Objective To analyze the molecular characteristics of Salmonella paratyphi A ( S. pa-ratyphi A) strains prevailing in Hangzhou area in recent years. Methods Pulse field gel electrophoresis ( PFGE) and multiple-locus variable-number tandem repeat analysis ( MLVA) were performed for molecular typing and epidemiological analysis of 72 S. paratyphi A strains isolated in Hangzhou area during 2002 to 2013. Results The 72 S. paratyphi A strains were divided into 11 PFGE ( by using restriction enzymes of Xba Ⅰ and Bln Ⅰ) and 6 MLVA types. Among the selected 34 variable number tandem repeat ( VNTR) sites, 4 sites (1188K, 2075K, 2201K and 4346K) showed high polymorphism, in which PFGE displayed a higher resolution than MLVA. Except for the 5 PFGE types of X4B5, X7B7, X8B8, X9B9 and X10B10, the other 6 PFGE types belonged to a same clone sharing a similarity of greater than 95%, and the S. Paratyphi A strains in that clone accounted for 93. 1% of the total strains isolated in Hangzhou. Conclu-sion The occurrence of paratyphoid A in Hangzhou area from 2002 to 2013 was mainly caused by S. para-typhi A strains belonging to the same clone. Combination of PFGE with MLVA was conducive to epidemiolog-ical investigation of paratyphoid A.

Objective To identify the key factors triggering medical disputes induced by differences in cognition between doctors and patients in order to control the occurrence and development of medical disputes. Methods From 2002 to 2006,the outpatients, inpatients and medical staff of 3 hospitals in Shanghai(class 3 first level general hospital,n=2;class 3 first level special hospital,n=1) were investigated, and random sampling method was employed for the research analysis. Results There were significant differences between doctors and patients in cognition of influential factors triggering medical disputes such as health law, medical services, rights and obligations. Conclusion The differences in cognition between doctors and patients are important causes for medical disputes, and effective prevention and intervention measures must be taken.

Objective To evaluate a combination therapy for huge tumors in the buttocks. Methods A total of 11 patients from our hospital were collected, among them 5 cases were of hemangioma,4 cases of neurofibroma,2 cases of soft tissue sarcoma. Before definite surgical resection all cases received tumor embolization with silk thread and gelatin-sponge article using Seldinger's technic.Subsequently, all patients underwent a successful tumor resection. Results Superselective embolization for all the cases'feeding arteries resulted in recession of the tumors and relatively well-demarcated margins, and all the lumps became softer.and the local pain was alleviated.Surgical resection could be radical with avoidance of fatal intraoperative hemorrhage. The 5 cases of hemangioma had a average operative bleeding of 450 ml,4 cases of neurofibroma had 420 ml,2 cases of soft tissue sarcoma had 150 ml.No patients needed intraoperative and postoperative blood transfusion. The operation time was about 2-3 hours, the normal tissues were preserved and the contour and function of the diseased limbs were very good. One case had a delayed incision healing, and the others had a healing by the first intention. There was no recurrence and other complications (like deep venous thrombosis)during a follow-up period of 4-8 months. Conclusions Surgical resection combined with interventional embolism for the treatment of huge tumors in the buttocks can reduce the risk of bleeding effectively during operation. It can improve the success rate of operation leading to satisfactory results.

Objective To study the reproductive health status in adolescents underwent repeated abortion and their psychosocial behavioral status and to understand their demanding on the knowledge of contraception, so as to provide suggestions on post abortion care. Methods From July ,2007 to May,2008,400 unmarried adolescents who visited doctors and met the following criteria were recruited in the study:①Younger than 24 years old;②Had artificial abortion at least once;③Have been pregnant for 5-10 weeks and asked for artificial abortion. Data were collected through symptom checklist 90 (SCL-90)and anonymous reproductive health questionnaire. The situation of reproductive tract infections (RTIs) were detected through gynecological examination and laboratory tests. The data was analyzed by SPSS 11.5 statistical software. Results The mean age of the subjects was 21. 18 ±2. 46 years. The mean time of previous artificial abortion was 1.96 ± 1.44,with 52% of ≥2 times. Aged less than 20 years old at the first abortion, having sex more than 3 times per week, more than 2 sex partners, and history of painless abortion were associated with repeated abortion in unmarried adolescents. The SCL-90 scores showed that the subjects had higher score of depression, terror, psychiatric symptoms, somatization, interpersonal sensitivity, anxiety and hostility factors than normal population (P < 0. 05). Of the subjects ,34. 5% had history of RTIs, and the current prevalence of RTI was 59. 5% ,and 58. 5% with one kind of RTI and 36. 0% with two kinds of RTI. Of these RTI patients,33. 5% did not present any clinical symptoms. Abortion for more than twice, first sex intercourse occurred at age younger than 18,history of RTI,sex intercourse of more than 3 time per week, multiple sex partners,especially18,history of RTI,sex intercourse of more than 3 time per week, multiple sex partners, especiallyhaving multi sex partner at the same, and poor sex sanitation were risk factors of RTI. Conclusions There are various reproductive health problems among repeated abortion adolescents in mental and physical aspects, lack of knowledge on reproductive health and contraception is the main reason. Improving post-abortion services will be helpful to prevent repeated abortion in adolescents and protect their physical and mental health.
