Objective To explore the clinical effectiveness of emergency upper gastrointestinal bleeding channel for patients with acute upper gastrointestinal bleeding(AUGIB)by the inverse probability of treatment weighted(IPTW)method. Methods A retrospective study method was used. The clinical information was collected on 299 AUGIB patients who belonged to the First People's Hospital of Guiyang, where they were admitted from January 2018 to December 2021.AUGIB patients admitted before the establishment of emergency treatment channel(from January 2018 to December 2019)were selected as the routinel group(152 cases),while AUGIB patients admitted after the establishment of emergency treatment channel(from January 2020 to December 2021)were selected as the channel group(147 cases).IPTW was used to balance multiple confounding variables[gender, age, history of previous underlying diseases, history of non-steroidal anti-inflammatory drug(NSAID)administration, personal history,Glasgow-Blatchford score(GBS),different bleeding etiologies, etc.],a consistent distribution of confounding variables among the groups was achieved after IPTW treatment. The difference of clinical treatment effects between the routine group and the channel group was performed, including time to hemostasis, recurrent bleeding rate, volume of blood transfusions, length of hospital stay, hospitalization cost, intensive care unit(ICU)transfer rate, and mortality etc.were compared. Results After IPTW,the confounding variables were well-balanced between groups. The time to hemostasis[hours:7.90(5.36,11.42)vs.9.92(6.25,18.15)],recurrent bleeding rate[23.1%(34/147)vs.40.1%(61/152)],length of hospital stay[days:8.00(7.00,10.34)vs.9.00(7.00,13.00)],ICU transfer rate[8.8%(13/147)vs.17.7%(27/152)],and mortality[0.7%(1/147)vs.4.5%(7/152)]in channel group were significantly lower than those in the routine group(all P<0.05).There were no significant difference in transfusions volume and hospital cost between channel group and routine group[transfusions volume(U):2(0,4)vs.2(0,4),hospitalization cost(ten thousand yuan):1.35(1.03,2.00)vs.1.16(0.71,2.29),both P>0.05].Conclusion The emergency treatment channel can reduce the recurrent bleeding rate,ICU transfer rate, and mortality rate, shorten the time of hemostasis and length of hospital stay, and has a good treatment effect.

Objective:To evaluate the skin development and repair process of X-ray radiation damage in rat with non-invasive two-photon excitation fluorescence (TPEF) imaging technology in vivo. Methods:Totally 24 SD rats were randomly divided into four groups including X-ray irradiated group (25, 35 and 45 Gy) and non-irradiation control group. At different times after irradiation, the degree of skin injury was evaluated, and the pathological changes of nicotinamide adenine dinucleotide (phosphate) [NAD(P)H] and collagen fiber fluorescence signals in epidermal cells were detected in vivo by TPEF imaging technology. Results:At 10 d post-irradiation, the skin of irradiation groups showed erythema and desquamation. At 15-20 d post-irradiation, the skin of radiation groups developed progressive exudation, edema and ulcers with increasing radiation dose. On day 25, the skin began to repair in the 25 Gy group, however, the skin of other groups still had exudation and ulcers. On day 10, NAD(P)H fluorescence signal in epidermal cells of irradiation groups decreased and the fluorescence signal of collagen fibers in papillary layer and reticular layer of irradiation groups reduced, which were significantly lower than that of normal control group ( t=24.145, 28.303, 26.989, 6.654, 7.510, 7.997, P<0.05). On day 30, fluorescence signal of NAD(P)H and collagen fibers in epidermal cells and dermis began to repair, the cell from stratum granulosum, stratum spinosum, and stratum basale in the 25 Gy group showed fluorescence signal, the other groups did not show. The fluorescence signal of collagen fibers in the 25 Gy group were gradually increased in papillary layer and reticular layer, however, they were significantly lower than normal control group ( t=115.133, 17.431, P<0.05), the skin of 45 Gy group did not show fluorescence signal of collagen fibers. Conclusions:The damage and repair process of epidermal cells and dermal collagen fiber can be detected noninvasively by TPEF imaging technology after X-ray irradiation in vivo.

Objective To observe the efficacy and safety of repetitive transcranial magnetic stimulation(rTMS)on refractory tinnitus and the differences of resting-state functional magnetic resonance imaging(rs-fMRI)imaging between before and after treatment, and to explore the possible central mechanism of rTMS regulation of tinnitus. Methods Thirty-seven patients with refractory tinnitus admitted in Affiliated Hospital of North Sichuan Medical College from September 2022 to February 2023 were selected and were divided into experimental group(n=20)and control group(n=1 7).The experimental group was given true rTMS treatment, and the control group was given sham stimulation with the same parameters.Tinnitus handicap inventory(THI)score, tinnitus loudness visual analogue scale(VAS)score and rs-fMRI scan were performed before and after treatment.Regional homogeneity(ReHo)was calculated after scanning, and the different brain regions were selected as the area of interest(ROI)and the whole brain functional connection(FC)was performed. Results There were no significant differences in age, gender, education level, tinnitus side, course of disease, hearing level,self-rating depression scale,self-rating anxiety scale the experimental group and control group. There were no significant differences in THI and VAS scores between the two groups before treatment;the THI and VAS scores in the experimental group decreased after 2 weeks of rTMS treatment(P<0.001),while there was no significant difference in the two scores in the control group before and after treatment.Only 3 patients in the experimental group experienced left facial muscle tremor or transient mild scalp pain during treatment, without other serious side effects. The ReHo of the left cerebellar area 9 increased in the experimental group after rTMS(P<0.005);the ReHo values in the right inferior temporal gyrus, left posterior central gyrus and left anterior central gyrus increased in the control group after intervention(P<0.005).The FCs between the right inferior temporal gyrus and the left orbital inferior frontal gyrus, the left anterior cingulate gyrus increased in the experimental group(P<0.005),and FC between the right inferior temporal gyrus and the left superior marginal gyrus decreased(P<0.005).The FCs between the right cuneus and the left fusiform gyrus, right superior occipital gyrus decreased in the experimental group after rTMS(P<0.005).The FC between the right cuneus and the left fusiform gyrus increased in the control group after intervention(P<0.005),while other FCs remained unchanged. Conclusions rTMS has a certain therapeutic effect on refractory tinnitus with higher safety;regulation of auditory brain network and related non-auditory brain network may be one of the central mechanisms of rTMS treating refractory tinnitus.
