1.Nursing of patients with high grade intraepithelial neoplasia treated by endoscopic submucosal dissection

Objective To explore the experience in nursing the patients with esophageal high-grade intraepithelial neoplasia lesions treated with endoscopic submucosal dissection. Method A retrospective analysis was carried out among the patients with esophageal high-grade intraepithelial neoplasia lesions to look into the nursing care to the patients. Results Thirty patients were successfully stripped, and no surgical complication occurred. After intensive care, they were cured and discharged. Conclusion Professional and meticulous care can effectively improve the success rate of endoscopic submucosal dissection and reduce the incidence of complications, and promote the rehabilitation of patients.
2.Investigation on infections of human intestinal parasites in Taixing City

Objective To understand the current status of infections of intestinal parasites of population in Taixing City,Ji-angsu Province. Methods The infection rates and densities of human intestinal parasites were investigated according to the methods of the National Investigation Scheme on Human Principal Parasites,and the data of society,economy and disease con-trol were collected and analyzed. Results Among 2 556 people investigated in five villages,16 persons were found with intesti-nal parasites, with an infection rate of 0.63%. The infection rate was higher in residents with a low education level than in others and it was higher in the age group over 50 years than in the group under 50 years. The infection density was mild and the most was the single parasite infection. Conclusions The current status of intestinal parasite infections of population in Taixing City has reached the county-level control standard of Jiangsu Province. Therefore,the preventive strategy and measures should be ad-justed and the monitoring work should be strengthened.
3.Clinic feature and surgical treatment for degenerative lumbar scoliosis

Objective Degenerative lumbar scoliosis is frequent clinic spine malformation ,and it is complicated by lumbar inter-vertebral disc ,joint of lumbar vertebra cataplasia and lumbar spinal stenosis .The aim of this paper is to study surgical therapeutic regimen for analyzing clinic feature of degenerative lumbar scoliosis .Methods We comprehended symptom and analyzed imageology feature for lumbar spinal stenosis through reviewing 48 cases of operation from August 2003 to August 2010 ,and then approached its therapeutic principle and regimen .Results There were good therapeutic effect on the basis of comprehending degenerative lum-bar scoliosis and designing different treatment plan by different case feature .Conclusion Ii is need to know degenerative lumbar scoliosis again ,in order to work out individual therapeutic regimen based on clinic feather ,process segment ,state of spinal stenosis , angle of lumbar scoliosis ,degree of vertebra rotation and lumbar destabilizing .
4.Resistant Genes and Cluster Analysis in Multidrug-resistant Pseudomonas aeruginosa

OBJECTIVE To investigate resistant genes encoding ?-lactamases and aminoglycoside modifying enzymes in Pseudomonas aeruginosa isolated from clinical specimens,and phylogenetic analysis was performed.METHODS Antimicrobial susceptibility test was performed by PhoenixTM-100 system.Resistant genes encoding ?-lactamases,aminoglycoside modifying enzymes and antiseptic resistance were detected by PCR amplification and verified by DNA sequencer.RESULTS The resistant rates of ?-lactams including ampicillin/sulbactam,piperacillin,piperacillin/tazobactam,cefotaxime,ceftazidime,cefepime,imipenem and meropenem in 190 strains of P.aeruginosa were 98.9%,59.5%,45.8%,77.4%,34.2%,38.4%,15.3% and 6.8%,respectively.Ciprofloxacin and levofloxacin still showed powerful activities with resistance being 15.3% and 21.0%.The positive rates of blaVEB, blaGES and blaCARB genes were 9.5%,9.5% and 57.1% in 21 isolates.Twenty strains lost oprD2 genes.However,the ?-lactamase genes of TEM,SHV,OXA,PER,IMP,VIM,SPM,GIM and DHA were not found.Three resistant genes encoding aminoglycoside-modifying enzymes were found in 21 isolates,such as aac(6′)-Ⅰ,aac(6′)-Ⅱ and ant(2″)-Ⅰ,and they accounted for 9.5%,61.9% and 66.7%,respectively.The positive rate of qacE△1-sul1 genes was 66.7% in 21 isolates.CONCLUSIONS P.aeruginosa isolated in clinic has carried many resistant genes.The loss of oprD2 gene may be the important cause of P.aeruginosa resistant to imipenem.Cluster analysis indicates that the spread of clones occurred in our hospital.
5.Short hairpin RNA-mediated silencing of APRIL gene inhibits proliferation and metastasis of human colon carcinoma SW480 cells in vitro

Objective To study the influence of short hairpin RNA-mediated inhibition of APRIL gene on proliferation,invasion and metastasis of human colon carcinoma SW480 ceils in vitro. Methods After treatment with shRNA, the expression level of APRIL protein in human colon carcinoma SW480 cells was detected by western blotting. And cell adhesion, cell migration and cell invasion of SW480 cells transfected with sh637 were analyzed respectively as compared with non-transfected SW480 cells and mocktransfectant. Results The expression of APRIL protein in SW480 transfected with sh637 were significantly lower than mock-plasmid groups and untransfected ones (F=42.15, P=0.00). Cell proliferation was markedly inhibited, compared with untransfected groups and negative control ones (F=24.76, P<0.05).And the average absorption of cell adhesion in transfected groups, mock-plasmid and non-transfected ones were 0.343, 0.409, 0.412, respectively. Cell adhesion decreased 39% compared with untransfected groups. Similarly, there was significant difference for cell migration (F=65.53, P<0.01) between transfected groups (The average ceil number in exposed area to migrate was 47.89±13.16) and nontransfected (98.78±23.26) as well as mock-plasmid ones (108.01±39.11). Then, in view of the average absorption of cell invasion between transfected groups(0.58±0.10) and non-transfected (0.94±0.23) as well as mock-plasmid ones(1.11±0.21), a prominently difference for cell invasion was also found between them (F=5.771, P<0.05). Conclusion The results suggest APRIL has a proliferation-inducing effect and probably be involved in cell invasion and cell metastasis of colon carcinoma and it could enhance capacity of invasion and metastasis of SW480 cells.
6.Effect of Wuzi Yanzong Fang Against Cyclophosphamide Induced DNA Damage in Mice

Objective To study protective effects of Wuzi Yanzong Fang on DNA damage induced by cyclophosphamide (CTX) in mice, and explore its mechanism. Methods BalB/c mice were randomly divided into normal group, model group, Wuzi Yanzong Fang low dose group and Wuzi Yanzong Fang high dose group. Mice in Wuzi Yanzong Fang groups were pretreated with Wuzi Yanzong Fang for 7 days, then the mice in Wuzi Yanzong Fang groups and model group were intraperitoneally injected with CTX (100 mg/kg) every other day for three times, and mice in Wuzi Yanzong Fang groups were continued administered with Wuzi Yanzong Fang. Animals were sacrificed in twelve hours after the final treatment of CTX. ELISA was used to detect 8-OHdG content in serum, and single cell gel electrophoresis to detect DNA damage in bone marrow cells. Results Wuzi Yanzong Fang low dose group and high dose group reduced the level of 8-OHdG in serum. Wuzi Yanzong Fang significantly decreased Olive tail moment, tail moment, tail length and tail DNA%in mouse bone marrow cells. Conclusion Wuzi Yanzong Fang has good protective effects on DNA damage caused by CTX.
7.Supported Ionic Liquid Solid Phase Extraction Coupled to Electrochemical Detection for Determination of Trace Bisphenol A

A simple and green method for the determination of trace bisphenol A( BPA) was established by coupling supported ionic liquid solid phase extraction to β-cyclodextrin modified ionic liquid carbon paste electrode-based electrochemical detection ( SILs-SPE-ED) . The synthesized imidazolium ionic liquid modified styrene type macroporous resin was used as adsorbent for SPE of BPA. The critical parameters that affect the extraction efficiency were optimized, including 0. 4 g of packing material quantity, 200 mL of sample solution at pH 7. 0, 5 mL of methanol as an eluent solvent, and 4. 5 mL/min of the SPE flow rate. Dynamic adsorption test showed that the maximum adsorption capacity of BPA on the SILs-SPE cartridge was 10. 1 mg/g, and the enrichment factor was 40 . The calibration curve showed a good linearity between the anodic current and the BPA concentration in the range of 1. 0×10-8-1. 0×10-6 mol/L (i. e. 2. 3-228μg/L). The detection limit was 4. 16×10-9 mol/L (equal to 0. 95 μg/L). The SILs-SPE-ED method was applied to the analysis of water and plastic samples and the results agreed well with HPLC method.
8.Clinical effect observation of expandable cage in treatment of lumbar spondylolisthesis

Objective To study the clinical effect of posterior lumbar interbody fusion (PLIF) using expandable cage in treatment of lumbar spondylolisthesis.Methods Twenty patients who underwent PLIF using expandable cage for lumbar spondylolisthesis were retrospectively analyzed.The operative time and transoperative bleeding were recorded.Postoperative regularity follow-up X-ray and CT,the olisthy rate,intervertebral height index and intervertebral fusion were observed.The lumbar function improvement was assessed by Oswestry disability index (ODI).Results All the patients were successfully operated,the operative time was 100-160 (123.3 ± 16.4) min,transoperative bleeding was 350-600 (464.0 ± 78.7) ml.The follow-up time was 12-36 (24.3 ± 7.3) months,symptoms were significantly improved.During the follow-up duration,20 patients obtained bony fusion.According to the reforming Macnab grade standard,excellent was in 14 cases,good was in 4 cases,acceptable was in 2 cases,the rate of excellent and good was 90.0%(18/20).The ODI score and olisthy rate postoperative 6 months and last follow-up were significantly lower than preoperative,and ODI score last follow-up was significantly lower than that postoperative 6 months,there were statistical differences (P < 0.05).The intervertebral height index postoperative 6 months and last follow-up were significantly higher than that preoperative,there were statistical differences (P <0.05).There was no statistical difference in olisthy reset rate between postoperative 6 months and last followup (P >0.05).The last followed-up,There were no expandable cage subsidence or displacement,intervertebral space collapse and intervertebral height loss.Conclusions As a new production,expandable cage is an effective tool on treating lumbar spondylolisthesis,it can effectively restore the intervertebral height and the biological mechanical properties of the lumbar vertebrae.This study indicates that expandable cage is an effective and safe products for lumbar interbody fusion,which maintained a lower complication rate and better results.
9.Excision-alcoholization-replantation method in management of bone tumors

Objective To evaluate the functional outcomes and complications of patients with bone tumors who were treated with the excision-alcoholization-replantation (EAR) method. Methods From August 1965 to August 2003, 191 patients treated with the EAR method who had suitable follow-up were evaluated in our department, including 102 males and 89 females with an average age of 20 years (range, 10-62). On the basis of Enneking tumor staging, 136 patients were identified II B stage, 25 I B stage, and 30 invasive benign tumors and tumor-like lesion. The length of the resected part ranged from 5 cm to 26 cm, with marginal resection in 30 cases, wide resection in 116. The oncological parameters that were evaluated included survival of the patients, local recurrence, and metastasis. Complications included non-union, infection, and fracture of the grafts. Mankin evaluation system was used to assess functional outcomes. Results The mean follow-up time was 32 months (range, 1-372), 52 patients died with disease at the final follow-up. Five-year survival rates of high grade sarcoma and low grade sarcoma were 59% and 90%, respectively, with statistical significance (P=0.02). Lung metastasis were demonstrated in 52 patients (27.2%, 52/191), 51 (26.7%, 51/191) had local recurrence. Except the tumor factor, the total complication rate was 50.3%(96/191). Local complications included 33 nonunion (17.3%, 33/191), 39 fracture of the grafts (20.4%, 39/191), 39 infections of the graft (20.4%, 39/191), 15 fracture of internal fixation (7.9%, 15/191), 5 instability or semiluxation of the joint (2.6%, 5/191). The 5-year survival rate of inactivated bone was 55%. On the basis of Mankin functional evaluation, the satisfied rate was 50.3%. At last there were 141 patients (73.8%, 141/191) preserved their extremities. Conclusion Compared to other methods in the same period, EAR method is considered on the same level of safety in oncology, which superiorities are economic-applicable to patients and the well fitness of bone graft with the defects.
10.APRIL mediates migration and invasion of colorectal cancer cells via regulation of matrix metalloproteinases

Objective To investigate the effects of a proliferation-inducing ligand(APRIL) on migration and invasion of colorectal cancer (CRC) and matrix metalloproteinases (MMPs) expression in order to observe the role of APRIL in CRC metastasis.Methods The siRNA plasmid vector targeting APRIL gene (siRNA-APRIL) was transfected into SW480 cells and recombinant human APRIL(rhAPRIL) was used to stimulate HCT-116 cells.Tumor cell migration and invasion were measured by Transwell chambers.RT-PCR and ELISA were applied to examine the expression level of MMPs.Results Metastatic and invasive capacities of siRNA-APRIL transfected SW480 were significantly inhibited,and these capacities of APRIL stimulated HCT-116 cells were significantly enhanced compared with their respective controls( all P<0.05 ),accompanied with the alterations of MMPs mRNA and secreted protein expression( P<0.05).The number of invading cells of SW480 control and rhAPRIL stimulated HCT-116 was significantly decreased by a MMP inhibitor GM6001 ( P<0.05 ).Conclusion APRIL facilitates migration and invasion of CRC via regulation of MMPs,which suggests that APRIL might be used as a new target for the intervention and treatment of CRC metastasis.
