Objective To investigate the clinical value of artery-sparing high-site transvaginal subtotal hysterectomy(TVSH) for the treatment of uterine body leiomyoma. Methods High-site transvaginal subtotal hysterectomy with the preservation of bilateral uterine arteries was performed in 23 patients with uterine body leiomyoma from January 2002 to December 2004 in this hospital. The vesicouterine peritoneum was opened through the vagina to mobilize the uterus outside. With bilateral uterine arteries preserved, the muscular layer was opened 1 cm medial to the uterine horn until the site 3 cm above the isthmus of the uterus. Then wedge resection of the anterior and posterior uterine wall was conducted with the endometrium 2 cm in thickness preserved. Results All the operations were successfully completed without conversions to open surgery. The operating time was 45~80 min(mean,55 min),and the intraoperative blood loss was 80~250 ml(mean,150 ml).The time to first passing flatus was 6~27 h(mean,11 h).The length of postoperative hospital stay was 3~8 d(mean,4 d).The pyrexia rate after operation was 8.7%(2/23). Normal sexual life recovered within 3 months in 18 patients. Conclusions Artery-sparing high-site TVSH has advantages of high safety, effectiveness, minimal invasion, rapid recovery, and fewer complications.

Objective To investigate the clinical value of transvaginal enucleation of benign ovarian cysts. Methods Transvaginal enucleation of benign ovarian cysts was carried out in 195 patients with surgical indications between January 2000 and June 2004.Relative surgical parameters were observed and recorded. Results Transvaginal operations were successfully completed in 190 patients, while conversions to open surgery were required in 5 because of massive pelvic adhesion. The operating time was 45~83 min(mean,50 min),the intraoperative blood loss was 30~90 ml(mean,45 ml),the time to first passing flatus was 4~24 hours(mean,8.5 hours),and the rate of postoperative pyrexia,18.4%(35/190).Follow-up for 2 weeks showed that patients resumed daily activities and regular works in a percentage of 72.1%(132/183) and 48.1%(88/183),respectively.Conclusions Transvaginal enucleation of benign ovarian cysts has been shown to be a safe and mini-invasive procedure that improves the(patient's) quality of life.

AIM:To study the effect of caffeine on the large conductance calcium activated potassium (KCa) channels by patch-clamp technique on smooth muscle cells enzymatically isolated from the porcine coronary artery (PCASMC),and to investigate the effect of ryanodine on KCa being activated by caffeine.METHODS:Using the single channel patch-clamp technique, single PCASMC was isolated by collagenase, the activity of single KCa channel was recorded in porcine coronary artery smooth muscle cells.RESULTS:Caffeine (0.1-10 mmol/L) enhanced the open probability (Po) of KCa channels in a dose-dependent manner in the intracellular side of inside-out patches and its effect was almost completely abolished by washout. Caffeine decreased the mean close time markedly, but had no effect on the amplitude of KCa channels. However, ryanodine (10-40 ?mol/L) decreased Po of KCa channels activated by caffeine in a dose-dependent manner in cell-attached patches. The mean open time also decreased.CONCLUSION:Caffeine directly activates KCa channels of porcine coronary artery smooth muscle cells in inside-out patches, the activity of single KCa channel is inhibited by ryanodine indirectly in cell-attached patches.

The composite material PLGA compounded with β-tricalcium phosphate (β-TCP) was prepared by melt blending method, and the absorbable interface screw was prepared by injection molding process. Prepare PBS buffer that simulates human body, conduct in vitro degradation experiments on interface screws according to relevant national and industry standards, then test and characterize interface screws at different time points for degradation of intrinsic viscosity, average molecular weight distribution, mass loss, mechanical properties and thermal properties. According to the degradation performance-time curve, determine the time node at which the interface screw loses the mechanical properties. In this paper, the in vitro degradation behavior of interfacial screws prepared from PLGA and β-TCP composites was studied in detail, providing a reference and basis for the degradation behavior of absorbable products prepared from PLGA and β-TCP composites.

Objective To explore the therapeutic effect of transumbilical single-port laparoscopic high uterosacral ligament suspension(HUS)and iliacpubic ligament fixation(ILF)in the treatment of pelvic organ prolapse(POP)mainly caused by middle pelvic defects. Methods Clinical data of 15 cases of POP with middle pelvic defects from June 2017 to June 2023 were analyzed retrospectively. The HUS and ILF were performed by transumbilical single-port laparoscopy. At one year after operation, gynecological examination(POP-Q system)and maximum straining pelvic MRI examination were performed for objective evaluation. The Pelvic Floor Distress Inventory Short Form(PFDI-20)and Pelvic Organ Prolapse-Urinary Incontinence Sexual Questionnaire(PISQ-12)were used for subjectiveevaluation. Results Theoperationtimewas 65-135 min(mean,102.9±16.3 min),thebleedingvolumewas 20-50 ml,and the postoperative hospital stay was 6-9 d.Follow-up time ranged from 6 to 72 months(mean,42.8±22.3 months),among which 14 cases were≥12 months. No pelvic pain, lumbosacral pain, urinary retention and thread exposure occurred.Stress urinary incontinence(SUI)occurred in 1 case at 1 year after operation.Surgical failure was noted in 1 case(degree Ⅱ),the objective success rate being 93.3%(14/15).At 1 year after operation, the distance from the anatomical landmarks of pelvic MRI to the pubococcygeal line(PCL)was significantly higher than that before operation(P<0.01),and the PFDI-20 and PISQ-12 scores were significantly better than those before operation(P<0.01).Conclusions Transumbilical single-port laparoscopic HUS and ILF is a safe and effective mutual aid procedure for the treatment of POP mainly caused by middle pelvic defects with surgical indications, which can improve pelvic floor function and quality of life.It is simple and easy to perform with low cost of consumables, which is suitable for primary hospitals.

OBJECTIVETo evaluate the storage performance of the domestically made platelet storage bags (experimental group) and the United States Trima set platelet storage bags (control group).
METHODSThe manually separated platelets were divided in two equal parts, which was added to control blood bags and experimental blood bags respectively, all samples were stored at a 22 °C ± 2 °C. The platelet count, mean volume, aggregation activity (ADP, THR), pH, glucose, lactate concentration, lactate dehydrogenase concentration, hypotonic shock reaction, CD62P and phosphatidic acid serine content were detected at day 0, 3, 5 and 7 of storage.
RESULTSThere was no significant difference of platelet quality at day 5 after storage between the experimental group and the control group (T-test, P > 0.05).
CONCLUSIONTwo kinds of platelet storage bags have the similar storage performance.
