis of Pioglitazone on Inflammation Factors in Patients with Bladder Cancer

Objecitve To study the inhibitory effect of pioglitazone on inflammation factors in patients with bladder cancer. Methods A total of 100 consecutives diagnosed as bladder cancer from Februray 2013 to Februray 2014 were individed ran-domly into experiment and control groups and each of 50 cases.All patients received the appropriate operation or chemother-apeutic regimens,and the patients in experiment group received pioglitazone (15 mg/d×12 weeks)at the same time.Then to compare expression differences of high-sensitive C reactive protein (hs-CRP),tumor necrosis factor alpha (TNF-alpha),in-terleukin (IL-6)and HOMA-IR,MCP-1,MIP-1 levels.Results The levels of hs-CRP,TNF-αand IL-6 in the two groups af-ter treatment were all lower (P <0.05),and in experiment groups they were significantly lower than control group (P <0.05).The levels of HOMA-IR,MCP-1 and MIP-1 in the two groups after treatment were all lower (P <0.05),and in ex-periment groups they were significantly lower than control group(P <0.05).The complication rate in the two groups were no statistical difference (P >0.05).Conclusion Pioglitazone could improve clinical effect and prognosis by lowing inflam-mation factors including hs-CRP,TNF-α,IL-6,HOMA-IR,MCP-1 and MIP-1 in patients with bladder cancer.
7.Angiogenesis and scar inhibition after subcutaneous implantation of Shengji Yuhong collagen

BACKGROUND:Shengji Yuhong col agen showed good curative effect of promoting angiogenesis and tissue healing compared with Shengji Yuhong Gao and col agen alone or gelatin alone.
OBJECTIVE:To explore the curative effect and mechanism of subcutaneous implantation of Shengji Yuhong col agen in rabbits in promoting angiogenesis and repair.
METHODS:Shengji Yuhong col agen as the experimental group and collagen as the control group was implanted inside the rabbit subcutaneous pockets of the back of New Zealand rabbits. The implanted samples and
surrounding tissues were obtained at 3, 7, 14, 28 and 56 days fol owing surgery. Pathological sections were made and the repair of surrounding tissue was observed. Hemoglobin levels in col agen were measured.
Immunofluorescence and CD34 dyeing marking method were utilized to observe capil ary angiogenesis. Western blot assay was employed to examine vascular endothelial growth factor and angiogenin-1 expression.
Immunohistochemistry was used to observe the secretion of typeⅠ and Ⅲ col agen on the surrounding tissues.
RESULTS AND CONCLUSION:The experimental group showed increased subcutaneous vascularization. There were reduced inflammatory exudation, granulation tissue hyperplasia, and mature fiber connective tissue at 28 days.
Angiogenesis and hemoglobin contents were greater in the experimental group than in the control group (P<0.05 or P<0.01). At 3 and 7 days fol owing surgery, vascular endothelial growth factor and angiogenin-1 expression was greater in the experimental group than that in the control group (P<0.05 or P<0.01). The secretion of type Ⅰ col agen was
identical between the experimental and control groups. However, the secretion of type Ⅲ col agen was higher in the experimental group than in the control group at 28 and 56 days (P<0.05), and the proportion of type Ⅰ and Ⅲ
col agen was lower in the experimental group than in the control group at 28 and 56 days (P<0.01). These suggested that Shengji Yuhong col agen can significantly promote angiogenesis in the surrounding tissues with the possible
mechanisms of adjusting the protein expression of vascular endothelial growth factor and angiogenin-1. At the same time, it has the function of regulating col agen formation with better ratio of typeⅠ and type Ⅲ col agen to acquire higher
quality of wound healing with reduced scar formation.
8.Clinical Study of Self-made Hong-huang Antioxidant Inhibiting Oxidative Stress Reaction During Chemotherapy in Patients with Breast Cancer

ObjectiveTo observe the hong-huang antioxidant on oxidative stress in patients with breast cancer during chemotherapy, including their related blood indexes, blood rheology changes, and the effects on TCM clinical symptoms and symptoms of stress.MethodsA total of 60 cases of breast cancer patients during chemotherapy from Jiangsu Province Hospital of TCM was randomly divided into treatment group and control group, 30 cases in each group. On the basis of conventional therapy, patients in treatment group were given hong-huang antioxidant (100 mL per bag) from the 1st day to the 14th day of chemotherapy, 2 bags for each day (morning and evening). Patients in control group were given foundation treatment the same as the treatment group. Patients in the two groups had their serum NO, the content of SOD, and blood rheology tested on the day before chemotherapy, and the 4th, 7th, 14th days during chemotherapy. Meanwhile, their symptom score and the integral of stress reaction and TCM symptoms were also assessed. ResultsOn the 4th day, serum NO of treatment group decreased, while SOD content increased,without statistical significance between the two groups (P>0.05). Serum NO on the 7th, 14th days was significantly lower than that in the control group, but the content of SOD was higher than that in the control group, with statistical significance between the two groups (P<0.05). Hemorheology on the 4th day significantly decreased after treatment (P<0.05), and was significantly better than that in the control group (P<0.05); Clinical symptoms and stress symptoms integral in the treatment group were significantly lower than those in the control group on the 4th, 7th, 14th days of chemotherapy, with statistical significance (P<0.05).Conclusion Hong-huang antioxidant can significantly improve the oxidative stress status, serological indexes, related blood rheology indexes, and clinical symptoms in patients with breast cancer.
9.Lateralizing language area and assessing hemispheric memory function by intracarotid injecting propofol

Objective In the context of worldwide shortage of amytal,explore the intracarotidpropofol test for lateralizing language area and assessing hemispheric memory function.Methods Fourteen patients with refractory partial epilepsy who were candidates for surgical intervention were included in the study.With guide under a digital subtraction angiography,propofol was injected in bilateral intracarotidsequentially.Muscle power deceasing to level 0 at the contralateral limb and eyes gazing to contralateral side were used as the mark of hemispheric anesthesia completely.The immediate language alterations were recorded.To evaluate the bilateral language and memory functions,the visual and auditory memory tasks were performed sequentially once patient could concentrate his attention ; and after limb muscle power recovering to normal level,patients were required to perform a free recall test.Any abnormal responses were recorded.Results Language dominant hemisphere was determined in 14 patients.Nine patients were confirmed as left language dominance,2 patients were right language dominance.The remained 3 patients were considered as bilateral language dominance.Meanwhile,the hemispheric memory function was able be evaluated in 13 patients.More than 67% memory function was sustained in hemisphere contralateral to mesial temporal lesions.Transient responses including eye pain,facial muscle spasms,laughers and involuntary movements were observed.Conclusion Hemispheric language and memory functions can be assessed with direct intracarotidpropofol injection,and propofol could be an alternative drug to amobarbital used in the Wada test.
10.Atypical Takotsubo cardiomyopathy associated with neuromyelitis optica spectrum disorders: a case report

Recently, neuromyelitis optica spectrum disorders (NMOSD) appear to be a multi-organ disorder, however, the involvement of myocardium in NMOSD is extremely rare. In the present article, we present a young girl who manifested bilateral optic neuritis, area postrema syndrome, brainstem syndrome and transverse myelitis, as well as tachycardia, abnormal electrocardiograph, moderate elevation of myocardial biomarker and regional wall movement abnormalities, which confirmed the diagnosis of Takotsubo cardiomyopathy associated with NMOSD. The neurological deficits along with myocardial injury were recovered soon after the administration of intravenous methylprednisolone and intravenous immunogloblin. This is a rare case that should be paid attention and by which can not only broaden the symptom spectrum of NMOSD, but can also provide novel visions for further investigating the mechanism of organs damage.
