1.Correlation between polymorphisms of transforming growth factor-β1, tumor necrosis factor-related apoptosis-inducing ligand genes and nodular thyroid disease

Objective To study the correlation between individual gene polymorphisms of transforming growth factor (TGF)-β1 + 869 T/C,tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) + 1525 G/A genes and nodular thyroid disease.Methods From September 2007 to December 2009,a total of 544 patients with nodular thyroid disease diagnosed in the Department of Endocrinology,The First Affiliated Hospital of Baotou Medical College,Inner Mongolia University of Science and Technology were selected,including 136 cases of nodular goiter patients (node group),132 cases of thyroid tumor (adenoma group),146 cases of Graves patients (GD group),and 130 cases of Hashimoto's thyroiditis (HT group).One hundred and thirty-five healthy subjects were enrolled as control group.Two milliliters of fasting venous blood of all subjects were collected.Polymorphisms of the TGF-β1 + 869 T/C and the TRAIL 1525 A/G genes were identified by the polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) and the restriction fragment length polymorphism (PCR-RFLP) methods.Results TGF-β1 + 869 T/C:The CC genotypes and C allele frequencies of nodular goiter group [47.0%(64/136),63.2%(172/ 272)] were significantly higher than those of normal control group [18.0%(22/135),45.2% (122/270); x2 =30.76,17.79,all P < 0.05].The genotypes and allele frequencies of adenoma group[42.4% (56/132),59.1% (156/264)] were significantly higher than those of the normal control group (x2 =24.40,10.34,all P < 0.05).The risk of population carrying the C allele suffering from nodular goiter was 2.086 times of those carrying the T allele (OR =2.086; 95% CI:1.480-2.943).The risk of population carrying the C allele suffering from adenoma was 1.752 times of those carrying the T allele (OR =1.752,95% CI:1.244-2.469).TRAIL + 1525 G/A:the genotypes and allele frequencies of nodular goiter group [40.4% (55/136),62.9% (171/272)] were significantly higher than those of normal control group [12.0% (16/135),48.5% (131/270); x2 =9.176,11.307,all P < 0.05].The genotypes and allele frequencies of adenoma group[53.3% (70/132),73.1% (193/264)] were significantly higher than those of the normal control group (x2 =9.806,33.82,all P < 0.05).The risk of population carrying the G allele suffering from nodular goiter was 1.796 times of those carrying the A allele (OR =1.796,95% CI:1.275-2.531).The risk of population carrying the G allele suffering from adenoma was 2.884 times of those carrying the A allele (OR =2.884,95% CI:2.009-4.142).Conclusions TGF-β1 + 869 T/C and TRAIL + 1525 G/A gene polymorphisms may be related to the incidence of nodular thyroid diseases; G allele of TRAIL and C allele of TGF-β1 may be predisposing genes of patients with nodular goiter.
2.The research of Lp-PLA2 in patients with atherosclerosis acute ischemic stroke

Objective To observe the changes of lipoprotein‐associated phospholipase A2(Lp‐PLA2) in patients with athero‐thrombosis acute ischemic stroke and the correlation of Lp‐PLA2 with cerebra infarction volume ,neurologic impairment and early prognosis .Methods A total of 104 patients with atherothrombosis acute ischemic stroke and 100 healthy people(controls) were collected .The levels of Lp‐PLA2 were detected by immunoturbidimetry .The cerebral infarction volume was checked by Cranial MRI or CT .The neurological impairment degree was assessed by National Institute of Health Stroke Scale (NIHSS) score .The early prognosis was assessed by Barthel index .The changes of serum Lp‐PLA2 levels in patients with different infarct volumes and neurologic impairment were compared ,the influence of early prognosis were analysised .Results The levels of serum Lp‐PLA2 in the patients with atherothrombosis ischemic stroke were significantly higher than those in control group (P<0 .05) ,and signifi‐cantly correlated with the levels of serum LDLC (r=0 .859 ,P<0 .05) .The levels of serum Lp‐PLA2 were significantly correlated with the increasing of cerebral infarction volume and cerebral neurological impairments (r=0 .531 ,P<0 .05 ;r=0 .623 ,P<0 .05) . The patients with higher level of Lp‐PLA2 has poor prognosis(P<0 .05) .Conclusion The level of Lp‐PLA2 was significantly higher in patients with atherothrombosis ischemic stroke and may reflect the severity and early prognosis in patients with athero‐thrombosis acute ischemic stroke .
3.Differentiation of mesenchymal stem cell in the microenviroment of retinitis pigmentosa

AIM: To assess the differentiation of rat mesenchymal stem cell (MSC) in the microenvironment of retinitis pigmentosa(RP) induced by the administration of sodium iodate. METHODS: In vitro cultured Lewis rat MSC were injected into the subretinal space of NaIO3 induced RP rat eyes (30g/L NaIO3 100mg/kg). To observe the trace and differentiation of MSC by immuno-fluorescent method successively in 5 weeks after the surgery.RESULTS: The majority of the transplanted cells stay in retinal pigment epithelium(RPE) layer and cones and rods layer. From the 2nd week after transplantation, the engrafted MSC expressed PCK and rhodopsin under fluorescent microscope.CONCLUSION: MSC can survive mainly in the outer layer of retina in the microenvironment of RP and differentiate forward the RPE cell and photoreceptor.
4.Clinical effect of circumcision stapler in the treatment of phimosis and redundant prepuce.

OBJECTIVETo observe the clinical effect and safety of circumcision stapler in the treatment of phimosis and redundant prepuce.
METHODSWe treated 120 patients with redundant prepuce or phimosis using circumcision stapler and another 60 by conventional dorsal-incision circumcision. We observed intraoperative blood loss, operation time, postoperative pain, wound healing time, cosmetic appearance of the penis, and postoperative complications and compared them between the two groups of patients.
RESULTSStapler circumcision showed obvious advantages over the conventional method in intraoperative blood loss ([2. 3 ± 1. 3] vs [15.6 ± 2.9] ml), operation time ([7.1 ± 1.4] vs [22.6 ± 4.6] min), wound healing time ([12.0 ± 2.9] as [16.3 ± 3. 1] d), postoperative pain score (1. 9 ± 1. 3 vs 5. 2 ± 1. 7), incision edema, and cosmetic appearance of the penis (all P <0. 05). Besides, stapler circumcision exempted the patients from stitch-removal pain. However, the incidence rate of postoperative local ecchymosis was significantly higher in the circumcision stapler group than in the conventional circumcision group (20. 8% vs 8. 3% , P <0. 05).
CONCLUSIONCircumcision stapler, with its advantages of easier manipulation, shorter operation time, better cosmetic penile appearance, less pain, and fewer complications, is superior to conventional circumcision in the treatment of phimosis and redundant prepuce.

5.Effect of interferon on HEL cell apoptosis and JAK2 V617F mutation gene expression

Objective To observe the effect of Interferon-α2b on HEL cells (human erythroleukemia cell line) growth, apoptosis and JAK2 V617F mutation gene expression. Methods HEL cells were placed in RPMI1640 containing 10% FBS and incubated in a cell incubator. Cells in the logarithmic growth phasem were collected, adjusting the cell density to 1 × 105/mL for experimental research. The interferon concentration in five groups were 0, 5 × 105, 10 × 105, 50 × 105, 100 × 105 U/L, with different incubation time (0, 24, 72, 120 h), respectively. The cell growth status in different groups was observed in the inverted optical microscope; MTT was used to detect the inhibition of interferon on HEL cell proliferation. Cell apoptosis was detected by flow cytometry. Fluorescence quantitative PCR was used to detect the mutation gene of JAK2 V617F expression. Results Inhibition rates of Interferon on the HEL cell proliferation in 5 × 105 U/L, 10 × 105 U/L, 50 × 105 U/L, 100 × 105 U/L groups were 18.57%, 25.10%, 42.10%, 57.00%, respectively. JAK2 V617F/GAPDH by fluorescence quantitative was 1.556, 1.213, 0.870 respectively under the concentration of interferon 100 × 105 U/L for 24, 72, 120 h. Conclusions Interferon-α2b can inhibit HEL cells proliferation and induce HEL cells apoptosis. Increasing concentration of interferon increases HEL cell apoptosis rate. Interferon can inhibit JAK2 V617F expression of HEL cells in a dose-dependent manner.

Objective To investigate the imaging manifestations of lung neoplasms in pre- and post- treatment with CT-guided Argon-Helium cryoablation.Methods All the lung neoplasms in 96 patients have been treated with CT-guided percutaneous Argon-Helium targeted cryoablation.All patients have pre- and post-treatment CT scanning in measurement of lesion size and CT value.The CT scanning has been rerpeated afterl,3,6,12 months of treatment.Results Among total 96 cases,there are 82 cases of lung cancer and 14 cases of metastasis with 110 lesions(89 cases with single lesion,7 cases with multiple lesions).The Ar-He cryoablation has been given 103 times in total.The size of each lesion ranged from 1.2 cm to 15.0 cm in diameter with mean value of(4.0?2.5)cm,including 12 lesions less than 2 cm,51 lesions between 2— 4 cm,24 lesions between 4—6 cm,23 lesions over 6 cm.There are 25 patients whose lesions covered by iceball with 1 cm of overloaping it's margin.There are 63 lesions with diameter less than 4 cm gained 100% ablation rate,24 lesions with 4—6 cm diameter gained 95.8% ablation rate,and 23 lesions with over 6 cm diameter gained 69.6% ablation rate.The post-treatment CT show a progressively enlarged round,low density refrigerant area which clearly demarked with non- refrigerant area.The center of each refrigerant area has negative CT value,the mean decreased CT value of lesion instantly after the treatment are about 30— 50 HU with P
7.Clinical effect of EGFR-TKIs combined with platinum-containing chemotherapy and whole brain radiotherapy in the treatment of non-small cell lung cancer with brain metastasis

Objective To explore the clinical effect of epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKIs) gefitinib combined with platinum-containing chemotherapy and whole brain radiotherapy in the treatment of non-small cell lung cancer (NSCLC) with brain metastasis. Methods One hundred and fifty-six patients of NSCLC with brain metastasis were divided into observation group (82 cases) and control group (74 cases) according to the therapeutic regimens. On basis of whole brain radiotherapy, the patients in control group received platinum-containing chemotherapy (paclitaxel+cis-platinum+pemetrexed), while the patients in observation group received gefitinib. The serum levels of carcino-embryonic antigen (CEA), cytokeratin 19 fragment antigen (CYFRA21-1) and EGFR, short-term effects, progression-free survival (PFS) and incidences of adverse reactions between two groups were evaluated. A subgroup analysis was carried on the 41 cases of patients with EGFR mutant type. The short-term effects and PFS between two groups were compared. Results The levels of CYFRA21-1 and EGFR in observation group were obviously lower than those in control group (P<0.01). The ORR and DCR in observation group was obviously higher than that in control group (81.71% vs. 36.49%, 93.90% vs. 82.43%) (P < 0.05 or < 0.01). The median PFS in observation group and control group was 12 months and 6months. There were statistic differences between two groups (P < 0.05). Subgroup analysis showed that the ORR and DCR in observation group was obviously higher than that in control group (72.0%vs. 25.0%, 96.0%vs. 31.25%)(P<0.01). The median PFS in observation group and control group was 13 months and 5 months. There were statistic differences between two groups (P<0.05). There was no statistic difference in incidences of adverse reactions between two groups (P>0.05). Conclusions For patients with NSCLC with brain metastasis, compared with the therapeutic regimen of whole brain radiotherapy + platinum-containing chemotherapy, whole brain radiotherapy + platinum-containing chemotherapy+EGFR-TKIs (gefitinib) could more effectively reduce the levels of EGFR and CYFRA21-1, improve short-term effects, prolong PFS and do not increase the incidence of adverse reactions.
8.Isolation and Resistance of Acinetobacter calcoaceticus-baumannii in Hospital: A 7-Year Surveillance Study

OBJECTIVE To study the isolation and resistance tendency of Acinetobacter calcoaceticus-baumannii to antimicrobial agents from 1998 to 2004 to provide valuable data for infection prevention and therapy. METHODS We reviewed the isolation rates,distribution in clinical specimens and wards,and the resistance rates of(A.calcoaceticus-baumannii)to 14 kinds of antimicrobial agents from 1998 to 2004. RESULTS There was an increasing tendency of isolation rates of A.calcoaceticus-baumannii every year,which was 0.18% in 1998 but 1.48% in 2004.In the seven years,there was the highest isolation rate of 70.58% in specimens from respiratory tract,the next was from the urine(9.42%),and blood(4.63%).Concerning the wards distribution,ICU had the highest rate of 47.28%.In 1998,A.calcoaceticus-baumannii had resistance rates more than 50% only to one kind of antimicrobial agents(aztreonam),but in 2004,it had increased to thirteen kinds(except cefoperazone/sulbactam).About the fourteen kinds of antimicrobial agents we inspected,that were increased in their resistance rate.The highest increasing of resistance rate was ceftazidime from 11.1% in 1998 to 88.9% in 2004,the imipenem was second for 0.0% to 64.8%,and the third was sulfamethoxazole/trimethoprim form 0.0% to 64.0%,while there still was an increasing resistance tendency to them. CONCLUSIONS The clinical isolation rate of A.calcoaceticus-baumannii is increasing,and it has higher resistance rates to many antimicrobial agents as well as an increasing resistance tendency to relatively susceptive antimicrobial agents every year.So physicians should prescribe on the basis of antimicrobial agents susceptibility tests in vitro.
9.Laparoscopic distal pancreatectomy for pancreatic benign and low-grade malignant tumor

Objective To explore the surgical techniques in laparoscopic distal pancreatectomy for benign and low malignant tumors in the body or tail of the pancreas.Methods The clinical data of 21 cases of benign and low-malignant tumors in the body or tail of the pancreas undergoing LDP from Jan 2015 to Mar 2017 in the First Hospital of Shanxi Medical University were analyzed retrospectively.Results One patient was converted to open surgery(4.76%),13 patients underwent laparoscopic spleen-preserving distal pancreatectomy (Kimura procedure).The other 7 patients underwent LDP with splenectomy.The average size of the tumor was (6.0 ±3.1)cm;the operation time was 190 to 421 mins with mean time of (288.4 ± 56.9)min;the intraoperative blood loss was 30 to 800 ml with the mean volume of(235.7 ± 202.6)ml;Postoperative hospital stay was 6 to 17 days with the mean time of (8.9 ± 3.1) days.Six patients suffered from type A pancreatic fistula,and were cured by conservative treatment.Conclusions Laparoscopic spleen-preserving pancreatectomy for benign or low-grade malignant body and tail pancreatic tumors is feasible and safe.
10.Laparoscopic pancreaticoduodenectomy via a "G "-shaped approach: experience of 33 cases

Objective To investigate the feasibility and safety of "G"-shaped surgical approach in laparoscopic pancreaticoduodenectomy.Methods The clinical data of 33 patients who received " G"-shaped laparoscopic pancreaticoduodenectomy in the First Hospital of Shanxi Medical University from April 2015 to March 2018 were retrospectively analyzed.Results All patients underwent surgery successfully.The time required for surgery ranged from 340 to 498 min,the blood loss ranged from 150 to 800 ml,and the specimen resection time ranged from 135 to 270 min.There were 10 cases of biochemical leakage,3 cases of B grade pancreatic fistula and 1 case of bile leak.Postoperative pathology confirmed lower bile duct adenocarcinoma in 17 cases,duodenal papillary adenocarcinoma in 11 cases and poorly differentiated adenocarcinoma of the pancreatic head in 5 cases.Conclusions " G"-shaped approach as a simple,effective and safe way for LPD,is particularly helpful for inexperienced hands.
