1.Detection of circulating cardiac troponin I autoantibodies and its clinical significance

Objective To detect the positivity of circulating autoantibodies against cardiac troponin Ⅰ (cTnI) in acute myocardial infarction (AMI) and myocarditis patients and investigate the interference of the antibodies with the detection of cTnI. Methods cTnI ELISA was established for assessment of sera obtained from 121 patients with AMI, 24 with myocarditis and 210 healthy subjects. Binding specificity of cTnI antibody from positive sera by ELISA was confirmed with Western Blot. The recovery of cTnl studies was employed to evaluating the effects of cTnl autoantibodies on cTnI immunoassays. Results Thirteen of the 121 AMI patients (10.74%) and 2 of the 24 myocarditis patients (8.3%) had positive anti-cTnI antibodyas compared with none in the healthy subjects. The recovery of cTnI by adding cTnI-C fusion proteincorresponding to final cTnI concentration of 0.625-100 ug/L to sample with anti-cTnI antibody wasinhibited significantly (Spearman correlation coefficient r=0.943, P=0.005 ). There was no significantchange of recovery of cTnl when adding it to a normal sera(Spearman correlation coefficient r=0.377,P=0.461). When cTnI-C complex corresponding to 20 ug/L cTnI was added, 5 of the 15 sera with anti-cTnIantibody were found with the inhibition of recovery (<80%). Conclusions Autoantibodies against cTnIwere increasing in patients with AMI and myocarditis, yet it appears that these autoantibodies could interferenegatively with the cTnI immunoassay. Thus, it should be paid more attention to cTnI immunoassays.
2.A preliminary evaluation of blood pressure and cardiovascular conditions in kidney transplant recipients

Objective To study the blood pressure and cardiovascular conditions in kidney transplant recipients in order to prevent the cardiovascular complications in such patients. Methods The blood pressure and ECG were monitored day and night by noninvasive means for 86 patients after transplantation.The results were analysed and compared with the pretransplantation studies. Results Hypertension occurred in 55 out of 86 patients.There was a significant difference of average blood pressure and the occurrence of abnormal ST segment or T wave between day and night whereas no significant change was observed in blood CsA concentration,blood creatinine,serum cholesterol,blood sugar,triglyceride and K+,N+,Cl-,TCO 2,Ca 2+ ,etc. Conclusions Cardiovascular complications in kidney transplant recipients is related to the high blood pressure during night,the normal day-night blood pressure rhythm being violated.It is not related to the immunosuppresives used.
3.Negative interference of circulating troponin autoantibodies in five commonly used cardiac troponin Ⅰ detection systems

Objective To explore the possible negative interference of circulating cardiac troponin Ⅰ(cTnI) autoantibody on the immunoassay of cTnI in five commonly used cTnI detection systems. Methods Thirteen patients with positive cTnI autoantibodies in their serum samples were firstly screened and selected from 121acute myocardial infarction (AMI) patients using ELISA assay. The serum cTnI values and their recovery rates were then carefully measured and analyzed. Results cTnI values in these 13 samples showed amazing difference in the five detection systems, demonstrating various degrees of pseudo-drop, or even false-negative. One sample with low recovery was detected in Access-2 system. One sample with low recovery as well as one sample with moderate recovery were detected in Architect i2000 (Abbott). Two samples with moderate recovery and one sample with low recovery were detected in Axsym(Abbott). Three samples with moderate recovery and two samples with low recovery were detected in Dimension X Pand (Dade Behring)and one sample with moderate recovery together with four samples with low recovery were detected in Vidas (Biomerieux). And the serum levels of autoantibodies (A450) positively correlated with the degrees of their negative interference for the detection of cTnI. The R2 and P values on each system were 0. 841 (P <0. 01)vs Access-2, 0. 808 (P < 0. 01) vs Architect i2000 (Abbott), 0. 772 (P < 0. 01) vs Axsym (Abbott), 0. 707 (P < 0. 01) vs Dimension X Pand (Dade Behring) and 0. 424 (P < 0. 05) vs Vidas (Biomerieux), respectively. Conclusion Circulating autoantibodies of cTnI can induce considerable negative interference in all the 5 commonly used cTnI detection systems, which might then lead to incorrect judgments of the obtained results of cTnI in daily clinical work.
4.Effects of NF-kappa B on doxorubicin induced myocardium injury and melatonin treatment in rats

OBJECTIVE To investigate the effect of NF-κB on cardiomyopathy induced by doxorubicin(Dox) in rats and the effect of melatonin(MT). METHODS Thirty male Sprague-Dawley rats were randomly divided into 3 groups: normal control group; Dox model group; Dox+MT group. The content of nitric oxide (NO) and the activity of inducible nitric oxide synthase (iNOS) in cardiac tissues were examined by spectrophotography method; the activity of NF-κB in myocardial cell was observed by immunohistochemistry method; the apoptosis information of myocardial cell was detected by TUNEL method. RESULTS There was significant difference in activity of NF-κB between normal control and the Dox model group. MT could inhibit the activation of NF-κB(P<0.05). There was significant difference in the content of NO and the activity of iNOS between normal control and Dox model group, MT could decrease content of NO and the activity of iNOS(P<0.05); apoptotic rate in Dox model group was significantly increased compared with that in normal control group, MT could significantly decrease myocardial cell apoptosis (P<0.05). CONCLUSION The activation of NF-κB plays an important role in cardiomyopathy induced by Dox, NF-κB seems to increase the activity of iNOS, and then significantly increase the content of NO in cardiac muscle; NF-κB possibly enhances myocardial cell apoptosis in cardiomyopathy induced by Dox. MT may inhibit the activation of NF-κB, and prevent excessive oxidative stress and suppress myocardial cell apoptosis, MT has protective effect against cardiomyopathy induced by Dox.
5.Research and implementation of cooperative cache for PVFS

At present, there are many effective ways to achieve high performance in cluster system storage management, including server-end disk, server-end caching, local caching and cooperative caching. The cooperative caching mechanism shares caches among different clients so as to avoid expensive disk access costs and to improve overall throughput of cluster system. In this paper, a Single Copy Cooperative Cache model is proposed together with block lookup algorithm, block replacement algorithm and the consistency algorithm based on the model. Meanwhile, the prototype system of the model is implemented in PVFS file system. Finally, the performance of this system is tested in InfiniBand Framework, the result of which shows that in contrast to the original PVFS system, read performance of PVFS file system is improved by about two times, while write performance is reduced by nearly ten percent.
6.Influences of clock resolution of bandwidth measurement on packet pair algorithm

Influences of the clock resolution of bandwidth estimator on the accuracy and stability of the packet pair algorithm was analyzed. A mathematic model has been established to reveal the relationship between the result deviation coefficient and the packet size, clock resolution and real bandwidth(value)of the measured route. A bandwidth self-adapting packet pair algorithm was presented based on the mathematic model to reduce the estimation error resulting from the clock resolution and to improve the accuracy and stability of measurement by adjusting the deviation coefficient. Experimental results have verified the validity and stability of the algorithm.
7.X-ray Diagnosis of Osteogenesis Imperfecta:A Report of 10 Cases and Literature Review

Objective To discuss the X-ray appearances of osteogenesis imperfecta.Methods The clinical and radiological manifestations of osteogenesis imperfecta in 10 child-patients were studied retrospectively.Results ①The change of osseous density:generalized osteoporosis with abnormal fragility of bone;②According to the abnormal contour of limbs bone,the patterns of lesions(10 cases) were categorized to four subtypes including long and thin type(3 cases),stumpy type(3 cases),pocketed and swelled lesions(2 cases) and blended appearances(2 cases);③Among the 10 cases,the lesions complicated by arthropathy were seen in 5 cases;④complicated by abnormal spines in 5 cases,pelvis deformity in 3 cases,chest deformity in 4 cases.Conclusion Radiological manifestations have certain clinical value in the diagnosis and typing of osteogenesis imperfecta.
8.Surgical treatment for prosthetic heart valve dysfunction

Objective To review our clinical experience with 41 reoperatioas of prosthetic heart valve dysfunction be-tween October 1996 and October2008. Methods There were 16 malas and 25 females with an average age of (44.5±12.6)years. All 41 patients underwent reoperation under hypothermic cardiopulmonary bypass, including 38 heart valve replace-ments, 2 disc rotations and cut of the excessive knot in 1 case. There were 13 emergency and 28 elective procedures. Results The median time for extubation was 15.3 hours and tracheostomy was needed in 5 cases. There were 6 deaths, all of which occurred in emergency cases, with a mortality rate of 15.4%. No death occurred in patients implanted with a bioprosthetic valve previously. Postoperative complications included 1 infective endocarditis, 1 intractable hiccup and 1 wound infection. Conclusion Prosthetic heart valve dysfunction is catastrophic. Early diagnosis and reoperation is mandatory.
9.Application of 3D printing technique in bilateral sagittal split osteotomy.

OBJECTIVETo investigate the application of osteotomy template manufactured via 3D printing technique in bilateral sagittal split osteotomy (BSSO).
METHODSThe patient group consisted of 32 patients (17 males and 15 females) with mandibular hypoplasia who underwent a BSSO setback (Hunsuck modification; 64 splits). The mean age at the time of surgery was 23.5 years (range 1.9-35 years). All patients were scanned using cone beam computed tomography (CBCT) to create a 3D model of the mandibular, and the osteotomy template of the inner horizontal ramus of mandible bone incision was manufactured via 3D printing technique. All splits underwent operation with or without the osteotomy template by either a young doctor (attending doctor) or a doctor with extensive orthognathic surgery experience (chief doctor). The time it took to perform the mandible bone incision of the inner horizontal ramus of each group was recorded. Postoperative CBCT scan was performed, and the lingual split scale was used to assess the effect of the operation.
RESULTSPrimary healing of incisions was observed in all patients, and no serious complications occurred. The time it took to perform the mandible bone incision of the inner horizontal ramus was significantly different in the four groups (F=30.059, P<0.05), and it was longer in the group of the attending doctor without osteotomy template than in the other three groups (P<0.05). No significant difference was observed in the other groups (P>0.05). Although all splits (n=64) were performed according to the standardized protocol, only 59.38% (38/64) of the fracture lines run according to the Hunsuck's description. By contrast, only 21.88% (14/64) run through the mandibular canal and 18.75% (12/64) was split. The database was analyzed using crosstabs and via Fisher exact test. The split pattern was influenced by the application of an osteotomy template (P<0.05).
CONCLUSIONThe application of osteotomy template manufactured via 3D printing technique in BSSO was effective, which enabled the young doctor to complete the operation faster and with good results.

10.CD3 + CD56 + NK T-like cells are significally increased in the peripheral blood of patients with newly diagnosed cancer

Objective To investigate the presences of regulatory T cells (Treg) and natural killer T cell (NKT)-like T cells in the peripheral blood of healthy subjects and patients with newly diagnosed cancer. Methods Patients were enrolled into four groups: healthy subjects (Group A), patients without cancer (Group B), patients with newly diagnosed stage Ⅰ~Ⅱ cancer (Group C), and patients with newly diagnosed stage Ⅲ~Ⅳ cancer (Group D). Flow cytometry was performed to detect the percentage of CD4+ CD25high CD127low cells in CD4+ T lymphocytes (Treg) and the percentage of CD3+ CD56+ cells in CD3+ T lymphocytes (NKT-like). Results The percentage of Treg cells in CD4+T lymphocytes was(6.72 ± 3.16)%, (6.49 ± 2.83)%,(6.80 ± 3.07)%and (7.63 3.47)% in Group A, B, C and D, respectively, with no significant differences among these groups (P>0.05). However, the percentage of CD3+CD56+T cells in CD3+T lymphocytes was (3.13 ± 1.66)%, (2.56 ± 1.27)%,(7.22 ± 2.70)% and (7.72 ± 3.10)% in Group A, B, C and D, respectively, with marked increases in Group C and D compared to those in Group A and B (P < 0.05). Conclusion Patients with newly diagnosed cancer demonstrated increasing trends in the percentage of peripheral NKT-like cells.
