1.Clinical Experience on Three-port Laparoscopic Cholecystectomy:Report of 364 Cases

Objective To explore the clinical feasibility of three-port laparoscopic cholecystectomy(LC).Methods From April 2006 to April 2007,364 patients with acute or chronic cholecystitis underwent LC using three-port method.The clinical data of the patients were analyzed retrospectively.Results All of the 364 patients were cured.Three-port LC were completed in 357 of them with a success rate of 98%.Seven patients were converted to four-trocar LC because of frozen Callot's triangle and gallbladder atrophy.The operation time was 30-80 min(mean,55 min).No severe complications including bile duct injury,bile leakage,and hemorrhage,occurred in our patients.Among the patients who were treated with three-trocar LC,250 were followed up for 1 to 13 months with a mean of 5 months.During the follow-up,no bile duct stenosis,gallbladder pouch,or residual stones were found.Conclusion Three-port LC is safe,effective,and feasible for patients with cholecystitis,if the surgeons have grasped the techniques of four-port LC.
2.The clinical application of rectangle window method treating pseudocyst of auricle.

OBJECTIVE:
To summarize pseudocyst of auricle governor square included surgical curative effect, the advantages of the analysis of the operation.
METHOD:
The governor of the central square in auricle pseudocyst incision, 56 cases were retrospectively analyzed clinical data, surgical curative effect and advantages were summarized.
RESULT:
Adopt the auricle pseudocyst governor of the central square of incision, postoperative follow-up of 6 months did not relapse.
CONCLUSION
Rectangle window method effective method of treating pseudocyst of auricle, able to cure pseudocyst of auricle, is a kind of simple, practical, and value in the basic-level hospital surgical method.

3.Clinical treatment of 368 sepsis patients induced by severe surgical abdominal infection

Objective To investigate and find a multiple treatment to reduce the mortality of sepsis patients induced by severe surgical abdominal infection.Methods While treating to severe surgical abdominal infection,inflammatory mediator bacteria,extoxin and endotoxin,immunity,dysfunction of microcirculation,nutrition and metabolism and the function of organs should be paid more attention on and considered as a whole.We also carried out 14 concrete treating measurement.Combined high dosage of anisodaminum and dexamethason were used in sbort-term.Bring forward nourishment support according to different stage of MODS and applying it in clinic could significantly reduce the companion syndrome.Oral administration of“JIE-DU-GU-BEN-TANG”which developed by our division could regulate the imbalance of immunity and inflammatory mediator.Results There were 46 patients died in 368 patients,mortality was 12.50%. Conclusion It was difficult to treat sepsis patients induced by severe abdominal infection and our new multiple treatment could significantly reduce the mortality of severe sepsis.
4.In vivo transdermal penetration enhancing activity of essential oil from Zanthoxyli Pericarpium by using microdialysis technique.

The aim of this paper is to investigate the topical pharmacodynamics behavior of different lipophilic model drugs after treatment with essential oil from Zanthoxyli Pericarpium by using the cutaneous microdialysis technique, and then evaluate its in vivo transdermal penetration enhancing properties. Two traditional Chinese medicine active components, namely tetramethylpyrazine and puerarin, were chosen as lipophilic and hydrophilic model drugs, respectively. Firstly, the concentration difference method was employed to measure the in vitro recovery rate and loss of the microdialysis probe, and the in vivo recoveries of two model drugs were determined by using the retrodialysis method. Secondly, the skin pharmacodynamics behaviors of two model drugs were studied after treatment with different concentrations of the essential oil, and the well-established and standard penetration enhancer Azone was selected as a positive control. It was found that the recovery of microdialysis probe was equal to its loss for two model drugs, with no interaction between drugs in dialysis membranes. The retrodialysis studies revealed that the in vivo recovery of tetramethylpyrazine and puerarin were 59.17%, 19.85%, respectively. The skin pharmacodynamics studies showed that the essential oil could facilitate the transdermal absorption of tetramethylpyrazine in a concentration-dependent manner, and the enhancement ratio (ER) for 5% essential oil was 98.64, which was higher than that of the optimum concentration of Azone (3% Azone, ER=89.11). Meanwhile, the Zanthoxyli Pericarpium could effectively promote the transdermal permeation of the puerarin in a concentration-dependent manner. Hence, this study further confirmed that the Zanthoxyli Pericarpium had excellent penetration-enhancing activity as a natural transdermal penetration enhancer, providing data support for its application in traditional Chinese medicine external preparations.
5.Microvascular decompression for the hemifacial spasm with endoscopy.

OBJECTIVE:
To investigate the value of the endoscopy in the operation of microvascular decompression (MVD) for the hemifacial spasm by approach of postauricular suboccipital to the cerebellopontine angle (CPA) with posterior auricular small incision.
METHOD:
Two hundred and eighty-six cases of hemifacial spasm had received the operation of MVD with endoscopy by approach of postauricular suboccipital to the CPA.
RESULT:
Responsible blood vessels were found in 285 cases (99.7 percent), including 264 cases of anterior inferior cerebellar artery, 21 cases of basilar artery. The root entry zone of the facial nerve were completely decompressed with Teflon. There is no responsible blood vessels but adhesion in 1 cases (0.3 percent). After surgery, hemifacial spasm immediately disappeared. House-Brackmann(1985) grading system was used to evaluated the recovery of facial nerve function. After 1 week of operation 196 cases' facial nerve function are stage 1/6, 62 cases' are stage 2/6, 23 cases' are stage 3/6, 4 cases' are stage 4/6, 1 cases' is stage 5/6. And after Six months of operation, 274 cases' are stage 1/6, 10 cases' are stage 2/6, 2 cases' are stage 3/6. After 1 month of operation there is no significant change of hearing in 238,there are 35 cases of hearing loss less than 20 dB, 10 cases of hearing loss more than 20 dB, but less than 50 dB,3 cases of hearing loss more than 50 dB. Ear-nose cerebrospinal fluid leakage occurred in 2 cases are cured. During 1 year to 4 years following-up, only 3 (1.0 percent) preliminary suffered relapse,among which 1 case was cured by the secondary operation. The long term cure rate was 99.3 percent without occurrence of serious complications such as death.
CONCLUSION
The microneurosurgery of MVD for the treatment of hemifacial spasm is an ideally functional and etiotropic operation. It is useful not only to discover the responsible blood vessels which are regarded as those pressing the root entry zone of facial nerve,but also to protect the function of the brain tissue and nerves as well. It is a safe, minimally invasive and efficient operation. To avoid the complications, enough knowledge of the craniotopography and skilled technique of endoscopic operation are primary.

6.Treatment of lumbar spondylolisthesis by posterior restoration and three-column fixation.

OBJECTIVETo investigate the late results of using posterior restoration and three-column fixation to treat lumbar spondylolisthesis.
METHODSOne hundred and eighty-four patients with lumbar spondylolisthesis were collected from March 1999 to May 2007, they were treated by posterior restoration and fixation with single nail-grooved tail steel plate and fixed with cage (WDFC). Among these cases, 87 cases were fixed with one WDFC, 97 cases were used two WDFCs.
RESULTSAll patients were followed up for 8 to 69 months(averaged 23 months). According to Nakai standard, the results was excellent in 142 cases, good in 34, fair in 8, the excellent and good rates were 95.6%. Seventy-nine vertebraes with I degree spondylolisthesis were reduced after surgery. Eighty-seven vertebraes with II degree spondylolisthesis were reduced except 9 with I degree spondylolisthesis left. Twenty-one with III degree spondylolisthesis were reduced except 5 with I degree spondylolisthesis left; In 2 with IV degree spondylolisthesis, one with I degree spondylolisthesis left and the other with II degree spondylolisthesis left. The follow-up results showed that there was no statistical significance in the height of intervertebral space between preoperation and post-operation, and no recurrence was observed and no single nail-grooved tail steel plate and WDFC were loose or crashed.
CONCLUSIONPosterior restoration and three-column fixation is a positive modus operandi to treat lumbar spondylolisthesis,which can reduce excellently,keep the height of intervertebral space and stabilization of segment, obtain high rate of fusion, and cut down complication.

7.Diversity and Antiaflatoxigenic Activities of Culturable Filamentous Fungi from Deep-Sea Sediments of the South Atlantic Ocean

Despite recent studies, relatively few are known about the diversity of fungal communities in the deep Atlantic Ocean. In this study, we investigated the diversity of fungal communities in 15 different deep-sea sediments from the South Atlantic Ocean with a culturedependent approach followed by phylogenetic analysis of ITS sequences. A total of 29fungal strains were isolated from the 15 deep-sea sediments. These strains belong to four fungal genera, including Aspergillus, Cladosporium, Penicillium, and Alternaria. Penicillium, accounting for 44.8% of the total fungal isolates, was a dominant genus. The antiaflatoxigenic activity of these deep-sea fungal isolates was studied. Surprisingly, most of the strains showed moderate to strong antiaflatoxigenic activity. Four isolates, belonging to species of Penicillium polonicum, Penicillium chrysogenum, Aspergillus versicolor, and Cladosporium cladosporioides, could completely inhibit not only the mycelial growth of Aspergillus parasiticus mutant strain NFRI-95, but also the aflatoxin production. To our knowledge, this is the first report to investigate the antiaflatoxigenic activity of culturable deep-sea fungi. Our results provide new insights into the community composition of fungi in the deep South Atlantic Ocean. The high proportion of strains that displayed antiaflatoxigenic activity demonstrates that deep-sea fungi from the Atlantic Ocean are valuable resources for mining bioactive compounds.
