Objective To analyze the morphologic characteristics of the single infarct in the territory of perforating arteries which arise from the middle cerebral artery (MCA),and further to investigate the association between the type of cerebral infarction and the isolated stenotic disease of the ipsilateral MCA.Methods Fifty-five patients with acute ischemic stroke who were admitted to the Department of Neurology,Peking Union Medical College Hospital,Beijing,China during January 1,2005 and December 31,2006 were identified.All of the patients had a single acute infarction in the territory of the MCA perforating arteries revealed on diffusion weighted imaging (DWI).Transcranial Doppler (TCD) and magnetic resonance angiography (MRA) were performed for all patients.Patients with stenesis >50% of proximal internal carotid artery and potential cardiac sources of embolism were excluded from the study.Patients were classified into 2 groups according to the presence of the atberosclerotic stenosis of the ipsilateral MCA: patients with or without MCA stenosis.Size of the lesions was measured including the diameter,area and volume.The infarcts with a diameter less than 2 cm were classified as lacunar infarcts; those bigger than 2 cm were classified as the striatocapsular infarcts.The infarcts on DWI was seen in basal ganglia,the body of lateral ventricle and beth.The concomitance of subcortical multiple small old infarcts or leucoariosis on T2>WI-MRI between the two groups was assessed.Results Among these 55 patients,14 (25.5%) had stenosis of the ipsilateral MCA and 41 (74.5%) had a normal MCA.In the group of MCA stenosis,71.4% patients were lacunar infarcts; 67.3% patients were also lacunar infarcts in the normal MCA group.There was no significant difference between the two groups (χ2=0.147,P=0.701).No significant difference in the diameter,area and vo|ume of the infarcts was found between the two groups.The basal ganglia,the body of lateral ventricle and both of these places involvement accounted for 31.7% ,17.1%,51.2% in the normal MCA group; 35.7%,28.6%,35.7% in the stenotic MCA group.No significant difference was found (χ2=1.272,P=0.529).Twenty-three (56.1%) patients had concomitant of small old subcortical multiple infarcts or leucoariosis in the normal MCA group and 3 (21.4%) in the MCA stenosis group,and there was a significant difference between the two groups(χ2=5.033,P=0.025).Conclusions A series of pathologic mechanisms are supposed to cause the single infarction in the territory of MCA perforating arteries.Stenosis of MCA is an underlying cause in addition to perforating artery diseases.There is no significant difference in the size,volume or distribution of the lesions between the two groups.The concomitant of subcortical multiple small old infarcts or leucoariosis may demonstrate the perforating artery disease.

OBJECTIVETo study clinical results of reduction and fixation under arthroscopy for the treatment of osteochondrotical lesion in the knee.

METHODSFrom December 2012 to December 2013, 4 patients with the knee joint osteochondritis lesion and free bone fragments were treated with arthroscopy to detect the stripped cartilage, and then the cartilages were reduced with small incision and fixed by cartilage nail. There were 3 males and 1 female, with age of 15, 15, 20 and 27 years old. The durations of the disease were 1 d, 5 d, 1 month and 1 year. All the patients had swelling and effusion in the knee joint, and the floating patellar test was positive. CT examination showed bone defects and loose bodies. The Lysholm, VAS, and Tegner scale were used to evaluate the knee joint functions before and after operation.

RESULTSAll the patients underwent successful surgery. Postoperative CT showed good location of the cartilage without loss of reduced bone and cartilage. All the patients were followed up, and the duration ranged from 10 to 13 months, with a mean of 12 months. In one patient, arthroscopy examination was conducted for a second time to examine the connection of the fracture part to the surrounding cartilage, and it showed that the internal fixator was not absorbed. The Lysholm, VAS and Tegner scale of all patients were better than those before operation.

CONCLUSIONTreatment of osteochondritis lesion with the fixation of absorbable cartilage nails may reconstruct the integrity of articular surface and recover the stability of joints, and it is an effective treatment method.

OBJECTIVE To study the bactericidal efficacy of glutaraldehyde,and its stability and change in pH value.METHODS The carrier quantity bactericidal test,field mimic bactericidal test,stability test,pH value detection,periodical biological surveillance and follow-up investigation of hospital infection were used.RESULTS After using of 20 000 mg/L of neutral glutaraldehyde on Candida albicans(ATCC 10231),Pseudomonas aeruginosa(ATCC27853),Escherichia coli(ATCC 25922) and other 3 clinical isolates separately for 3,5,and 10 min,the average killing rate was 97.90%,99.83% and 100.00%;on the field mimic bactericidal test with MRSA contaminated gastroscope,the average killing rate was 87.84%,99.50%,and 100.00%,respectively.The concentration of disinfectant after storaging 14d on 54℃ was decreased,only loss 1.46%,the concentration loss of disinfectant on using endoscope 5,7,and 14d after disinfection was 2.64%,0.69% and 0.94%,respectively.The value of pH was decreased from 7.22 to 6.00,6.50,and 6.20.The qualification rate of 260 various endoscopes after surveillance was 100.00%.Their was one of hospital infection happened among 3 200 patients after endoscopy which were followed up.CONCLUSIONS The results show that,only under the following circumstances,namely the concentration of glutaraldehyde being no less than 20 000mg/L and the action time being no less than 10 min,timely alteration of glutaraldehyde,and regular biological and concentration monitoring could the disinfection of endoscope be qualified so that hospital infection can be prevented,and medical dispute may be put to an end.

Quorum sensing systems of pathogens are central regulators for the expression of virulence factors. Increasing evidence implies that targeting the quorum sensing system of many pathogenic bacteria is a promising therapeutic approach to control infections. In this work,we isolated 47 strains of actinomycetes from the mud sample of Jiaozhou Bay. Quorum sensing inhibitory activity was monitored by Chromobacterium violaceum CV026. As a result,the culture broth extract of actinomycetes WA-7 was found to have significant quorum sensing inhibitory activity. This strain was assigned to the genus Streptomyces based on its 16S rDNA sequence. Further investigation revealed that the extract could inhibit the quorum sensing-controlled violacein and proteases production of C. violaceum in a concentration-dependent manner.

OBJECTIVETo observe preventive and therapeutic effects of Tanshinone IIA (T II A) on oxaliplatin induced peripheral neuropathy (OlPN) and to explore its effects on the expression of calcitonin gene related peptide (CGRP) and never growth factor (NGF).

METHODSTotally 36 phase II - III patients with malignant tumor of digestive tract undergoing chemotherapy program with oxaliplatin, were equally assigned to the T II A group (using THA at 80 mg/day 1 day before oxaliplatin chemotherapy for 3 successive days) and the control group (using chemotherapy program with oxaliplatin alone) by segmented randomization. After 4 cycles of chemotherapy, the incidence degree and incidence of OlPN were evaluated. Sensory nerve conduction velocity (SNCV) and motor nerve conduction velocity ( MNCV) were tested by EMG evoked potential device. Serum levels of CGRP and NGF were also detected in the two groups before and after chemotherapy. The correlation of serum levels of CGRP and NGF to OIPN was assessed using linear correlation analysis.

RESULTSAfter chemotherapy the OlPN incidence was 27.8% (5/18 cases) in the T II A group, obviously lower than that in the control group (55.6%, 10/18 cases; P < 0.05). Compared with before treatment in the same group, SNCV and MNCV of common peroneal nerve were slowed down, serum NGF levels decreased, and serum CGRP levels obviously increased in the two groups (all P < 0.05). Compared with the control group after treatment, SNCV and MNCV of common peroneal nerve were obviously accelerated, serum NGF levels increased, and serum CGRP levels obviously decreased in the THA group (all P < 0.05). Results of linear correlation analysis indicated serum NGF level was negatively correlated with peripheral neuropathy (PN), serum CGRP expression was positively correlated with neurotoxicity (P < 0.05).

CONCLUSIONT II A could reduce the incidence of OlPN, which might be associated with inhibiting the expression of CGRP and up-regulating NGF activities.

Objective To prepare monoclonal antibody against Prostate Stem Cell Antigen(PSCA) and explore the inhibitory effects of Anti-PSCA mAb in treatments of human prostate cancer xenografts in mice. Methods Balb/c mice were immunized by PC-3 cell line. After fusing and screening, the anti-PSCA McAb’s characterizations were determined. Solid tumors in mice were produced by subcutaneous injection with PC-3 cells in the flanks of the mice.We picked out 10 mice bearing human prostate cancer xenografts.They were divided into a treatment group (n_1=5) and a control group (n_2=5).200 ?g Anti-PSCA mAb was injected into abdominal cavity of each mouse of the treatment group and PBS for them of the control group.Anti-PSCA mAb and PBS were administered once every three days for consecutive three times.The mice survival conditions of two groups were recorded during 5 weeks.The serum PSA, the tumor weights and dimensions of survived mice were measured.The tumor volume inhibition rate was calculated. T-test was performed to compare differences of PSA in serum,tumor weights and volumes between the treatment and control groups.Routine pathological slides of tumor tissue were observed under light microscope to evaluate the range of tumor tissues damaged by Anti-PSCA mAb. Ultrastructure was observed with transmission electron microscope. Results A marine McAb was produced, which raised against PSCA,belonged to IgG1 subclass. The average PSA serum level of the two groups were(3.28?0.55)ng/ml and(7.26?0.43)ng/ml.The weights of tumors of the two groups were(0.95?0.17)g and(3.08?0.18)g.The volumes of them were(164.59?14.08)mm3 and(548.49?19.79)mm3.There are remarkable differences between the treatment group and the control group(P

Objective To compare the consistence and difference between the assay results of the second generation of Tg (Tg Ⅱ) and the first generation of Tg (Tg Ⅰ) immunoassay,as well as to evaluate the impact of Tg Ⅱ on the clinical management of thyroid diseases.Methods Serum samples of 249 patients (30 with benign thyroid disorders and 219 with DTC;64 males and 185 females,average age 43.0 years)were collected and assayed by Tg Ⅱ and TgⅠ kits simultaneously.The measuring ranges of TgⅠ and TgⅡ were 0.10-1 000.00 μg/L and 0.04-500.00 μg/L,respectively.Data were analyzed by the Wilcoxon rank sum test and Spearman correlation analysis using IBM SPSS 19.0.Results The assay results of TgⅡ and TgⅠ strongly correlated (rs =0.979,P＜0.05).However,the median value of TgⅡ (2.31 (0.06-13.17) μg/L) was lower than that of TgⅠ(3.63(0.41-16.84) μg/L)(z=-13.25,P＜0.001).The difference between Tg Ⅱ and Tg Ⅰ got bigger when TgⅠ value decreased more.TgⅡ values were 11.09％ lower than TgⅠ (5.61(1.07-26.39) μg/L) vs 6.31(2.07-33.93) μg/L;z=-4.78,P＜0.05) in 30 patients with benign thyroid disorders and 37.71％ lower (2.18(0.07-7.47) μ.g/L) vs 3.50(0.39-10.18) μg/L;z=-9.02,P＜0.001) in 108 DTC patients without 131 Ⅰ treatment.But the above changes had no influence on clinical diagnosis and treatment.In the 71 DTC patients post 131Ⅰ treatment with low TSH and normal TgAb,there were 3 cases with TgⅠ＞1.0 μg/L but TgⅡ＜1.0μg/L,and 12 cases with TgⅠ＞0.1 μg/L but TgⅡ＜0.04 μg/L.Conclusions Serum TgⅡand Tg Ⅰ assay results are strongly correlated,though Tg Ⅱ value is slightly lower than Tg Ⅰ value.This difference may have no significant influence on the clinical diagnosis of thyroid diseases.However,TgⅡ may be better to evaluate the curative effect in some DTC patients post 131Ⅰ therapy.

OBJECTIVETo explore the improvement of dendritic cells' (DCs) functions in chronic hepatitis B (CHB) patients by two different drugs plasma, i.e., Shen supplementing and detoxification (SSD) and Pi invigorating and detoxification (PID), thus comparing which method was more effective to activate DCs to improve T lymphocyte functions.

METHODSTotally 30 CHB outpatients were recruited. They were assigned to the immune tolerant group and the immune clearance group, 15 in each group. Totally 60 mL peripheral blood was extracted to isolate and develop mature DCs. Chinese compound containing (Liuwei Ganlu Syrup for SSD) and (Sijun Ganlu Syrup for PID) plasma were added to promote the maturation of DCs on the 7th day. Besides, non-drug plasma was taken as the control. On the ninth day, HBV core 18-27 loaded core peptide and its own T lymphocyte were co-cultivated for 72 h. Then T lymphocytes were collected. The expression levels of CD3, CD28, CD4, and CD8, programmed death-1 (PD-1) were detected using flow cytometry.

RESULTSCompared with non-drug plasma, the expression levels of CD3, CD4, and CD28 could be improved, and the expression levels of CD8 and PD-1 could be reduced by the two methods, showing statistical difference (P < 0.05). Besides, SSD containing plasma showed better effect in improving the molecular CD28 expression rate, and reducing the molecular PD-1 expression rate on the T cell surface, showing statistical difference when compared with that of PID containing plasma (P < 0.05).

CONCLUSIONSIn vitro intervention of DCs by SSD and PID containing plasmas combined co-cultivation of its own T lymphocytes could promote the activation of DCs to improve the function of T cells and the expression of T cell surface molecules. Besides, SSD showed more significant effect on infection immune of HBV patients in the tolerance stage.
