1.Influence of early rehabilitation for cerebral hemorrhage patients on neurology and serum neuron-specific enolase

Objective To evaluate the influence of early rehabilitation for cerebral hemorrhage patients on neurology and serum neuron-specific enolase.Methods 82 cerebral hemorrhage patients were selected,30 patients without rehabilitation were set as the normal group,26 patients treated with early rehabilitation were set as the early rehabilitation group,and the other 26 patients with on-early rehabilitation were set as the normal early rehabilitation group.Neurological function,Barthel score and NSE score were compared in three groups.Results At 6 months, MESSS score of early rehabilitation group was (12.74 ±4.77)points,which were better than those of the other two groups (t=18.495,P=0.001;t=8.867.P=0.003);BI score was (78.32 ±8.44)points,which were better than those of the other two groups (t=16.231,P=0.002;t=7.428,P=0.002).At the 14th day,NSE of early rehabilita-tion group was (12.11 ±1.96)μg/L,which were better than those of the other two groups (t=6.886,P=0.004;t=6.728,P=0.004).At the first month,NSE of early rehabilitation group was (10.43 ±2.15 )μg/L,which were better than those of the other two groups (t=9.562,P=0.003;t=9.793,P=0.003).Conclusion Early rehabili-tation is helpful for cerebral hemorrhage patients to improve neurological function with good safety performance,so it can be applied in clinical treatment.
2.Clinical study of CT virtual endoscopy in diagnosing polyps of the gallbladder

0.05). CTVEGB could more correctly demonstrate the surface details of polyps which were viewed in a 3D fashion in any projection than ultrasound. CTVEGB corresponded well with color Doppler ultrasound and operation and pathology in demonstrating the size, configuration and location of the lesions. Conclusion CTVEGB is a noninvasive and accurate procedure, which has a important value in clinic applications.
3.The correlation between MRI features and pathological characterization of ovarian thecoma-fibroma

Objective To analyze the correlation between MRI features and pathological characterization of ovarian thecoma-fibro-ma.Methods MRI manifestations of 21 patients with ovarian thecoma-fibroma proved by pathological diagnosis were analyzed retro-spectively and compared with pathologic findings.Results The 21 cases involved 22 lesions (one patient had bilateral masses of the-cofibroma),including 13 fibrothecoma,4 thecofibroma and 5 fibroma.All lesions presented with well-defined margin,and cystic changes were found in 1 5 cases,7 lesions were solid masses ,1 9 lesions were round or oval masses ,2 lesions were lobulated mas-ses,and 1 lesion was irregular mass.The diameters of tumor ranged from 3.4 to 18.5 cm with mean diameter of 8.9 cm.On MR images,all lesions showed isointense or slight hypointense on T1 WI.On T2 WI,most tumors showed isointense or slight hypoin-tense,along with hyperintense cystic areas in 1 5 lesions,patchy slight hyperintense edema areas in 4 lesions,and homogeneous isointense or slight hypointense in 3 lesions.After contrast enhancement,all lesions demonstrated mild-to-moderate enhancement and delayed progressive enhancement slightly.13 cases associated with varied degrees of pelvic effusion,1 case with thickening of endometrium,and 1 case with endometrial carcinoma.Two patients had a higher level of CA125.Postoperative pathology demon-strated that all lesion were benign.Conclusion The MRI findings of the ovarian thecoma-fibroma are considerably characteristic and indirectly reflect pathology of tumor,thus beneficial to the correct diagnosis of tumor with relevant clinical information.
4.MR quantitative study of cerebrospinal fluid flow in sella region

Objective To quantitatively study the features of cerebromspinal fluid(CSF)flow dynamies in normal sella region in MRI with phase-contrast method.Methods Seventeen healthy volunteers were studied.The CSF flow image in sella region was positioned at the middle sagittal T1WI or T2WI image.This pulse sequence used a encoding velocity of 20 cm/s.The waveforms were analyzed for the maximum flow velocity,flow volume rate and the change of the figures.From the velocity and area measurements on the cine images,mean CSF flow was calculated in millimeters per second and milliliters per cardiac cycle.Results The normal CSF flow of the sella region had two directions which was downward(caudal)flow during thesystolic period and upward(cranial)flow during the diastolic period of the cardiac cycle.The downward and upward peak flow velocity,mean downward and upward flow velocity and mean flow velocity was(1.44±0.99)cm/s,(302.71±248.15)ms,(1.16±0.64)cm/s,(331.00±225.38)ms,(0.49±0.39)cm/s.(0.67±0.44)cm/s,(0.54±O.30)cm/s,respectively.The downward and upward peak flow volume rate.mean downward and upward flow volume rate and mean flow volume rate was (0.014±0.009)ml/s.(0.012±0.006)ml/s,(0.047±0.041)ml/s,(0.053±0.003)ml/s,(0.005±0.003)ml/s,(0.034±0.031)ml/s,respectively.The mean cycle was(775.25±173.06)ms.Conclusion Phase-contrast method in MRI cine is a noninvasive method to study the CSF flow in physiological and pathological conditions for determining the pattern,direction,speed and quantity of the CSF flow.Therefore it is better than other invasive research modalities and has an important value in clinical application.
5.Proton MR spectroscopy in multiple sclerosis:a preliminary study

Objective To study the characteristics and the regularities of the metabolites in multiple sclerosis(MS) with proton MR spectroscopy, and to investigate the clinical value of MR spectroscopy in multiple sclerosis. Methods The findings of conventional MRI and of ~1HMRSI from 8 cases of acute and chronic multiple sclerosis as and from 12 normal volunteers were comparatively analyzed, its all were using Philips Intera Nova Dual 1.5T MR system. Results ~1HMRSI of all multiple sclerosis as revealed different spectral peaks, including reduced N-acetylaspartate (NAA), creatine (Cr)and NAA/Pcr, prominent phosphorcreatine (Pcr),prominent choline (Cho), prominent lactate,(Lac), and prominent myoinositol (MI) in the acute MS. Normal Cho、Cr、Pcr、MI,void Lip reduced NAA and NAA/Pcr, in the chronic MS. Conclusion MRS is a noninvasive and accurate procedure ,it can reveal organic metabolizability and have capacious foreground.
6.Clinical Value of the Thickness of Pharyngeal Wall of Nasopharynx in Adenoid Hypertrophy Examined with CT

Objective To investigate the clinical value of the thickness of pharyngeal wall of nasopharynx in adenoids hypertrophy examined with CT. Methods CT manifestations in 117 patients suspected with adenoids hypertrophy were analysed. The thickness of adenoid and pharyngeal wall were observed. The width of adenoid/pharynx(A/N) ratio were measured and the correlation of pharyn-geal wails with A/N ratio were analysed. Results A/N ratio was ≥0.70 in 50 cases (group Ⅰ), between 0.60 ~ 0.70 in 35 cases (group Ⅱ) ,≤0.60 in 32 cases (group Ⅲ) ,the thickness of pharyngeal wall were (1.26±0.33) cm, (0.99±0.41) cm and (0.86± 0.19) cm respectively. There were significant differences between group Ⅰ and Ⅱ, Ⅲ in the thickness of pharyngeal wails(P<0.01). The relationship of linear regression of A/N ratio with pharyngeal wall was found and the equation of the thickness of pharyngeal wall was 1.69×A/N ratio-0.08. Conclusion There is correlation between A/N ratio and thickness of pharyngeal wall, which is helpful for clinical diagnosis of adenoid hypertrophy.
7.Application of Dynamic Susceptibility Contrast-Enhanced MR Imaging in Evaluation of Brain Tumor after Operation

Objective To evaluate the value of dynamic susceptibility contrast-enhanced MR imaging (DSC)in evaluation of brain tumor after operation.Methods 41 cases with brain tumors verified histologically,including 8 cases of high-grade gliomas,16 of low-grade gliomas,7 of metastases and 10 of meningiomas underwent DSC examination.All the cases at 3.0T MR unit.All primitive data were sent to ADW4.3 workstation for post-processing.The mean maximal rCBV and rCBF ratios of tumors,1 cm and 2 cm regions of peri-tumor were obtained respectively.The relativity of MR perfusion parameters(rCBV and rCBF)between diferent tumors were statistically evaluated by using Pearson correlative analysis.Results In the solid region of tumors,rCBV and rCBF were statistically significant differences (P<0.05)between low-grade gliomas and the other three groups of tumors; in 1 cm region of peri-tumor between high-grade gliomas and the other three groups,and between low-grade gliomas and metastases,between the solid region and both regions of 1 cm and 2 cm peri-tumors at the same tumor were all of statistically significant differences(P<0.05),between 1 cm and 2 cm region of peri-tumors at high-grade gliomas and metastases,the rCBV and rCBF were also of statistically significant differences.Pearson correlative analysis showed that there were significant positive correlations between the mean maximal rCBV and rCBF ratios at the same region in the same tumor's group.Conclusion DSC is very valuable in evaluation of the histopathological grade and the differentiation of brain tumor.
8.Establishment and evaluation of anesthesia nurses training system for postoperative delirium assessment

Objective:To establish training system for postoperative delirium (POD) assessment and evaluate the efficacy of training for anesthesia nurses.Methods:Sixteen nurse anesthetists of both sexes in our hospital were selected and received the systemic training for POD assessment.The training system included questionnaire survey, theoretical teaching, simulated visit, clinical observation, independent evaluation, centralized question-answering, evaluation of efficacy and random inspection.The level of POD knowledge tests were performed before the training and at the end of the fourth week of independent evaluation, respectively.At week 1 and 4 of independent evaluation, the diagnostic rate of POD and sensitivity and specificity of the assessment were calculated, and Kappa consistency analysis was used to assess the consistency between anesthesia nurses and training group in diagnosis of POD.In the first week of the third month after the end of training, the evaluation results were randomly inspected, the POD diagnosis rate was calculated between the anesthesia nurses and the training group, and the consistency analysis was conducted.Results:Compared with the scores of POD knowledge questionnaire and sensitivity of the assessment of the anesthesia nurses in the first week of training, the scores were significantly increased ( P<0.05), and no significant change was found in the POD diagnosis rate in the fourth week of training ( P>0.05). Compared with the training group, the diagnosis rate of POD of anesthesia nurses was significantly decreased in the first week of training ( P<0.05), and no significant change was found at the fourth week of training ( P>0.05). In the first and fourth weeks of training, the Kappa value of anesthesia nurses and the training group was 0.676 and 0.954 ( P<0.001), respectively.In the first week of the third month after the end of training, the Kappa value between anesthesia nurses and the training group in diagnosis of POD was 0.862 ( P<0.05). Conclusion:The training system of POD assessment has been successfully established, and the standardized anesthesia nurses training of POD has been achieved with good results.
9.Relationship between prognostic nutritional index and myocardial injury after non-cardiac surgery

Objective:To evaluate the relationship between prognostic nutritional index (PNI) and myocardial injury after non-cardiac surgery (MINS).Methods:This was a retrospective cohort study. The clinical data of adult patients ( n=2 203) who underwent liver resection surgery with general anesthesia at our center from January 2016 to August 2019 were retrospectively collected. The predictive value of preoperative PNI for MINS and the optimal cut-off value of PNI were evaluated and determined according to the receiver operating characteristic curve, and the patients were divided into 2 groups based on the cut-off value: high PNI group and low PNI group. Logistic regression analyses were applied to investigate the relationship between preoperative PNI and MINS. According to the same inclusion and exclusion criteria, the clinical data of patients at our center from January 2022 to December 2023 were collected as the validation set ( n=2 525), and they were grouped using the same PNI cutoff value. Logistic regression analyses were used to verify the relationship between PNI and MINS. Results:The receiver operating characteristic curve analysis showed that the area under the curve of preoperative PNI for predicting MINS was 0.651 (95% confidence interval [ CI] 0.602-0.699), with an optimal cut-off value of 46.193, and the specificity and sensitivity were 0.729 and 0.519 respectively. The integer 46 was considered as the optimal cutoff value for PNI, and the patients were divided into low PNI group (PNI<46, n=606) and high PNI group (PNI≥46, n=1 597). Both univariate and multivariate logistic regression analyses showed that preoperative low PNI was an independent risk factor for the occurrence of MINS (univariate: OR=2.873, 95% CI 2.063-4.003, P<0.001; multivariate: OR=1.844, 95% CI 1.241-2.600, P=0.003). The results in the validation set were still robust (univariate: OR=2.694, 95% CI 1.890-3.833, P<0.001; multivariate: OR=1.602, 95% CI 1.071-2.385, P=0.021). Conclusions:Preoperative low-level PNI is an independent risk factor for MINS, with a certain predictive value.

The use of data ferry technology avoids hidden safety hazards in hospitals, realizes the integration and sharing of perioperative medical data of various medical institutions, and forms a complete data chain combining patient medical data and follow-up data.
