1.Effect of HPMC on release characteristics of cefaclor sustained-release tablets in vitro

Objective To investigate effect of hydroxypropyl methyl cellulose (HPMC) to in vitro release characteristics of cefaclor sustained-release tablets.Methods Collected 10 batches of HPMC from different manufacturers.Determined the viscosity, molecular weight and the distribution of molecular weight of HPMC.HPMC from different manufacturers was used as blocking agent for the preparation of cefaclor sustained-release tablets according to the same prescription.Results The molecular weight was positively related to the viscosity of HPMC.The more molecular weight was, the slower the drugs release rate was,the better controlled released.Conclusion HPMC from different manufacturers show different quality stability and effect in vitro release of cefacor sustained-release tablets.
2.The effect of adaptive cognitive training on cognitive function in the elderly: a review

Current studies have found that cognitive training has a significant effect on the improvement of training task performance in the elderly, but the training effects are difficult to transfer to untrained cognitive tasks.Recent studies have found that adaptive cognitive training can obtain better transfer effects.This article focuses on the transfer and maintenance effects of adaptive cognitive training; reviews recent research on brain mechanisms related to adaptive cognitive training; analyzes existing problems in the field, and proposes further studies to explore ways for effective implementation of adaptive cognitive training with continuously challenging tasks and to elucidate the cognitive and neural mechanisms related to the transfer effects.
3.Clinical observation on Yi Jin Jing(Sinew-transforming Qigong Exercises)in improving anxiety for asymptomatic patients with COVID-19 infection during quarantine

Objective:To observe the effect of Yi Jin Jing(Sinew-transforming Qigong Exercises)intervention on anxiety in asymptomatic patients with Corona Virus Disease 2019(COVID-19)infection during quarantine.Methods:A total of 160 asymptomatic patients with COVID-19 infection were stratified by gender and divided into an observation group and a control group by the stratified randomization method,with 80 cases in each group.The control group was given basic nursing in the cabin,and the observation group was given additional Yi Jin Jing exercises once a day,20 min each time,and trained continuously until 1 d before leaving the cabin.The Hamilton anxiety scale(HAMA)score was compared between the two groups.Results:A total of 154 cases were included for data analysis in this study,including 74 cases in the observation group and 80 cases in the control group.After intervention,the HAMA scores in both groups increased(P<0.05),while the score in the observation group was lower than that in the control group(P<0.05).In the control group,the HAMA score of females after intervention was higher than that of males.The scores of mental anxiety and somatic anxiety in both groups were higher than those before intervention,while the score of mental anxiety in the observation group was significantly lower than that in the control(P<0.05).However,there was no statistical difference in the somatic score between the two groups(P>0.05).Conclusion:Patients with COVID-19 infection will get anxious during quarantine.Yi Jin Jing exercise can effectively reduce the aggravation of anxiety in asymptomatic patients with COVID-19 infection during concentrated quarantine.
4.Clinical and prognostic analysis of seven cases of neonatal congenital hyperinsulinemia

Objective:To explore the clinical features, treatment, prognosis and genetic mutation in neonatal congenital hyperinsulinemia(CHI).Methods:Neonates with CHI admitted to the neonatal intensive care unit of Hebei Provincial Children′s Hospital from February 2017 to August 2020 were selected, and their clinical characteristics, diagnosis and treatment, prognosis and genetic mutation were retrospectively analyzed.Results:A total of seven neonates were enrolled.The average gestational age was(38.1±1.5)weeks with two cases gestational age<37 weeks.The mean birth weight was(3 608±906)g with three cases birth weight>4 000 g. The common clinical manifestations included lethargy, poor feeding, cyanosis, seizures, and tremble.Non-specific manifestations were observed in two premature infants, whose blood glucose were found very low during the routine monitoring at 1 hour and 3 hours after birth respectively.Among the seven cases, six cases needed high glucose infusion rate(GIR)[>10 mg/(kg·min)] to maintain the serum glucose at the normal level from the beginning.Only one case needed lower GIR[3-5 mg/(kg·min)] on admission while gradually increased to 8 mg/(kg·min) maximumly during hospitalization.All seven neonates were treated with diazoxide orally, and two cases(2/7) were effective, including one case who discontinued the drug at six months after birth with normal blood glucose level.The remaining five neonates(5/7) were diazoxide resistant due to mutations in the ABCC8 gene encoding the K ATP+ -channel of the pancreatic beta cell and then treated with octreotide.Two cases(2/5) of them were effective to octreotide and the other three cases(3/5) were both diazoxide and octreotide resistant.One case died after withdrawal from the treatment and the other one lost follow-up.The other five cases were followed up until now.Normal neurological development were found in three cases.Two cases were found with epilepsy and moderate developmental delay in language and social competence ability during the follow up.Mutations in ABCC8 were the most common in seven cases, of which six cases were heterozygous mutation of ABCC8 and one case was heterozygous mutation of GLUDI. Conclusion:The clinical manifestations of CHI are non-specific.The blood glucose level of the high-risk neonates should be timely monitored.Neonates who needs lower GIR[<8 mg/(kg·min)] at the early stage can not be completely excluded CHI.Some CHI cases may self-resolved after several months.Molecular diagnosis can identify the pathogenic genes, which is important to achieve accurate diagnosis and treatment, and thus improve the prognosis of patients with CHI.
5.An exploration of the training and evaluation systems of medical undergraduates in the context of tutorial system

Objective To investigate the training process of clinical practice ability in medical undergraduates in the context of tutorial system,as well as new clinical training models and evaluation methods.Methods A total of 40 medical undergraduates who entered the stage of clinical practice were randomly divided into tutorial group and control group,with 20 undergraduates in each group.A diversified evaluation system was used to perform a comprehensive evaluation of clinical practice ability of medical undergraduates at different stages,with a combination of professional theory examination,Mini-Clinical Evaluation Exercise,Direct Observation of Procedural Skills,and clinical case report.All the data were analyzed by SPSS 23.0,t test was used for statistical analysis.Results Compared with the control group after the internship ended,the tutorial group had significantly higher scores of clinical professional theory (86.24 ± 2.93),clinical practice ability (7.53 ± 0.48),clinical operation skills (4.35 ± 0.47),and clinical thinking ability (6.31 ± 0.52) (all P<0.05).Conclusion The application of the tutorial training mode during the internship of medical undergraduates can improve their comprehensive qualities such as professional quality,medical theory,and practical skills.The diversified evaluation system can evaluate the teaching effect and reflect the cultivation of comprehensive quality among students.

7.Four-dimensional Path Interpretation of Ideological and Political Construction of Doctor-patient Communication Course

The development of medical and health services calls for a harmonious doctor-patient relationship. From the perspective of education, good communication is a positive factor in building a harmonious doctor-patient relationship ecology. The action path to strengthen the ideological and political construction of the doctor-patient communication course is the need to cultivate high-quality medical students, which is conducive to the ideological and political education of medical students and the teaching effect of the doctor-patient communication course. To improve the ideological and political construction system of the doctor-patient communication course, efforts should be made in four dimensions: strengthen the ideological and political quality of teachers, excavate and make good use of ideological and political elements of the doctor-patient communication course, enhance the dominant and recessive dual educational narration, form a joint force mechanism for the ideological and political system of the doctor-patient communication course.
8.The formula of ω-3 polyunsaturated fatty acids improving cognitive impairment in patients with depres-sion:a clinical randomized double-blind controlled trial

Objective To investigate the effects of different ratios of ω-3 polyunsaturated fatty acids(ω-3 PUFA)on depression and cognitive impairment in patients with major depression.Methods A randomized,double-blinded controlled trial was used to randomly assign patients with depression to a cognitive improvement group,a depression improvement group,and a placebo group.The cognitive improvement group took 1388 mg of docosahexaenoic acid(DHA)and 692 mg of eicosapentaenoic acid(EPA)every day and the depression improvement group took 1248 mg of EPA and 832 mg of DHA every day.The placebo group took the same dose of soybean oil for 12 weeks,during which psychiatric medication was maintained.The 24-item Hamilton depression scale(HAMD-24)was used to evaluate depressive symptoms,and the standardized MATRICS consensus cognitive battery(MCCB)was used to evaluate cognitive function after 6 weeks and 12 weeks,respectively.Results The study recruited a total of 46 patients with depression including 22 in the cognitive improvement group,12 in the depression improvement group,and 12 in the placebo group.After 6 weeks of treatment,the HAMD-24 scores were significantly lower in the depression improvement group(19.00±10.70)and cognitive improvement group(16.58±9.39)than in the placebo group(31.10±10.03)(P<0.01).After 12 weeks of treatment,HAMD-24 scores were significantly lower in the depression improvement group(13.58±8.43)than in the placebo group(28.10±15.04)(P=0.02).No significant interaction effect was found on the cognitive assessment scores in any dimension after 6 weeks and 12 weeks of treatment(P>0.05).The incidence rate of adverse events in the depression improvement group was 16.7%(2/12),and no adverse events were reported in the other two groups.There was no significant difference in the incidence of adverse events among the three groups(P=0.13).Conclusion Treatment with ω-3PUFA for 6 weeks can improve the depressive symptoms of patients with depression.The formula with a higher ratio of EPA exhibits higher effectiveness while the two groups of ω-3PUFA formulas with different ratios do not improve cognitive function.
9.Association between interferon gamma receptor 1-56C/T gene polymorphism and tuberculosis susceptibility: a meta-analysis.

BACKGROUNDGenetic variations in the interferon-gamma (IFN-γ) receptor 1 gene (IFNGR1) may contribute to tuberculosis (TB) risk in different populations. Many studies have investigated the relationship between IFNGR1 56C/T polymorphism and the susceptibility to TB, but have yielded conflicting results. A comprehensive meta-analysis is needed to provide a more accurate estimation of the relationship between them.
METHODSA literature search based on a combination of manual and computer-based methods was conducted on four English databases (PubMed, Science Direct, SpringerLink, and EBSCO) and three Chinese databases (Wanfang, CQVIP, and Chinese National Knowledge Infrastructure databases). Pooled odds ratios (ORs) and 95% confidence intervals (95% CIs) were calculated using either the fixed-effects model or the random-effects model for different genetic models based on the heterogeneity examination.
RESULTSA total of six studies comprising 1 497 confirmed TB cases and 1 802 controls were included in this meta-analysis. Overall, no significant association was observed between IFNGR1 -56C/T polymorphism and TB susceptibility (C vs. T, OR = 0.90, 95% CI 0.69-1.17; CC vs. TT, OR = 0.87, 95% CI 0.65-1.18; TC vs. TT, OR = 1.031, 95% CI 0.872-1.219; CC+TC vs. TT, OR = 0.89, 95% CI 0.64-1.26; CC vs. TC+TT, OR = 0.92, 95% CI 0.66-1.29). In subgroup analysis, a significant association was found in the dominant model (CC+TC vs. TT, OR = 1.24, 95% CI 1.02-1.51) in Africans, but not in Asians or Caucasians.
CONCLUSIONSOur meta-analysis did not provide enough powerful evidence to identify a significant association between IFNGR1 -56C/T polymorphism and TB susceptibility in the overall population. In subgroup analysis, it indicates that IFNGR1 -56C/T is possibly associated with increased TB risk in Africans, but not in Asians or Caucasians. However, larger sample size and better-designed case-control studies are needed to validate these findings.
