1.Correlation of the expression of Noxa with hepatocellular carcinoma

Purpose To explore the correlation of Noxa gene with clinical pathology and its effect on proliferation and apoptosis of HepG2 cells.Methods Noxa protein in 100 hepatocellular carcinoma tissues and 100 paracancerous tissues were detected by imnunohistochemical technique,clinicopathological parameters and follow-up data were statistically analyzed.The recombinant eukaryotic expression plasmid pIRES2-EGFP-Noxa was transiently transfected into HepG2 cells with lipofectamine.Both Noxa mRNA and protein were detected by RT-PCR and Western blot respectively.The inhibition of cell proliferation was evaluated by MTT assay.The cell apoptosis was detected by flow cytometry (FCM).Results Immunohistochemistry showed that in tumor tissue of hepatocellular carcinona the positive rate of Noxa protein expression was 50%,while it was 78% in cancer adjacent normal mucosal tissue,with statistically significant difference (P < 0.05).The expression of Noxa was related to TNM staging and the tumor differentiation.Exotic Noxa gene was expressed successfully in HepG2 cells after transfected with lipofectamine.The expression of Noxa mRNA and protein of HepG2 cells was significantly up-regulated after transfected 24 h,48 h and 72 h (P < 0.05).MTT assay showed that the Noxa expression inhibited HepG2 cell proliferation in a time-dependent manner for 24 h,48 h and 72 h (P < 0.05),which was significantly different from that of the control group (P < 0.05).Apoptosis rate after transfected 24 h,48 h and 72 h was (15.5 ± 0.9)%,(24.6 ±0.8) % and (35.4 ±0.7) %,respectively.There were significant difference between Noxa and the control group (P < 0.05).Conclusion The expression of Noxa in hepatocellular carcinoma tissue is closely related to TNM stage and the tumor differentiation.Noxa gene overexpression is able to effectively inhibit the proliferation and promote the apoptosis of HepG2 cell line.
2.Effect of muscarinic acetylcholine receptor agonist on glycinergic spontaneous inhibitory postsynaptic currents and micro-inhibitory postsynaptic currents in spinal lamina Ⅱ neurons of rats

Objective To investigate the effect of the mascarinic acetylcholine receptor (mAChR) agonist Oxo-tremorine-M (Oxo-M) on glycinergic spontaneous inhibitory postsynaptic currents (sIPSCs) and micro-inhibitory postaynaptic currents (mIPSCs) in lamina Ⅱ neurons in the spinal cord of rats. Methods Glycinergic IPSCs (sIPSCs and mIPSCs) in lamina Ⅱ neurons of spinal slices were recorded using the whole-cell voltage-clamp method. The non-selective mAchR ngonist Oxo-M was applied through bath perfusian. The effects of Oxo-M 1, 3, 5 and 10 μmol/L on sIPSCs and mIPSCs were examined. Results Oxo-M at the concentrations of 3-10 μmol/L significantly increased the frequency of sIPSCs without changing the amplitude in 16 lamina Ⅱ neurons tested. Interestingly, when the concentration of Oxo-M was increased to 10 μmol/L, the potentiating effect of Oxo-M on the frequency of slPSCs was decreased as compared with 3 μmol/L Oxo-M in the above 16 neurons. The slPSCs were completely abolished by 2 μmol/L strychnine. Atropine, the specific mAChR antagonist, completely blocked the effect of Oxo-M on the frequency of sIPSCs. In 9 additional lamina Ⅱ neurons, 1-10 μmol/L oxo-M failed to alter significantly the frequency and amplitude of glycinergic mIPSCs. Conclusion Activation of mAChRs in the somatodendritic site of glycinergic interneurous increases the synaptic glycine input to spinal dorsal horn neurons, but not in a dose-dependent manner.

4.T-cell Subpopulations, ASO, IL-6 and IL-8 in Children with Tic Disorder

Objective: To observe the changes of blood T-cell subpopulations, ASO,IL-6 and IL-8 in children with Tic disorder and explore the pathogenesis. Methods: T-cell subpopulations and concentration of IL-6, IL-8, ASO were measured in 52 children with tic disorder and 30 normal children using SAP and ELISA.Results:The numbers of CD3+、CD4+ and CD8+ cells had no significant difference between 32 children with tic disorder and 30 normal children, and the concentration of ASO in serum of children with tic disorder was markedly higher than those in normal children. The concentrations of IL-6 and IL-8 in serum had no significant difference between the two groups.Conclusion: There may be dysfunction in humoral immunity in tic disorder.
5.Mini-Incision Dismembered Pyeloplasty Assisted by Retroperitoneal Laparoscopy in the Treatment of Adult Ureteropelvic Junction Obstruction

Objective To compare the efficacies of mini-incision dismembered pyeloplasty assisted by retroperitoneal laparoscopy and routine retroperitoneal laparoscopic dismembered pyeloplasty for ureteropelvic junction(UPJ) obstruction.MethodsThe clinical data of 47 patients with UPJ obstruction,who underwent dismembered pyeloplasty from January 2004 to November 2006 in our hospital,were retrospectively analyzed.Of the patients,22 were treated by mini-incision dismembered pyeloplasty assisted by retroperitoneal laparoscopy(group A),in whom a 4-cm incision was made under the costal margin at the posterior axillary line;and the other 25 cases underwent retroperitoneal laparoscopic dismembered pyeloplasty(group B).The clinical data of the two groups,including the operation time,blood loss,average airway resistance and partial pressure of carbon dioxide,recovery of intestinal function,length of drainage and hospital stay,incidence of urinary leakage,and remission rate of hydronephrosis and improvement of GFR,were statistical analyzed by SPSS 13.0.ResultsNo intra-operative complications occurred in either group.Compared with Group B,Group A had shorter operation time [(92.9?16.3) min vs(155.8?18.6) min,t=-12.251,P=0.000],more blood loss [(18.9?6.3) ml vs(13.6?6.7)ml,t=2.782,P=0.008],lower average airway resistance [(15.6?2.6) cm H2O vs(26.9?4.3) cm H2O,t=-10.715,P=0.000] and partial pressure of carbon dioxide [(36.0?6.9) cm H2O vs(51.6?6.7) cm H2O,t=-7.855,P=0.000].No statistical differences were detected in the recovery of intestinal function,postoperative length of drainage and hospital stay,incidence of urinary leakage,and remission rate of hydronephrosis and improvement of GFR.ConclusionMini-incision dismembered pyeloplasty assisted by retroperitoneal laparoscopy is a safe,effective,and minimally invasive therapy for UPJ obstruction.
6.Hypercatecholaminism(report of 95 cases)

Objective To evaluate the improvement of diagnosis and treatment of hypercatecholaminism. Methods A total of 95 cases of hypercatecholaminism were reviewed.Of them 74 cases were of pheochromocytoma and 21 cases were of adrenal medullary hyperplasia (AMH).In pheochromocytoma group,67 cases (90.5%) had symptom of hypertension.Increase in 24 h urinary VMA occurred in 62 cases (83.8%) and elevated urinary catecholamine level in 67(90.5%).All thses had positive findings of ultrasound,CT scan and MRI.In AMH group,all the patients were hypertensive and their 24 h urinary VMA was increased.Most of them (15/16) had higher urinary catecholamine level than normal;14 out of the 21 cases had positive findings of B-ultrasound examination. Results Surgical operations were performed in the 74 patients with pheochromocytoma including 9 cases of extra-drenal pheochromocytoma.Of them 5 cases were confirmed as malignant pheochromocytoma.In the 21 cases of AMH,18 undewent surgical operation.Of them 12 received unilateral resection and 6 received bilateral resection of adrenal gland.The pathological examination confirmed the diagnosis of AMH. Conclusions The diagnosis of pheochromocytoma should be focused on endocrinological examination,ie,qualitative examination,whereas it is diffucult to localize the lesion in AMH compared with pheochromocytoma.The establishment of notion of AMH as an isolated clinicopathological entity broadens and perfects the theory of hypercatecholaminism.
7.Modulatory effects of midazolam on the expression of vascular endothelial growth factor after cerebral ischemia-reperfusion injury in Mongolian gerbil

Objective To study the effects of midazolam on expression of vascular endothelial growth factor (VEGF) of gerbils following total cerebral ischemia-reperfusion injury to look for an experimental basis for the rational clinical use of midazolam. Methods Seventy-two male gerbils (Mongolian gerbil) were randomly assigned into three groups (24 each): sham injury group, injury group and midazolam treatment group. Total cerebral ischemia was reproduced by blocking the bilateral carotid arteries for 10 minutes with bulldog clamps. When reperfusion began, with release of the clamps, 5mg/kg of midazolam was intraperitoneally injected to the animals in midazolam group, and 50ml/kg of normal saline was given by the same way in the gerbils in injury group. Then the parameters listed below were observed: positron emission tomography (PET) images at 6h, 1d, 3d and 7d after reperfusion, and the expression of VEGF in cerebral tissue was immunohistochemically assessed. 9.Application of a self-made distraction reductor in percutaneous minimally invasive treatment of calcaneal fractures

Objective:To evaluate our self-made distraction reductor used in the percutaneous minimally invasive treatment of calcaneal fractures.Methods:A retrospective study was conducted of the 32 patients (37 feet) who had been treated at Department of Orthopaedics, The First Affiliated Hospital to Chongqing Medical University from January 2017 to December 2019 for calcaneal fractures. They were 26 males and 6 females, aged from 20 to 67 years (average, 46.5 years). The height and length of the calcaneus were restored by our self-made tri-plane distraction reductor in all patients; the displaced fracture blocks were reset in a minimally invasive manner in patients with intra-articular fracture; final fixation was performed with a minimally invasive plate and screws. Time for reductor installation, operation time and intraoperative blood loss were recorded; complications, fracture union and reduction were observed; height, length and width of the calcaneus, ankle-hindfoot scores of American Society of Foot and Ankle Surgery (AOFAS), Maryland scores, and visual analogue scale (VAS) were recorded at the last follow-up; B?hler and Gissane angles were compared between pre- and post-operation.Results:All patients were followed up for 12 to 36 months (average, 17.2 months). Time for reductor installation averaged 5.0 min (from 3 to 8 min), operation time 91.5 min(from 70 to 110 min), and intraoperative blood loss 25.2 mL (15 to 50 mL). Superficial infection of traction track occurred in one patient and sural nerve injury in one patient. All fractures united without any reduction loss by the last follow-up. On average, the last follow-up observed a calcaneal height of 39.3 mm, a calcaneal length of 70.6 mm, a calcaneal width of 32.7 mm, an AOFAS score of 87.8, a Maryland score of 86.7 and a VAS score of 2.2. The calcaneal B?hler angle was recovered significantly from 3.7°±13.7° preoperatively to 25.8°±6.4° at the last follow-up, and the calcaneal Gissane angle from 112.2°±21.3° preoperatively to 125.8°±5.7° at the last follow-up ( P< 0.05). Conclusion:In percutaneous minimally invasive treatment of calcaneal fractures, application of our self-made tri-plane distraction reductor can lead to fine clinical efficacy.
10.Clinical application of duct-to-mucosa anastomosis technique for pancreaticoduodenectomy

Objective To investigate the effect and safety of duct-to-mucosa anastomosis technique for pancreaticoduodenectomy(PD).Methods The clinical data,including pancreatic fistula and other complications,of 189 patients underwent PD with end-to-side pancreaticojejunostomy at our institution from Jan 2001 to Jan 2009 were analyzed retrospectively.The definition of pancreatic fistula was threefold increase over the serum amylase level 7 days after operation,and draining volume was more than 50 ml per day.Results Totally 177 Whipple procedures were performed,while 12 pylorus-preserving pancreateduodenectomy procedures were performed.Five patients developed pancreatic fistula with a incidence of 2.65%(5/189).In which 3 were mild Cases,who fully recovered after conservative management,and the other 2 cages were cured by surgical intervention.Other complications included 9 cases of wound infection(4.76%,9/189),11 cases of empty dysfunction(5.82%,11/189),5 Cases of delayed hemorrhage(2.65%,5/189),and 4 cases of intra-abdominal infection(2.12%,4/189),and 2 patients died due to severe intra-abdominal infection and acute pulmonary infarction.Conclusions Duet-to-mucesa anastomosis technique resembles physiological state with low incidence of pancreatic fistula and delayed anastomosis hemorrhage.It may be used for different kinds of anastomosis for pancreatic stump.
