1.Titanium Clips in the Treatment of Defects Caused by Endoscopic Mucosal Resection:Report of 62 Cases

Objective To evaluate the therapeutic effects of titanium clips occlusion for the defects and complications caused by endoscopic mucosal resection(EMR).Methods Totally 62 patients with protuberant lesions in the esophagus or gastrointestinal tract(≤15 mm in diameter) were treated by EMR in our hospital.During the operation,69 lesions were cut and the surgical defects were clipped using titanium clips.The intra-and post-operative complications of the patients were reviewed.Six weeks after the treatment,the patients received re-examination by endoscopy to observe the healing of the mucosal defects.Results The 69 lesions in the 62 cases were resected completely.At each defect,1-3 titanium clips were used,no perforation or hemorrhage occurred in the patients.Endoscopy performed in 6 weeks showed that all the defects were healed without ulcer,stenosis or recurrence.The titanium clips dropped 65 of the defected mucosa.Conclusion Titanium clips is effective and safe for defects caused by endoscopic mucosal resection.

Obiective To introduce the design and implementation of system for health examination and health monitoring according to the needs and workflow of the Health Examination Center. Methods The system has an effective configuration based on HL7 and smart client technology. Results The system can truly meet the actual needs and realize the digitizing management of the health examination center. Conclusion The system plays an important role in optimizing the wotkflow and can improve service quality and efficiency of health examination to promote digital construction in hospital.

4.Clinical examination and evaluation of stereoacuity with multi-dimensional spacial perception model in children with strabismus and amblyopia

Background There is multi-dimensional order of spatial stereopsis perception in human,however,current stereopsis examination is zero-order of position disparity.A multi-dimensional space perception model is very important for the detection of stereoacuity.Objective This study was to screen the deficit of zero-order,first-order,second-order multi-dimensional spatial stereopsis perception in amblyopia and strabismus children and to explore the association of zero-order,first-order,second-order spatial perception deficit.Methods Multidimensional spacial perception was examined in 79 children aged 4-14 years in Beijing Children' s Hospital.Nineteen normal children,19 children with ametropia amblyopia,12 children with anisometropic amblyopia,18 children with strabismus and 11 children with strabismus combined amblyopia were included this study.The random-dot and line spatial stereopsis perception in zero-order,first-order and second-order were examined with a new system of multidimensional space perception screening.Written informed consent was obtained from each subject or custodian before any ocular examination associated with this study.Results Absence of zero-order,first-order,second-order random-dot channel was found in 24 children (24/79,30.4%),18 children (18/79,22.8%) and 24 children (24/79,30.4%) respectively,with an average percentage of 27.9%.Absence of zero-order,first-order,second-order line channel was examined in 37 children (37/79,46.8%),37 children (37/79,46.8%),32 children (32/79,40.5%),with an average percentage of 44.7%.In the children with a deficiency of the zero-order spatial perception,the children who still remained the first-order or/and second-order spatial perception of random-dot accounted for 41.6% and that of lines accounted for 43.2%.In children without deficiency of zero-order random-dot or lines spatial space perception,deficiency of first-order and/or second-order spatial perception was in 37.5% children.Various order spatial perception deficiency was seen in children suffering from amblyopia or strabismus compared with normal group(P < 0.05).Conclusions There exists spatial perception deficiency in children with amblyopia or strabismus.The patients with zero-order spatial perception absence partially remain a first-order or/and second-order spatial perception;while the patients with normal zero-order spatial perception might have first-order or second-order spatial perception deficiency.The multi-dimensional space perception model has a directive role for the training of visual information process and the treatment of spatial perceptual learning in children with amblyopia or strabismus.
5.Healthcare-associated methicillin-resistant Staphylococcus aureus infec-tion outbreak in neurosurgical intensive care unit

Objective To investigate the causes of an outbreak of healthcare-associated infection with methicillin-resist-ant Staphylococcus aureus (MRSA)in a neurosurgical intensive care unit(NSICU).Methods Epidemiological investigation on 8 patients with lower respiratory tract infection (LRTI)in a NSICU between June 15 and June 28,2104 were performed by combination methods of prospective and retrospective survey.Results The attack rate of MRSA LRTI in NSICU patients was 22.86%,a total of 16 MRSA isolates were detected from patients’clinical specimens,nasal vestibule,as well as hospital surroundings during the period,pulsed-field gel electrophoresis (PFGE)result revealed that infection outbreak was caused by two subtypes of MRSA;risk factors analysis showed that long length of stay in ICU and aspiration of spu-tum through bronchoscopy were risk factors for MRSA LRTI.Conclusion Contamination of bronchoscope was the key factor for this epidemic spread of healthcare-associated MRSA infection.
6.Applications of three-dimensional visualization technology in individualized diagnosis and treatment oh hepatic hemangioma

Objective To investigate the values of three-dimensional visualization technology in the diagnosis and surgical treatment of hepatic hemangioma. Methods Thirty two patients with hepatic hemangioma who had been hospitalized during the period from July 2010 to January 2014 in our hospital were scanned by 64-slice spiral computerized tomography (CT) before surgical treatment. Three-dimensional (3D) reconstruction based on the CT data was carried out to achieve dignosis and surgical planning. Assisted with the 3D model , we chose the best surgical procedure for liver resection, protecting the major blood vessels near hemangioma and retaining normal liver tissue as far as possible. Results The location, size and shape of hepatic hemangioma, vascular variation, and spatial relationship with intrahepatic vessel were shown factually by three-dimensional reconstruction. All the hemangiomas were preoperatively assessed to be resectable. The compliance rate for preoperative surgical planning to actual surgery was 100%. Under assistance of the 3D model during surgery , 14 patients received laparoscopic hepatectomy and 18 underwent hepatectomy. Pringle′s maneuver was applied in 18 patients , with blocking time of (15.32 ± 7.12) min and blood loss of (188.63 ± 66.37) mL. The postoperative complications included pleural effusion in 5 patients and incision infection in one patients. Conclusions Three-dimensional visualization technology for the individualized diagnosis and treatment of hepatic hemangioma helps reduce surgical trauma and incidence of postoperative complications.
7.Monitoring rehabilitation system for the cervical spine based on computer software

BACKGROUND:Conventional rehabilitation evaluation call not provide more objective data for analysis,so all innovative method to monitor rehabilitation of the cervical spine is introduced.OBJECTIVE:The monitoring rehabilitation system of the cervical spine develops independently under Visual Basic 6.0 environment.It can gather both video and photos at dynamic and static states.According to the suitable feature points of the video,the system could calculate characteristic parameters,such as moving angles,distances,speed,acceleration,and integral curves.Thus,the system could create patients'database.RESULTS:The system could calculate different kinds of patients'characteristic parameters through data recording.Through the objective comparison of characteristic parameters,the turning flexibility of head and controlling capability for head turning could be reflected.CONCLUSl0N:The design is an innovation in the monitoring rehabilitation systems of the cervical spine.It makes up the shortcomings of conventional rehabilitation evaluation and objectively reflects the rehabilitation status of patients.
8.The effect of HIFU ablation combined with UCA on normal tissue around VX2 liver tumor in rabbits

Objective To explore the optimized range of normal tissue ablation around the tumor by HIFU in combination with UCA for VX2 liver tumors in rabbits. Methods In this study,51 rabbit models with implanted fiver VX2 tumors were randomly divided into 3 groups:ablating the tumor only(group A)、ablating the tumor and 2 mm eircumferential normal tissue(group B)、and 4 mm normal tissue around the tumor(group C).After HIFU+UCA treatment,the pathology of the targeting tissues,the level of serum ALT,the metastases in the liver、lungs and abdominal cavity,and the survival time were compared with each other. Results After treatment coagulative necrosis was observed in all the target tissues.Serum ALT level between each two groups were statistically different on d3 and d5(F=12.933、18.37,P<0.01、<0.01),and the level of ALT in group C was the hightest.The ALT level in group A was significantly higher than group B or group C on the 14th day(F=17.2,P<0.01).Metastases in group A was significantly higher than group B or group C,and there were no statistically different between group B and C (P=0.353、0.332、0.380).The survival time both in group B and group C were significantly longer than group A(48±13.6,51±12.1 vs 29±5.4),and there were no statistically difference between group B and C(P>0.05). Conclusions Tumor eradication can be significantly promoted when 2mm normal liver tissue around the tumor is ablated by HIFU+UCA.
9.Clinical observation of therapeutic effects of endoscopic retrograde cholangiopancreatography in elderly patients over 80 years old with biliary-pancreatic diseases

Objective To evaluate the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) in elderly patients over 80 years old with biliary-pancreatic diseases. Methods One hundred and twenty patients over 80 years old who underwent ERCP between January 2004 and April 2008 were retrospectively analyzed. The clinical efficacy, safety and complication were observed and followed up. Results One hundred and seventeen patients successfully underwent therapeutic ERCP and the success rate was 97.5%. All diseases of the patients included cholangiocarcinoma in 22 cases, duodenal papilla carcinoma in 3 cases, pancreatic carcinoma in 8 cases, acute pancreatitis in 15 cases, chronic pancreatitis in 5 cases, acute suppurative cholangitis in 9 cases and choledocholithiasis in 58 cases. During the procedure of ERCP, 3 cases failed to insert the duodenoscope, 3 cases were found hemobilia and 2 cases were found acute pancreatitis. Conclusions Therapeutic ERCP is effective and safe in elderly patients over 80 years. Age is not the contraindication of therapeutic ERCP.

And then the application of data mining technology in Chinese medicine pair research was classified and summarized, such as the compatibility characters, characteristic pairs, dosage-effect relationship and property compatibility, which provide the direction and data base for modern research of Chinese medicine pair.
