Objective To explore the clinical diagnosis,medication,curative effect,and prognosis of lymhocyte subsets and humoral immunity in children with Kawasaki disease(KD)complicated with coronary artery dilatation.Methods One hundred and seventy-one children(age of 4 months to 9 years)from Jun.2000 to Dec.2007 with KD in Capital Institute of Pediatrics were taken as research objective,65 healthy children(age of 5 months to 7 years)were taken as controls,the difference of the level variation of lymhocyte subsets and humoral immunity was detected and analyzed between them.The flow cytometry and scattering turbidimetry method were used to detect the numbers of T cell subsets [CD3+,CD3+CD4+,CD3+CD8+,CD19+,natural killer(NK)cell] and levels of immune globulin(IgG,IgA,IgM)of the peripheral blood of 171 patients,compare them with those of the control group,and compare them between patients with and without coronary artery dilatation.Results Compared with control group,in the acute period of KD the levels of CD3+,CD3+CD4+ decreased significantly(t=0.01,0.02 Pa0.05);of all the 171 children patient,53 cases were coronary artery expansion patients.Compared with non-expansion patients,levels of CD4+/CD8+,CD3+CD8+,IgG of coronary artery expansion patients were significant different and had statistical difference(t=0.02,0.04,0.03 Pa

3.The combined application of PBPK model and PopPK model in the dose selection of pediatric drug development: an example of rivaroxaban

Model-informed drug development (MIDD) in the development of pediatric drugs is drawing more and more attention due to the insufficiency of subjects, lack of research on ontogeny, and the limitation of ethic. The core of MIDD used for dose selection includes the population pharmacokinetic (PopPK) model and physiologically based pharmacokinetic (PBPK) model, as well as model-based simulation and prediction. PBPK model has the advantage of predicting the optimal pediatric dose before the clinical trials and has the ability of extrapolation from adult model to pediatric model. PopPK model characterizes the pediatric PK feature based on the analysis of clinical data and can be used to explore the significant covariates, which is a power tool for individualized medicine in children. With their own advantages and disadvantages, PBPK and PopPK model should be jointly used in the pediatric drug development to refine the dose regimen for children at different ages. In this study, the pediatric drug development of rivaroxaban was taken as an example to introduce the combined application of PBPK model and PopPK model in the design and validation of pediatric dose regimen in Phase Ⅰ, Ⅱ and Ⅲ trials, which may provide reference to MIDD in other pediatric drug development.
4.Antifungal susceptibility testing of clinical isolates of itraconazole-resistant Aspergillus fumigatus to four antifungal drugs

Objective To evaluate the susceptibility of itraconazole-resistant Aspergillus fumigatus isolates to four other antifungal drugs.Methods Six strains of Aspergillus fumigatus were serially isolated from a patient with lung aspergilloma,who was resistant to itraconazole therapy.Clinical Laboratory Stan- dard Institute broth microdilution method M38-A and E-test were applied to test the susceptibility of A.fumi- gatus strains to amphotericin B,itraconazole,voriconazole,caspofungin and micafungin.Results With M38-A method,the minimal inhibitory concentration(MIC)of itraconazole was 0.5?g/mL for 2 strains and more than 16?g/mL for the other 4 strains;the MICs of amphotericin B and voriconazole were 1?g/mL and 0.25-1?g/mL respectively for all the 6 strains;and the minimal effective concentration(MEC)of caspofungin and micafungin were no more than 0.03?g/mL for all the 6 strains.Moreover,E-test results also revealed that caspofungin and voriconazole had a good antifungal activity against all the strains,including the 4 itraconazole-resistant ones.Conclusion The itraconazole-resistant A.fumigatus strains isolated from a patient with lung aspergilloma are susceptible to amphotericin B,voriconazole,caspofungin,and micafungin.
5.Progress on application of central composite design-response surface methodology in optimization of preparations in China

Central composite design (CCD) is one of the most commonly used design methods in response surface optimization and has been widely applied in the field of pharmaceutics to optimize preparations. On the 20th anniversary of the introduction of CCD into China, the paper reviews its application in domestic pharmaceutical researches. Based on the brief introduction of basic principle and operation steps of CCD, the mistakes emerging in the application of CCD are summarized, including conceptual confusion with Box-Behnken design and face-centered CCD as well as wrong designs. Besides, the issues concerning the selection of factors and responses are discussed. The article is helpful for researchers to comprehensively understand the CCD and facilitates the rational application of this method.

· AIM: To observe the inhibitory effect of triamcinolone acetonide (TA) on the proliferation of monkey choroidretinal endothelial cells (RF/6A) in hypoxia or normal conditions.retinal endothelial cells of rhesus monkey (RF/6A). The effect of TA on the cellular activity was observed by MTT,the effect on cellular proliferation and apoptosis was detected by flow cytometry (FCM).cycle were reduced and the proportion of cells in G2-M phase was increased under the hypoxia condition .TA had a great effect on the cell cycle of choroid-retinal endothelial cells of rhesus monkey and it induced apoptosis of endothelial cells. It relatively increased the S-phase cells and reduced G2-M phase cells under both normal and hypoxia conditions, which indicates its role in blocking cell cycle from s-phase to G2-M phase and reducing mitosis.RF/6A cells while TA has the opposite effect in both normal and hypoxia conditions. TA can also induce apoptosis of endothelial cells.
8.An intensity window in the effects of pulsed electromagnetic fields on bone metabolism in ovariectomized rats

Objective To investigate the effects of pulsed electromagnetic fields (PEMFs) of different intensities on bone metabolism in ovariectomized rata. Methods Sixty 10-month-old female Sprague-Dawley rats were randomly divided into a normal group, an ovariectomized control group, a 0. 14 mT group, a 0. 16 mT group, a 0.18 mT group and an estrogen group. All rats except those in the normal group were subjected to bilateral ovariectomy. Beginning one week after the operation, the rats in the 0. 14 mT, 0. 16 mT and 0.18 mT groups were treated with PEMFs at 50 Hz, 60 min daily for 90 days, while those in the estrogen group received estrogen instead. During the experiment, the bone mineral density (BMD) of the whole body was observed dynamically, and local BMDs and biochemical indexes were measured after 3 months of treatment. Results Alkaline phosphatase (ALP) activity in the normal group was significantly lower than in the other groups. Tartrate-resistant acidic phosphatase 5b (TRAP5b)activity was elevated significantly, in the control and 0.14 mT groups compared with the normal group. After 2 months of treatment, whole body BMD was reduced significantly in the 0.14 mT group compared to the normal group. After 3 months of treatment, whole body BMD in the ovariectomized controls and the 0.14 mT group was reduced significantly compared to the normal group, but significantly elevated in the 0. 16 mT, 0.18 mT and estrogen groups when compared to the ovariectomized control group. At the end of 3 months of treatment, the trends in lumbar vertebral BMD were similar to those of the whole body BMD, with the femoral BMD in the ovariectomized group and the 0.14 mT group significantly lower than in the normal group, though the differences among the other groups were not statistically significant. Conclusions PEMF treatment at 50 Hz and 0.16 mT or 0.18 mT can promote bone formation. PEMFs at 50 Hz and 0.14 mT might stimulate bone resorption. An intensity-window effect exists in the action of PEMFs on bone metabolism in ovariectomized rats.
9.HIF-1α expression and retinal cell apoptosis in rat retina ischemia-reperfusion injury

AIM:Investigating the expression of HIF-1α,apoptosis of retinal cells and the role of HIF-1α in apoptosis in rats' retinal ischemia-reperfusion injury.METHODS:The rat model of experimental retinal ischemia-reperfusion injury was established by increasing the intraocular pressure to 110mmHg(1kPa=7.5mmHg)in rat eyes.At different time points of post ischemia,the expression of HIF-1α of the retina was detected by immunohistochemicalstaining,and apoptosis of the retinal cell was detected by terminal deoxynudeotidyl transferase medialed deoxyuridine triphosphatebiotin nick end labeling(TUNEL).RESULTS:HIF-1α appeared in the cells of retinal ganglion layer and inner nuclear layer at 2 hours after ischemia.The expression reached to a peak,12 hours after retinal ischemia-reperfusion,then the expression was dedined.The apoptotiC Cells were mainly in inner nuclear layer and could be detected at the 12th, 24th and 48th hour after ischemia,the peak value was the group of 24th hour.CONCLUSION:Expression of HIF-1α in the rats' retina is greatly enhanced after ischemia-reperfusion,which may be involved in the retinal injury:the injury of retinal neurons Occurs partly in the form of apoptosis.The expression of HIF-1α may play an important role in cell apoptosis.
10.Fingerprint analysis of Resina Draconis from different manufactuers by UPLC coupled with chemometrics.

This study is to establish an UPLC fingerprint of Resina Draconis from different manufacturers, which can provide a comprehensive evaluation for its quality control. The analysis was performed on a Phenomenex Kinetex 2.6 μ C18 100A column by agradientelution program with acetonitrile-water as mobile phase at a flow rate of 1.7 mL x min(-1). The column temperature was 40 degrees C and the detection wavelengthwas 280 nm. The fingerprints of 18 batches of Draconis Resina were further evaluated by chemometrics methods including similarity analysis (SA), hierarchical clustering analysis (HCA) and principal component analysis (PCA). As a result, there were 15 common peaks, 13 of which had been identified by LC-Q-TOF MS, and the similarity degrees of 15 batches of the samples was more than 0.9, and the samples were divided into 4 clusters by their quality difference. The method is reproducible, simple and reliablethat it can be used for quality control and evaluation of Resina Draconis from different manufacturers.
