1.A study of relationship between serum SIL-2R titer and severity of involved muscles in patients with myasthenia gravis

Objective To study the relationship between serum soluble interleukin-2 receptor (SIL-2R) titer and severity of involved muscles and transient aggravation due to large dose corticosteroid therapy in patients with myasthenia gravis.Methods 30 severe patients with MG were classified according to improved Osserman method. The involved voluntary muscles were divided into seven groups. The severity of MG was recorded with five grades and four parts formulation. The absolute integral was taken as the index of MG degree. ELISA was used to measure the titer of serum SIL-2R.Results Remarkable difference was found between patient group and control group both in the SIL-2R titer and the severity of MG(all P0.05) when the cases aggravated transiently during large-dose corticosteroid therapy.Conclusion Cell immunity is abnormal in patients with MG. SIL-2R may reflect the severity of involved muscles.

3.Integrin ?_1 expression during mandibular fracture healing

砄bjective:To investigate the expression of integrin ? 1 during mandibular fracture healing.Methods:Rabbit mandibular fracture model was used,the fractured bone tissues were obtained and paraffin slices were made on the day 1,3,5,7,14,30,60 and 90 after surgery respectively.Non fractured mandible was used as the control.The expression of integrin ? 1 in bone tissue, especially in osteoblasts and osteoclasts, was studied with LsAB immunohistochemical method.Results:Integrin ? 1 was widely espressed in the bone tissure close to fracture and surrounding soft tissue.In osteoblasts increased expression of integrin ? 1 was observed on day 7,maximum on day 14~30 and normal on day 90.While in osteoclasts,it was increased and kept at maximum 14~90 days aftere fracture.Conclusion:During mandibular fracture healing,the expression of integrin ? 1 in bone,especially in osteoblasts and osteoclasts,was increased.Integrin ? 1 has a critical influence on bone fracture healing.

To investigate the effect of selective N-methy-D-aspartate receptor antagonist MK-801 on dentate granule cell neurogenesis after diffuse brain injury in the adult rat, so as to explore the role of glutamic acid on dentate granule cell neurogenesis after diffuse brain injury. 45 adult male SD rats were subjected to diffuse brain injury and randomized into MK-801-treatment group, vehicle-treatment group and control group (n=15 each). By using bromodeoxyuridine ( BrdU ) labelling method and immunohistochemistry to observe dividing cells, the proliferation rates of neural precursor cells in the dentate gyrus(DG) were compared between MK-801-treatment group and corresponding control groups on 2, 4, 6, 8, and 12 days after diffuse brain injury. To improve higher training quality and capacity of orthopedic postgraduates,early contact education,problem-base learning combined with co-supervisor with radiologists were engaged for orthopedic postgraduates since 2008.Effect of the practice was assessed by postgraduates one year later.Investigation revealed that practice aroused learning enthusiasm and initiative,promoted comprehensive understanding of orthopedic knowledge,set up social responsibility,and constructed innovative thinking.
7.Indications for screw fixation of posterior malleolas fractures

BACKGROUND:Posterior maleolar fractures are often accompanied by ankle joint instability,if the stability of ankle joint is not recovered,it is prone to traumatic arthritis of the ankle.However,the indications of internal fixation of posterior maleolar fractures remain controversial.OBJECTIVE:To explore the indications for internal fixation of posterior maleolar fractures by comparing the clinical effects of posterior maleolar fractures treated with internal fixation or not.METHODS:42 patients with maleolar fractures involving posterior ankle were recruited from the First Affiliated Hospital of Chongqing Medical University from January 2007 to January 2012.According to preoperative CT scans of ankle joint,42 cases were divided into the internal fixation group and the non-fixation group.27 cases in the fixation group had posterior maleolar fractures in more than 10%of the distal tibial articular surface and/or dislocation of the posterior maleolar fractures greater than 2 mm,and were treated with screws.15 cases in the non-fixation group had posterior maleolar fractures in less than 10%of the distal tibial articular surface anddislocation of the posterior maleolar fractures less than 2 mm,and were treated with non-operation.The average healing time of posterior maleolar fractures,postoperative complications,ankle-hindfoot scores of American Orthopedic Foot and Ankle Society were compared during the folow-up postoperatively.RESULTS AND CONCLUSION:Al of 42 patients with ankle fractures achieved bony union within 6 months.In the fixation group,1 case had postoperative superficial infection of the wounds in the medial ankle and lateral ankle,and the wound got healed completely with anti-infection therapy.One case in each group had postoperative superficial necrosis in the incisional edges,and got healed by dress changing.For the other cases,there was no wound infection,internal fixation loosening,breakage or failure and other adverse events during the folow-ups.Ankle functions were evaluated one year after operations according to the ankle-hindfoot score standard of American Orthopedic Foot and Ankle Society.The score of the fixation group was (83.74±10.35) points,with excelent ankle functions in 10 cases,good in 12 cases,fair in 5 cases,and no poor case,and the rate of patients achieving excelent and good ankle functions reached 82%; the score of non-fixation group was (85.60±10.40) points,with excelent ankle functions in 7 cases,good in 5 cases,fair in 3 cases and no poor case.The rate of patients achieving excelent and good ankle functions reached 80%.The ankle function evaluation results of both groups showed no statistical difference (P>0.05).It is reasonable to consider posterior maleolar fractures is higher than 10%of the distal tibial articular surface and/or dislocation of the posterior maleolar fractures greater than 2 mm as the indications for internal fixation of posterior maleolar fractures.

In forensic case, the location diagnosis determination of drowning has been one of the most important and dififcult diagnosis. Then diatom test is considered to be a relatively reliable method for the diagnosis of drowning. According to community characteristics inferred from diatoms into the different water region has been credibility. The study of the diatoms in different waters identiifcation is reference to the determination of the body drowned into the water area. In this paper, the research progress of diatom relevant biological characteristics and test methods on review for forensic workers is related to the further research and reference case in practice.
9.Biomechanic analysis of the stability and vertebra strain distribution of asymmetrical posterior internal fixation in minimally invasive transforaminal lumbar interbody fusion

Objective To assess the biomechanical stability and vertebra strain distribution of asymmetrical posterior internal fixation for minimally invasive transforaminal lumbar interbody fusion ( MI-TLIF) . Methods Range of motion ( ROM) and strain distribution testing were performed in 8 fresh-frozen calf lumbar spine motion segments in flexion/extension, lateral bending, and axial rotation using 5. 0 Nm torques at the L4-5 motion segment. The sequential test configurations included intact motion segment, TLIF with unilateral pedicle screw ( UPS) , TLIF with UPS plus transfacet pedicle screws ( UPS+TFPS) , and TLIF with bilateral pedicle screw ( BPS) . The ROM was deter-mined to assess the construct stability. Strain distribution was recorded along with flexion and lateral bending configurations. Results In flexion/extension, lateral bending, and axial rotation, there was no significant difference in the ROM between BPS and UPS+TFPS fixation after TLIF. After TLIF, the UPS construct provided less segment stability than BPS and UPS+TFPS fixation in flexion, lateral bending. Strain distribution under UPS+TFPS fixation was respectively 21. 8% and 24. 2% higher than that under BPS fixation along with flexion and lateral bending. Conclusion UPS+TFPS fixation provides stability comparable to that of MI-TLIF with bilateral PS, with better load share with the vertebrae body.
