1.Role of ImmunoCyt in the early diagnosis of bladder cancer

Objective To evaluate the effect of ImmunoCyt on the early diagnosis of bladder cancer by comparing the result of cystoscopy with that of cytology. Methods Eighty-six consecutive patients with bladder cancer (mean age,63 years) were included in this group.Among them 38 had undergone transurethral resection of superficial transitional cell carcinoma and had been followed up and 48 had hematuria.These patients were evaluated with standard cytology and ImmunoCyt test.Then all the patients underwent cystoscopy and suspected lesions were biopsied. Objective To evaluate the biocompatibility,immunology and physiologic features of encapsulated Newborn Porcine Islets(NPI) in the liver of the recipient dogs with type I diabetes.Methods Type I diabetic dogs were perfused with 400 000-600 000 encapsulated NPI(group A,n = 15)or unencapsulated NPI(group B,n = 15)through the hepatic artery without immunosuppressive treatment.Liver function and CD4/CD8 in the recipients were measured before and after the transplantation.The livers from all NPI recipient dogs were analyzed by histopathology 6 months after transplantation(Tx).Results Insulin dose administrated to group A was reduced gradually within one week after Tx,from 22 u before Tx to 5 u after Tx,exogenous insulin required for group B was decreased from 24 u to 10 u.However,2 to 3 weeks after Tx,the insulin dose given to group B returned to the original level before Tx.In contrast,the amount of insulin administrated to group A was continually reduced to 8 u.Moreover,CD4+ cells in the blood of group B recipients were higher than that before Tx,whereas no significant alteration of CD4+ cells and CD8+ cells in the blood of group A after Tx.All NPI recipient dogs demonstrated a normal function and structure of the liver after Tx.Conclusion Microcapsulated NPI has a good biocompatibility in recipients livers providing prolong-ation of xenograft survival,and correcting the hyperglycemia of diabetic canines.(J Intervent Radiol,2006,15: 607-610)
3.Evaluation of the curative effect of functional endoscopic sinus surgery by acoustic rhinometry.

OBJECTIVE:
To evaluate the curative effect of Functional endoscopic sinus surgery (FESS) who suffered with chronic rhinosinusitis by acoustic rhinometry.
METHOD:
We collected 60 patients who accepted FESS for chronic rhinosinusitis, and calculated the mean minimal cross-sectional area (MCSA), nasal volume (NV), nasal airway resistance (NAR) by acoustic rhinometry within endoscopy to estimate the curative effect of FESS.
RESULT:
Four weeks after FESS, the patients' ethmoid sinus,maxillary sinus,frontal sinus and sphenoid sinus were clear by endoscopy. Meanwhile, the mean MCSA and NV by acoustic rhinometry were increased, NAR by acoustic rhinometry were descended. There were significant differences between the quantitative levels before and 4 weeks after FESS.
CONCLUSION
As the determining methods of nasal function before and after FESS, acoustic rhinometry and endoscopy can be used to comprehensively evluate the curative effect of FESS.

5.Advance in the islet transplantation for the treatment of diabetes

Islet transplantation is effective in treating diabetes, however, its clinical use is highly restricted by a series of influencing factors. This review elucidates the non-immune factors including islet purification, instant blood-mediated inflammatory response and revascularization deficit on the development of islet transplantation, and also highlights the application and perspective of islet xenotrasplantation in the treatment of diabetes.
6.Evolution logic of integrated medical delivery system in the UK, the USA and Australia and lessons to learn

Medical service system integration follows the logic of system innovation and development.Case studies of the UK,the USA and Australia found that medical delivery system integration is the product of a series of mixed factors.Health care financing and payment mechanism reform,physician team building,stair-cased movement to grassroots of healthcare service,shared clinical norms utilization as well as its gradual development constitute the general logic of such an integration.In the evolution process,the check and balance between the government and market,equity and efficiency have been affecting the integration process all the way.In light of these logics,China should speed up its transform of the integration concept,improve the integration elements,and strengthen the guidance of integration principles during its integration of the medical service integration.
7.Changes and maintenance measures of liver physiological function in donor of brain death

The quality and function of the donor liver is one of the main factors which influence the success and prognosis of liver transplantation.At present,the major source of donor liver for transplantation is Donor of Brain Death (DBD) all over the world,which has unstable circulation.When the brain death occurs,a series of serious physiological function changes will be induced within a few minutes and affect the hemodynamics and homeostasis of the body,which will greatly influence the liver quality and function,and consequently the success and prognosis of liver transplantation,finally leading to the loss of potential donor organs.Therefore,it is necessary to identify the physiological function changes induced by the process of brain death and its injury to liver and take immediate proper protective measures,which can effectively reduce the organ injury,improve liver function and enhance the organ utilization and liver transplantation success.In this paper,the changes and maintenance measures of liver physiological function in DBD will be reviewed.
8.Development and clinical application effect of venipuncture auxiliary devices under special condi-tions

Objective To design and develop a kind of venipuncture auxiliary device to use in the absence of light condition or patients with poor vein condition, and validate its clinical effect. Methods A total of 70 patients with shock were divided into experimental group (venipuncture auxiliary device group with 37 cases) and control group (non-device group with 42 cases) by random digits table method. The one-time puncture success rate and operating time were compared between two groups. Results The one-time puncture success rate was 94.59%(35/37) in experimental group and 78.57%(33/42) in control group, and there was significant difference, χ2=4.214, P<0.05. The time of establishing a single vein passage was (73.63±29.35) s in experimental group and (107.61±45.94) s in control group, and there was significant difference, t=3.656, P<0.05. Conclusions The device is small in size, low cost, easy to carry, the screen is clear, is a rapid infusion with explore vascular equipment. It can guide nurses accurately on vascular puncture, fast, smooth and effective transfusion puncture process.
9.Endovascular treatment for aorto-bi-iliac artery total occlusive disease

Objective To investigate the feasibility, safety and efficiency of endovascular treatment for patients with aorto-bilateral-iliac artery total occlusive disease.Methods A total of 35 patients with aorto-bi-iliac artery total occlusive disease treated with endovascular therapy in Peking Union Medical College Hospital and the First Hospital of Shijiazhuang between Jan 2012 and Dec 2013 were retrospectively analyzed.Results There were 33 males and 2 females, mean age (67 ± 6) years treated during the study period.Technical success rate was 100%.129 bare stents and 4 covered stents were implanted.There were no peri-operative death.Postoperative leg ankle brachial index (ABI) improved significantly (0.86 vs.0.28, P < 0.28).Postoperative complications occurred in 2 patients (5.7%), including brachial artery thrombosis and rupture of external iliac artery post-dilation.The mean follow-up period was 16.5 months (2-28 months).Two patients (5.7%) were lost to follow up.Re-intervention was performed in 3 patients (8.6%) due to reocclusion of the stents.Primary patency was 91% (30/33) Conclusions Endovascular treatment is effective for aorto-bi-iliac artery total occlusive disease with low complications and acceptable mid-term patent rate.
10.Assessment of Therapeutic Effect in Acupuncture Treatment of Intermittent Lameness Caused by Lumbar Stenosis Syndrome

Purpose: To explore the therapeutic method for lameness caused by lumbar spinal stenosis. Methods: 28 cases of the patients with lameness were treated by puncturing Dachangshu (BL 25), Weizhong (BL 40), Chengshan (BL 57) and Kunlun (BL 60) and simultaneously another 26 cases were treated with routine method for control study. Results:After the treatments for four weeks, the scores of the symptoms in the two groups were remarkably elevated. In the acupuncture treatment, the results showed remarkable effect in 4 cases, effect in 21 cases. In the routine treatment, the results showed remarkable effect in 4 cases and effect in 8 cases. Conclusion: acupuncture treatment is better than the routine conventional treatment for lameness caused by lumbar spinal stenosis.
