1.Rational use of antibiotics in children with mycoplasma pneumoniae encephalitis

Mycoplasma pneumoniae encephalitis is one of the extrapulmonary complications of mycoplasma pneumoniae infectiontion children.The clinical diagnosis of mycoplasma pneumoniae encephalitis is difficult, so it is necessary to make stratified diagnosis based on the diagnosis of encephalopathy and the reliability of pathogen detection evidence.In addition to considering whether antibiotics are sensitive to mycoplasma pneumoniae, the selection of antibiotics should also consider the tissue penetration of antibiotics and the safety of children′s medication.These problems may lead to the delay in diagnosis as well as treatment of mycoplasma pneumoniae encephalitis in children and affect the prognosis of children.This paper intended to introduce and summarize the diagnosis of mycoplasma pneumoniae encephalitis and the selection principles and characteristics of main antibiotics for mycoplasma pneumoniae encephalitis, hoping to be helpful to clinicians.
4.Clinical Observation of Runchang Xuanfei Shugan Formula in Treatment of Slow Transit Constipation

Objective To observe the clinical efficacy of Runchang Xuanfei Shugan Formula in the treatment of slow transit constipation. Methods Totally 94 patients were randomly divided into treatment group and control group, with 47 patients in each group. Treatment group was given Runchang Xuanfei Shugan Formula, one dose per day, half a dose for morning and half a dose for evening, and the control group was given lactulose oral liquid, 15 mL each time, for morning and evening, and citrate moser Billy 5 mg, three times a day orally taken before meals. The treatment course for the two groups were 4 weeks, with three-month follow-up. The defecation symptom score, anxiety and depression score were observed, the colonic transit test meter was performed, and the side effects were recorded. Results The degree of defecation difficulty, defecation frequency, defecation time, abdominal distension and stool quality score of the two groups significantly decreased after treatment (P<0.05), and the treatment group was much lower than the control group after treatment and three months after treatment (P<0.05). The score of anxiety and depression (SAS, SDS) of the treatment group decreased significantly after treatment and three months after treatment (P<0.05), and had significant differences with the control group (P<0.05). The colon function was significantly improved after treatment in both groups (P<0.05), without statistical significance (P>0.05). Conclusion Runchang Xuanfei Shugan Formula can improve clinical symptoms of slow transit constipation, ease anxiety and depression of patients, and promote colon transmission function.
5.L-Arginine-NO pathway inhibits the hypertrophic response of cultured cardiomyocytes induced by angiotensin Ⅱ

Aim This study is designed to analyze the influence of L-arginine (L-Arg) on expression level of angiotensin Ⅱ receptors (ATR) and the mitogen-activated protein kinase (MAPK) in cultured cardiomyocytes, with the intention to clarify the mechanisms relative to L-Arg's inhibitory effects on formation of cardiac hypertrophy. Methods Five groups of cardiomyocytes were established: control group, Angiotensin Ⅱ (ATⅡ) group, ATⅡ + Saralasin group, ATⅡ + L-Arg group, ATⅡ + L-Arg+L-NAME (N-nitro-L-arginine methyl ester) group. After 48 hours in supplemented culture, synthetic velocity of protein, NO production, expression level of ATR and p38 MAPK in cardiomyocytes were detected through the [3H]-leucine incorporation method, colorimetry, RT-PCR and western blotting, respectively. Results L-Arg could decrease the expression level of ATR1 and phosphorylated p38 MAPK, enhance NO production and reduce the synthetic velocity of protein in cultured cardiomyocytes stimulated by ATⅡ. Both ATⅡ and L-Arg had no influence on the expression level of angiotensin receptor type 2(ATR 2). Correlation analysis revealed that the relationship of negative correlation was significant between NO production and each of following factors: ATR1 expression level and phosphorylated p38MAPK expression level; As indicated by multiple stepwise regression analysis, not ATR2 expression level, but ATR1 expression level acted as the regression predictor of expression level of phosphorylated p38MAPK. Conclusion By enhancing myocardial NO production, L-Arg-NO pathway inhibits the p38MAPK activation mediated by ATR1, leading to inhibition of the hypertrophic response of cardiomyocytes. ATR2 seems to be independent from the activation process of p38 MAPK.
6.Effect of short-term treatment with metformin on psychological distress and metabolic feature in patients with polycystic ovary syndrome

Objective To observe the effect of short-term treatment with metfbrmin on psychological distress and metabolic feature in patients with polycystic ovary syndrome(PCOS).Methods Ninety women were diagnosed as cases of PCOS based on the 2003 Rotterdam criteria.These patients were divided into three groups:group A,consisted of 26 subjects treated with mefformin only; group B,36 cases treated with metformin plus Dane-35 ; and group C,28 cases treated with placebo and Dane-35.The treatment was carried out for 3 months.Clinical and metabolic parameters were observed.The psychological distress was evaluated by the hospital anxiety and depression scale(HAD scale).Results Compared with group C,patients in group A and B showed significantly lower body mass index(BMI),waist circumference (WC),waist-hip ratio (WHR),serum fasting insulin,homeostasis model assessment for insulin resistance (HOMA-IR),total cholesterol (TC),triglyceride (TG),low density lipoproteincholesterol(LDL-C) levels as well as anxiety and depression scores,but higher high density lipoprotein-cholesterol (HDL-C) levels (all P<0.05).Compared with group A,patients in Group B had significantly higher WC,WHR,and LDL-C levels(all P<O.05).No significant differences in BMI,TG,HDL-C,anxiety and depression scores were found between group A and B (all P>0.05).In group A,a positive correlation was found between the changes of metabolic feature (BMI,WHR,and HOMA-IR) and the changes of anxiety and depression scores (all P <0.05).Conclusion Short-term treatment with metformin will benefit patients with polycystic ovary syndrome,regarding psychological distress and metabolic features.
9.Mine Disaster Rescue and Medicine Supply before Hospitalization

OBJECTIVE: To provide reference for rapid and effective first aid to mine disaster before hospitalization. METHODS: The status quo of emergent rescue for mine disaster in China was reviewed. The key to rescue, main countermeasures and necessary drug of first aid before hospitalization were analyzed according to the characteristics of mine disaster and troubles of medical rescue. RESULTS & CONCLUSIONS: Ambulance corps cooperated with medical staffs to set ICU at disaster site. Scientific rescue provided by professional team can improve survival rate. Meanwhile, a perfect mine disaster rescue system should be set up.
10.Screening of miRNAs in ovarian cancer patients and its correlation with clinicopathological features

Background and purpose:The prognosis of ovarian cancer is poor and the diagnosis is relatively late. It is needed to search for early diagnosis and treatment for ovarian cancer. This study investigated the expression of serum miRNAs in patients with malignancy or benign ovarian tumor preoperativey and analyzed its correlation with clinicopathological progress and prognostic features of epithelial ovarian cancer.Methods:Forty-eight miRNAs which have been reported to be related to ovarian cancer were ordered. The differential expression of 48 miRNAs in the serum of patients with malignant or benign epithelial ovarian tumors was detected by TaqMan low density array. The differentially expressed miRNAs were further confirmed by real-time fluorescent quantitative polymerase chain reaction (RTFQ-PCR). The relationship between the expression level of selected miRNA and clinical clinicopathological factors, progress and prognosis were analyzed.Results:TaqMan low density array and further RTFQ-PCR showed that only miR-125b was sig-nificantly increased in 135 ovarian cancer patients as compared with 38 individuals with benign tumor. The expression of miR-125b was higher in early stage patients than that in advanced stage patients (P=0.039). The patients without residual tumor expressed more miR-125b than patients with residual tumor (P=0.013). The high level of miR-125b was signifi-cantly correlated with longer progress free survival (PFS) (P=0.003), but not correlated with overall survival (OS) (P=0.069). Conclusion:MiR-125b may play an important role in the pathogenesis of epithelial ovarian cancer and prognosis. It may be a potential gene to predict the recurrence of epithelial ovarian cancer, but the change of gene expression at different stages and its underlying mechanism need further research.
