1.Assessment of activated carbon in standard lymphadenectomy in gastric cancer

ObjectiveTo assess activated carbon in the standard lymphadenectomy of gastric cancer.Methods183 cases undergoing radical gastrectomy before 1997 served as control group,87 consecutive cases after 1997 received intumour injection of activated carbon(AC) before undergoing gastrectomy.The areas and number of dissected lymph nodes,operative complication and hospital stay in subtotal gastrectomy and total gastrectomy were analyzed respectively.ResultsIn subtotal gastrectomy,the ratio of D 1 ,D 2 ,D 3 and D 3 +PAL over the total cases was 4 7%,10 3%,14 9% and 16 1% respectively in AC group and 25 7%,21 3%,9 9% and 2 7% respectively in control group.In total gastrectomy,the ratio of D 1 ,D 2 ,D 3 and D 3 +PAL was 3 4%,5 7%,21 9% and 23% respectively in AC group and 15 8%,14 8%,8 7% and 1 1% respectively in control group.The differences of the four procedures were all statistically significant between the two groups.The number of dissected lymph nodes in each case in AC group was more than that of control group.ConclusionsLocal injection of activated carbon is a simple,easy and safe method to increase the dissection of lymph nodes in radical gastrectomy.
2.A preliminary study on evaluation the performance of hospital academic papers by Hirsch index

Objective:To explore the feasibility of using Hirsch index(H index) to evaluate the performance of academic papers published by medical talents.Methods:67 medical talents of Wuxi People′s Hospital Affiliated to Nanjing Medical University were enrolled in the study. The numbers of papers published and citations of these talents which have been included by PubMed, Chinese Citation Database, CNKI, Wanfang, and Baidu Scholar. The H value of these talents was calculated according to the list method proposed by Rousseau R. The H index was compared with the traditional evaluation indexes of scientific research performance, such as the number of papers published, the total number of citations and the average number of citations. Pearson test was used to explore the correlation between H index and traditional evaluation index.Results:The H value of 67 talents ranged from 1 to 14. Pearson test showed that the H index was significantly correlated with the total number of papers, the total number of citations and the average number of citations at the level of 0.01, and their Pearson value was 0.746, 0.932, and 0.648 respectively.Conclusions:As a new scientific research performance evaluation system, the H index is recommended in the evaluation of medical personnel, but it needs further and sustained optimization in terms of database inclusion timespan, research work timespan, and authors ranking limit among others.

Objective To investigate the efficacy of sodium hyaluronate on reducing adhesions in patients undergoing peritoneal cavity surgery. Methods Peritoneum and the surface of the related organs was coated with 3~15 ml sodium hyaluronate gel before closing the peritoneal cavity. Findings on 23 reoperations out of 1 360 sessions of primary abdominal surgery with sodium hyaluronate were compared with that of 26 reoperations without the use of sodium hyaluronate in their first episode of laparotomy during the same period. Results The severity of adhesion was graded as 4 degrees. In treatment group, adhesion Ⅰ?,Ⅱ?,Ⅲ?,Ⅳ? were in 11,9,3 and 0 cases, respectively. OBJECTIVE: To investigate the anti-tumor effect of bufalin and its regulation on Bcl-2 and Bax proteins in orthotopically transplanted tumor of human hepatocellular carcinoma in nude mice. METHODS: Orthotopically transplanted tumor of human hepatocellular carcinoma was established in nude mice. The mice were randomly divided into five groups: high-dose bufalin-treated group (1.5 mg/kg), medium-dose bufalin-treated group (1 mg/kg), low-dose bufalin-treated group (0.5 mg/kg), adriamycin-treated group (8.0 mg/kg), and normal saline-treated group. After 25 days, mice were sacrificed. The tumor volume was measured, and the pathological changes of tumor tissues were detected by HE staining to observe the tumor necrosis degree. Cell morphological changes were also observed by an electron microscopy. Label index of tumor cell apoptosis was assessed by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL), and the expressions of Bcl-2 and Bax proteins were determined by immunohistochemical method. RESULTS: The tumor volume in the bufalin-treated groups was shrunk significantly compared with that in the normal saline-treated group (P<0.01). The survival time of the bufalin-treated groups was prolonged compared with that of the adriamycin-treated group and the normal saline-treated group P<0.05. Apoptotic characteristics could be seen in tumor tissues of the bufalin-treated groups. The label index of tumor cell apoptosis in the bufalin-treated groups (5.87+/-2.13, 8.86+/-2.96 and 10.60+/-3.42 in low-, medium- and high-dose groups respectively) was higher than that in the adriamycin-treated group (3.28+/-0.98) (P<0.05, P<0.01). The expression of Bax was up-regulated, while no changes were detected as to Bcl-2 protein in tumors of the bufalin-treated groups. CONCLUSION: Bufalin has significant anti-tumor effect on the orthotopically transplanted tumor of human hepatocellular carcinoma in nude mice. Its effect might be related to up-regulation of Bax protein and inducement of the tumor cell apoptosis.
6.Effects of continuous blood purification on inflammation and nutritional status in patients with sever sepsis

Objective To study the effect of continuous blood purification on inflammation and nutritional status in patients with sepsis, and to study relationship between inflammatory, malnutrition and illness. Method Forty-eight patients with severe sepsis were randomly divided into two groups: continuous renal replacement treat-ment (CRRT) group (n=27) and control group (n=21). The flowing biomarkers including albumin, preallbu-min, transfenin, insulin-like growth factor-1 (IGF-1), tumor necrosis factor (TNF-α) and C reactive protein (CRP) were determined before,and 1 week and 2 weeks after treatment. Results Compared with control group, levels of albumin, prealbumin and IGF-1 in treatment group increased significantly at 14 days after CRRT (P<0.05), and levels d CRP and TNF-α decreased significantly (P<0.05), resulted in increase in survival rate, shortened me-chanical ventilation time and decreased SOFA scores (P<0.05). The IGF-1 and prealbumin had a negative cor-relations with CRP and TNF-a in both groups (P<0.05). The survivors in both groups had significantly higher levels of IGF-1,prealbumin and TNF-α than the deads before trearment(P<0.05). Conclusions Continuous blood purification can effectively attenuate inflammation and improve nutritional status in patients with severe sep-sis.Maybe IGF-1 and prealbumin act as prognostic markers more sensitive in severe sepsis.

Objective To compare the values of a screening methods by OC-Hemodia and chemical fecal occult blood test (COB) for colorectal cancer in elderly people. Methods The screening programme targeted colorectal cancer in old people was conducted on 1 100 participants aged 60-92 years (mean was 81.3 years). All subjects received OC-Hemodia and COB examinations. Positive subjects were examined by colonoscopy and/or barium enema. Out of 1 100 subjects 1 046 were followed up in the next five years. Results Among 1 100 subjects, 231 subjects (21.0%) were positive by OC-Hemodia test and 159 subjects (14.5%) were positive by COB. Objective To understand and to grasp the basic situation on blood donors who were ineligible by ALT dry chemistry testing before blood donation.Methods The ALT rejection ratios used in initial screening and not used in two blood collection point were compared at the same time.The relativity between the dry chemistry testing and rate method were compared.An association was explored in various influencing factors on the activity of ALT.Results The use of ALT initial screening in blood donation had a significantly lower unqualified rate than not using it in early screening (P<0.05).Dry chemical,biological method and the results had good correlation rates (r=0.993,P<0.05).The B MI,drinking,sleeping,sports,diabetes,liver disease were independent factors affecting ALT activity (P<0.05).Conclusion ALT initial screening can significantly reduce the unqualified rates and disposal of blood.
10.Gene polymorphisms of esxA, esxH and fbpB in clinical Mycobacterium tuberculosis isolates from tuberculosis children

Objective To sequence the antigen-encoding genes esxA,fbpB and Rv2660c from clinical isolates of Mycobacterium tuberculosis (MTB) in tuberculosis children and study the polymorphisms.Methods A total of 135 isolated strains were amplified for gene sequences of esxA,fbpB and Rv2660c by PCR,and the products were sequenced respectively.The results of sequencing were compared with the corresponding gene sequences of standard strain H37Rv and human T and B cell epitope sequences from immune epitope database (IEDB).The genetic diversities of the gene sequences of esxA,fbpB and Rv2660c were analyzed.Results Among the 135 strains,no gene polymorphisms were found in esxA and Rv2660c.But fbpB had genetic diversities in 66 clinical isolates (48.89%).The 5 single nucleotide polymorphism (SNP) mutations in fbpB affected 8 (14.81%) T cell epitopes and 9 (24.32%) B cell epitopes.The dN and dS values of whole fbpB gene were 0.000 120 and 0.002 126,respectively.The dN and dS values of T epitopes areas were 0.000 067 and 0.002 382 respectively,those of non-T epitopes area were 0.000 408 and 0.000 470,respectively,those of B epitopes areas were 0.000 063 and 0.00 2380,and those of non-B epitopes areas were 0.Conclusion The gene sequences of esxA and Rv2660c are conservative,and that of fbpB contains genetic diversities.Its polymorphisms may influence the function of antigen Ag85B,and thus affect the efficacy of vaccine H56.
