1.Optical quality evaluation of pseudophakic eyes implanted different materials of IOL by optical quality analysis system

Background Phacoemulsification with IOL implantation can reduce the light scattering caused by cataract,but many patients with IOLs implantation complained of glare or blurred vision.IOLs from different materials will affect IOL refractive index (RI) and optical design,which affect the value of scattered light of IOLs implantation.Objective This study was to analyze the effect of IOL materials on optical quality in pseudophakic eyes with optical quality analysis system (OQAS).Methods The clinical data of 95 eyes from 65 patients with age-related cataract who received phacoemulcification with IOL implantation in Tianjin Eye Hospital from March to Octomber were retropectively analyzed.Three types of lOL,including AR40e,Acrysof SA60 AT and Rayner 620H,were implanted in 32 eyes,30 eyes and 33 eyes,respectively.The visual quality indexes were detected and evaluated using OQAS,including uncorrected distance visual acuity (UCDVA,LogMAR),objective scatter index (OSI),modulation transfer function cut off (MTF cut off),strehl ratio (SR),Profile width at 50% point scatter function (PSF),Profile width at 10% PSF and contrast visual acuity under the 100%,20%,9% contrasts.Results The UCDVA was ≥ 0.3 in the eyes of AR40e group,SA60AT group and 620H group 1 month after surgery.No significant difference was found in the best corrected distance visual acuity (BCDVA,LogMAR) among three groups (F =1.362,P =0.246).The OSI was 1.72±1.11,1.89±1.07 and 2.13±1.13 in the AR40e group,SA60AT group and 620H group,with a significant difference among the groups (F =3.638,P =0.045),and the OSI was significantly lower in the AR40e group than that in the SA60AT group or 620H group (both at P<0.05).No considerable difference in MTF cut off among the groups (F =2.401,P =0.192).The SR value was significantly elevated and the PSF at both 50% and 10% was declined in the AR40e group compared with the SA60AT group and 620H group (all at P<0.05).A significant difference was found in 9% contrast visual acuity among the three groups (F=3.866,P=0.038),and 9% contrast visual acuity was better in the AR40e group than that in the SA60At group or the 620H group (both at P< 0.05).Negative correlations were found betwee OSI and BCDVA in the AR40e group,SA60AT group and 620H group (r=-0.275,P=0.041;r=-0.287,P=0.042;r=-0.233,P=0.037).Conclusions OQAS can evaluate optical quality of cataract patients after opeartion objectively.Material and RI of IOL have a great effect on visual quality after phacoemulcification with IOL implantation,especially under the low contrast environment.The operated eyes implanted hydrophobic acrylic IOLs with a lower RI can get better optical quality in comparison with the eyes implanted hydrophilic IOLs.

3.Resiniferatoxin inhibits neuropeptide expression in rat with prostatitis

Objective To study the therapeutic effect of resiniferatoxin(RTX)on prostatic pain from chronic prostatitis.Methods Complete Freund's adjuvant(CFA)was injected into rat bilateral ventral prostate to form chronic prostatic inflammation in 15 male SD rats.Two weeks later,RTX was injected into lumbosacral vertebral canal(n=5)or bilateral ventral prostate(n=5).Another 5 SD rats served as normal control.Neuropeptide level and the mRNA expression of substance P(SP)and calcitonin gene-related peptide(CGRP)in lumbosacral spinal cord and dorsal root ganglia(DRG)were measured by immunohistochemistry,radioimmunoassay,and RT-PCR.Results SP and CGRP level and their mRNA expression increased significantly in rat with prostatitis.Neuropeptide level and their mRNA expression were decreased after RTX was injected into vertebral canal and prostate.Conclusion Local injection of RTX can effectively inhibit synthesis and release of SP and CGRP,suggesting that RTX may be helpful to the analgesic therapy of chronic prostatitis.
4.Deficit of social cognition with patients following the temporal lobes infarction

Objective To investigate the ability of social cognition in patients with right temporal lobe infarction.Methods Fifteen patients with temporal lobe infarction (TLI) and 20 healthy controls (HC) were examined with the recognition of faux pas and reading the mind in the eyes tasks for assessing the ability of social cognition.Results The TLI group performed significantly worse when compared with the HC group (28.6 ± 6.1,36.4 ± 4.3 respectively) as revealed in the total faux pas-related scores (U =41.00,P <0.01).For the control question,there was no significant difference between the TLI group and the HC group (9.86 ±0.35,10.00 ± 0.00 ; U =130.00,P =0.097).In the reading the mind in the eyes task,patients with TLI performed worse than the HC group(17.13 ±6.41,29.75 ±2.07 respectively),and the difference had statistical significance (U =0.00,P < 0.01).But there was no significant difference in gender recognition judgment (30.37 ± 1.16,31.35 ± 1.08 respectively; U =106.00,P =0.129).Conclusions The right temporal lobe involves in social cognition.Moreover,the right temporal lobe not only involves in the verbal related social cognitive processing,but also involves in the no-verbal related social cognitive processing.
5.Expression and significant of EphA2、 EphrinAl and E-cadherin in pancreatic carcinoma

Objective To investigate the relationship between EphA2, EphrinAl and E-cadherin expressions and tumor stage and prognosis in pancreatic cancer. Method EphA2, EphrinAl and Ecadherin expressions were detected by immunohistochemistry in the tumor tissue and normal tissue specimens from 48 patients with primary pancreatic cancer. Results The expressions of EphA2 and EphrinAl were higher in the pancreatic carcinoma tissues than in the normal pancreatic tissues (P<0. 05). The E-cadherin expression was lower in the pancreatic cancer tissues than in the normal pancreatic tissues (P<0. 05). With decreasing histological differentiation, the expressions of EphA2 and EphrinAl in carcinoma tissues increased significantly (P<0. 05), while the E-cadherin expression decreased significantly (P<0. 05). The positive expressions of EphA2 and EphrinAl in the primary tumor significantly increased in stageⅢ and Ⅳ than in stage Ⅰ and Ⅱ (47. 9%vs 6. 25% , P<0. 05;47. 9% vs 8. 3%, P<0. 05), while the negative expression of E-cadherin was reversely correlated with these tumor stages (14. 6% vs 64. 6%, P<0. 05). Cox multivariate analysis showed that the clinical stage, EphA2 positive expression and E-cadherin negative expression were significantly associated with survival. Conclusion Abnormal expressions of EphA2, EphrinAl and E-cadherin were involved in the progression of pancreatic cancer and they were useful in predicting prognosis.
6.The analytical performance assessment and clinical practice of three sensitive cardiac troponin Ⅰ assays

Objective To assess the analytical performance of three sensitive cardiac troponin Ⅰ assays and compare the clinical application to provide help in choosing the detection method for clinical laboratory. Methods A total of 474 serum samples were collected from apparently healthy subjects and a total of 112 serum samples were collected from patients presenting with symptoms suggestive of acute myocardial infarction. The functional sensitivities of three assays from Abbott, Beckman-Coulter and Ortho were determined ( CV = 10% ). The reference ranges have been established. The analytic performance was compared according to the assessment mode described by Apple. The relationship was compared among the different assays. The preliminary clinical application value for different detection methods has been evaluated and validated with self-established reference ranges. Results The functional sensitivities ( CV = 10% ) of the cTnI assays for Abbott, Beckman-Coulter and Ortho were 0. 030, 0. 04 and 0. 013 μg/L, respectively.The 99th percentiles of cTnI in healthy volunteers were 0. 021, 0. 02 and 0. 026 μg/L respectively. The analytical data of ROC curve showed that the area under curve (AUC) of the cTnI assays for Abbott,Beckman-Coulter and Ortho for diagnosis of AMI was 0. 852,0. 909 and 0. 910,respectively. There was no statistical difference between any two methods(Z1 = 1.18 ,Z2 = 1.21 ,Z3 =0. 026,all P >0. 05). There were good consistency between the 99th percentile obtained from our laboratory and suggested by manufacturers (Kappa value were 1. 000, 0. 730 and 0. 893 respectively, all P < 0. 01 ). Conclusions The analytical performance of two cTnl assays is "clinical accepted" ,the other one is accepted according to guideline. All of them could detect cTnI in apparently healthy subjects. There exist differences among three assays, but their diagnostic characteristics for AMI are not significantly different.
7.Systematical Review and Meta Analysis of the Efficacy of Acupuncture and Moxibustion plus Cognitive Rehabilitation Training in Treating Post-stroke Cognitive Disorder

ObjectiveTo systematically assess the efficacy of acupuncture and moxibustion plus cognitive rehabilitation training in treating post-stroke cognitive disorder.MethodA computer searched CNKI, CBM, VIP, WANFANG DATA, and Pubmed and Cochrane Library for randomized controlled trials (RCT) of acupuncture and moxibustion plus cognitive rehabilitation training for the treatment of post-stroke cognitive disorder. The reference literature in the included literature was also retrieved. Retrieval time limit for both of them was from Jan. 1st 1990 to Jan. 1st 2015. Two reviewers independently selected the trials, extracted the data and assessed the quality of methodology. Meta analysis was then performed using RevMan 5.3 software.ResultEleven trials with a total of 789 subjectswere finally included. The results of Meta analysis showed that the following aspects were better in acupuncture plus cognitive rehabilitation training than in cognitive rehabilitation training or medication alone and there were statistically significant differences: the total efficacy rate [RR=1.58, 95%CI(1.10,2.26),P=0.01], the MMSE score [MD=2.64, 95%CI(1.78, 3.50),P<0.00001], P300 latency [MD=﹣18.46, 95%CI(﹣30.51,﹣6.41),P=0.003], P300 amplitude [MD=1.23, 95%CI(0.82, 1.63),P<0.00001] and activities of daily living (ADL) [SMD=0.52, 95% CI(0.31, 0.73),P<0.00001]. Based on the results of a systematical review, thequality of evidence was assessed using GRADE system recommended classification method. The results showed that the level of evidence was low and the strength of recommendations was weak.ConclusionThe results of Meta analysis showed that the therapeuticeffect was better in acupuncture and moxibustion plus cognitive rehabilitation training than in cognitive rehabilitation training or medication alone. Because of low quality of all the original literature, high-quality, multicenter and large-sample randomized blind controlled trials are still needed for validation.
8.Long-term follow-up study of titanium implant impact on pediatric mandibular growth and development.

OBJECTIVETo explore the impact of titanium implant on the growth and development of pediatric mandible after suffering from mandibular fracture and undergoing open reduction and internal fixation (ORIF) compared with those that underwent titanium plate removal postoperatively.
METHODSFifteen pediatric patients with mandibular fracture who underwent ORIF were included in this study. Eight patients did not undergo titanium implant removal postoperatively, whereas the other seven patients underwent the routine. The postoperative data of the pediatrics were collected for comparative analysis by taking the patients' frontal and lateral photos, recording the inter-incisor distance, and measuring the height of mandibular ramus, length of the mandibular body, and combined length of the mandible in three-dimensional reconstruction image.
RESULTSAll patients had acceptable facial contour, mouth opening, and occlusion, without obvious abnormalities. The radiography showed no significant difference between the bilateral mandibular lengths in the two groups of patients (P>0.05).
CONCLUSIONThe titanium plants have no significant impact on the growth and development of pediatric mandible postoperatively; hence, the question on whether the titanium plates should be removed or not may be neglected. The removal operation may lead to secondary trauma; thus, performing titanium plate removal routinely is not recommended.

9.Study of the location of coronary ostial in the artic valve stenosis by the real-time three-dimensional transesophageal echocardiography

Objective To study the the feasibility of real-time 3D transesophageal echocardiography (RT-3D TEE) to measure the distance between the coronary ostial and the junction of left ventricle and artery.Methods The RT-3D TEE image of the aortic root was collected intraoperative in 65 cases who were refered to our center for valve replacement.The reconstruction of the aortic annulus was done in the QLAB quantitative analysis software and the height parameter of the annulus could be generated.Meanwhile the coronary artery position was observed and the distance between the coronary ostial and the junction of left ventricle and artery was measured respectively.Results In the 65 cases,49 cases clearly showed bilateral coronary ostial,three cases only showed the left coronary ostial,six cases only showed the right coronary ostial,seven cases of both sides of the coronary ostial were unclear.The shows rate of right coronary ostial was 84.61% (55/65),with the left coronary ostial 81.54% (52/65).The measured value of the L-AA in systole and diastole were (15.5 ± 1.9)mm,(12.8 ± 1.5)mm,the R-AA systole and diastole measured values were (13.7 ± 1.8)mm,(12.4 ± 1.7)mm,respectively,there was significant difference between systole and diastole respectively.The aortic annulus height (H) got by 3D reconstruction of the aortic annulus was lower than ventricular-arterial junction to the coronary artery distance in both systole and diastole.Conclusions Not only can RT-3D TEE reconstruct the annulus,but also can show and measure the distance between the coronary ostial and the junction of left ventricle and artery respectively.
